Experience in the surgical correction of congenital ptosis in 203 children operated on at The Hospital for Sick Children, Toronto, demonstrated that because of the different types and complicated nature of ptosis the choice of operation in each case is most important. Occasionally ptosis is associated with other anomalies such as blepharophimosis, epicanthus, and the jawwinking syndrome, which further complicate its repair. Although the results in complicated cases are not perfect, these children should be given the benefit of surgery, since almost all can be greatly improved. The best time for surgery is after three years of age but before the child starts school. If adequate levator action is present, shortening this muscle is the operation of choice. If levator function is poor, the lid is elevated by the frontalis muscle and fascia lata. Achievement of symmetry in the position of the lid and width of the palpebral fissure is the key to real success in ptosis surgery.

Sixty-five patients, in whom eyelid cysts and tumours were surgically excised and examined histopathologically, were followed up. The general principles of treatment are outlined. The procedures used in the reconstruction of the eyelids, which depended upon the extent of lid-loss; the results obtained; and the complications encountered, are described.Surgical excision with subsequent repair is preferred to radiation therapy in view of the unique accessibility, general low malignancy and infrequent metastases of these tumours. Such excision and repair produces a better cosmetic and functional result.

Enterochromaffin-like (ECL) cells are the dominant endocrine cell type in the oxyntic mucosa. Normally regarded as histamine-producing cells, they are exquisitely sensitive to the trophic action of gastrin and undergo a hyperplastic increase in a variety of hypergastrinemic conditions. A hyperplasia-neoplasia sequence of ECL-cell proliferations has been recently proposed, following the realization that increasingly severe degrees of ECL-cell hyperplasias over a period of several years can progress to ECL-cell carcinoids. Such carcinoids arising in patients with chronic hypergastrinemia differ both in their clinical and pathologic profiles from the sporadic carcinoids that occur in normogastrimenic individuals and, therefore, need to be distinguished from them. This distinction is particularly important for their clinical management, since antrectomy appears to be of benefit in ECL carcinoids of hypergastrinemic patients.
