Drug interaction between cyproheptadine and tipepidine. Effect of single and repeated administrations.

In order to elucidate the interaction between cyproheptadine (CPH) and tipepidine (TP), the disposition of CPH and its metabolites from plasma and the hepatic drug metabolizing enzyme activities in rats dosed singly or repeatedly were investigated. The elimination of CPH and its metabolites, desmethylCPH (DMCPH) and DMCPH-epoxide (DMCPHEPO), from plasma after a single intravenous (i.v.) administration of both CPH and TP was not significantly altered compared with that after CPH alone, although the i.v. administration of DMCPH and TP enhanced the plasma levels of DMCPHEPO. The elimination of TP from plasma was not affected by the coadministration with CPH. The single oral administration of both CPH and TP (50 mg/kg) significantly enhanced the plasma levels of CPH and DMCPH compared with those after CPH alone, consequently resulting in their delayed elimination. However, the coadministration with TP at a low dose (5 mg/kg) hardly affected the plasma decay of CPH and its metabolites. The repeated dosing of them for 7 d obscured the interactive effect. The hepatic drug-metabolizing enzyme activities, cytochrome P-450 and aminopyrine demethylase activity, were greatly increased after the repeated administration of CPH, especially showing much more increased activities after the coadministration with TP. These results suggest that the competition of hepatic oxidative metabolism between CPH or its metabolites and TP based on the depletion of the enzymes might largely be involved in the drug interaction on a single dosing of them and that the repeated dosing of them would dissolve the depletion due to their strongly inductive effect.

Effects of gomisin A on hepatocarcinogenesis by 3'-methyl-4-dimethylaminoazobenzene in rats.

We examined the effects of gomisin A on tumor promotion in the liver after a short-term feeding of 3'-methyl-4-dimethylaminoazobenzene (3'-MeDAB) to rats, compared with the effects of phenobarbital. Male Donryu rats were fed ad libitum a diet containing 0.06% 3'-MeDAB and 0.03% or 0.01% gomisin A or water containing 0.05% phenobarbital. Gomisin A and phenobarbital did not cause any proliferative and neoplastic lesions by themselves in 40 weeks of feeding. Altered foci in the liver increased with a peak at 12 weeks after the rats were fed 3'-MeDAB. Gomisin A decreased the number of hepatic altered foci such as the clear cell and basophilic cell type foci in the early stages. Phenobarbital enhanced neoplastic alterations so that the number and size of the foci were much larger in the phenobarbital-combined group than in the 3'-MeDAB-control group. Thus, phenobarbital acted as a promoter of cells initiated by 3'-MeDAB; on the other hand, gomisin A showed a weak suppressive effect on tumor promotion.

[Radionuclide ventriculographic evaluation of exercised left ventricular performance in asymptomatic diabetic patients].

Radionuclide ventriculography was made in 49 asymptomatic diabetic patients, aged 30-70 years, to investigate cardiac function. Comparisons were made with 33 age- and sex-matched non-diabetic controls. Radionuclide ventriculography was performed at rest and during dynamic exercise by multigraded, supine bicycle ergometer. The resting left ventricular ejection fractions were similar between the diabetic patients and control subjects. No significant rise in the left ventricular ejection fractions during dynamic exercise was observed in the diabetic patients [58.4 +/- 9.8% (mean +/- SD) to 60.3 +/- 9.9]. In the control subjects, the left ventricular ejection fractions increased during dynamic exercise [59.3 +/- 8.4 to 63.0 +/- 11.4 (p less than 0.05)]. Peak ejection rates were similar in both groups at rest [-2.62 +/- 0.64 (s-1) vs -2.66 +/- 0.52] and during dynamic exercise [-3.25 +/- 0.74 vs -3.23 +/- 0.90]. Time to end-systole were similar in both groups at rest [315 +/- 42 (ms) vs 309 +/- 42] and during dynamic exercise [258 +/- 37 vs 262 +/- 37]. The resting peak filling rates were similar in both groups [2.27 +/- 0.62 (s-1) vs 2.45 +/- 0.58], and the peak filling rates increased (p less than 0.001) during dynamic exercise in both groups [2.68 +/- 0.61 vs 3.28 +/- 0.74]. However, the percent changes of the peak filling rates were significantly lower in diabetic patients (p less than 0.001). In four of the diabetic patients, hypokinesis of the intraventricular septum was observed.(ABSTRACT TRUNCATED AT 250 WORDS)

In vitro and in vivo studies of the growth inhibitory effect of a newly synthesized peptide, obiopeptide-1, on mice-bearing methylcholanthrene-induced murine tumours.

Novel synthetic Obiopeptide (Obi-1), Glycil-penta-Glutamate, as an immunoregulator, injected intramuscularly to methylcholanthrene (MC) induced tumour bearing mice at 30-100 micrograms per mouse weekly, significantly inhibited tumour growth, compared to that of the untreated control. Numbers of mononuclear cells (MNC) in the spleen obtained from Obi-1 treated and untreated mice were reduced to 64% and 10% of the normal levels, respectively. In histopathological findings, variations of MNC in the spleen was observed in colonies of large MNC and lymphocytes in Obi-1 treated red marrow, while untreated one showed only scattered number of small lymphocytes. A significant O2- production was noted in monolayers of normal macrophages incubated with 0.5 mg Obi-1/ml for 48 hr. Cytotoxicity of spleen MNC cultured with Obi-1 against MC-tumour, Meth A, P-815 and YAC-1 target cells showed less than 10% cytotoxic activity in almost all experiments.

Automatic monitoring system for the measurement of body weight, food and water consumption and spontaneous activity of a mouse.

The simultaneous recording system for body weight, food and water consumption and behavior (spontaneous motor activity and drinking and feeding behavior) of a mouse was developed. The body weight and food consumption were measured by force transducers. Food and water consumption and drinking and feeding behavior were measured by an infrared luminous diode and a phototransistor. Spontaneous motor activity was measured by photosensors. The system control and data acquisition were performed by using a personal computer. Every parameter could be monitored with a desired time interval. All the data collected by this system revealed apparent circadian rhythm. In conclusion, this system would be a powerful tool for pharmacological and/or toxicological research.

[A case of psoas abscess due to renal pelvic carcinoma complicated with non-ketotic hyperosmolar diabetic coma].

A 85-year-old man was admitted to our hospital because of semicomatous status. Laboratory data on admission showed elevation of blood sugar (823 mg/dl) and serum osmotic pressure (345 mOsm/l), but ketonuria was not detected. Non-ketotic hyperosmolar diabetic coma was diagnosed. The insulin infusion and physiological saline improved the blood sugar level and consciousness within a day. The abdominal ultrasound examination revealed an abscess in the left kidney and right psoas muscle. The same findings were seen by abdominal computed tomography but the possibility of malignant neoplasm of the left kidney could not be ruled out because of a swelling of the left adrenal gland. Pain associated with psoas abscess and low grade fever were observed. Because of his poor general condition, drainage of the abscess was not performed and conservative therapy using antibiotics was administered. Without any improvement of the abscess, he died due to general deterioration four months later. Autopsy findings showed carcinoma of the left renal pelvis and metastasis to the right psoas muscle, left adrenal gland, liver, bilateral lungs and lymph modes. Psoas abscess is a relatively uncommon disease, especially in elderly patients. The etiology of the disease is divided into primary and secondary causes. Most secondary psoas abscess cases are caused by intestinal diseases, and Crohn's disease has been related to the highest incidence. A few cases of psoas abscess caused by colorectal carcinoma have been reported. Ultrasound and computed tomography are useful in diagnosing this disease and drainage of an abscess is necessary for therapy and proving the cause. Cancer metastasis should considered in differential diagnoses, when psoas abscess is seen in elderly patients.

Improvement in iron deficiency anemia through therapy with ferric ammonium citrate and vitamin C and the effects of aerobic exercise.

In 781 female college students, there were 41 cases of iron deficiency anemia, 209 of latent iron deficiency, 3 of other anemias, and 528 normal cases. Fifty-four volunteers recruited from the iron deficiency anemia and severe latent iron deficiency groups were randomly divided into 4 study groups. Groups I and III received 500 mg of vitamin C daily, and groups II and IV received ferric ammonium citrate (FeAC; equivalent to 6 mg iron) in addition to vitamin C for 9 weeks. Groups I and II were loaded by aerobic exercise at 50% VO2 max. Significant differences between groups were noted in serum ferritin (SF) in III/IV, hematocrit (Ht) in II/III and III/IV, and reticulocytes (RET) in I/II, I/IV, and III/IV. Hemoglobin (Hb) and other iron-related blood indices tended to normalize in groups II and IV when compared with the pre-values. VO2 max was elevated in groups I and II regardless of iron treatment, but was augmented more in group II than group I.

[Bilateral renal metastases of lung adenoid cystic carcinoma].

A 79-year-old male was admitted for macroscopic hematuria and right back pain. A mass shadow in the right lower lung fields had been present for three years previously with unconfirmed diagnosis. Excretory urogram, ultrasonogram and computed tomographic scan revealed bilateral renal tumors. Selective right renal arteriogram demonstrated a slightly hypervascular mass in the upper pole of the right kidney. Chemotherapy was not effective. Slow but definite malignant progression developed and the patient died two and a half years later. Autopsy finally revealed the adenoid cystic carcinoma of the right lung with bilateral renal metastases. The renal metastasis of this tumor is the first case in Japan to our knowledge.

Hemostasis activation during esophageal variceal sclerotherapy with thrombin in cirrhotics.

Sclerotherapy of bleeding esophageal varices in liver cirrhotics is a common procedure, but little is known about the possible entry of sclerosants into the systemic circulation. We injected a mixture of thrombin, sodium tetradecyl, and cefazolin and studied the effect of this sclerosant on selected hemostasis parameters. Twenty-four patients with liver cirrhosis (Child's Classification C) were studied 29 times. Blood samples were drawn before and immediately after the injection of the sclerosant. In seven patients we collected a sample 30 minutes and 24 hours after treatment. Before injection, almost all patients had elevated D-dimer, t-PA and PAI-1 levels. Fibrinogen, antithrombin, alpha-2 antiplasmin, and protein C were decreased. Only thrombin/antithrombin III complex (TAT) levels were within normal ranges. Immediately after the injection, TAT, D-dimer, and t-PA levels rose significantly (P less than 0.001, P less than 0.01, P less than 0.001), PAI-1 and PC levels decreased (P less than 0.01), while antithrombin, alpha-2 antiplasmin, and fibrinogen concentrations were unchanged. TAT and D-dimer levels were still elevated after 24 hours (P less than 0.05). These data indicate that thrombin entered the systemic circulation (elevated TAT) and that the hemostasis system was briefly systemically activated (elevated D-dimer). In spite of these changes in the hemostasis system, clinically there were no detectable thrombotic or hemorrhagic complications.

Regional lymph node metastasis of early gastric cancer.

Of 122 patients hospitalized for early gastric cancer in 1974-1989, 15 were histologically found to have regional lymph node metastases. Tumor invasion was limited to the mucosa in 3 cases and to the submucosa in 12. An ulcer or ulcer scar was found in the tumor in 12 cases. Metastasis had occurred only to the perigastric nodes in 13 cases and also along the left gastric and/or common hepatic arteries in two. Preoperative endoscopic injection of Chinese ink into the submucosa revealed a high rate of uptake in regional lymph nodes along the left gastric, splenic and common hepatic arteries. Patients with early cancer should undergo gastrectomy with lymphadenectomy, particularly if the cancer contains an ulcer or has invaded the submucosa. If the patient's general health and life expectancy are poor, restricted surgery or endoscopic treatment should be considered.

[Ectopic salivary amylase-producing IgA- lambda-type multiple myeloma with expression of MDR-1/P-glycoprotein].

We report a case of ectopic salivary amylase-producing IgA- lambda-type multiple myeloma. A 70-year-old man was admitted because of anemia and renal failure. After chemotherapy for eight months, the serum amylase markedly increased. Amylase activity in the supernatant of cultured myeloma cells, which were obtained from the bone marrow, also increased. The myeloma cells expressed MDR-1/P-glycoprotein). The case implies the association of drug resistance and the ectopic amylase production in a case of multiple myeloma.

In vitro chemosensitivity of lymphoblasts at relapse in childhood leukemia using the MTT assay.

Lymphoblasts from 21 previously untreated patients with acute lymphoblastic leukemia (ALL) and 31 patients in relapse were tested for chemosensitivity. Blast cells were cultured with 22 anticancer drugs for 4 days and assayed by MTT dye using a scanning microplate photometer. The percent cytotoxicity index (%CI) and LD50 (micrograms/ml) were calculated for each drug. The mean absorbances (+/- S.D.) of 1 x 10(5) cells in the untreated group and relapsed groups in control wells were 0.219 (+/- 0.126) and 0.385 (+/- 0.147), respectively (p less than 0.01). Cells in the untreated group were more sensitive in vitro to vincristine, prednisolone, L-asparaginase (L-ASP), vinblastine, 5-fluorouracil, epirubicin, bleomycin (BLM), and etoposide (VP16) with respect to the %CI value and to L-ASP, VP16, BLM, and mitoxantrone with respect to the LD50 value than those in the relapsed groups. In contrast, no significant differences were observed for the other 13 drugs. There was also a significant difference in sensitivity within the relapsed group--13 having good clinical response and 15 showing no response to chemotherapy--with regard to four drugs, mitomycin C, neocarzinostatin, L-ASP, and teniposide. Cells in the relapsed group had more heterogeneous chemosensitivity than those in the untreated group, and divided into sensitive and resistant types, but large interindividual differences existed. The MTT assay and LD50-drug resistance percentile curves are useful for the selection of effective drugs in both untreated and relapsed patients with acute leukemia.

[Intraglomerular coagulation and fibrinolysis in human primary glomerular diseases].

It is a well known fact that intraglomerular coagulation plays an important role in the development of human primary glomerular diseases. However, the precise mechanism of intraglomerular coagulation, and intraglomerular coagulability and/or fibrinolytic activity remains obscure. The present study was aimed to elucidate the role of the intraglomerular coagulation and fibrinolysis in human primary glomerular diseases. Subjects enrolled in this study were 27 patients with minimal change nephrotic syndrome (MCNS), 14 patients with focal glomerular sclerosis (FGS), 36 patients with membranous nephropathy (MN), 161 patients with mesangial proliferative glomerulonephritis (mesPGN), 9 patients with membranoproliferative glomerulonephritis (MPGN), and 40 healthy volunteers as controls. Normal human renal cortex as controls of isolated intraglomerular plasminogen activator activity (PAA) was obtained at the time of nephrectomy from the normal pole of kidneys removed because of an opposite pole tumor. Urinary urokinase (UK), fibrinopeptide A (FPA) and fibrinopeptide B beta 15-42 (B beta 15-42) antigens were measured by RIA. Urinary tissue plasminogen activator (t-PA) antigen was measured by ELISA. Urinary fibrin/fibrinogen degradation products (FDP) were measured by latex agglutination method. Moreover, PAA was measured by 125I-fibrin films. The following results were obtained: 1) In primary glomerular diseases, levels of urinary UK and t-PA were significantly lower than those in healthy volunteers, 2) Urinary UK and t-PA showed gradual decrease along with the development of mesangial proliferation, 3) Urinary UK and t-PA were significantly correlated with both the urinary FPA and B beta 15-42, 4) In mesPGN and FGS, PAA was significantly lower than that in normal controls, 5) PAA was significantly correlated with urinary UK, t-PA, FPA and B beta 15-42, 6) Urinary UK and t-PA in the patients with urinary FDP were significantly lower than those in patients without urinary FDP, 7) Urinary UK, t-PA and PAA were significantly lower in patients with intraglomerular fibrin deposition than those in patients without fibrin depositions. These findings suggest that the decrease of urinary UK and t-PA levels and the diminution of isolated intraglomerular plasminogen activator activity contribute to the progression of primary glomerular diseases.

[Diaphragmatic fatigue and its recovery are influenced by markedly decreased cardiac output: possible involvement of neuromuscular junction].

Effects of highly decreased cardiac output on the development and recovery of diaphragmatic fatigue were studied in dogs. The fatigue was induced by supramaximal electrical stimulation (20 Hz) of phrenic nerves for 30 minutes. Cardiac output was reduced during this period to 30% of control value by mechanically obstructing inferior vena cava with a balloon in animals with lowered cardiac output group (lowered Qt group). Cardiac output was maintained at normal value throughout the experiment in animals of the other group (control group). The animals were observed for recovery for 60 minutes after induction of the fatigue. The standardized transdiaphragmatic pressure (Pdi/Pdi 100) and integrated electromyographic activity (Edi/Edi 100) elicited by electrical test stimuli (20, 100 Hz) were significantly lower in lowered Qt group (P less than 0.01) during entire fatigue and recovery period. The decrease of Pdi/Pdi 100 and Edi/Edi 100 at high frequency test stimuli (100 Hz) was observed only in lowered Qt group. These results suggest that when cardiac output is severely decreased the diaphragm is more susceptible to fatigue, and that this may be caused by a failure of neuromuscular junction as well as by an impaired excitation-contraction coupling.

[Effects of diaphragm plication for phrenic nerve paralysis on respiratory mechanics and diaphragm function].

The purpose of this study is to examine the effects of unilateral diaphragm plication for phrenic nerve paralysis on the respiratory mechanics and diaphragm function. Study 1: We performed thoracotomy (BASELINE), left phrenicotomy (INJURY) and left diaphragm plication (PLICATION) in ten dogs. After chest was closed in each condition, measured were the tidal volume (TV), respiratory rate (f), esophageal and gastric pressure swing (delta Pes, delta Pga), dynamic compliance (Cdyn) the ratio of gastric to esophageal pressure swing (delta Pga/delta Pes) and the work of breathing per liter (WOB/L) under stable spontaneous breathing. Study 2: In eight other left-phrenicotomized dogs, Pdi and fractional shortening of the costal diaphragm during contralateral electrophrenic stimulation were measured before and after plication. After plication TV, delta Pga/delta Pes, Cdyn, Pdi and fractional shortening during stimulation increased significantly, whereas Pes and WOB/L decreased significantly compared with INJURY condition. In conclusion unilateral diaphragm plication after paralysis improves the intact hemidiaphragm contractility and diaphragmatic contribution to breathing.
