[Relation of the serosal invasion mode to a recurrence in advanced gastric carcinomas].

Discussed are the attributable factors affecting the type of carcinomatous recurrence seen in 126 patients who had been operated on for their gastric carcinomas from 1979 to 1982 and who later showed a macroscopically positive serosal invasion (S0) and a histologically ss alpha approximately se invasion and required a curative resection. Forty-six percent of the patients with an ss beta approximately se invasion had a peritoneal recurrence, 21% a liver recurrence, and 33% a recurrence of some other type, whereas those with ss alpha had no such recurrence. Peritoneal recurrence tended to increase with the increase in the length of the serosal invasion and its rates were: one third in serosal invasions of less than 3 cm, one third in invasions of 3 approximately 6 cm and another third in invasions of over 6 cm. The smaller ratio of submucosal length to subserosal length, especially when less than 1.0, meant a greater frequency of peritoneal recurrence. The histologic characteristics of carcinomas that developed a peritoneal recurrence were poorly differentiated, contained INF alpha and had weaker cellular cohesion, whereas those that developed a liver metastasis were well differentiated carcinomas, contained INF beta and had a tighter cellular cohesion.

[PTCA and coronary artery bypass operation prior to subsequent surgery of gastric cancer in patients with coronary artery disease].

Two patients with coronary artery disease required operation for gastric cancer. In one patient with single vessel disease, 37 days later after successful PTCA, gastric resection for early gastric cancer was performed without any cardiac complication. The other patient with severe three vessel disease initially underwent coronary artery bypass operation, and 13 days later gastric resection for advanced cancer was performed. This patient developed supraventricular arrhythmia as a sole complication which was easily controlled by medical treatment. In both cases, fentanyl was used as an anesthetic agent and nitroglycerin was continuously administered intravenously during the perioperative period. ECG, central venous pressure, and direct arterial pressure were also carefully monitored. We feel that noncardiac operation such as gastric resection can be performed without major cardiac complications even shortly after coronary artery bypass operation.

[Surgical problems encountered in cases of S0 (serosal invasion-free) gastric cancers].

In this paper we have studied 649 resected cases of S0 (macroscopically negative serosal invasion) gastric cancer, this number of S0 gastric cancer resections experienced from a total number of 1692 patients treated at our hospital during the previous 16 years. The five-year survival rate of S0 cancers was 94% for patients given curative resections. Most of S0 cancers were histologically restricted within the pm layer but a few had reached the ss gamma or se layer. Non-curative factors in S0 cancers we recomposed of cancer positive stumps in the IIb-like margin, liver metastasis in the differentiated type carcinomas with ss invasion, and surgically left lymph node metastasis in the advanced carcinomas. As for lymph node dissections, our study reveals that an R2 dissection in the C and M regions, and a R3 or R2 + No.12 dissection for the A region are recommended. Modes of recurrence were characterized by liver or other hematogeneous metastasis in pm cancers or lymph node-positive cancers at the antrum, and by a peritoneal recurrence at the corpus in cases of advanced cancers. Histological ps(-) cancers showed a different survival result than did the S0 cancers and S(+) cancers.

Monoclonal antibodies against synthesized short peptides corresponding to human AA amyloid protein.

Monoclonal antibodies (McAbs) were raised against the synthesized short peptides corresponding to 37-47 residues in amino acid sequence of human AA protein. The McAbs reacted immunohistochemically to amyloid tissues from cow, mouse, swan, and human AA amyloidosis. We concluded that the McAbs were useful for identification of AA type amyloidosis of various species, and that the 37-47 residues were effective antigenic sites in AA protein.

Effects of aldose reductase inhibitor (M79175) on ERG oscillatory potential abnormalities in streptozotocin fructose-induced diabetes in rats.

The effects of an aldose reductase inhibitor (M79175) on electroretinogram (ERG) oscillatory potential abnormalities was studied in rats with streptozotocin fructose-induced diabetes. The ERG oscillatory potential was recorded under scotopic conditions without general anesthesia in the diabetic rats, before and 4, 8 and 12 weeks after administration of M79175. The ERG tracings were analyzed for peak latencies, peak intervals and amplitudes; the peak latencies and intervals were prolonged and the amplitudes were reduced in untreated diabetic rats, but the prolongation of the peak latencies and intervals were suppressed in diabetic rats receiving M79175. The amplitudes were reduced in the treated rats as in the untreated rats. The results of this investigation suggest that M79175 is effective to suppress progression of diabetic retinopathy in its early stage.

Ultrastructure of posterior subcapsular cataract in human lens.

In 20 eyes with posterior subcapsular cataract, ultrastructural changes were observed in the capsule and the subcapsular cortex. In 13 eyes, lens fibers of the subcapsular cortex were markedly swollen and liquefied, and contained granules of various sizes. In 4 eyes, various vacuoles or wide separations of the cell membrane were seen. In 3 eyes, the lens fibers became narrow and showed high electron density. The structure of human posterior subcapsular cataract affects the operation for posterior subcapsular cataract. In 1 case of posterior subcapsular cataract, a lamellar structure containing fine fibrils was observed in the deep layer of the posterior capsule and the epithelial cells which had a long nucleus and poor cytoplasm were scattered just under the posterior capsule. It is considered that these changes in the cells indicated a reduction of cellular function.

Argentaffin carcinoid tumor of the rectum.

A case of rectal carcinoid tumor in a 79-year-old Japanese man is reported. The tumor, 1.5 x 0.9 cm, was localized in both mucosa and submucosa of the rectum with neither invasion nor metastasis. Microscopically, neoplastic cells were mainly arranged in rosette-like and trabecular structures within thin fibrous stroma. Histochemically, both argyrophil and argentaffin reactions were positive. At the ultrastructural level, two distinctive types of neurosecretory granules were found in the cytoplasm of tumor cells.

Successful management of iliac mycotic aneurysm secondary to Klebsiella endocarditis--report of a case.

A case of common iliac mycotic aneurysm that presumably developed secondary to Klebsiella endocarditis was described. Recently, reports on gram negative septicemia and endocarditis have been on the increase. However, mycotic aneurysms secondary to bacterial endocarditis and particularly to Klebsiella endocarditis are rare. Inadequately treated serious gram negative septicemias have a high mortality rate. Early diagnosis and adequate combination chemotherapy with prompt surgical intervention were proven to be important factors in the successful management of such a complication.
