[Usefulness of magnetic resonance imaging for the diagnosis of superior lumbar hernia: a case report].

A 48-year-old man consulted us with the chief complaints of right flank mass. On examination, there was a soft bulge on the right superior lumbar area. Since computed tomographic scanning showed subcutaneous fatty tissue, we suspected a superior lumbar hernia. The diagnosis was confirmed by magnetic resonance imaging (MRI) that revealed a lumbar muscle defect and prolapsed fatty tissue around the kidney. During the operation, a small hernia hiatus, 2 cm in width, opened in the superior lumbar triangle. The herniated fatty tissue was excised, and the defect of fascia was closed by overlapping the adjacent fascial structure. No signs of recurrence were found at 3 months postoperatively. The usefulness of MRI for the diagnosis is discussed and previous literature is reviewed.

Familial prostate cancer in four brothers.

We identified a family of 4 brothers with prostate cancer. The proband was the second son who was seen at the Department of Urology, Gunma University, Gunma, Japan, in May 1995, with a chief complaint of dysuria. The serum level of PSA was high, and a prostate needle biopsy revealed a moderately-differentiated adenocarcinoma. Subsequently, a total prostatectomy was performed. The family history revealed that the fourth son had been under treatment at the Saitama Cancer Center Hospital for prostate cancer since 1994. Therefore, the other 2 brothers became healthy male family members of the familial prostate cancer line, and their serum PSA levels were examined after obtaining their informed consent. Both had a high serum PSA level and biopsies revealed moderately-differentiated adenocarcinomas in both. Human leukocyte antigen class II gene typing was conducted on 3 of 4 brothers and 4 healthy family members who gave informed consent, and revealed the DRB1*0901-DQB1*0303 haplotype in the first, second and third sons. Familial prostate cancer is well documented in Europe and the US where the incidence of prostate cancer is high, but this is the first report of prostate cancer in 4 brothers in Japan.

[Severe pulmonary hypertension due to mitral regurgitation without overt sign of congestive heart failure: a case report].

A 49-year-old man was found to have a heart murmur at a local hospital and was referred to our hospital for further examination. Although he had no signs of congestive heart failure, echocardiography and cardiac catheterization showed severe pulmonary hypertension caused by severe mitral regurgitation. Mitral valve replacement was performed, followed by reduction of pulmonary artery pressure. This patient had an unusual combination of no signs of lung congestion despite severe mitral regurgitation with pulmonary hypertension.

[Clinical study on prostate cancer initially presenting with disseminated intravascular coagulation syndrome].

We experienced five patients with prostate cancer with disseminated intravascular coagulation syndrome (DIC) at the first presentation at Gunma University Hospital and affiliated institutions between 1991 and 1997. Their average age was 68 years, average DIC score at the first presentation was 10 and prostate specific antigen (PSA) level was more than 700 ng/ml. All of them had multiple bone metastases. The therapy for DIC and hormonal therapy for prostate cancer were simultaneously started at the first presentation before prostate needle biopsy, but all patients died. The average number of days from the start of DIC to death was 685 days. The patients initially showed a good response to therapy, but their conditions soon aggravated. The prognosis was extremely poor, but some proper therapies lead to the prognosis which was equal to that of prostate cancer in Stage D2 without DIC.

Carcinogenesis in accessory sex organs of rats induced by 3,2'-dimethyl-4-aminobiphenyl: response to androgen deprivation.

It is generally accepted that early human prostate cancers reveal higher androgen dependency than do advanced ones. In the present study, we examined whether the animal model of prostate cancer has already lost androgen dependency at the early stages of carcinogenesis. At experimental week 46, androgen deprivation was induced in rats and the incidences of atypical hyperplasia and cancer were examined in the ventral, dorsolateral prostate, coagulating glands, and seminal vesicles. Androgen deprivation significantly lowered the incidence of atypical hyperplasia in all four organs. As for the incidence of cancer, no significant differences were observed in the coagulating glands and seminal vesicles. Regarding atypical hyperplasia, androgen deprivation significantly decreased the proliferative cell nuclear antigen labeling index in the coagulating gland and seminal vesicles. The presence of cancer was also decreased in the coagulating gland but not in the seminal vesicles. With control group specimens, more intense staining of androgen receptor was observed in atypical hyperplasias than in cancers. Compared with the atypical hyperplasias, the cancers revealed low androgen dependency at the early stages of carcinogenesis. The cancers in the seminal vesicles also revealed higher androgen independency than did those in the coagulating gland.

Stimulative effect of transforming growth factor-beta on collagen synthesis by human prostatic stromal cells in vitro.

BACKGROUND: Expression of transforming growth factor-beta (TGF-beta) in the prostate has been reported. The purpose of this study was to evaluate the effect of TGF-beta on collagen synthesis by stromal cells isolated from a patient with benign prostatic hyperplasia (BPH). METHODS: Human prostatic stromal cells (HPSC) derived from BPH tissue were cultured in serum-free medium. After incubation with TGF-beta1, the concentration of procollagen type I C-peptide (PIP) in the HPSC-conditioned medium was measured by enzyme immunoassay while the concentration of procollagen type III N-peptide (PIIIP) was measured by radioimmunoassay. Per-cell production of each type of collagen was calculated by multiplying the measured concentration by the volume of medium and dividing by the number of harvested cells. RESULTS: One ng/mL of TGF-beta1 significantly (P < 0.05) increased collagen production by HPSC, to 220% and 120% of control for types I and III, respectively. Increasing the amount of TGF-beta1 to 10 ng/mL had no further effect. TGF-beta1 did not significantly affect HPSC number at concentrations of either 1 or 10 ng/mL. There was a strong correlation between PIP and PIIIP production (r = 0.929, P < 0.001). CONCLUSION: These findings suggest that TGF-beta1 stimulates accumulation of extracellular matrix in stromal BPH tissue without affecting proliferation of stromal cells.

Effect of sex hormones on the tissue localization of nuclear estrogen receptor positively stained cells in the seminal vesicle of immature castrated rats.

We studied the changes in the tissue localization of nuclear estrogen receptor (ER) positively stained cells in the seminal vesicle of immature castrated rats under various environmental conditions of sex hormones by immunohistochemical methods. In castrated rats of 6 weeks of age, the percentage of nuclear ER positively stained cells showed remarkable increase in the periglandular stroma region, but not in the epithelium and the peripheral stroma region. Estrogen administration to castrated rats dramatically increased the percentage of nuclear ER positively stained cells in both the epithelium and the peripheral stroma region, whereas cessation of estrogen treatment caused a significant percentile decrease. These results suggest that the nuclear ER expression in both the epithelial cells and the peripheral stromal cells seems to respond to estrogen. The concomitant treatment of estradiol-17 beta (E2) with 5 alpha-dihydrotestosterone (DHT) completely inhibited these E2 mediated ER expression in the epithelium and the stroma. This result suggests that ER works only when E2 is given in the absence of DHT in the seminal vesicle of immature castrated rats.

Expression and degradation of rat androgen receptor following castration, testosterone replacement and antiandrogens administration: analysis by Western blot and immunohistochemistry.

To elucidate the autoregulation of androgen receptor (AR) by androgen and antiandrogen, Western blot analysis and immunohistochemical study were performed. Castration reduced the immunodetected AR content, and nuclear staining was lost without cytoplasmic staining. Testosterone (T) supplement restored AR content. Quick response of AR content restoring following single administration of T was observed 48 hours after castration. The recovery of AR content detected by Western blot under each condition was accompanied by recovery of the reduced unclear staining intensities in the epithelia. Neither steroidal nor non-steroidal antiandrogens, chlormadinone acetate and flutamide, altered the AR content in normal rat ventral prostate 5, 12, 24 or 48 hours after single administration. Furthermore, neither of the drugs at various doses altered AR levels 12 hours after single administration. In summary, the rat AR is upregulated by androgen. Single administration of antiandrogens have no effect on immunodetected AR content.

[Etiology of prostate cancer and significance of screening for early prostate cancer].

The incidence and mortality rate of prostate cancer in Japan are lower than in Western countries. Though studies of migrating populations would suggest that dietary or environmental factors may influence the etiology of the disease. The Westernized lifestyle after the Second World War may have influenced the increasing incidence of this disease in Japan. The screening by prostate specific antigen (PSA) on an asymptomatic healthy population can detect early prostate cancer. It is expected that the mortality may be decreased through such a system. However, some scientists hesitate to perform an aggressive program of early detection because of the possibility of latent cancer detection, the difficulty of distinguishing between clinically important and unimportant cancer, a high false positive rate to detect early stage cancer, and the lack of well-executed randomized prospective studies. Current clinical studies of the prostate cancer have demonstrated that the unimportant cancer detection rate by PSA was less than 10%. When a 4.0 ng/ml PSA cut-off value is applied for early detection, the false positive rate is approximately 80%. It is important to make subjects examined by the screening program aware of its the meaning. However, there is not a complete and cost-effective diagnostic modality to detect early prostate cancer. Moreover, it is confirmed that radical prostatectomy improves survival, but it has not been confirmed to improve the mortality based on population. We propose that well-executed randomized studies by the end point of the survival, cancer death rate, cost effective, QOL and etc. are urgently needed.

Serial changes in proximal urethral function after transurethral balloon laser hyperthermia of the prostate (TUBAL-H) in a canine model.

We performed transurethral balloon laser hyperthermia of the prostate (TUBAL-H) in 6 mongrel dogs. To evaluate the changes in proximal urethral function after TUBAL-H, the urethral cross sectional area (CSA, cm2) and urethral pressure (Pu, cmH2O) were measured using a balloon probe that allows their simultaneous measurement, and the urethral compliance (Comp, cm3/cmH2O) was calculated from these parameters and serially evaluated. In addition, the changes in Pu, CSA and Comp after administration of an alpha-adrenoceptor antagonist were evaluated before and 16 weeks after TUBAL-H. Before TUBAL-H, the mean Comp was 0.013, and the mean maximum CSA (MCSA) was 0.66. Eight weeks after TUBAL-H, the mean Comp was 0.038, and the mean MCSA was 1.39, showing a significant increase (p < 0.01). Sixteen weeks, after TUBAL-H, the mean Comp was 0.026, and the mean MCSA was 1.21, being lower than those at 8 weeks after TUBAL-H, but significantly higher than those before TUBAL-H (p < 0.05). After administration of the alpha-adrenoceptor antagonist, phentolamine (1 mg/kg), before TUBAL-H, the mean Comp significantly increased to 0.046 (p < 0.05), and the mean MCSA to 1.40 (p < 0.01). The mean Comp was significantly increased to 0.033 (p < 0.05), by phentolamine administration 16 weeks after TUBAL-H, but no other changes were observed. After TUBAL-H, urethral elasticity increased, and this increase persisted for 4 months. The responses of Comp and MCSA to alpha-adrenoceptor antagonist administration before and 16 weeks after TUBAL-H suggested that part of the effects of TUBAL-H is due to damage to alpha-adrenoceptors.

Effects of a new steroidal antiandrogen, TZP-4238 (17 alpha-acetoxy-6-chloro-2-oxa-4, 6-pregnadiene-3, 20-dione), on spontaneously developed canine benign prostatic hyperplasia.

The effects of the new steroidal antiandrogen, TZP-4238 on spontaneously-developed canine prostatic hyperplasia (BPH) were studied in comparison with those of chlormadinone acetate (CMA), a steroidal antiandrogen used for the treatment of BPH and prostatic cancer in Japan. Aged beagle dogs (5-9 years old) with spontaneously developed BPH (mean prostate volume, 17.7ml) were treated orally with a placebo, TZP-4238 (0.1 mg/kg/day, 0.01 mg/kg/day), or CMA (3 mg/kg/day), for 25 weeks. Prostate volume was measured by transrectal ultrasonography before treatment and every 5 weeks during treatment. TZP-4238 produced a regression in spontaneously developed canine BPH, its effects being more potent than those of CMA. TZP-4238 reduced the content of testosterone, dihydrotestosterone (DHT) and androgen receptor in the prostates of these animals, suggesting antiandrogenic mechanisms of the agent. TZP-4238 also appeared to reduce 5 alpha-reductase activity by prevention of the androgen action in prostate as described above.

[Effects of antiandrogen TZP-4238 on the prostatic androgen receptor and prostatic androgen in rat].

TZP-4238, a new steroidal antiandrogen, is an orally active drug in rats and dogs. The effects of TZP-4238 on the androgen receptor and androgen content were examined in comparison with other antiandrogens in rats. The prostate DHT levels decreased markedly within 4-8 hr after a single oral administration of TZP-4238 8mg/kg. The prostatic testosterone concentrations fell below the detection limit (5pg/g of tissue) at 4-12 hr after the initiation of treatment. The plasma testosterone level also fell to 60% of the control level after 4-8 hr and then returned to the normal range. Eight percent of DHT present in normal prostatic tissue was located in the nuclear fraction. TZP-4238 reduced the concentration of DHT in nuclei to 50% of the normal level. The concentration of the plasma drug which induced a 50% decrease in the prostatic DHT, the IC50, was about 10ng/ml, while the IC50 value for plasma testosterone was 30-50ng/ml. After oral administration of 15-OH TZP-4238, the main metabolite, the level of plasma testosterone was significantly elevated above the control level. The androgen receptor level was markedly reduced at 24 hr following castration and returned to the normal range within 5 hr of a single injection of testosterone. TZP-4238 reduced the nuclear androgen receptor level to 60% at 24 hr after a single oral dose and then, the receptor content returned to its original level. Both 15-OH TZP-4238 and cyproterone acetate also reduced the androgen receptor and DHT contents to 50%. A series of in vivo studies demonstrated that TZP-4238 inhibited the uptake of testosterone and the decrease of DHT and testosterone, and decreased the nuclear androgen receptor in the rat prostate.

Exon switching and activation of stromal and embryonic fibroblast growth factor (FGF)-FGF receptor genes in prostate epithelial cells accompany stromal independence and malignancy.

Stroma and the heparin-binding fibroblast growth factor (FGF) family influence normal epithelial cell growth and differentiation in embryonic and adult tissues. The role of stromal cells and the expression of isoforms of the FGF ligand and receptor family were examined during malignant progression of epithelial cells from a differentiated, slowly growing, nonmalignant model rat prostate tumor. In syngeneic hosts, a mixture of stromal and epithelial cells resulted in nonmalignant tumors which were differentiated and slowly growing. In the absence of the stromal cells, epithelial cells progressed to malignant tumors which were independent of the stroma and undifferentiated. The independence of the malignant epithelial cells from stromal cells was accompanied by a switch from exclusive expression of exon IIIb to exclusive expression of exon IIIc in the FGF receptor 2 (FGF-R2) gene. The FGF-R2(IIIb) isoform displays high affinity for stromal cell-derived FGF-7, whereas the FGF-R2(IIIc) isoform does not recognize FGF-7 but has high affinity for the FGF-2 member of the FGF ligand family. The switch from expression of exclusively exon IIIb to exclusively exon IIIc in the resident FGF-R2 gene was followed by activation of the FGF-2 ligand gene, the normally stromal cell FGF-R1 gene, and embryonic FGF-3 and FGF-5 ligand genes in malignant epithelial cells. Multiple autocrine and potentially intracrine ligand-receptor loops resulting from these alterations within the FGF-FGF-R family may underlie the autonomy of malignant tumor cells.

[The characteristics of hormone responsiveness of glandular epithelium and stroma in male accessory sex organs].

We investigated the hormonal responsiveness of gland and stroma in the male accessory sex organs (ventral prostate, dorsolateral prostate and seminal vesicle). Immature rats (3 weeks old) were castrated and left untreated for 4 weeks and then distributed at random into 3 groups, A, B, C. The rats in groups A, B and C were injected subcutaneously with 0.2 ml of soybean oil, 5 alpha-dihydrotestosterone (DHT, 500 micrograms/day) or estradiol-17 beta (E2-17 beta, 5 micrograms/day), respectively, for 14 days before they were killed. DHT administration in prepuberal castrates stimulated collagen synthesis and accumulation in the stroma of male accessory sex organs as well as the proliferation and differentiation of the glandular epithelium in these organs. In E2-17 beta treated rats, the glandular epithelium of ventral prostate and seminal vesicle had mostly a single layer structure, but the glandular epithelium of dorsolateral prostate had a mostly multilayer structure. On the other hand, in E2-17 beta treated rats, the EMBP content, an indicator of glandular epithelium differentiation in male accessory sex organs, did not increase even in the dorsolateral prostate which showed a multilayer gland epithelium. Collagen synthesis and accumulation in the seminal vesicle was stimulated by E2-17 beta treatment several times as much as in the ventral and dorsolateral prostate. Histological investigation demonstrated that the seminal vesicle in prepuberal castrates treated with E2-17 beta resembled that of fibromuscular type human benign prostatic hyperplasia.
