[Transmembrane controls in cultured human thyroid carcinoma].

It is well known that many of thyroid carcinoma are capable of responding to TSH, but our studies shown that there are some alteration in this responsiveness. The adenylate cyclase responsiveness to TSH was usually greater in thyroid carcinoma than in adjacent histologically normal thyroid tissue. The level of increased response of adenylate cyclase were correlated with the level of enhanced expression of ras oncogene product p21 assessed by Western blotting analysis. The TSH induced desensitization of adenylate cyclase was not observed in some differentiated carcinoma. This loss of desensitization may be reflect the change in ADP-ribosylable Gi protein. In the differentiated carcinoma, the capacity of EGF receptor was higher than that in normal thyroid. The EGF binding to cultured carcinoma cells did not increase in response to TSH. These altered properties of transmembrane control in human thyroid carcinoma may be related to the neoplastic growth.

[Rapid bone marrow dissemination of gastrointestinal lymphomas after surgical resection].

We describe two cases of gastrointestinal lymphoma associated with rapid bone marrow dissemination after surgical resection. Case 1: A 73-year-old male was diagnosed as having malignant lymphoma originating from ileocaecal region (diffuse medium-sized, B cell type). Tumor (8 x 8 cm) was resected but infiltrated to the peritoneum and curative operation could not be done. Two weeks after operation, elevation of LDH, pancytopenia and bone marrow infiltration of lymphoma cells developed and he died of respiratory failure. Case 2: A 69-year-old female was diagnosed as having remnant gastric lymphoma (diffuse large, B cell type). Tumor size was 5 x 4 cm and swelling of the third lymph nodes was found, so curative operation could not be done. Two months after operation bone marrow infiltration of lymphoma cells was observed and she is now undergoing chemotherapy. Surgical resection is performed in the majority of patients with localized gastrointestinal lymphoma. But the operation of the advanced case must be carefully done, because the operative procedure may sometimes facilitate growth and metastasis of tumor.

[Outburst of fusidic acid resistant Staphylococcus aureus].

Fusidic acid (FA) resistance (MIC greater than or equal to 12.5) was found in only five of 123 strains of S. aureus isolated during the period from January, 1987, to March, 1988; the FA resistance rate soared up to 42 of 81 strains isolated during the period from April, 1988, to October, 1988. In contrast, all strains of S. aureus isolated in Kochi prefecture during the period from September, 1987, to September, 1988, were susceptible to FA. Our data clearly demonstrate an explosive increase of FA resistant S. aureus in Japan. However, regional differences currently exist in the emergence rate of FA resistant S. aureus.

[Cis-dichlorodiammineplatinum in osteosarcoma. Osteosarcoma Cooperative Study Group report].

Thirty-two patients with primary osteosarcoma and 18 patients with advanced osteosarcoma were treated by iv or ia infusion of cisplatinum at a dose of 100 mg/m2 every three weeks. The efficacy of the agent for primary osteosarcoma was mainly estimated by X-ray findings and histologic examination. One patient had a partial response, and 7 patients had a minor response. Pathologic evaluation of the extent of the primary tumor necrosis was performed on 27 resected specimens. Eight of 27 cases showed a good response (Ayala III A less than). One of 18 patients with advanced osteosarcoma had a partial response, and 2 a minor response. Nausea and vomiting (88%), liver dysfunction (42%), leukopenia (36%), nephrotoxicity (20%) and auditory disturbance (20%) were the main side effects in 50 patients. However, side effects of cisplatinum were generally reversible. The results suggest that cisplatinum is effective against osteosarcoma and may enhance the therapeutic results in osteosarcoma.

[Clinical evaluation of various tumor markers in pleural effusion and in the serum].

Various tumor markers and enzymes in the pleural effusion and serum have been measured in 47 patients with carcinoma and in 43 patients with benign disease, by means of a radioimmunoassay and biochemical methods. CEA in the pleural effusion showed a high specificity and sensitivity for the detection of the malignancy. In patients with a lung cancer, measurement of the NSE in the serum was more useful than in the pleural effusion. Further, both CA19-9 and SCC in the pleural effusion showed a high specificity in a differential diagnosis of cancer and benign diseases. On the other hand, CA 125, TPA and IAP in the pleural effusion and in the serum showed a high sensitivity, but a low specificity for diagnosing the malignancy. The levels of ADA were significantly higher in tuberculous pleural effusions than in carcinomatous effusions. Therefore, this suggests that measurement of the various tumor markers in both the pleural effusion and in the serum is useful in achieving a differential diagnosis of malignant and benign diseases.

[Therapeutic effect of interferon-alpha on hairy cell leukemia].

Hairy cell leukemia (HCL) is characterized by slow proliferation of special mononuclear cells. Although splenectomy has been evaluated to be the most effective traditional treatment for HCL, the response of patients to splenectomy is not necessarily satisfactory. Recently, the treatment of HCL with Interferon-alpha has been reported to be effective, and we report a female case of HCL who showed marked improvement in her hematological findings with this therapy.

[Evaluation of the effect of treatment in patients with liver tumors using 18F-fluorodeoxyglucose PET].

To evaluate the glucose metabolism in the liver tumor, 18FDG PET (Positron Emission Tomography) was performed in 9 patients with primary and metastatic liver tumors. And usefulness of 18FDG PET in clarifying the effect of treatment on liver tumor was examined. Glucose metabolism of tumor was showed in terms of the coefficient 'C' calculated by Patlak's plot. Each tumor's coefficient 'C' was 0.016-0.054 and it decreased after treatment. Compared with pathological findings, the coefficient 'C' of necrosis part of tumor was 0.004 and fibrous change after irradiation was 0.007. It suggested that coefficient 'C' reflected activity of tumor and it was a useful index to know viability of tumors in the treatment for patients with liver tumors.

[Axilla-iliac bypass in a patient with aortic obstruction progressed from atypical coarctaion].

A case of a 13 year-old girl with abdominal aortic obstruction is presented. At the age of nine years she underwent an evaluation for a continuous murmur heard over the left costal margin. The blood pressure was 210/125 mmHg in the ascending aorta, and an aortography revealed a critical stenosis in the descending aorta proximal to the origin of the celiac artery. No surgical treatment was recommended at this time. At the age of 13 years she developed severe headache and was admitted for the second time. Upon catheterization the ascending aortic pressure was 242/150 mmHg, and an aortography revealed total obstruction of the abdominal aorta in the segment proximal to the celiac artery. An extraanatomical bypass was constructed between the right axillary artery and the right common iliac iliac artery with a 10 mm dacron graft. Two weeks after this procedure, the blood systolic pressure was 110 mmHg in the upper extremities. It has been 12 months since the patient had this procedure, and she continue to do well, i.e., she has no particular limitations of the hip joint movement, has no headache, and remains to be normotensive.

[The sedative effects of a low dose of butorphanol combined with diazepam during spinal-epidural anesthesia (H-type anesthesia)].

The sedative effects of the low dose of butorphanol combined with diazepam during spinal-epidural (H-type) anesthesia were studied in 20 urological or gynecological patients of ASA class I or II. Patients were premedicated with 0.5mg of atropine sulfate and 50mg of hydroxyzine hydrochloride 1 hour before the operation. Spinal anesthesia was performed with 2.4 ml of 0.3% dibucaine or 2.5 ml of 3% lidocaine and if necessary 1-2% mepivacaine was administered via the catheter for maintenance of the epidural anesthesia. Butorphanol 0.5 mg and diazepam 5 mg were administered intravenously after the establishment of spinal anesthesia. We observed the condition of sedative effects, the changes in blood pressure, heart rate and respiratory rate, before the administration of butorphanol, 2 and 5 minutes after the administration of the drug and then every 5 minutes until the recovery was noticed. We found that the sedative effects of butorphanol combined with diazepam were clinically sufficient. Patients were sleeping for 40 minutes and then drowsy for 30 minutes with sufficient sedation. There was no complication except slight respiratory suppression. Our results suggest that the butorphanol combined with diazepam is a useful method for the sedation during spinal-epidural anesthesia.

Induction of follicle stimulating hormone receptor by erythroid differentiation factor on rat granulosa cell.

Erythroid differentiation factor (EDF), inhibin beta A-homodimer, induced expression of follicle stimulating hormone receptors on rat granulosa cells prepared from diethylstilbestrol primed immature female rats. After 3 day incubation with EDF, the number of FSH receptors on the granulosa cells was increased to about 3.5 times of the control value in a dose dependent manner with an ED50 value of 61 ng/ml. On the other hand, EDF related peptides, i.e., bovine 32K Da inhibin A and TGF beta, had no effect on the FSH receptor induction. The present observation suggests that EDF may play a role in the initiation of the cytodifferentiation of ovarian granulosa cells.

Calcitonin gene-related peptide(CGRP) stimulates the release of atrial natriuretic peptide(ANP) from isolated rat atria.

The effect of calcitonin gene-related peptide(CGRP) on the release of atrial natriuretic peptide(ANP) was studied in spontaneously beating, isolated rat atria. CGRP stimulated the ANP release in a dose-dependent manner. When the atria were incubated with a combination of phentolamine, propranolol, and atropine, these antagonists blocked neither the rise in ANP release nor the positive chronotropic and inotropic effects of CGRP. Therefore, we conclude that CGRP stimulates ANP release as well as cardiac contractility independently of adrenergic and cholinergic receptors.

[Biochemical diagnosis of pheochromocytoma by determining normetanephrine and metanephrine concentrations in single voided urine].

Diagnosis of pheochromocytoma has been made by the determination of urinary noradrenaline and adrenaline excretion for 24 hours. The assay procedure and the collection of urine for 24 hrs. are intricate. In the present study, we have ascertained the clinical significance of urinary normetanephrine (NM) and metanephrine (M), chemically stable metabolites of catecholamines, in single voided urine for a diagnosis of pheochromocytoma. Urine and plasma samples were collected from 361 normal subjects, 59 patients with essential hypertension, 22 patients with chronic renal failure and 22 patients with pheochromocytoma. Urinary NM and M concentrations were determined by radioimmunoassay with prior hydrolysis by acidification with 1N HCl. Plasma NM and M concentrations in normal subjects were 71.8 +/- 30.7 pg/ml and 41.5 +/- 8.61 pg/ml, respectively. Plasma NM was increased in 8 and plasma M was increased in 20 of 21 patients with pheochromocytoma, although many of these overlapped with those patients with chronic renal failure (NM, 285.9 +/- 175.1 pg/ml; M, 206.3 +/- 186.7 pg/ml) and essential hypertension (NM, 107.7 +/- 90.7 pg/ml; M, 46.7 +/- 20.2 pg/ml). Urinary NM and M concentrations did not show specific diurnal variation and there was significant correlations between the values in single voided urine and those in the 24 hour urine. Urinary NM and M concentrations in normal controls were 197.5 +/- 46.7 ng/mg Cr. and 125.3 +/- 37.1 ng/mg Cr., respectively. Urinary NM concentration was increased in 14 and urinary M concentration was increased in all of 17 patients with pheochromocytoma. In addition, urinary M concentration was higher in most of the 17 patients with pheochromocytoma than that in the patients with chronic renal failure and essential hypertension. However, the values in three patients with Sipple's syndrome with a small adrenal tumor or recurrent cases overlapped with those in other diseases. Relationships between urinary concentrations of NM and/or M and tumor size showed positive correlations. Urinary NM and M concentrations showed significant decreases after surgical removal of the tumors. These results suggest that NM and/or M concentrations in single voided urine could be a sensitive and specific diagnostic tool for pheochromocytoma.
