Malignant melanoma arising in ovarian cystic teratoma.

A 65-year-old Japanese woman with a primary malignant melanoma arising within a mature ovarian cystic teratoma is presented along with details of morphological examinations. The melanoma, found incidentally upon microscopic examination of the mature teratoma, consisted of cells showing radial and vertical growth and was surrounded by epithelium showing melanosis. This report describes the detailed morphological features of the malignant ovarian melanoma, which was of interest because of its small size and lack of peritoneal tumor cell dissemination.

Experimental otitis media with effusion induced by lipopolysaccharide from Klebsiella pneumoniae. Mucociliary pathology of the eustachian tube.

Otitis media with effusion (OME) is generally benign and self-limiting but is often recurrent and can change into a chronic type. We have previously reported that intratympanic inoculation of lipopolysaccharide (LPS) from Klebsiella pneumoniae can develop experimental OME in the guinea pig. However, OME induced by 10 micrograms/ml of LPS was of a self-limiting type. Our hypothesis was that a higher concentration of LPS might develop a chronic OME because LPS reduces the ciliary activity in the tubotympanum in a dose related manner. In our present study we inoculated 100 micrograms/ml of LPS from K. pneumoniae into the tympanic cavity of the guinea pig, in the hope of illuminating a possible mechanism predisposing to the chronicity of the disease. A longer-term OME was observed after tympanic inoculation of 100 micrograms/ml of LPS. Ciliary activity in the Eustachian tube was depressed for a longer term by this dose than by the previous one. In addition to mucociliary pathologies and general inflammatory changes similar to those induced by 10 micrograms/ml of LPS, intracytoplasmic ciliary cysts were observed after inoculation of 100 micrograms/ml of LPS, which appeared to show disturbed ciliogenesis and chronic mucociliary lesions. In conclusion, intratympanic inoculation of a higher concentration of LPS developed a longer-term OME in the guinea pig, which was supported by functional as well as morphological examinations.

Histamine-induced mucociliary dysfunction and otitis media with effusion.

Histamine, which has been found in middle ear effusions, is a potent pharmacological mediator released at an early stage of allergic reactions or general inflammatory process, increasing permeability of small blood vessels. Histamine might be involved in the origin and chronicity of middle ear effusions. In this study we studied the effect of histamine on the mucociliary function. First we examined the effect of histamine and its H1 and H2 blockers on the ciliary activity in the middle ear. 10(-2) M of histamine deteriorated ciliary activity; however, at lower concentrations ciliostimulatory effects were demonstrated for histamine (between 10(-8) M and 10(-4) M). Such ciliostimulatory effects were not affected by diphenhydramine (H1-blocker) but were reduced by cimetidine (H2-blocker). Thus histamine stimulates ciliary activity by combining with H2-receptor. Intratympanic injection of 10(-4) M of histamine induced accumulation of middle ear effusions (MEEs). The volume of MEEs was largest at 1 day postinjection when its mucociliary clearance time was longest. Then the mucociliary clearance time became shorter, but it was still significantly longer than that of the control animal. Ciliary activity in the tubotympanum showed no recovery through the observation period. On the other hand, intratympanic injection of 10(-6) M of histamine produced MEEs at 1 and 3 days postinjection when the mucociliary clearance time was longer than that of the control group. At 8 days, when most ears did not demonstrate MEEs, the mucociliary clearance time and ciliary activity in the Eustachian tube and tympanic orifice reached the level of the control group. Our present study demonstrates that histamine can induce mucociliary dysfunction of the tubotympanum resulting in middle ear effusions, and that cilia, especially those present in the tube and the tympanic orifice, have a significant role in eliminating middle ear effusions.

Influenza A virus-induced otitis media and mucociliary dysfunction in the guinea pig.

There is much epidemiologic, clinical and laboratory evidence that viral infection is involved in otitis media with effusion (OME). However, few studies have demonstrated any direct influence of viruses on the tubotympanum. The purpose of this study was to establish the effect of influenza A virus having invaded the tubotympanum and so elucidate the possible mechanism by which this virus contributes to the pathogenesis of OME. Eighty guinea pigs with normal otoscopic findings were inoculated with 0.2 ml suspension of influenza A (3.3 x 10(8) PFU/ml) into the tympanic cavity through the tympanic membrane. To serve as controls, the same number of guinea pigs were injected with 0.2 ml of physiologic saline solution into the tympanic cavity. At 3, 7, 14, and 28 days postinoculation, they were used for examination of the mucociliary function. Middle ear effusions as well as mucociliary dysfunction were observed only in the animals inoculated with the virus. The ciliary activity in the bulla was declined at any time examined. On the other hand, the ciliary activity in the Eustachian tube and the tympanic orifice was slightly lowered between 7 and 14 days, but the level was not different from that of the controls. However, the number of active ciliated cells (showing more than 500 beats/min) was significantly smaller than that of the controls. The mucociliary clearance time of the tubotympanum was more prolonged than that of the controls at 3, 7, and 14 days, and returned to the control level at 28 days. A variety of morphological changes were observed in the tubotympanum treated with the virus. Major pathologies observed included general inflammatory cell infiltration, vacuolation and other degeneration of ciliated cells, and vascular damage and increased vascular permeability. Regeneration of cilia or ciliated cells followed the degeneration, which included an increased number of basal cells and newly formed centrioles. However, the viral infection also affected the epithelial cells with new centrioles. Our study demonstrates that viral infection can evoke mucociliary dysfunction of the tubotympanum and create increased susceptibility to bacteria. Therefore, viral infection may enhance bacterial infectious processes in the tubotympanu thereby contributing to the occurrence of OME.

Whole blood histamine release rate during immunotherapy for nasal allergy.

In treating nasal allergies, desensitization is widely used on a daily basis, and its efficacy has been highly valued. With the purpose of predicting therapeutic efficacy before carrying out immunotherapy, we compared whole blood histamine release rate and various subjective symptoms before and after immunotherapy. We also compared the degrees of efficacy. Immunotherapy brought about a tendency for a reduction in the whole blood histamine release rate. This tendency appeared most marked in those patients in whom an improvement of subjective symptoms was considerable. Among subjective symptoms, the reduction in whole blood histamine release rate was strongest against nasal discharge and sneezing. In patients with a low histamine release rate prior to the initiation of immunotherapy, the improvement in subjective symptoms tended to be low.

Distribution of immunoglobulins and secretory component in gastric cancer of the aged.

The secretory immune system plays an important role in the local humoral immunity of the gastrointestinal tract. In order to evaluate humoral immunity in gastric cancer, distribution of immunoglobulins (Ig) and secretory component was immunohistochemically studied in 74 early and 15 advanced primary gastric cancers. In non-cancerous gastric mucosa, IgA and IgM, and secretory component were mainly identified in the cytoplasm of the intestinal metaplasia. In early gastric cancer of well-differentiated type, the localization of IgA and IgM, and secretory component was similar to that of intestinal metaplasia. In advanced gastric cancer, they were faintly observed and showed low positivity. The number of Ig-containing cells infiltrating cancerous stroma was enumerated. Immunoglobulin A-containing cells were dominant in the stroma of early gastric cancer. On the other hand, there were few Ig-containing cells in the stroma of advanced gastric cancer, and the number of IgA-containing and IgM-containing cells was also decreased. These results suggest that local humoral immunity is suppressed in gastric cancer, especially in advanced gastric cancer.

A comparison of patterns of metastasis in gastric cancer by histologic type and age.

Patterns of metastasis were examined in 173 autopsy cases of gastric cancer in which the primary tumor had not been resected and the death was mainly caused by growth and metastasis of the cancer. The results were compared by different histologic types as well as different age groups. From the histologic viewpoint, gastric cancer of glandular type showed preferential metastasis to the liver, whereas the nonglandular type showed a preference for peritoneal involvement and lymph node metastasis. With regard to age, peritoneal involvement was more frequently observed in younger patients for both glandular and nonglandular types. Younger patients also had a higher incidence of lymph node metastasis of gastric cancer of nonglandular type, but not for the glandular type. Regarding liver metastasis, there was no difference in incidence between different age groups.

Immunopathological analysis of interstitial renal lesions in elderly people.

The incidence of focal lymphoid infiltrates in the renal interstitium was examined in autopsy cases of young and old subjects, and the infiltrating lymphocytes were immunohistologically characterized by a panel of monoclonal antibodies. Histologically, 198 and 227 autopsy cases over 60 years of age (87.2%) were shown to have mononuclear cell infiltrates of varying degree in the renal interstitium, whether or not these were accompanied by progressive arteriosclerotic changes. Above all, severely infiltrating foci in the renal interstitium were frequently found in the elderly over 70 years of age overlapping arterio-artherolosclerotic changes. In contrast, in the 54 younger control subjects under 49 years of age, the incidence of such a lesion was less (5.6%). An immunohistologic study revealed that the infiltrating mononuclear cells were predominantly composed of CD4+ cells, whereas CD22+ B cells were apparently lesser in number. Moreover, a considerable proportion of T cells was activated as judged by IL-2 receptor expression. From these findings, we now propose that susceptibility to the development of interstitial renal lesions in the elderly involves the cellular immune response, and may be related to an age-associated disturbance in regulatory T-cell function.

[Immunohistologic study on focal lymphocytic infiltration in the liver of the elderly autopsy cases].

Immunologic and immunohistologic analyses were performed on lymphocytic infiltration in periportal areas of the liver obtained from 105 autopsy cases over 60 years of age and 63 cases under 60 years of age, all cases showing no symptoms of overt liver diseases. Lymphocytic infiltration in the periportal areas began to be found in the livers of young autopsy cases in the 4th decade, with an incidence of more than 60%. The incidence reached 100% in cases of 6th decade and over, accompanied by increased severity. The infiltrating lymphocytes consisted mainly of T cells, (approximately 85%), regardless of sex, age and underlying diseases. In terms of T cell subsets composing the infiltrating lymphocytes, there were 3 types; A:CD4+ cells greater than CD8+ cells. B:CD4+ cells = CD8+ cells. C:CD4+ cells greater than CD8+ cells. Type A was seen in 60/105 cases (57%), type B in 10/105 (9%) and type C in 35/105 (34%). The presence of cases with predominant CD8+ cells appeared to be characteristic of lymphocytic infiltration lesions in the liver, and was different from lesions in other organs. It was suggested that lesion of lymphocytic infiltration in the periportal areas of the liver might be of an autoimmune nature, gradually advancing with age.

Establishment of a monoclonal antibody against senile plaques and its application for immunohistological and immunoelectron microscopical studies in the brain of the elderly.

A monoclonal antibody (Am-3) was produced against senile plaques in the brain of a patient with Alzheimer's disease. Am-3 was reactive with senile plaques of typical, primitive and diffuse type not only in the brain used as immunogen, but also those in the brain of 15 out of 25 autopsy cases of the aged people. Moreover, Am-3 was also reactive with granular materials of various sizes scattered in the 1st, 3rd and 4th layers of the cerebral cortices of the cases with severe dementia. Am-3 was also reactive with vessel wall of the congophilic angiopathy. By immunoelectron microscopic examination, Am-3 was positive with amyloid fibril in the core and crown of senile plaques, and in the congophilic angiopathy.

An experimental study of mucociliary pathology of the eustachian tube in otitis media with effusion induced by irradiation.

We have examined the function and fine structure of the mucociliary system of the eustachian tube in an experimental study of otitis media with effusion induced by X-ray irradiation. Functional examination demonstrated that the ciliary activity was diminished in such a condition, while morphological observations showed pathological findings including compound cilia, vacuolation of ciliated cells and expansion of intercellular space. These findings show that irradiation-induced otitis media with effusion results in impairment of the mucociliary system. As evidenced by these studies, the mucociliary system in the eustachian tube has an important role in the clearance of fluid produced in the tympanic cavity as well as affording improvement in this disease.

Acute effects of sulfur dioxide exposure on the middle ear mucosa.

A variety of atmospheric pollutants are known to depress mucociliary function in the respiratory system. Since the mucociliary function in the middle ear is similar, and the middle ear may be invaded by atmospheric pollutants, we decided to investigate the possible contribution of sulfur dioxide to middle ear effusion. Guinea pigs were exposed for 24 hours to 300 ppm of sulfur dioxide or air. Immediately after exposure, ciliary activity and epithelial structure were examined close to the tympanic orifice (proximal site) and more distal to it (distal site). In the animals exposed to sulfur dioxide, no effusion was found in the tympanic cavity. Ciliary activity was reduced only in the distal site. Electron microscopy demonstrated hypersecretion in the proximal site and severe pathologic changes in the distal site. Although the normally functioning cilia in the proximal site may prevent retention of surplus secretions in the ear, sulfur dioxide may promote middle ear effusion when combined with other detrimental factors, because it stimulates mucus secretion in the proximal site and impairs ciliary function in the distal site.

Increased ciliary beating frequency of nasal mucosa following immunotherapy for allergy.

The present study was designed to elucidate the effect of immunotherapy on the beating frequency of nasal cilia in patients with nasal allergy. Of 40 patients with nasal allergy due to Dermatophagoides farinae, 20 were treated with immunotherapy by the use of D farinae extracts, and 20 control patients were treated with antihistamine tablets. Mucosal pieces were taken from the right inferior turbinate before and 1 year after the initiation of treatment, and the ciliary beating frequency (CBF) was examined by a photoelectric method. The use of antihistamine tablets did not increase CBF even when it relieved the nasal symptoms. Immunotherapy, on the other hand, increased CBF in 66.7% of patients when it alleviated their nasal symptoms. The CBF before immunotherapy of patients who showed an increase CBF after treatment was statistically higher than that of patients who did not.

Mucociliary disease of the middle ear during experimental otitis media with effusion induced by bacterial endotoxin.

Lipopolysaccharide (10 micrograms/mL) derived from Klebsiella pneumoniae was injected into the middle ear of guinea pigs. The animals were killed painlessly on days 1, 3, and 7 after inoculation, and the mucosal samples from two sites within the tympanic cavity, close to the tympanic orifice and distal to the orifice, were examined for ciliary activity and epithelial morphology. At day 1 and day 3 serous effusion was observed and deterioration of ciliary activity and morphologic changes were observed. No effusion was recognized at day 7, when the ciliary activity in the distal mucosa was still diminished and that in the proximal mucosa had recovered to a normal level. Our data have shown that lipopolysaccharide extracted from K pneumoniae can produce otitis media with effusion in laboratory animals, and dysfunction of cilia due to lipopolysaccharide probably is responsible for the accumulation of middle ear effusion. The mucociliary system is indeed an important defense system and failure of such a system, especially in the mucosa close to the tympanic orifice, can cause the buildup of effusions.

An experimental study on the respiratory toxicity of isopropyl alcohol.

Guinea-pigs were exposed to 400 ppm or 5500 ppm of isopropyl alcohol (IPA) for 24 successive hours, and they were killed immediately after the exposure period. The ciliary activities of the nose and trachea were examined. In addition, the epithelium of the nose and trachea was observed by scanning and transmission electron microscopy. The present study showed that exposure to 400 or 5500 ppm of IPA vapor caused deterioration of the ciliary activity and also some pathological changes. Although our study revealed that exposure to 400 ppm of IPA vapor can affect the mucosa of the nose and trachea, the ciliary activity of the 400-ppm exposure group was not too poor and morphological changes were rather mild. Recovery from such degeneration might be rapid. Therefore, our study supports the justification of the allowable IPA level recommended by ACGIH.

Pharyngeal ciliary activity in guinea pigs with otitis media.

The function of cilia present in the tubotympanum plays an important role in the pathogenesis and chronicity of otitis media with effusion (OME). We used a photoelectric method to examine the ciliary activity in the eustachian tube, in the central pharynx, as well as the nasal cavity of normal animals and animals with otitis media. In normal animals, the ciliary activity in the nasal cavity was not different from that in the eustachian tube. In some animals with otitis media, however, there was a significant difference between the ciliary activity in the nasal cavity and that in the eustachian tube. On the other hand, no significant differences were recognized in any given group of animals examined between the level of the ciliary activity in the central pharynx and that in the eustachian tube. Our findings show that the function of the cilia present in the pharynx is a valid index of that in the eustachian tube. Our study further indicates that it may be possible to clinically assess the ciliary activity in the pharyngeal mucosa biopsied from patients with OME to determine objectively the effects of medical treatments of the disease.

Toxicity of isopropyl alcohol exposure on the nasal mucociliary system in the guinea pig.

The toxicity of isopropyl alcohol (IPA) on the nasal mucosa in the guinea pig was experimentally studied, with special reference to the recovery process of the organ. The results showed that 400 ppm of IPA has an acute effect on the nasal mucosa and the recovery from such degeneration can occur in 2 weeks. When functional and morphological damages were induced by a higher level (5500 ppm) of IPA, it took over 2 weeks for complete recovery from such damage. The conclusion is that the present allowable level of IPA is reasonable from a viewpoint of the effect of short-term exposure to IPA on the nasal mucosa. In addition, a higher level of IPA exposure has a longer-term effect on the nasal mucociliary system in the guinea pig, and workers exposed to such a higher level of IPA should be given careful otolaryngological observation.
