Relationships between C3b receptor (CR1) activity of erythrocytes and positive Coombs' tests.

Although positive direct Coombs' tests occur in most patients with active systemic lupus erythematosus (SLE), haemolytic anaemic associated with antibody to erythrocytes (E) occurs in less than 10%. Our studies show an association between positive direct Coombs' tests and both the presence of circulating immune complexes and diminished activity of the C3b receptor (CR1) of E. Data presented in this report suggest that in vivo binding of immune complexes and complement by the CR1 of E results in positive direct Coombs' tests in the absence of antibody to E. These observations explain the low frequency of haemolytic anaemia compared with the high frequency of direct positive Coombs' tests in patients with SLE.

Histopathologic classification of uterine choriocarcinoma.

Histopathologic features of uterine choriocarcinoma were studied to establish new criteria for grading malignancy of the disease. Thirteen items of histopathologic findings concerning the degree of differentiation and the forms of masses of trophoblasts (Trs), the manner of Tr invasion, and host response of surrounding tissues were studied with relationship to prognosis in 70 patients with uterine lesions (alive, 49; dead, 21). Chi-square test results were examined for each item in relation to prognosis of the patients. Four items were thought to have significance and were selected as criteria: (1) island formation; (2) massive proliferation of intermediate-type Trs; (3) rectangular infiltration of Trs to surrounding muscle fibers; and (4) atypia of Trs at the end-point of tumor invasion. A discriminant analysis was carried out (under the standardization of tumor extension and the historical staging of treatment). From the results obtained in discriminant analysis, scores were given to the four items that existed in the specimen. New criteria for grading malignancy are proposed based on scoring these four items. The algebraic sum of the scores had a possible range of +4 to -16. Patients with scores of -9 and above had a low-grade malignancy with a mortality rate of 7%. Patients with scores of -10 and lower had a mortality of 69% and were classified as having tumors of high-grade malignancy.

Anatomical and microscopic study of the volar dermal ridges of the rat (Rattus norvegicus).

The epidermal ridges of the rat (Rattus norvegicus), which are distributed only on the volar pads and digital apices, were studied. Examination of the ridges was difficult on the epidermal surface, as the undulations expressed on the epidermis are weak. Therefore, the dermal surface, prepared by alkaline solution treatment, was inspected by scanning electron microscopy and staining with toluidine blue. The dermis of the pads and digital apices is composed of ridged and rippled areas. The ridged area, where sweat ducts are distributed, is constructed of grooves and ramparts. Frequently, the sweat duct is surrounded by a dermal collar, and the groove is separated by a dermal partition. The grooves and ramparts display dermatoglyphic configurations, such as whorls, loops, cusps, triradii, and some other patterns, which are peculiar to each pad and digital apex and comparable to dermatoglyphic patterns of man and other primates.

[A long-term prognosis of ventricular septal defect associated with aortic regurgitation].

Fifty-one cases of ventricular septal defect associated with aortic regurgitation (VSD and AR) were examined for past twenty-two years. Only VSD closure was performed for AR cases under Sellers II regurgitation. After the average of 11.6 years' follow-up, 14 cases retained over 60 mmHg of diastolic pressure and belonged to NYHA functional class 1. VSD closure and aortic cusp suspension were performed for AR cases over Sellers II regurgitation. Three cases operated before 1969 died of cardiac failure and one case recently died of infectious endocarditis (IE). Because of residual AR, one case was performed re-aortic cusp suspension one year after the operation and another case was scheduled for re-aortic cusp suspension after three years and eight months. Eighteen cases with malfunctioned valve caused by calcification, etc. underwent the aortic valve replacement. Early death was due to low cardiac output syndrome (LOS). Five cases were followed up for eleven years on the average. Two cases were found atrial fibrillation and myocardial infarction, respectively. Four cases were VSD + AR with the past history of IE and one died of LOS after surgery. Two cases were postoperative IE. One died three months after the operation and the other is doing well after the re-surgery by translocation. A long-term finding of SV1 + RV5 by electrocardiogram showed a significant decrease in left ventricular overload. Significant decrease of cardiothoracic ratio was observed in AR group under Sellers II regurgitation. Pulse pressure was significantly decreased as well.

High melanosome engulfing activity of cutaneous fibroblasts in macular amyloidosis: an electron microscopic study.

Six patients with macular amyloidosis were investigated by electron microscopy and various morphological changes in fibroblasts were identified. Many cells, which seemed to be macrophages by light microscopy, proved to be fibroblasts. The digesting action of fibroblasts was well developed and many melanosomes were taken into the cells making the cells resemble melanophages. The fibroblasts extended long, thin and branched cytoplasmic processes to surround the amyloid mass, and the cells selectively extended these into the narrow spaces between collagen and amyloid. The fibroblast showed highly developed endocytotic activity and probable ingestion of amyloid by pinocytosis or phagocytosis. The developed rough endoplasmic reticulum contained much protein and was active in secretion.
