[Solitary infected renal cyst: a case report].

A 48-year-old man was admitted to our hospital because of high fever and left flank pain. Laboratory findings revealed a high white blood cell count, high C-reactive protein level, and severe pyuria. Sonographic examination revealed an enlargement of the cyst at the upper pole of the left kidney that had already been detected. Percutaneous drainage was performed for the cyst and 60 ml of purulent fluid was obtained. Bacterial culture of the fluid was positive for Propionibacterium acnes and gamma-Streptococcus. The drainage and administration of povidone-iodine was continued for 7 days. The size of the cyst was reduced with disappearance of symptoms.

[A case of Cushing's syndrome due to adrenal black adenoma].

Functional black adenoma of the adrenal gland is rare, and there are few reports. We report a case of functional black adenoma of the adrenal gland associated with Cushing's syndrome. The case was in a 59-year-old female, whose chief complaints were weight gain and general fatigue for 7 years. Clinical and laboratory findings were characteristic to those of Cushing's syndrome. With computed tomography and ultrasonography, a round tumor about 18 x 12 mm was detected at the left suprarenal region. Left adrenalectomy was done and a black tumor, 5.0 g in weight, was obtained. Most of the adenoma cells contained numerous pigmented granules which were considered as lipofuscin. The patient is alive without any symptoms of Cushing's syndrome 8 months after the operation. The Japanese literature of the functional black adenoma was also reviewed.

Evidence for the in vivo formation of ascorbic acid 2-O-alpha-glucoside in guinea pigs and rats.

In vivo formation of ascorbic acid 2-O-alpha-glucoside (AA-2G) in guinea pigs and rats given ascorbic acid (AA) orally in combination with maltose was examined. A metabolite of AA which has the same HPLC retention characteristics as authentic AA-2G was detected in the blood, urine and liver of guinea pigs 1-2 hr after their administration. The metabolite was isolated from the urine by chromatographic procedures and identified as AA-2G by its UV spectrum, non-reducibility, susceptibility to alpha-glucosidase hydrolysis, HPLC profile and elementary analysis. The same glucoside was also synthesized by rats and found in the urine, although it could not be determined qualitatively in the blood. AA-2G-forming activities of tissue homogenates from both animals were apparently correlated with their alpha-glucosidase activities and, moreover, both activities were completely inhibited by a specific neutral alpha-glucosidase inhibitor. Thus, we conclude that AA-2G is a possible metabolite produced by enzymatic alpha-glucosidation after a combined administration of AA and maltose to guinea pigs and rats.

Immunological comparison between prostate-specific antigen and gamma-seminoprotein.

Prostate-specific antigen (PA) and gamma-seminoprotein (gamma-Sm) were compared by immunocytochemical, immunodiffusion and immunoblotting methods using rabbit anti-PA antibody and rabbit anti-gamma-Sm antibody. Enzyme immunoassays (EIAs) were developed for measurements of PA and gamma-Sm to determine a correlation between serum PA and gamma-Sm levels in patients with prostate cancer. The patterns of localization and distribution of PA and gamma-Sm were identical in prostate tissue sections, including benign and cancerous human prostates. The immunodiffusion study showed that the antigens with which anti-PA antibody and anti-gamma-Sm antibody reacted in seminal plasma and prostate tissue homogenates were identical to each other. In the immunoblotting study, anti-PA antibody and anti-gamma-Sm antibody recognized a single antigen corresponding to a molecular weight of approximately 33,000 both in seminal plasma and prostate tissue homogenates. The EIAs developed in this study were sensitive, specific, and reproducible, and the correlation between serum PA and gamma-Sm values determined by these EIAs was highly significant (r = 0.99, P less than 0.001). These results indicated that PA and gamma-Sm were immunologically identical and that serum PA and gamma-Sm determined by immunoassays using anti-PA antibody and anti-gamma-Sm antibody should be evaluated as identical tumor markers for serodiagnosis of prostate cancer.

Graves' thyrotoxicosis and Moyamoya disease.

Cerebrovascular accidents due to Moyamoya disease, a disorder characterized by arterial stenosis at the base of the brain accompanied by typical net-like collateral vessels, occurred in two young Japanese women with Graves' disease when they were in thyrotoxicosis. In one patient, a second attack of cerebral infarction occurred with the recurrence of thyrotoxicosis. Association of Moyamoya disease and Graves' thyrotoxicosis is rare and the pathegenetic relationship is discussed.

Simultaneous development of insulin dependent diabetes mellitus and alopecia areata universalis.

Alopecia areata universalis developed gradually over nine months in a 25-year-old woman. When her scalp hairs were totally lost and other body hair began to fall out, the symptoms of insulin dependent diabetes mellitus, an organ-specific autoimmune disease, developed rapidly and progressed to diabetic coma. Alopecia did not regress after the metabolic state improved following insulin therapy. Biopsy of the scalp skin revealed significant reduction of the total number of hair bulbs and prominent lymphocyte infiltration into the remaining hair follicles. Insulin dependent diabetes mellitus and alopecia areata universalis are both thought to be related to the autoimmune mechanism and sometimes coexist. However, simultaneous development of these two disorders is extremely rare.

[Detection of Chlamydia trachomatis by polymerase chain reaction].

A polymerase chain reaction (PCR) procedure was developed for detection of Chlamydia trachomatis. Two oligonucleotide primers based on sequences within the major outer membrane protein gene from C. trachomatis serovar L2 were used. A single DNA fragment was amplified, when C. trachomatis DNA was template for the PCR. No amplified product was detected in Chlamydia psittaci DNA, Chlamydia pneumoniae DNA or other bacterial DNAs. The amplified DNA fragment was detected, when DNA of greater than or equal to 10(2) C. trachomatis per reaction was used as template for the PCR. Thus, the PCR was shown to be specific for C. trachomatis and more sensitive than the enzyme immunoassays for detection of chlamydial antigen and the chlamydial rRNA:DNA probe hybridization method.

[Detection of Chlamydia trachomatis from urethral swab in male urethritis by polymerase chain reaction].

A polymerase chain reaction (PCR) with Chlamydia trachomatis-specific primers was applied for detection of C. trachomatis from urethral swab in male urethritis. The results were compared with those of culture method for detection of C. trachomatis. Of 18 clinical specimens tested in this study, inclusion bodies of C. trachomatis were detected in 11 specimens by the culture method. For PCR, sample DNA was prepared from transport medium in which urethral smear was suspended and two oligonucleotides based on sequences within the major outer membrane protein gene from C. trachomatis serovar L2 were used as extension primers. In 12 of the 18 specimens, 242bp DNA fragment was amplified by PCR and demonstrated to be the DNA fragment of C. trachomatis by Southern blot hybridization. No DNA of 242bp was amplified by PCR from five specimens in which any inclusion bodies of C. trachomatis were observed or from a specimen in which one inclusion body per cover slip was detected by culture method. C. trachomatis DNA of 242bp was amplified from all specimens in which 14 and more inclusion bodies per cover slip were detected by culture method. In two specimens concluded s negative by culture method, amplified C. trachomatis DNA were detected by PCR. Thus, the PCR would be a more simple and sensitive method for detection of C. trachomatis, compared with the culture method.

[Clinical study of urinary fungal infection: survey of patient background and fungal strains from the urine].

Clinical features of urinary fungal infections were analyzed in 152 patients in whom fungi were cultured from urine and mycologically identified between February, 1989 and June, 1990. The average age was 66 years old, and approximately 70% of the patients were 60 years old or older. Systemic and urinary tract underlying diseases were observed in 145 patients (95.4%) and 70 patients (46.1%) had urinary indwelling catheters. Previous use of antimicrobial agents before isolation of urinary fungi was recorded in 103 patients (67.8%), the 2nd and 3rd generation cephems and new quinolones being most frequent. A mixture of fungi and bacteria was observed in 68 patients (44.7%). Gram-positive cocci were isolated from 50% of them. Of 173 strains of fungi, C. glabrata was the most frequent (31.2%), followed by C. albicans (27.8%), C. tropicalis (17.3%) and T. beigelii (15.0%). All strains of C. glabrata, C. albicans, C. tropicalis and T. beigelii were highly sensitive to flucytosine (5-FC), miconazole (MCZ) and amphotericin B (AMPH), except for 3 strains which were resistant to 5-FC. The consecutiveness was confirmed by repeated culture in 59.4% of the patients. Fungi in the urine, however, spontaneously disappeared in the other patients.

[Studies on 5-FU concentration in serum and prostate cancer tissue after oral administration of UFT].

The serum, urine and tissue concentrations of 1-(2-tetrahydrofuryl)-5-fluorouracil (FT), 5-fluorouracil (5-FU) and uracil were estimated in 11 patients with prostate cancer (4 cases treated by total prostatectomy and 7 cases by transurethral resection (TUR-P) after oral administration of UFT. The concentration of FT, 5-FU and uracil in the tumor tissue (micrograms/g) were 5.920 +/- 5.902, 0.018 +/- 0.012 (T/S; 2.20) and 7.785 +/- 4.151 in 4 patients treated by total prostatectomy and 1.943 +/- 1.355, 0.024 +/- 0.010 (T/S; 1.46) and 4.616 +/- 2.848 in 7 patients treated by transurethral resection of prostate. The concentrations of FT, 5-FU and uracil in the tumor tissue did not increase as compared with those in normal tissue in 4 cases treated by total prostatectomy.

[A case of adrenal myelolipoma].

A case of myelolipoma of the right adrenal gland is reported. A 43-year-old female was admitted to our hospital, Department of 1st Internal Medicine, because of acute hepatitis. CT incidentally showed a well circumscribed mass, as an area of low density, arising from the right adrenal gland. Although sonography showed a markedly echogenic tumor, it was not visualized on the angiogram. Results, of hormonal examinations, were unremarkable except for low levels of ACTH. We confirmed a myelolipoma of the right adrenal gland, and performed right adrenalectomy. The tumor was 65 x 50 x 35 mm in size, 95 g in weight. Pathology disclosed an admixture of mature adipose tissue and hematopoietic elements resembling bone marrow.

[Flucytosine in the treatment of urinary fungal infections. Clinical efficacy and background factors].

Flucytosine (5-FC), an oral antifungal agent, was used in the treatment of 37 patients with urinary tract infections due to fungi. Criteria for treatment included pyuria of greater than or equal to 5 WBCs/HPF, fungal colony counts in urine greater than 10(3) CFU/ml and the presence of candiduria in 2 consecutive urine culture with interval of 7 days. Patients received 2 approximately 8 g/day of 5-FC for 7 to 14 days. The clinical efficacy was evaluated in 27 patients with 14 day-treatment and overall efficacy rate was 77.8%. In patients with indwelling catheter, overall efficacy rate was lower than that in patients without catheter. Twenty-two of 30 strains (73.3%) were eradicated by the treatment. The MIC of 5-FC for isolates was less than 0.78 micrograms/ml except for 5 strains. The secondary resistance during 5-FC treatment was observed in early phase in 6 patients whose clinical efficacy was poor. The loss of appetite and mild leukocytopenia were observed as adverse effects. From the results of our study, 5-FC appears to be effective and safe in the treatment of urinary tract infection due to fungi.

[Spontaneous rupture of the renal pelvis in a female patient with defect of the uterus and hypoplasia of the vagina: a case report].

A 17-year-old woman consulted our clinic with the complaint of gross hematuria and lower abdominal pain. Blood examination showed severe anemia and renal failure. Emergent hemodialysis and blood transfusion were performed. CT scanning revealed left retroperitoneal hematoma, right severe hydronephrosis and loss of uterus shadow. Under diagnosis of left renal rupture, left nephrectomy and removal of retroperitoneal hematoma were performed. The area of rupture in the left renal pelvis was identified on its posterior wall. Postoperatively renal function did not improve and periodic hemodialysis has been done.

[The significance of serum fructosamine measurement in patients with thyroid diseases].

Serum fructosamine was measured in patients with Graves' disease and primary hypothyroidism. Fructosamine levels and fructosamine per albumin ratio were significantly lower in patients with Graves' disease than in normal subjects, while they were significantly higher in patients with primary hypothyroidism. Fructosamine levels were normalized by treatment in the euthyroid state in patients with Graves' disease. Along with the normalization of thyroid stimulating hormone levels, fructosamine levels returned to the normal range in patients with primary hypothyroidism with treatment. There were significant correlations between fructosamine levels and free triiodothyronine levels, free thyroxine levels, thyroid stimulating hormone levels, hemoglobin A1C levels, albumin levels, and creatine phosphokinase levels. We concluded that it was useful to measure serum fructosamine as an indicator of peripheral metabolic function in patients with thyroid diseases.

Structure of the D-mannan of Candida stellatoidea IFO 1397 strain. Comparison with that of the phospho-D-mannan of Candida albicans NIH B-792 strain.

The structure of the D-mannan of Candida stellatoidea IFO 1397 strain, which has properties identical to those of the phospho-D-mannan of C. albicans serotype B strain, does not contain phosphate groups, and its 1H- and 13C-n.m.r. spectra are quite similar to those of the phospho-D-mannan of C. albicans NIH B-792 strain. However, the 1H-n.m.r. and 1H-13C-correlation n.m.r. spectra of the products obtained by digestion with alpha-D-mannosidase of C. stellatoidea D-mannan considerably differed from those of the corresponding digestion products of the C. albicans phospho-D-mannan. Additionally, the enzyme-linked immunosorbent assay, by means of a monoclonal antibody corresponding to (1----2)-linked beta-D-oligomannosyl residues, of the phospho-D-mannan of the same C. albicans strain indicated that the C. stellatoidea D-mannan does not contain any (1----2)-linked beta-D-oligomannosyl residues. The absence of these residues may be used as one of the criteria of chemotaxonomical identification of C. stellatoidea spp.

Electron microscopic study of acute retrograde pyelonephritis in mice.

A strain of Escherichia coli O6:H-, which was isolated from a patient with acute pyelonephritis, was used to produce ascending pyelonephritis in mice. The histologic features at the early stage of acute pyelonephritis and the pathways of bacterial invasion and distribution in the infected kidneys were studied by using transmission electron microscope. The histologic changes were characterized by degeneration and destruction of renal pelvic and tubular epithelial cells, and a massive infiltration of polymorphonuclear leukocytes. Bacteria invaded the pelvic surface and spread over the renal medulla and cortex by contiguity twelve hours after infection. It was also shown that bacteria ascended the tubules, multiplied in the tubular lumina, destroyed the tubular epithelial cells, and spread over the renal cortex by twelve hours. In addition, bacteria and leukocytes phagocytizing bacteria were present in the capillaries in the renal interstitium twelve hours after infection.

[Study on porcelain veneer restorations. 2. Influence of hydrofluoric acid on bonding strength at the porcelain-resin interface].

Recently, porcelain veneer restoratives have been introduced to the general practice, and their clinical performances have been confirmed through many longterm clinical investigations. It is expected that porcelain veneer restorations will perform successfully in esthetic, conservative and abhesive dentistry. It is an well known fact that the micro-mechanical bonding strength at the porcelain-resin interface which is achieved through the application of hydrofluoric acid to the porcelain surface is quite a strong bonding mechanism. However, there are very few studies reporting on the acid treatment of porcelain surfaces. The authors have been studying the influence of hydrofluoric acid on porcelain surfaces, and in our previous report we reported, the degrees of corroded porcelain treated with different concentrations of hydrofluoric acid for different durations of application. In the present study, shear bonding strength was measured between resin cements and porcelain surfaces treated with different concentrations (4, 6, 8%) of hydrofluoric acid and for different durations (1 to 24 min.), and the appropriate treatment of porcelain surfaces with regard to the bonding strength was determined. The results obtained were as follows. 1. As the treating time increased with any concentration (4, 6, 8%) of hydrofluoric acid, corrosion of the porcelain surface became more intense. Hardly any evidence of corrosion was observed on any porcelain surface treated for one minute, so it seems that the treatment of porcelain surfaces using 4 to 8% hydrofluoric acids should be continued for over three minutes. 2. Observation of the surface profile by SEM showed no significant differences between the surfaces treated for 3, 6, 12 and 24 minutes. 3. It was not clear as to how the differences of hydrofluoric acid concentrations (4, 6, 8%) plus the differences in the kinds of porcelain (Super Porcelain AAA, NORITAKE Co. Ltd., VMK 68, Vita Zahnfabrik Gmbh & Co., Cosmotech Porcelain, G-C Co. Ltd.) had affected the differences in the amount of corrosion. 4. Regarding to the shear bonding strength at the porcelain-resin cement interface, in comparison with control specimens which were only sandblasted, about all two to three times higher bonding strength were obtained in specimens treated with any concentration (4, 6, 8%) of hydrofluoric acids and for any duration between 3 to 24 minutes. Specimens treated for one minute were an exception.

[A case of silicate urolithiasis].

We report a case of silicate calculi with no history of taking magnesium trisilicate. A 33-year-old woman was sent to our hospital as an emergency case because of severe right lower flank pain. Physical examination was unremarkable except for severe right cost-vertebral angle knock pain. She denied administration of a magnesium trisilicate anti-acid before. She was admitted to the urologic ward since the pain did not relieve in spite of several analgesics. The stone passed spontaneously on the third hospital day. Analysis by infrared spectrophotometry demonstrated the composition to be over 98% of silicate. A review of the literatures discloses only 21 cases of silicate stones.
