Apnea associated with Chiari malformation: medullary hemorrhage revealed by MRI.

Medullary hemorrhage in the medulla oblongata of an infant with Chiari malformation, hydrocephalus, meningomyelocele and hypogenesis of the corpus callosum is reported. Four days after placement of a ventriculoperitoneal shunt, spontaneous respiration diminished. Magnetic resonance imaging (MRI) successfully revealed a small medullary hemorrhage.

Protective activity of anti-exotoxin A monoclonal antibody against mice infected with toxin-producing Pseudomonas aeruginosa.

The neutralizing and protective effect of murine monoclonal antibody (MAb) 3C7 (IgG1) against Pseudomonas aeruginosa exotoxin A was examined in an experimental mouse model of infection with exotoxin A-producing strains. Treatment with MAb 3C7 blocked the reduction of functional elongation factor 2 (EF-2) in the liver of mice but could not clear the bacteria. Administration of gentamicin caused bacteria to be cleared but did not block reduction of hepatic EF-2 level. Treatment with either MAb 3C7 or gentamicin individually did not prolong time to death; however, the combined therapy with both MAb 3C7 and gentamicin cleared bacteria and blocked the reduction of hepatic EF-2 level, resulting in a significant increase in the survival rate of mice. These results suggest that anti-exotoxin A MAbs show effectiveness against pseudomonal infection caused by exotoxin A-producing strains.

Effect of acute maternal alcohol consumption on the fetal ductus arteriosus in the rat.

This study was conducted to determine whether acute alcohol consumption in near-term pregnant rats results in constriction of the fetal ductus arteriosus (DA). Twenty milliliters per kilogram of 30% (v/v) alcohol was administered via a stomach tube 0.5, 2, 4, 6 and 12 h prior to cesarean section and fetal sacrifice, at 1.00 p.m. on the 20th gestational day (day 20.5), in experimental groups A1-A5, respectively. Controls were given water alone. The calibers of the DA and pulmonary artery (PA) were measured using the whole-body freezing method with direct exposure of the DA and PA by shaving the frozen chests of the fetuses. The DA was significantly constricted 30 min after alcohol treatment. The constriction was also observed in groups A2-A4, but not in group A5. The PA was also less markedly but significantly constricted 30 min after alcohol administration, but groups A2-A5 showed normal PA caliber. It is concluded that alcohol has a constrictive effect on the DA when administered close to the end of gestation in the rat.

An electron microscopic study on follicular formation and TSH sensitivity of the fetal rat thyroid gland in organ culture.

Thyroids of 15-day rat fetuses prior to culture had no follicles. After 2 days in culture, regardless of added TSH in the medium, thyroids had small, colloid-storing follicles. In the follicular cells, the rER and Golgi apparatus were somewhat well developed as compared with the thyroids prior to culture. Thyroids of 17-day fetuses prior to culture were first inlaid with follicles. With TSH, 2-day cultures of 17-day thyroids showed a more marked dilation of rER and a further development of Golgi apparatus than those without TSH. These results indicate electron microscopically that TSH has no effect on the first formation of follicles as well as on the initial development of follicular cells, but that TSH can promote the further development of follicular cells themselves of once-developed follicles.

Constrictive effect of thyroxine on the ductus arteriosus in fetal rats.

This work was conducted to know whether thyroxine (T4) when injected into fetal rats would induce a constriction of the ductus arteriosus (DA). In Experiment 1, fetal rats on day 20 of gestation were given a subcutaneous injection of 1 or 10 micrograms T4 through the maternal uterine wall, and were autopsied 3 or 6 hr later. Similarly, in Experiment 2, the mother rats were given an injection of hydrocortisone (10 mg/kg) just after fetal T4 injections. In either series of experiments, uninjected littermates served as controls. According to the whole-body freezing and shaving method, the DA was exposed and calibrated under a dissecting microscope. In Experiment 1, the DAs of the T4-injected fetuses, 3 hr later, were significantly smaller in caliber than those of their controls, but recovered 6 hr later. In Experiment 2 with fetal T4 injection followed by maternal hydrocortisone injection, the DAs of the T4-injected fetuses were further smaller in caliber than those of their controls which were clearly shrunken as compared with those of controls in Experiment 1. These results indicate that T4 exerts a constrictive effect on the DA, an effect which is strengthened in the presence of hydrocortisone.

Developmental changes in paramesonephric and mesonephric ducts and anogenital distance in fetal Mongolian gerbils during sexual differentiation.

On day 13 of gestation in Mongolian gerbil fetuses, the mesonephric (Wolffian) duct was already visible. On day 14, the anlage of the paramesonephric (Müllerian) duct was first observed close to the mesonephric duct in both sexes at the cranial part of the mesonephros. Thereafter, the paramesonephric duct grew caudally in parallel with the mesonephric duct. On day 19, the duct reached the urogenital sinus. At this stage, the anogenital distance was significantly longer in male than in female. Also in male, the mesonephric duct was increased and the paramesonephric duct was decreased in diameter compared with those in female. These suggest that both testosterone and anti-Müllerian hormone are produced in fetal Mongolian gerbils at least before day 19 of gestation.

Amino-acid sequence of the alpha D- and beta-polypeptide chains of the Japanese quail hemoglobin.

Two hemoglobin components are recognized in the erythrocytes of the adult Japanese quail: a major (Q-II) and a minor (Q-I) component. We have determined the amino-acid sequence of the alpha D-globin of the minor component and the beta-globin which is common to both components by conventional protein sequence analysis. The sequences of both the alpha D- and the beta-globins showed close homology with those of their counterpart constituents in other avian hemoglobins. Proline at position 119 in the alpha D-globin which is known to be critical for the altitude respiration by the alpha 1 beta 1 interface is conserved in the Japanese quail hemoglobin.

Sinus and atrioventricular nodal re-entrant tachycardia.

A case of a 42-year-old woman who had combined sinus and atrioventricular nodal tachycardia is reported. Programmed atrial stimulation study showed the induction of the two types of paroxysmal supraventricular tachycardia. The mechanisms of these supraventricular tachycardias were both sinus nodal and atrioventricular re-entrant tachycardia. Sinus nodal re-entrant tachycardia could be induced only after injection of atropine. The tachycardias could no longer be induced after the administration of verapamil.

[Prevention of postoperative recurrence after hepatic resection for metastatic colorectal cancer by adjuvant locoregional chemotherapy].

Postoperative adjuvant locoregional chemotherapy was performed on patients who underwent hepatic resection for metastatic colorectal cancer. To prevent recurrence in the residual liver after hepatic resection. 7 patients received intra-hepato-arterial infusion of MMC, 5-fluorouracil (5-FU), doxorubicin + Lipiodol emulsion. Sixteen cases did not receive such treatment. The 1-, 2- and 3-year cumulative disease free rates were 100, 80 and 40% in the patients with this adjuvant chemotherapy, against 61.4, 54.6 and 28.1% in those without this locoregional therapy, respectively. The 1-, 2- and 3-year cumulative survival rates were 100, 100 and 60% in the TAI-E group, and 73.7, 63.3 and 63.3% in the patients without TAI-E, respectively. Postoperative adjuvant locoregional chemotherapy is considered effective to prevent the recurrence of metastatic colorectal cancer in the residual liver after hepatectomy.

Cellular localization of type 1 plasminogen activator inhibitor messenger RNA and protein in murine renal tissue.

Type 1 plasminogen activator inhibitor (PAI-1) may be markedly increased in the plasma of patients with endotoxemia and/or renal disease. To investigate renal PAI-1 production during acute endotoxemia, a murine model system was used. Mice were injected with either saline alone or saline containing 50 micrograms endotoxin, and sacrificed 3 hours later and their tissues analyzed for PAI-1 messenger RNA (mRNA) and antigen. Northern blot analysis confirmed that the level of renal PAI-1 mRNA was greatly increased in the endotoxemic mice relative to the saline controls. In situ hybridization was then performed to determine the cellular localization of PAI-1 mRNA within the renal tissues. In the control kidneys, low levels of PAI-1 mRNA were detected in the renal papilla and in the muscular walls of renal arteries. However, in the endotoxemic mice, an intense hybridization signal for PAI-1 mRNA was observed in glomerular and peritubular cells. These cells also stained positively for von Willebrand factor antigen, an endothelial cell-specific marker. The PAI-1 mRNA hybridization signal could further be observed in peritubular endothelial cells in the medulla and in endothelial cells of veins and arteries throughout the kidney. Immunochemical analysis revealed that PAI-1 antigen co-localized to the cytoplasm of cells expressing PAI-1 mRNA. This study provides the first direct evidence that PAI-1 is induced in endothelial cells of the kidney during endotoxemia in vivo and suggests a role for PAI-1 in the pathogenesis of renal disease.

Isolation and characterization of the chicken bcl-2 gene: expression in a variety of tissues including lymphoid and neuronal organs in adult and embryo.

The expression of human bcl-2 gene is de-regulated by t(14;18) translocation in most of follicular lymphoma. Recent studies indicated that the bcl-2 gene product has an ability to block apoptosis of hematopoietic cells. To facilitate the analysis of the role of this gene in normal development using an animal model, we have isolated and partially characterized the chicken homologue of human bcl-2 gene. The analysis of nucleotide sequence showed that the organization of the chicken bcl-2 gene is very similar to that of human bcl-2 gene. The primary transcript is spliced to encode a 25,687 dalton (233 a.a.) protein. The chicken Bcl-2 protein has two regions highly homologous to human Bcl-2 protein surrounding a totally non-homologous region. The expression of the chicken bcl-2 gene was analyzed in various chicken tissues. In the adult chicken, bcl-2 transcripts were detected in thymus, spleen, kidney, heart, ovary and brain, with the highest levels being detected in the thymus. However, the bursa of Fabricius, which is the site of early B cell development, expressed much less amounts of bcl-2 RNA. On the other hand, in embryo, the gene is extensively expressed in the bursa, as well as in muscle and the above tissues. Our findings indicate that a homologue of the human bcl-2 gene does exist in the chicken and that its expression is developmentally regulated in some tissues.

Strong association of insulin autoimmune syndrome with HLA-DR4.

Insulin autoimmune syndrome is characterised by spontaneous hypoglycaemia without evidence of exogenous insulin administration, a high serum concentration of total immunoreactive insulin, and the presence of insulin autoantibodies in high titre. HLA typing of 27 patients with insulin autoimmune syndrome showed that all had DR4, which was present in only 43% of 51 healthy controls (odds ratio 72.1, p less than 2 x 10(-6), and 19 (70%) of the patients were positive for the allelic combination, Cw4, Bw62, and DR4. Analysis of the nucleotide sequences of the DRB1, DQA1, and DQB1 genes showed that all the patients had DRB1*0406, DQA1*0301, and DQB1*0302, compared with only 14% of the controls (odds ratio 281, p less than 1 x 10(-10). We conclude that the development of insulin autoimmune syndrome is associated with a strong genetic predisposition.

Human insulin-specific immunoglobulin G antibody and hypoglycemic attacks after the injection of gold thioglucose.

A 56-year-old woman with granulomas of gold thioglucose in her hips exhibited recurrent bouts of hypoglycemic attacks. The first attack occurred 2 years after the last injection of gold thioglucose, when large amounts of extractable insulin and human insulin-specific antibody were noted in her serum. Histological examination of the resected granulomas showed marked infiltration of lymphocytes, plasma cells, and macrophages containing yellowish-brown granules, which proved to be gold by electron microscopy using X-ray microanalysis. After resection of the granuloma, however, the frequency of the hypoglycemic attacks decreased remarkably as well as the levels of both extractable insulin and human insulin-specific antibody.

Nonresponsiveness of the thyroid gland to goitrogen in the early neonatal period in the rat: light- and electron-microscopic observations.

The thyroid response of fetal and neonatal rats to propylthiouracil (PTU) as a goitrogen was studied with observation of the thyroid glands by light and electron microscopy. On day 19 of gestation and on days 1, 3, 5 and 8 after birth, fetal and neonatal rats were given a subcutaneous injection of PTU and were autopsied 2 days later. PTU induced conspicuous goiters in fetal rats but did not in neonatal rats aged up to day 5 after birth. Beyond that age, PTU again induced goiters. Histologically, the follicular cell height in goitrous thyroid glands was significantly increased. Ultrastructurally, follicular cells in goitrous thyroid glands often had colloid droplets and lysosomes. It seems that nonresponsiveness of the thyroid glands in early neonatal rats to goitrogen is due to a temporary decline of the pituitary activity of thyrotropin secretion. About 5 days or more after birth, the pituitary-thyroid system begins to operate again in response to goitrogen.

Sensitivity of the fetal rat pituitary-adrenal system to corticotropin-releasing factor in organ culture.

Six groups of adrenal glands from 17-day fetal rats were explanted to organ culture for 2 days. In one group, adrenal gland was cultured alone, and in the remaining five groups adrenal gland was cultured with pituitaries from fetuses ranging in age from 14 to 18 days. In each of the groups, half of the cultures had corticotropin-releasing factor (CRF) added to the medium. A histometric parameter utilized the size of adrenocortical cells as an indicator of sensitivity of the pituitary-adrenal system. When 17-day adrenal gland was cultured alone, addition of CRF did not cause any enlargement of cortical cells. When the adrenal gland was cultured with two 14-day pituitaries, cortical cells were enlarged. Addition of CRF to this culture induced no further change. With two 15-day pituitaries in the presence of CRF, cortical cells were slightly larger than those in the absence of CRF. With 16- to 18-day pituitaries, a marked hypertrophy of cortical cells was induced, and the addition of CRF caused further acceleration in their enlargement. These results suggest that, in organ culture, 14-day pituitary can release some adrenocorticotropic hormone (ACTH) with or without additional CRF. Older pituitaries (16- to 18-day) can apparently release an amount of ACTH in the presence of CRF that is greater than their own spontaneous ACTH secretion.
