[Experimental study on immunity and burn wound healing].

The immune system is involved in the complicated process of wound healing. In this study, we at first set up an optimal immuno-suppression model with low-doze radiation with 60Co and found that both the ratio of Th/Ts in peripheral blood and the rate of T cells adhesion to tumour cells dropped persistently, and there were no significant injuries to skin tissue DNA and cells cycle in 20 days after 2Gy radiation with 60Co. Then we investigated the influence of immuno-suppression on burn wound healing. The results showed that both ability of collagen repair and rates of wound healing after burn injury were inhibited in immuno-suppression rats. This indicated that it was necessary to improve immunity condition to enhance wound healing after severe burn injury.

[The inhibitory effect of hydroprednisolone on tumor necrosis factor in rabbits' temporomandibular joint disturbance syndrome].

In order to study the mechanism of hydroprednisolone for temporomandibular joint disturbance syndrome (TMJDS), 12 white rabbits were injected TNF into the rabbits' right TMJDS for 2 to 3 times, 90,000 u each time, then another drug, 2 mg hydroprednisolone, was injected into these TMJDS again. The rabbits were killed after 8 to 18 days and the TMJDS were examined by microscope. We found the articular tissues only destroyed slightly and sometimes the cartilages were nearly normal. So, the study suggested that hydroprednisolone can reduce the destruction of TNF on rabbit's TMJ obviously. This may be the cause why hydroprednisolone is useful to TMJDS.

[The role of PMN CD11b/CD18 on the increasing PMN adhesion to endothelial cells induced by severe burn injury].

The process of leukocyte adhesion was mediated by the intercellular adhesion molecular. In this study, both the influence of burn serum on the expression of PMN CD11b/CD18 and the role of CD11b/CD18 on the burn serum stimulated PMN adhesion to endothelial cells (EC) were investigated. The expression of CD11b/CD18 on PMN incubated with burn serum for 2 hours was increased obviously. So was PMN adhesion to EC. Monoclone antibody to PMN CD11b/CD18 could reduce the normal PMN adhesion to EC by half and block the increasing PMN adhesion induced by burn serum. These indicated that PMN CD11b/CD18 may be the main adhesion molecular participating in the increasing PMN adhesion to EC due to severe burn.

[Sudden hearing loss in acoustic neuroma].

To avoid misdiagnosis and mistherapy of acoustic neuroma, 104 cases of acoustic neuroma treated between 1986 and 1995 were retrospectively reviewed. Among them, 23 patients (24 ears, 23%) presented with sudden hearing loss as the starting symptoms. The clinical and audiological analysis demonstrated that 54.2% of the ears showed hearing loss over 71 dB HL, and all ears had deteriorated ABRs. -SP/AP was measured in nine ears, seven showed -SP/AP greater than 0.4. Acoustic reflex was negative in all 11 ears tested, and 94.4% of the ears showed abnormal ENG. The rate of positive CT scan was 88.8%. The diagnosis was made with CT pneumo-encephalography or MRI in those with negative CT result. It was concluded that for patients with sudden deafness, ABR should be used as a routine test. When ABR was abnormal, CT scan of the internal auditory meatus was needed.

[Diagnosis and management of Mondini malformation].

Owing to the development of imaging technology and audiology, some of the sensorineural hearing loss cases that were previcusly considered to be of unknown cause have been found to be inner ear malformation. Five cases of Mondini malformation are reviewed in this paper. CSF otorhinorrhea occurred in four cases, Klippel-Feil syndrome in three, and concurrent otosclerosis in one patient. In discussion, the authors point out that: 1) CT scanning is an supplement to audiologic tests for such patients; 2) perilymph fistula in Mondini malformation is often found at the oval window and its vicinity, the foot plate and the round window; 3) the fistula can be treated by plugging with fascial tissue via tympanoplastic approach with endaural incision. The mucosa around the fistula should be stripped away and the plugging tissue should be of dumb-bell shape.

[Study on observation indexes of rational dosage of atropine in treatment of acute organophosphorus insecticides poisoning].

In order to obtain nursing observation indexes in treatment of acute organophosphorus insecticides poisoning (AOIP) with rational atropine dosage, three nursing observation indexes: abnormal secretion, body temperature and pulse were summarized with the theory obtained from a earlier study in our hospital. The rational atropine dosage for treatment of AOIP should be enough to antagonize the accumulation of acetylcholine but should not cause too much body feedback regulation. Regulate atropine dose by changes of these three indexes, 171 patients with AOIP (Group A) were treated and observed, and compared with 184 patients (Group B) treated with traditional observating method (atropinization index). The results showed that the atropine dose used in Group A was markedly less than that in Group B (P < 0.001), the medication duration was shortened (P < 0.001), the complication and fatality rate were decreased (both P < 0.05). It indicated that these observation indexes are scientific and reliable, practical and of spreading value.

Studies on efficacy of praziquantel and mebendazole-medicated salt in treatment of Echinochasmus fujianensis infection.

The therapeutic effect of praziquantel and mebendazole-medicated salt has been studied in 109 cases with Echinochasmus fujianensis infection. These cases were randomly divided into four groups: 2 groups with a single dose of praziquantel 5 mg/kg or 2.5 mg/kg; and other 2 groups with mebendazole 800mg or 400 mg in 10d table salt. Four weeks after treatment, the egg negative conversion rates were 100%, 92.3%, 85.2% and 71.4% respectively, the egg reduction rates were 84.8-100%, and side-effects were mild. The symptoms caused by infection such as abdominal pain, diarrhea, distension and anorexia were obviously relieved. These data indicated that praziquantel is the drug of choice in the treatment of Echinochasmus fujianensis. The dosage is only 2.5 mg/kg, and its egg negative conversion rate and reduction rate reach 92.3% and 95.4%, respectively. For convenience, the dosage can be made according to their age. Children under 12 take half a tablet (100 mg), and one tablet (200 mg) for those over 12. This dosage is approximately equal to 2.5-5.0 mg/kg. Although the efficacy of mebendazole is lower than praziquantel, its egg negative conversion rate also reaches 71.4-85%. Mebendazole-medicated salt can be used for treating Echinochasmus fujianensis infection as the presence of co-infection with nematodes.

[A quantitative study on the cells of cochlear nucleus in rats at different ages].

The volume of cochlear nucleus and nucleolus, neural populations were determined in rats of three groups aged (1, 2-3, 22-28 months). The study data indicated that there is no difference between the volume of cochlear nucleus in the aged and adult animals (P > 0.05). Compared with the other two groups, the aged animals show a reduction of volume of necleolus of cochlear nucleus neurons (P < 0.01), a loss of neurons (spherical cell and multipolar cell) (P < 0.01). On the other hand, it shows an increase in number for the glial cells in the aged animals (48.48%, P < 0.01). Quantitative results of the cell of cochlear nucleus were analysed. We suggest that central changes also play an important role in presbycusis.

[Serotyping of hemorrhagic fever with renal syndrome in Hubei province].

Ninety-two serum specimens, positive for antibodies against hemorrhagic fever with renal syndrome (HFRS) virus in initial screening with immunofluorescence assay technic (IFAT), were serotyped with micro cell pathogenic effects neutralization test based on preliminary epidemiological classification of epidemic foci of HFRS throughout the province to find out serological evidence of HFRS typing in Hubei Province. It was found that 48 of the specimens were belonged to Type I (HTN) accounting for 52.18 percent, 29 Type II (SEO) for 31.52 percent, and 15 undefined for 16.30 percent. Hubei Province was classified serologically as a mixed prevalent area with Type I as its major component, but all serotypes in different sub-areas have their own features and those in the old epidemic foci were more complex. It indicated that it was better to use a bivalent HFRS virus vaccine, or a single-valent vaccine consistent with local serotype. Serotyping of local HFRS conformed basically to that of epidemiological classification. Local HFRS should be serotyped periodically due to continuous changes in types of foci. Attention to reactions of vaccine immunization should be paid during observation of the effectiveness of the vaccine.

[Antagonistic effect of orphanin FQ on morphine analgesia in rat brain].

The discovery of a novel receptor-opioid receptor-like (ORL) receptor (1994) and its endogenous ligand-Orphanin FQ (OFQ) (1995) represented a new approach in the study of opioids and anti-opioids in CNS. Based on the high homology of ORL receptor and OFQ with their opioid family counterparts, as well as the high expression of ORL receptor mRNA and protein in the brain areas associated with nociception, the effect of OFQ on morphine induced analgesia in the rat brain was further investigated. The results showed that: (1) Intracerebroventricular (i.c.v.) injection of OFQ reversed the stress analgesia induced by i.c.v. injection of normal saline, which seems to be mediated by endogenous opioid peptides. (2) I.c.v. injection of OFQ dose-dependently antagonized morphine-induced analgesia dose-dependently. (3) I.c.v. injection of antisense oligodeoxynucleotide for the gene encoding ORL receptor to block the expression of ORL receptor in the CNS potentiated the analgesia induced by cumulative injection of morphine. The results suggest that OFQ seems to play a role of anti-opioid peptide in the rat brain.

Brain substrates activated by electroacupuncture of different frequencies (I): Comparative study on the expression of oncogene c-fos and genes coding for three opioid peptides.

Low and high frequency electroacupuncture (EA)-produced analgesia have been shown to be mediated by different brain substrates and different opioid peptides. In this study, Fos-like immunoreactivity (FLI) and in situ hybridization of the three opioid mRNAs were used to examine the effect of low (2 Hz) and high (100 Hz) frequency EA on neuronal activities, and the expression of opioid genes. 2 Hz and 100 Hz EA induced a markedly different spatial patterns of Fos expression in the rat brain, suggesting there are distinct neuronal pathways underlying EA of different frequencies. Likewise, 2 Hz and 100 Hz EA exert differential effects on opioid gene expression: while 2 Hz EA induced a more extensive and intensive preproenkephalin (PPE) mRNA expression than 100 Hz EA, it had no effect on preprodynorphin (PPD) mRNA expression which was significantly increased by 100 Hz EA stimulation. In contrast, EA of both frequencies did not affect POMC mRNA expression.

Brain substrates activated by electroacupuncture (EA) of different frequencies (II): Role of Fos/Jun proteins in EA-induced transcription of preproenkephalin and preprodynorphin genes.

Antisense oligodeoxynucleotides (ODNs) of c-fos and/or c-jun were used in this study to investigate the role of Fos and Jun proteins in electroacupuncture (EA)-induced transcription of the opioid genes, preproenkephalin (PPE), preprodynorphin (PPD) and proopiomelanocortin (POMC). As the results showed, EA-induced Fos and fun expression was blocked efficiently and specifically by e-fos and c-jun antisense ODNs, respectively. This treatment significantly prevented EA-induced PPD, but not PPE, mRNA expression. These results suggest that Fos and Jun proteins are involved in PPD rather than PPE gene transcription activated by EA stimulation.

Hsp90 regulates androgen receptor hormone binding affinity in vivo.

The regulation of human androgen receptor (AR) by the molecular chaperone Hsp90 was investigated using the yeast Saccharomyces cerevisiae as a model system. These studies were performed in strains expressing a conditional temperature-sensitive mutant allele of the hsp82 gene, which encodes Hsp90 protein. At the restrictive temperature in the mutant, there is a decrease in hormone-dependent transactivation by the AR, although steady state levels of AR protein are unchanged. Quantitative hormone binding studies at the permissive temperature revealed the presence of both high affinity and low affinity hormone binding states. At the restrictive temperature in the hsp82 mutant, the high affinity state was abolished, and only the low affinity state was observed. The change in hormone binding affinity was further investigated by a competition assay with the anti-androgen hydroxyflutamide. Under permissive conditions, hydroxyflutamide competes poorly for the synthetic androgen R1881, but under restrictive conditions in the hsp82 mutant strain, hydroxyflutamide was shown to be a potent competitive inhibitor. Our findings indicate that Hsp90 participates in the activation process by maintaining apoAR in a high affinity ligand binding conformation which is important for efficient response to hormone.

Prospective trial of combined hyperfractionated radiotherapy and bronchial arterial infusion of chemotherapy for locally advanced nonsmall cell lung cancer.

PURPOSE: This article is a prospective trial of hyperfractional radiotherapy with bronchial arterial infusion of anticancer drugs to treat locally advanced bronchogenic cancer. The treatment results, the complications of bronchial arterial infusion, the failure patterns, the relationship of technical aspects of delivery of radiotherapy, and the protocol of anticancer drugs are presented. METHODS AND MATERIALS: One hundred and twenty-six patients with locally advanced bronchogenic cancer, treated from January 1988 to January 1990, were divided randomly into four groups in our hospital. Group 1: combination of hyperfractional radiotherapy with bronchial arterial infusion of anticancer drugs (30 cases); Group 2: combination of conventional radiotherapy with bronchial arterial infusion of anticancer (33 cases); Group 3: combination of systemic chemotherapy and split-course radiotherapy (33 cases); Group 4: conventional radiotherapy only (30 cases). RESULTS: All the patients were followed for 3 years. The 1-year survival rates for Groups 1, 2, 3, and 4 are 80, 63.6, 48.5, and 30%, respectively. The 2-year survival rates for Groups 1, 2, 3, and 4 are 23.3, 15.15, 9.1, and 6.6%, respectively. The 3-year survival rates for Groups 1, 2, 3, and 4 are 10, 3.3, 0, and 0%, respectively. CONCLUSION: This study demonstrated that the combination of hyperfractional radiotherapy with bronchial arterial infusion anticancer drugs can be performed safely and effectively for locally advanced bronchogenic carcinoma.

Activation energy of the separation of DNA sequencing fragments in denaturing noncross-linked polyacrylamide by capillary electrophoresis.

We present the mobility of DNA sequencing fragments as a function of temperature in 5%T, 0%C noncross-linked polyacrylamide and at an electric field of 150 V/cm. The mobility of the fragments follows the Arrhenius equation. The activation energy for migration of a fragment through the polymer decreases monotonically with fragment length. The data were also analyzed in terms of the biased reptation model; the onset of biased reptation with stretching occurs for longer fragments as the temperature increase. These results are quite different from those observed for separation of DNA in cross-linked gels and represent a fundamental difference in the physics of the cross-linked and noncross-linked polyacrylamide.

PCR-based gene targeting of the inducible nitric oxide synthase (NOS2) locus in murine ES cells, a new and more cost-effective approach.

Gene targeting by double homologous recombination in murine embryonic stem (ES) cells is a powerful tool used to study the cellular consequences of specific genetic mutations. A typical targeting construct consists of a neomycin phosphotransferase (neo) gene flanked by genomic DNA fragments that are homologous to sequences in the target chromosomal locus. Homologous DNA fragments are typically cloned from a murine genomic DNA library. Here we describe an alternative approach whereby the inducible nitric oxide synthase (NOS2) gene locus is partially mapped and homologous DNA sequences obtained using a long-range PCR method. A 7 kb NOS2 amplicon is used to construct a targeting vector where the neo gene is flanked by PCR-derived homologous DNA sequences. The vector also includes a thymidine kinase (tk) negative-selectable marker gene. Following transfection into ES cells, the PCR-based targeting vector undergoes efficient homologous recombination into the NOS2 locus. Thus, PCR-based gene targeting can be a valuable alternative to the conventional cloning approach. It expedites the acquisition of homologous genomic DNA sequences and simplifies the construction of targeting plasmids by making use of defined cloning sites. This approach should result in substantial time and cost savings for appropriate homologous recombination projects.

Differential production of IL-12 in BALB/c and DBA/2 mice controls IL-4 versus IFN-gamma synthesis in primed CD4 lymphocytes.

The profile of cytokines produced by CD4 T cells is profoundly influenced by the presence of IL-12. Here we demonstrate that during re-stimulation of antigen-specific immune responses in vitro, antigen-primed lymph node cells from DBA/2 mice produced 3- to 30-fold more IL-12 than did cells from BALB/c mice, which are identical at the major histocompatibility locus. The strain differences in IL-12 production were observed only in antigen-driven responses (and not in responses induced by bacterial products), and were dependent upon an interaction between CD4 T cells and lymph node adherent cells. In addition, differences in the quantity of IL-12 produced by DBA/2 and BALB/c antigen-presenting cells (APC) was not dependent on differential production of IFN-gamma by T cells, since APC from DBA/2 mice still produced much greater quantities of IL-12 than did BALB/c APC when each was cultured with the same H-2d-restricted Th2 clones, in the complete absence of IFN-gamma, or when each was cultured with primed (BALB/c x DBA/2)F1 T cells. The level of IL-12 produced in the cultures critically affected cytokine production in CD4 T cells, since neutralization of endogenous IL-12 in DBA/2 cultures, which are predisposed towards developing Th1 responses, reduced IFN-gamma production and enhanced IL-4 synthesis to levels normally seen in BALB/c cultures, which are predisposed toward developing Th2 responses. We propose therefore that differential production of antigen-driven IL-12 is a mechanism by which the genetic background in DBA/2 and BALB/c mice can affect the pattern of cytokine synthesis by T cells during the development of adaptive immune responses.
