Inhibition of stress-induced gastric mucosal injury by a long acting superoxide dismutase that circulates bound to albumin.

To determine whether oxygen-derived free radicals play an important role in the pathogenesis of stress-induced tissue injury, the effect of a superoxide dismutase derivative, which binds to albumin and circulates with a half-life of 6 h in intact rats, on acute gastric mucosal lesion was observed in rats which were given water-immersion-restraint. This enzyme derivative also circulated bound to albumin with a half-life of 8 h in rats which were challenged with water-immersion-restraint. This treatment significantly perturbed systemic circulation of animals by decreasing the effective volume of circulating blood, increased vascular permeability of the gastric mucosa, and induced acute gastric mucosal lesion. Intravenous administration of this enzyme derivative normalized both systemic circulation and vascular permeability of the gastric mucosa and prevented the occurrence of stress-induced gastric injury. These findings suggest that the superoxide radical and/or its metabolite(s) plays an important role in the pathogenesis of stress-induced acute gastric mucosal lesion.

Flow cytometric DNA analysis of thyroid carcinoma.

Abnormal DNA content has been considered as an additional criterion for determining the biological behavior of a tumor. Flowcytometric DNA analysis was done on 121 patients with thyroid carcinoma encountered during the period between 1975 and 1987. Tumor tissues were sampled from paraffin-embedded blocks and the histology of thyroid carcinoma found to consist of 91 papillary, 23 follicular, 2 medullary, 1 squamous cell and 4 anaplastic carcinomas. The incidence of aneuploidy in thyroid carcinoma was 7.4 per cent (9 patients) while that of diploidy was 92.6 per cent (112 patients). The aneuploid specimens consisted of 6 papillary, 1 follicular, 1 medullary and 1 anaplastic carcinomas and, of 4 anaplastic carcinoma patients with subsequent death within 6 months, only 1 was aneuploid. As an indicator of proliferative potential, S-phase fraction (SPF) was also determined by flow cytometry, but this could not be used as an independent prognostic factor. The aneuploid patients showed a significantly decreased survival rate (p less than 0.01). Thus, although DNA measurement proved useful for predicting the survival of aneuploid patients, there is some discrepancy between DNA content and the biological behavior of the tumor.

Identification of HTLV-I sequence in cord blood mononuclear cells of neonates born to HTLV-I antigen/antibody-positive mothers by polymerase chain reaction.

We developed a polymerase chain reaction (PCR) method which has high sensitivity and simple technique in order to investigate the presence or absence of human T lymphotropic virus type I (HTLV-I) provirus in cord blood mononuclear cells of neonates born to HTLV-I carrier mothers. Out of 40, three subjects were found to contain the HTLV-I provirus genome. These three subjects remained HTLV-I sequence-positive in follow-up study. On the other hand, when examined by a conventional technique for detection of HTLV-I-associated antigen on peripheral mononuclear cells, all 40 neonates were HTLV-I-associated antigen-negative. These results suggest that PCR is more sensitive than the conventional antigen detection method and is useful in early detection of HTLV-I infection in neonates born to HTLV-I carriers.

Incontinentia pigmenti in identical twins with separate skin and neurological disorders.

Incontinentia pigmenti in female identical twins is reported. The first baby showed the typical pigmentation of incontinentia pigmenti, while the second baby had hydrocephalus (colpocephaly) without pigmentation. They were identical, with a rate of 99.9% in 18 blood-type studies. Virus was not detected and cytogenetic studies proved normal. Both showed peripheral eosinophilia. The individual expressions of Incontinentia pigmenti in these identical twins were separated into cutaneous lesions and lesion of the central nervous system (intra-uterine hydrocephalus). Cutaneous lesions developed after birth. Twins with Incontinentia pigmenti are extremely rare and in this family showed different expressions of this disease in space and time.

[Regulation of coagulo-fibrinolytic activity and lupus anticoagulants in systemic lupus erythematosus].

We determined the following coagulo-fibrinolytic activities in 24 patients with systemic lupus erythematosus (SLE) and 20 healthy adults: prothrombin time (PT), activated partial thromboplastin time (A-PTT), factor VIII: coagulant activity), von Willebrand factor antigen (vWF: Ag), antithrombin-III (AT-III), plasminogen (PLG), alpha 2 plasmin inhibitor (alpha 2 PI), alpha 2-plasmin inhibitor-plasmin complex (PIC), protein C (PC: activity and antigen concentration), and protein S (PS: total PS and free PS). PLG, AT-III, PC antigen concentration and total PS were significantly decreased in ten female controls as compared with ten male controls. Therefore, we used the ten healthy females as controls and excluded two male SLE patients in the analysis of the correlations of coagulo-fibrinolytic activities with lupus anticoagulant (LA), clinical and laboratory features in 22 female patients with SLE. In the SLE patients, PT was significantly shortened, while A-PTT was prolonged. PLG, PC activity and antigen, and total PS were significantly increased, and free PS levels were decreased in SLE. The shortened PT and decreased free PS suggest hypercoagulable states in SLE patients. A significant prolongation of A-PTT and a decrease of F VIII activity were observed in the six LA-positive SLE patients, and the results were considered as known effects of LA. Furthermore, vWF: Ag, AT-III and PC antigen levels were significantly increased in the LA-positive patients as compared with LA-negative patients. These changes indicate both vascular endothelial cell damages and a compensatory increase in coagulation inhibitors in the LA-positive patients.

Quantitative chemical analysis of nickel-chromium dental casting alloys.

Twenty-nine brands of dental casting nickel-chromium alloys made in Japan for small castings were analyzed by electron probe X-ray microanalyzer. Nickel-chromium alloys for metal-ceramic application were composed primarily of nickel, chromium, and molybdenum with the exception of one brand. Of the nickel-chromium alloys for inlay, crown, and bridgework applications, 11 of the 22 alloys were up to the standard of the Ministry of Welfare specifications. And additive metal elements of these alloys were molybdenum, iron, copper, manganese, aluminum, silicon, tin, indium, silver, titanium, and gallium.

Inhibition of corneal inflammation by an acylated superoxide dismutase derivative.

Superoxide radicals and their metabolite(s) have been postulated to play an important role in the pathogenesis of inflammation. Hence, superoxide dismutase (SOD) has been used to reduce tissue injury caused by reactive oxygens. However, protection of the cornea and other ocular tissues from oxygen toxicity could not be achieved by administering SOD presumably due to its unfavorable in vivo behavior. To scavenge superoxide radicals on the outer surface of corneal epithelial cells, the authors synthesized an acylated SOD derivative (AC-SOD) by linking capric acid. When instilled into rabbit eyes, a significant amount of AC-SOD remained bound to the corneal surface for a fairly long time. Intracorneal injection of lipopolysaccharide (LPS) triggered infiltration of polymorphonuclear leukocytes (PMNs) to the cornea and induced severe keratitis. Topical administration of AC-SOD to the LPS-treated cornea markedly inhibited the infiltration of PMNs and suppressed the occurrence of keratitis. Under identical conditions, topically administered SOD was rapidly removed by tears and, hence, did not inhibit LPS-induced keratitis. When the number of PMNs in the systemic circulation was reduced by intravenous administration of hydroxyurea, LPS-induced keratitis was inhibited markedly. These results indicate that superoxide radicals and circulating PMNs might play a critical role in LPS-induced keratitis.

p24 protein of a human T-lymphotropic virus type-I (HTLV-I) producing cell line (TCL-Kan) reacts with some normal pregnant women's sera.

Some HTLV-1 antibody carrying pregnant women, who are rated by the gelatin particle agglutination (PA) method, show no expression of HTLV-I antigen in their peripheral blood lymphocytes. Paying attention to this fact, the authors recently studied the reaction of the antibody contained in the PA-positive sera with the antigen protein used for the PA method. This study demonstrated that the protein responsible for non-specific reaction has a molecular weight of 24,000 and that the isoelectric point of this protein differs from that of HTLV-I specific protein.

[Experimental study on chronic entrapment neuropathy].

This study was undertaken to clarify the pathology and pathophysiology of chronic entrapment neuropathy. Ten mm long Silastic tubes with internal diameter of 1.5 mm were placed on the right sciatic nerves of adult Wistar rats. Morphological and functional changes were studied from one to 14 months after tubing. The earliest changes consisted of thickening of the epineurium and perineurium, followed by breakage of nerve-blood barrier. Intraneural edema and segmental demyelination were evident at the central and peripheral areas of funiculi from 4 months after tubing. At 14 months after treatment, intraneural fibrosis was recognized. Compound action potentials and motor nerve conduction velocity were decreased in 4 and 8 month specimens respectively. Tadpole deformity of the myelin sheath was observed at 4 months and later. The fast axonal transport declined at 8 months after tubing. Our results were compatible with clinical and pathological findings of human materials. This model was thought to be useful one to elicit the pathology and pathophysiology of clinical entrapment neuropathy.

[A primary malignant lymphoma of the appendix].

A malignant lymphoma (ML) of the gastrointestinal tract occurs most frequently in the stomach, and ML of the appendix is very rare, with only 16 cases reported in the Japanese literature. Reported is a case of a primary ML the appendix in a 48 year old male, in whom an ileocecal resection was carried out. This case was pathohistologically diagnosed as a diffuse large lymphoma of the B cell type, according to the LSG classification. Therapeutic problems were encountered because of the difficulty in making an early diagnosis.

[Prognostic significance of flow cytometric DNA analysis in colorectal cancer].

Significance of flow cytometric DNA analysis for assessing malignant potential and survival of colorectal cancer was investigated using paraffin-embedded materials from 144 patients with primary colorectal cancer who had been treated from 1971 to 1985. Forty-four percent of colorectal cancer were composed of diploid and 56 percent were aneuploid. DNA indices (DI) of aneuploid tumors showed a bimodal distribution. There was no significant correlation between ploidy pattern and clinicopathological factors. While, DI level showed significantly higher in poorly differentiated adenocarcinomas and in clinicopathological stage III and V tumors. Overall survival in the patients with aneuploid tumor was significantly worse than that in those with diploid tumor (p less than 0.001). Survival rate was poorer in the patients with aneuploid tumor than in those with diploid tumor, who were stratified according to categories of curable resection, stage, histological type, negative peritoneal or hepatic involvement and negative node metastases. However, there was no significant relation between DI and survival among the patients with aneuploid tumor. From these results, it was concluded that the nuclear DNA content of colorectal cancer may represent biological malignant potential of the disease, and that the DNA ploidy pattern may be an important prognostic indicator, being independent of clinicopathological factors.

[Detection of HTLV-I genome in infants born to HTLV-I seropositive mothers by polymerase chain reaction].

We developed a new method to detect human T cell leukemia virus type I (HTLV-I) provirus by amplifying the pX region of the HTLV-I genome by means of the polymerase chain reaction (PCR) method. This new method was used to examine babies delivered by HTLV-I carrier women for HTLV-I vertical transmission. It was found that intra-uterine infection with HTLV-I can occur, though rarely, and some babies carry HTLV-I provirus despite their negative response to HTLV-I antibody, and, noticeably, some of the babies found negative by the conventional antigen detection method may be positive when tested by the PCR method. It was also confirmed with the PCR method that the incidence of vertical transmission of HTLV-I can be reduced to about 1/5 by replacing breast feeding with bottle feeding and frozen mother's milk feeding. It was also found that vertical transmission of HTLV-I can occur at high incidence rates even when the duration of breast feeding is 3 months or less.

[Bacteriological and clinical studies on norfloxacin in the field of pediatrics].

Bacteriological and clinical studies on norfloxacin (NFLX) were carried out in the field of pediatrics, and the results obtained are summarized as follows: 1. The MICs of NFLX against clinically isolated organisms were determined to compare with MICs of nalidixic acid, amoxicillin, cefaclor, erythromycin and fosfomycin. NFLX had low MICs against both Gram-positive and Gram-negative bacteria and, particularly, showed higher antimicrobial activity to Escherichia coli, Salmonella sp., Klebsiella pneumoniae, Pseudomonas aeruginosa, Haemophilus influenzae and Campylobacter jejuni than any other drugs tested. 2. Seventeen pediatric patients (11 cases of enteritis, 4 cases of urinary tract infections, 1 case of tonsillitis and 1 case of pyoderma) were treated orally with NFLX in daily doses ranging from 3.1 to 16.7 mg/kg t.i.d. for 6-14 days. Rates of clinical effectiveness and bacterial elimination were 94.1% and 58.8%, respectively. 3. None of the treated children showed clinical symptoms indicating the occurrence of side effects nor abnormal laboratory findings except 1 patient with eosinophilia. These results indicate the usefulness of NFLX in the treatment of bacterial infections in the pediatric field.

Cross-linking of lipocortin I and enhancement of its Ca2+ sensitivity by tissue transglutaminase.

The stimulation of human epidermoid carcinoma A431 cells with the calcium ionophore A23187 resulted in the formation of high-molecular-weight lipocortins I, having apparent molecular weights of 75 kDa and 160 kDa as detected with specific anti-lipocortin I antibody. These immunoreactive proteins were identified to be covalently cross-linked multimers of lipocortin I, since essentially the same cross-linked multimers were observed when purified lipocortin I was incubated with tissue transglutaminase (TGase) in vitro. Classical amine substrates for TGase, such as dansylcadaverine and putrescine, were also incorporated stoichiometrically into lipocortin I. Cross-linking or amine incorporation was not observed with lipocortin II. Des 1-26 lipocortin I did not serve as a substrate for TGase, indicating that the N-terminal region of lipocortin I plays an important role in the formation of lipocortin I multimers. The cross-linking of lipocortin I by TGase resulted in a remarkable enhancement of calcium sensitivity for phospholipid binding; i.e., the free calcium concentration required for the cross-linked lipocortin I to attain 50% maximal binding to phosphatidylserine vesicles was as little as 3 microM, while that required for intact monomeric lipocortin I was 20 microM.
