[Effects of Shosaiko-to-go-keishikashakuyaku-to (TJ-960) on the valproic acid induced anomalies of rat fetuses].

Shosaiko-to-go-keishikashakuyaku-to (TJ-960) is an extract of nine herbal drugs (Paeoniae radix, Cinna momi cortex, Bupleuri radix, Zingiberis rhizoma, Glycyrrhizae radix, Ginseng radix, Scutellariae radix, Pinelliae tuber and Zizyphi fructus) that has a potent anticonvulsant action. The rat fetuses treated orally with TJ-960 during the organogenesis period (days 7-17 of gestation) revealed no anomalies (up to 3000 mg/kg/day). When TJ-960 was co-administered with sodium valproate (VPA, 400 mg/kg) during the organogenesis period, embryonic resorption, fetal body weight, ossification and skeletal variation or anomalies induced by VPA were markedly reduced. The maternal plasma and embryonic concentration of VPA with TJ-960, however, were not significantly different from VPA alone. These results suggest that TJ-960 has protective effects against the teratogenicity of VPA.

Models for association in bivariate survival data.

This paper reviews dependence models for bivariate survival data, classifying them into the four groups: the shock model, the Freund model, the Clayton model, and the mixture model. The paper then concentrates on the mixture model, discussing the testing problem for the equality of marginal distributions under the Weibull type baseline hazard assumption. The new test proposed recently by Fujii is introduced, and its characteristic is studied with respect to the test proposed by Nayak and the sign test by simulation study.

Fetal calf serum increased the zona pellucida penetrability of rat oocytes matured in vitro.

We examined the effect of fetal calf serum (FCS) on meiotic division, subsequent fertilization, and first cleavage to the 2-cell stage of rat oocytes during in vitro maturation. FCS had no effect on the nuclear progression from dictiate to metaphase of the second maturation in vitro and, FCS had no effect on the first cleavage to the 2-cell stage of fertilized oocytes. However, FCS efficiently increased penetration rate of oocytes and shortened the time required for dissolution of the zona pellucida by alpha-chymotrypsin. These results showed that FCS did not affect cytoplasmic maturation necessary for oocytes to develop to the 2-cell stages. We found that FCS only affects the zona pellucida and does not affect the nucleus or cytoplasm of rat oocytes. FCS may prevent hardening of the zona pellucida.

A new method for induction of experimental autoimmune uveoretinitis (EAU) in mice.

Experimental autoimmune uveoretinitis (EAU) was induced in two strains of mice by repeated-immunization protocol. SMA mice (H-2 nondefined) and C57BL/6 mice (H-2b) were immunized with S-antigen mixed with Klebsiella 03 lipopolysaccharide (K03 LPS) repeatedly at intervals of 1 to 4 weeks. Following the tertiary immunization, the mice exhibited histopathological changes of EAU as well as significant immune responses to the antigen. The antigen doses required for successful EAU induction were 4 micrograms or more at each immunization time. The histopathology of EAU was characterized by mild infiltration of mononuclear cells in the retina and the choroid, particularly, at the retinal blood vessels and the photoreceptor cell layer. The anterior segment of the eye was not affected by inflammation, and therefore clinical signs of EAU were not detected even under an operating microscope. Since the mouse is a genetically and immunologically well-defined species, this model is useful for study of immunopathogenic mechanisms of EAU.

[Establishment and characterization of human pancreatic adenocarcinoma cell line SOJ producing carcinoembryonic antigen and carbohydrate antigen 19-9].

A new tumor cell line derived from a human pancreatic exocrine adenocarcinoma was established in tissue culture and was transplantable in a nude mouse. In tissue culture, the neoplastic cells grew as epithelial-like, mucin-producing cells with a population doubling time of 50-70 hrs. Chromosomes ranged from 63 to 186 with a modal number of 77. Subcutaneous injection of 1 x 10(6) cultured neoplastic cells into nude mice resulted in tumor formation histologically closely resembling the original neoplasm. Ultrastructurally, the cell line showed characteristic ductal epithelium. Immunohistochemically, carcinoembryonic antigen (CEA). Carbohydrate Antigen 19-9 (CA19-9) and DU-PAN-2 antigen were demonstrated in the original tumor, the culture cells and the transplanted tumor. The cells secreted CEA (48.7 ng/1 x 10(5) cells/24 hrs) and CA19-9 (325 U/1 x 10(5) cells/24 hrs) in spent medium as well as sera of the nude mouse. This cell line has been passaged 30 times in vitro and maintained for more than one year. These characteristics will make the cell line SOJ a valuable tool in studying various aspects of biology of human pancreatic cancer.

Gamma interferon production and two-color fluorescence flow cytometry analysis of peripheral blood mononuclear cells in allogeneic bone marrow transplant recipients.

Gamma interferon (gamma-INF) production was studied and two-color fluorescence flow cytometry analysis was done on the peripheral blood mononuclear cells (PBMC) in allogeneic bone marrow transplant recipients. Gamma INF was not detected in any patients within a year after transplantation whether PBMC was stimulated with PHA or not. A year after transplantation, gamma-INF was produced in the normal level in the stimulated and unstimulated PBMC. The number of suppressor-inducer T cells (CD4+2H4+) was decreased and that of suppressor T cells (CD11+CD8+) was normal. The numbers of helper-inducer T cells (CD4+4B4+) and helper T cells (CD4+2H4-) were normal. The numbers of activated helper-inducer T cells (CD4+HLA-DR+) and suppressor-cytotoxic T cells (CD8+HLA-DR+) were elevated. In the NK cells, Leu7+ CD16-cells were elevated, whereas Leu7+CD16+ cells and Leu7-CD16+ cells were normal. Leu7+CD8+ cells were elevated. These results indicated immunodeficiency after transplantation.

Genetic and physiological characteristics of oral Streptococcus intermedius.

The taxonomic relationships among strains of oral Streptococcus intermedius and the related streptococci were determined by G + C mol%, DNA-DNA hydridization, phenotypical and serological tests. As to the results, group 1 strains of S. intermedius including the type strain ATCC 27335 and five isolates behaved distinctively with respect to genetic, phenotypic and serological characteristics compared with group 2 strains of S. intermedius including ATCC 31412 and two isolates. Therefore, nine strains of oral S. intermedius could be separated at least into two homology groups.

[A calcified mucinous adenocarcinoma of the stomach--case report].

The paper discusses the case of a 44-year-old woman with an epigastric discomfort at Hiroshima University Hospital. A barium study of the stomach showed rigidity and a giant fold. A subsequent abdominal roentgenogram and CT disclosed calcification throughout the stomach wall. Thus, is hopes of effecting a cure, a total gastrectomy, a partial resection of transverse colon, a splenectomy and a pancreatic tail resection were performed. A surgical specimen macroscopically presented an inelastic and thickened wall of the entire stomach, and deposits of whitish calcium were visible within the gastric wall. The pathological diagnosis indicated a mucinous adenocarcinoma, se, INF alpha, v2, ly3, n4 (No. 221), with calcification of this mucinous adenocarcinoma in the stomach.

IgG antibodies to asialoGM1 are more sensitive than IgM antibodies to kill in vivo natural killer cells and prematured cytotoxic T lymphocytes of mouse spleen.

IgG and IgM antibodies, which were isolated from the anti-asialoGM1 (GA1) serum, had different effects against natural killer (NK) and prematured cytotoxic T cells (CTLs) in in vivo administration and in in vitro treatment. In in vitro treatments, the IgM antibody killed NK cells of nude mouse spleen in the presence of complement (C') 12 times more potently than the IgG antibody did, and either antibody with C' killed pre-CTL. In in vivo administrations, only the IgG antibody was effective in diminishing NK activity of the nude mouse spleen cells and in suppressing antigen-specific CTL induction from primed spleen cells by in vitro stimulation with X-irradiated tumor cells. The IgM antibody was not effective at all in either system. The in vivo effect of the IgG on NK activity was blocked by preadministration with silica or carrageenan but not by that with cobra venom factor (CVF). These results indicate that in vivo administration of anti-GA1 antiserum leads to macrophage-mediated depletion of CTL precursors as well as NK cells.

[Impairment of neuromuscular transmission in fatigued canine diaphragm].

The role of impairment of neuromuscular transmission in fatigued canine diaphragm was investigated by comparing the function of fatigued diaphragm with that of neuromuscular junction (NMJ)-blocked diaphragm. Diaphragmatic fatigue was produced by intermittent supramaximal stimulation of bilateral phrenic nerves. Partial NMJ blockade was obtained by vecuronium bromide administered intravenously. During control study, no fade was observed either in integrated diaphragmatic electric activity (Edi) or transdiaphragmatic pressure (Pdi). In fatigued diaphragm, Edi generated by 100 Hz showed a remarkable fade. The fade was prominent in high frequency fatigue. In NMJ-blocked diaphragm, fade was observed in Pdi as well as in Edi produced by 100 Hz. The fade of Edi by 100 Hz was prominent. In low frequency fatigue, both Pdi and Edi produced by 20 Hz test stimulation were significantly lower than control values. These results show that in addition to impairment of excitation-contraction coupling, neuromuscular blockade is involved, at least in part, in low frequency diaphragmatic fatigue.

[Clinical experience with chemotherapy using sulbactam/cefoperazone for severe infections accompanying malignant hematological disorders].

Clinical and bacteriological efficacies of sulbactam/cefoperazone (SBT/CPZ) were studied in 44 patients with serious infections associated with hematological malignancy. 1. SBT/CPZ was clinically effective in 33 cases (76.7%). Excellent effects were obtained in 23 cases, good effects in 10 cases and fairly good effects in 7 cases. Clinical effectiveness of SBT/CPZ was not dependent on neutrophil number in peripheral blood. 2. Bacteriologically SBT/CPZ was effective against all of the isolated organisms from 21 cases. 3. Adverse reactions were not significant except one case with eruption, 2 cases with abnormalities in hepatic function tests and 3 cases with abnormalities in renal function tests.

[A case of triple carcinomas: stomach carcinoma surgically removed, renal tumor, and carcinoma of the remnant stomach].

Reported is the case of a 57-year-old male patient, who manifested tarry stool and who had undergone a subtotal gastrectomy at our hospital in 1983 for an early carcinoma, type IIc, which proved to be a well differentiated tubular adenocarcinoma. Three years later, he returned complaining of epigastralgia, although no evidence of a recurrence could be seen. An abdominal ultrasonogram and a CT scan, however, revealed a right renal tumor and he subsequently underwent an operation for the removal of a renal cell carcinoma, a clear cell type. In 1988, he again was readmitted to hospital, this time because he easily became fatigued. An upper GI series and gastroscopy located a polypoid lesion in the remnant stomach. Thus, a total gastric resection was performed, and the lesion was diagnosed as being a well differentiated tubular carcinoma, an early gastric cancer, type I. Triple carcinomas are very rare, and the details of this case are discussed.

[Effects of cardioplegic solution and calcium entry blockers on coronary artery contraction].

We studied the effects of the electrolyte composition (K, Ca, Na) and calcium entry blockers of the extracellular fluid on the tension development of isolated canine coronary arterial strips. In 20 mEq/l K solution, 4.7 mEq/l calcium produced coronary artery contraction. This Ca-induced contraction was inhibited dose-dependently by calcium entry blockers-nifedipine, nicardipine, diltiazem and verapamil. In the presence of 20 mEq/l K, the reduction of sodium concentration to 12 mEq/l increased the tension of coronary artery. Calcium entry blockers did not affect this tension development. After 5 or 30 min perfusion of calcium entry blockers, Ca-induced contraction was inhibited. It is concluded that low calcium, high sodium cardioplegic solution or addition of calcium entry blockers may relax the canine coronary artery.

[Experimental and clinical studies on liver tissue blood flow in obstructive jaundice].

To clarify some pathologic aspects of obstructive jaundice before and after bile duct drainage, changes in liver tissue blood flow were observed using an electrolytic historheometer, in combination with the FITC-dextran method, during experimental bile duct obstruction and after its elimination. Clinical investigations were based on histologic findings in liver tissue specimens collected by wedge biopsy at the time of laparotomy following bile duct drainage. Rats with bile duct obstruction showed significantly (P less than 0.01) lower values for liver tissue blood flow, with sinusoid dilation, in comparison to control rats, at 2 and 4 weeks following the initiation of experimental bile duct obstruction. Liver tissue blood flow disturbance subsided 3 days after eliminating the obstruction, at 2 weeks following its initiation, while no improvement occurred even following elimination of obstruction after 4 weeks. Clinically, in cases with a high degree of sinusoid dilation the effect of postoperative bile duct drainage was poor, with a high incidence of postoperative complication. These findings suggest that liver tissue blood flow disturbance might causatively affect the pre- and postoperative course of obstructive jaundice.

Involvement of IL-6 in mesangial proliferative glomerulonephritis.

In this study, we demonstrated that IL-6 was a possible autocrine growth factor for rat mesangial cells (MC). rIL-6 induced in vitro growth of rat MC at a concentration of 2 to 200 ng/ml and IL-6 activity was found in the supernatant of cultured rat MC. Northern blot analysis as well as in situ hybridization revealed that IL-6 mRNA was expressed in the cultured MC. Of urine samples from patients with mesangial proliferative glomerulonephritis (PGN) 50% were found to contain significant IL-6 activity (ranging from 30 to 126 pg/ml). Urine samples from other type of primary glomerular diseases such as minimal change nephrotic syndrome or healthy volunteers contained no detectable IL-6 activity. Only 2 of 27 urine samples from membranous nephropathy contained detectable amount of IL-6. Furthermore, there was some relationship between the levels of urine IL-6 and the progressive stage of PGN. Finally, by immunohistochemical staining using an anti-IL-6 mAb, it was shown that MC in the affected glomeruli of PGN patients produced IL-6, whereas MC obtained from the patients with membranous nephropathy, minimal change nephrotic syndrome or normal kidney were not found to produce IL-6. These data suggest that deregulated production of IL-6 is involved in PGN and the measurement of urine IL-6 is helpful for the differential diagnosis of PGN as well as for monitoring the progression of PGN.
