Persistence of human papillomavirus type 6e in adult multiple laryngeal papilloma and the counterpart false cord of an interferon-treated patient.

Human papillomavirus (HPV) type 6e DNA was detected in a laryngeal papilloma, in a papilloma recurring after an interval of six years and in the counterpart clinically-normal false cord of the same patient. No detectable subgroup change of HPV DNA occurred during the clinical course, which included therapy by CO2 laser followed by intramuscular alpha-interferon injection. The data demonstrated a single subtype of HPV to have persisted, and the prevention of a recurrence of laryngeal papilloma during our patient's long clinical course to have failed, in spite of the interferon treatment.

Specific base recognition of oligodeoxynucleotides by capillary affinity gel electrophoresis using polyacrylamide-poly(9-vinyladenine) conjugated gel.

Poly(9-vinyladenine) was synthesized and utilized as an affinity macroligand entrapped within the gel matrix. Base-specific separation of oligodeoxynucleotides was achieved with high resolution and high speed by electrophoresis, using capillaries filled with conjugated polyacrylamide-poly(9-vinyladenine) gel. Oligothymidylic acids were selectively separated from the mixture of oligothymidylic and oligodeoxyadenylic acids by utilizing a specific hydrogen bonding between poly(9-vinyladenine) and oligothymidylic acids. Migration time and resolution of oligodeoxynucleotides were influenced by several parameters, such as the size of poly(9-vinyladenine), capillary temperature, and concentrations of poly(9-vinyladenine) and urea. Some guidelines are presented, based on the theoretical formulation of the effect of these parameters, in order to find optimum electrophoretic conditions. Analytical capillary affinity gel electrophoresis was developed for the selective and sensitive base recognition of oligodeoxynucleotides with efficiencies as high as several 10(6) plates/m by using a urea-gel capillary with poly(9-vinyladenine) and temperature-programming.

Demonstration of systemic lymphnodes by bone scintigraphy in amyloidosis.

We report a case of primary amyloidosis with calcification of systemic lymphnodes which were demonstrated as positives by bone scintigraphy. Positive sites delineated by bone scintigraphy would seem likely to reflect avid calcification of amyloid deposits. The discovery of positive systemic lymphnodes by bone scintigraphy is very rare in a routine study and may allow for amyloidosis in a differential diagnosis.

Evaluation of oil-soluble FUdR ester for transcatheter arterial treatment of hepatomas.

From January 1987 to December 1989, we performed a prospective study of transcatheter arterial chemoembolization therapy with iodized oil (Lipiodol) mixed with an anticancer agent. A new oil-soluble modification of FUdR, FUdR-C8 ester, was developed and dissolved in Lipiodol as an embolic material, which was administered to 36 patients with hepatomas. Water-soluble adriamycin emulsified in Lipiodol was used as control in 67 patients with hepatomas. Initial effects on tumors were observed as decrease of tumor size in 25.0% for the FUdR group and 17.9% for the control group. Alpha-fetoprotein decreased more than half in 41.9% of the FUdR group, and 16.1% of the ADM group. Six-month and one-year survivals were 74.2% and 46.8% for the FUdR group (median survival 317 days), whereas the control group yielded 61.0% and 28.3% survivals, respectively (median survival 191 days).

Recognition of (d(TTTATT) and d(TTATTT) by capillary affinity gel electrophoresis (CAGE) using poly(9-vinyladenine)-polyacrylamide conjugated gel.

Sequence-specific recognition of oligodeoxynucleotide isomers (TTTATT and TTATTT) was achieved by using polyacrylamide-poly(9-vinyladenine) conjugated gel filled capillary affinity gel electrophoresis. It was found that the interaction between poly(9-vinyladenine) and the isomers was dependent on the sequential thymidylic acids of them.

[Multiorgan failure following intravesical bacillus Calmette-Guerin administration: a case report].

We describe a case of multiorgan failure after intravesical bacillus Carmette-Guern (BCG) immunotherapy for bladder cancer. A 58-year-old man with superficial transitional cell carcinoma of the bladder was initially treated by transurethral resection and intravenous chemotherapy, and then administered 11 sessions of BCG intravesically. He was administered BCG intravesically after cystoscopic examination. The next day he complained of nausea and malaise. He became hypotensive. The symptom progressed with multiorgan failure, disseminated intravascular coagulation and respiratory failure. The patient gradually improved with administration of antibiotics and corticosteroid, and hemodialysis, without antituberculous antibiotics. Intravesical instillation of BCG should not be carried out immediately after cystoscopic examination.

[Tubulo-interstitial nephritis (TIN) with no glomerular lesions, distal renal tubular acidosis and asteatosis cutis in a patient with systemic lupus erythematosus (SLE): a case report].

We report a 28-year-old woman with systemic lupus erythematosus (SLE) who showed tubulo-interstitial nephritis (TIN) without any glomerular changes. In 1990, she was admitted to our hospital, complaining of anorexia, vomiting and persistent high fever. Laboratory findings showed proteinuria, pancytopenia, hypocomplementemia and positive for antinuclear antibody, anti-DNA antibody, anti-Sm antibody, anti-SSA antibody and anti-SSB antibody. We made a diagnosis of SLE. Furthermore, distal renal tubular acidosis and asteatosis cutis were revealed. The diagnosis of Sjögren's syndrome was not made. We treated with high-dose prednisolone (60mg/day) and achieved improvement of symptoms and laboratory data. Open renal biopsy showed TIN without any glomerular changes. Predominant TIN is very rare in SLE. We discussed its pathogenesis and relation to the renal lesions of Sjögren's syndrome.

[Endoscopic treatment of early gastric cancer].

We assessed the results of endoscopic resection (ER) of early gastric cancer using electrocoagulation with high frequency current. Resection was performed utilizing endoscopic double-snare polypectomy (EDSP) and electrocoagulation. The initial endoscopic total resection rate was 65.0% (119/183 lesions). There were 32 lesions in which no residual cancer was present at follow-up biopsy or surgery, and when these were included the endoscopic resection cure rate was 82.5%. Extensive tissue necrosis and degeneration occurred in the resected stump as a result of using electrocoagulation. Since an increase in suitable patients and an expansion of the indications for endoscopic resection are anticipated in the future, careful clinical evaluation of endoscopic resection based on the results of pathological assessment of the resected tissue is needed.

[A case of familial lecithin: cholesterol acyltransferase deficiency].

Lecithin: cholesterol acyltransferase (LCAT) is an enzyme that catalyzes the esterifying reaction of cholesterol in plasma high density lipoprotein (HDL). Deficiency of LCAT is a rare hereditary disease characterized by several clinical symptoms such as proteinuria, corneal opacity, and anemia due to a shortened life span of erythrocytes. In this communication, we report a case of 40 year-old female patient of LCAT deficiency. She visited a hospital for work-up of proteinuria, corneal opacity and anemia. Activity of her serum LCAT was found to be extremely low, and characteristic changes in plasma lipids due to deficiency of LCAT was observed: those were marked decreases in HDL-cholesterol, degree of esterification in serum cholesterol, and apoprotein A-I, A-II, B and C-II levels. The diagnosis of LCAT deficiency was finally made. We studied about histopathological changes in the patient's kidney, and erythrocyte membrane lipid composition and fluidity. Histopathological findings in renal biopsy were follows: a) Light microscopy showed spherical deposits stained with periodic acid-Schiff in mesangial matrix and adjacent capillary loops, and hyaline deposits in arterioles, b) Electron microscopy showed vacuoles in mesangial matrix and along the glomerular basement membranes. In erythrocyte membrane lipids, increase of cholesterol to phospholipid molar ratio was evident, being accompanied by changes in phospholipid fractions: increase of phosphatidylcholine, and decreases of phosphatidylethanolamine, sphingomyelin and lysophosphatidylcholine. In phospholipid acyl chains, increase of C18:2 and decreased of C18:1 were evident in the patient. Erythrocyte membrane fluidity was found to be decreased in the patient in a measurement by pyrene, probably being related to the changes in membrane lipid composition.(ABSTRACT TRUNCATED AT 250 WORDS)

Evaluation of cancer prevention strategies by computerized simulation model: an approach to lung cancer.

A computerized simulation model was developed to evaluate the potential impact of primary and secondary prevention on lung cancer mortality in Japan. The natural history of lung cancer was modeled as a Markovian stochastic process from cancer-free to preclinical, clinical, and finally to terminal states. The increase in mortality rate of lung cancer among males aged 75 to 79 years has been the major force of increase in the total number of lung cancer deaths in Japan. The simulation showed that this tendency would continue until the late 1990s, presumably due to the increase in the proportion of ever-smokers in that cohort, who started smoking at an earlier age than did prior generations. It was shown that the number of lung cancer deaths can be reduced either by smoking cessation or screening programs, and that the reduction is proportional to the increase in the annual smoking-cessation rate and to the annual increment in the screening rate. However, only two to three percent reduction of lung cancer deaths in the year 2001 can be expected when the annual smoking-cessation rate is raised from the current value of 0.46 percent to five percent during the period from 1991 to 2000 or when the screening rate is increased by three percent annually for the same period.

Ceroid-containing histiocytic granuloma of the endometrium.

In an endometrial curettage specimen, taken from a 67-year-old woman with post-menopausal bleeding, the endometrium was replaced by aggregates of swollen histiocytes filled with yellowish-brown pigment. Histochemical and ultrastructural examination of the specimen revealed that the pigment was of ceroid nature. Previously reported cases with similar findings are reviewed, and the distinction of this type of endometrial lesion from 'foam cell reaction' is briefly discussed.

[Clitoral enlargement caused by prepucial hemangioma: a case report].

A 4-year-old girl visited with the chief complaint of clitoral enlargement which was first pointed out when she was one year old. She has received excision of the femoral tumor, which was histologically an arteriovenous hemangioma, at 4 years old. The prepuce was darkly purplish and the clitoral neck was enlarged without enlargement of the clitoral glans. The external genitalia otherwise were normal and there were no virilizing signs. The mass was excised at the preoperative diagnosis of prepucial hemangioma. The mass was enclosed in the prepuce and adherent to its skin and there was no enlargement of the clitoris itself. The histopathological diagnosis was arteriovenous hemangioma.

[Successful surgical management of postinfarction ventricular septal perforation in an 85-year-old woman].

The patient was an 85-year-old woman who developed chest pain on August 6, 1989, and was admitted to our hospital the next day. The diagnosis was acute inferior myocardial infarction. Her general condition was good at first; however, a harsh holosystolic murmur was suddenly heard 13 hours after the onset of infarction. Immediate cardiac catheterization revealed a ventricular septal perforation with a 2.2:1 shunt and total occlusion of the posterior descending artery. IABP was started 3 days after the onset of perforation. PCWP and shunt ratio decreased after starting IAPB; however, lung congestion continued to worsen. An emergency operation was performed on the 5th day after the onset of perforation. The post-operative course was uneventful. She is presently doing well 17 months after the surgery. Seven patients more than 80 years old have been surgically treated for postinfarction ventricular septal perforation in Japan and this patient is the oldest.

[Severe alcoholic cirrhosis associated with spur cell anemia and DIC].

A 29-year-old male who had a 15-year history of alcohol drinking was admitted with a 5-month history of jaundice in July 1989. Laboratory examinations revealed that he had hemolytic anemia and severe liver damage. Erythrocytes of peripheral blood showed typical spiculated cells on light microscopic and scanning electron microscopic studies. Free-cholesterol/phospholipid ratio of the erythrocyte membrane was elevated, and the level of chenodeoxycholic acid increased in serum. The patient was diagnosed as having advanced alcoholic cirrhosis associated with spur cell anemia. Two months after admission, he was complicated with chronic disseminated intravascular coagulopathy (DIC). Anemia, hepatic failure and DIC progressed, and the patient died six months after admission. The mechanism of the formation of spur cells was discussed.

[Thermography of collagen diseases with Raynaud's phenomenon].

In forty-four patients with collagen diseases accompanied with Raynaud's phenomenon(systemic lupus erythematosus(SLE)10, progressive systemic sclerosis(PSS)15, primary Sjögren's syndrome(PSJS)18 and mixed connective tissue disease(MCTD)1), we analyzed the thermography of fingers after thermal stimulation with cold water(15 degrees C, 30 seconds). In normal controls, the temperature of the distal points of fingers was higher and recovered more quickly than that of the proximal points(proximal interphalangeal joint; PIP). But in the patients with Raynaud's phenomenon, the temperature of distal point of fingers was lower and recovered more slowly than that of proximal points. Because the difference of the temperature between the distal and the proximal points (DP)is a good parameter to distinguish these two thermographic patterns, we analysed DP. DP is less influenced by room temperature than the temperature of fingers and the recovery temperature after thermal stimulation which have been used as indicators of Raynaud's phenomenon. DP in the normal controls was significantly higher than that in the patients. DP in PSS was lower than that in SLE and PSJS, and statistically associated with the markers of disease activities, such as erythrocyte sedimentation rate, titers of anti-RNP antibody and diffusing capacity of lung. These results show that thermographic pattern and the value of DP are the useful indicator of severity of Raynaud's phenomenon.
