Analysis of anchor residues in a naturally processed HLA-DR53 ligand.

The peptide motif of the HLA-DR53 (DRB4(*)0101) molecule, which is associated with autoimmune diseases including Vogt-Koyanagi-Harada's syndrome, was determined by peptide binding assay using human L plastin p581 - 595 peptide and its substituted analogues. L plastin p581 - 595 peptide is one of the naturally processed peptides bound to HLA-DR9/DR53 (DRB1(*)0901/DRB4(*)0101) molecules. The binding affinity of each peptide to the HLA-DR53 molecule was measured by fluorescence intensity of biotinylated peptides to L cell transfectants expressing HLA-DR53 molecules, followed by treatment with avidin-fluorescence. Binding of biotinylated peptides to HLA-DR53 molecules was not inhibited by all single-alanine-substituted nonbiotinylated peptides, indicating that the replaced position was important for binding to the HLA-DR53 moleule. The inhibitory motif is considered to be an HLA-DR53-specific binding motif, composed of a positively charged residue (K) at position 1, a hydrophobic residue (I) at position 4, positively charged residue (R or K) at position 8 or 9, and another hydrophobic residue (I) at position 10. This predicted motif is different from the binding motifs of other HLA-DR molecules.

Detection of patients with sick sinus syndrome by use of low amplitude potentials early in filtered P wave.

OBJECTIVES: This study sought to determine whether patients with sick sinus syndrome could be detected by analyzing the initial portion of the signal-averaged P wave corresponding to the electrical activity of the perinodal atrial myocardial cells. BACKGROUND: In sick sinus syndrome, pathophysiologic abnormalities have been shown not only in the sinus node, but also in the atrial muscle, especially the perinodal portion. METHODS: The study included 41 patients with sick sinus syndrome and 140 age-matched control subjects. Eighteen of 41 patients with sick sinus syndrome had paroxysmal atrial fibrillation. Signal-averaged P wave electrocardiograms (ECGs) were recorded through a bandpass filter of 40 to 300 Hz with a P wave-triggering technique. Signals of the orthogonal bipolar leads were combined into a spatial magnitude. The root mean square voltage for the initial 30 ms (EP30) and the duration of initial low amplitude signals < 4 microV (ED4) of the filtered P wave were measured. The root mean square voltage for the last 20 ms (LP20) and the duration of the filtered P wave were also measured. RESULTS: EP30 was significantly lower and ED4 was significantly longer in patients with sick sinus syndrome than in the control subjects (EP30 [mean +/- SD]: 2.18 +/- 0.90 vs. 3.94 +/- 1.45 microV, p < 0.0001; ED4: 31.7 +/- 14.5 vs. 14.0 +/- 7.4 ms, p < 0.0001), although there was no significant difference in LP20 between patients with sick sinus syndrome without paroxysmal atrial fibrillation and the control subjects. The duration of the filtered P wave was significantly but minimally longer in patients with sick sinus syndrome than in the control subjects (139.8 +/- 18.8 vs. 127.3 +/- 13.6 ms, p < 0.0001). The criteria of EP30 < 3.0 microV and ED4 > 22 ms as atrial early potential gave a sensitivity of 76%, a specificity of 91%, a positive predictive value of 74% and a negative predictive value of 93% for identification of patients with sick sinus syndrome. CONCLUSIONS: These results suggest that the long, low amplitude signals early in the filtered P wave on the signal-averaged ECGs are characteristic of sick sinus syndrome. Thus, the atrial early potential could be a useful marker to identify patients with sick sinus syndrome.

Changes in calcium transient and left ventricular function during positive inotropic stimulation and myocardial ischemia in indo-1-loaded beating guinea pig heart.

To elucidate the issues such as excitation-contraction coupling and myocardial ischemia, it is necessary to measure intracellular free Ca2+ concentration and mechanical function of hearts perfused via the normal arterial circulation. For this purpose, we simultaneously measured Ca(2+)-dependent indo-1 fluorescence and left ventricular (LV) pressure on a beat-to-beat basis in Langendorff guinea-pig hearts, and investigated the changes in Ca2+ transient and LV function during inotropic stimulation and myocardial ischemia. The indo-1 fluoresence ratio and LV developed pressure increased the perfusate [Ca2+] increased from 1.6 to 3.2 mmol/L, and there was a good correlation between Ca2+ transient and LV contractility. Digoxin (10(-6) mol/L) and milrinone (10(-5) mol/L) increased LV contractility with a concomitant increase in Ca2+ transient, and the relative increase of Ca2+ transient produced by milrinone was much more than that by digoxin. The reduction of coronary perfusion pressure from 80 to 40 mm Hg decreased LV contractility with an increase in indo-1 fluorescence ratio. These results suggest that Ca2+ responsiveness of contractile apparatus declines during inotropic stimulation by milrinone and during myocardial ischemia. Thus, this experimental technique is useful to investigate the interrelation of Ca2- regulation and LV function during a variety of pharmacological and physiologic perturbations.

Therapeutic efficacy of an antineutrophil monoclonal antibody, Urge-8, against acute necrotizing pancreatitis in rats.

BACKGROUND: Although they have critical roles in the defense mechanism against invading microorganisms, neutrophils may facilitate exacerbation of critical conditions associated with acute necrotizing pancreatitis by a discharge of granule contents into the organ tissues. Because of this constitution of neutrophils, the therapeutic efficacy of Urge-8, a mouse monoclonal antibody to neutrophils, on the survival of rats with experimentally induced acute necrotizing pancreatitis was investigated in vivo. METHODS: Acute pancreatitis was induced by injection of trypsin mixed with sodium chenodeoxycholic acid into pancreatic ducts. Urge-8 was infused intravenously 30 minutes after pancreatitis was induced, a series of vital signs was taken, and plasma amylase level was estimated. RESULTS: Hemorrhagic necrosis of the pancreas and intraabdominal bleeding were observed 1 hour after the pancreatitis-inducing drugs were injected, and death occurred at 240.9 +/- 24.6 minutes (mean +/- SD)in the control group. With Urge-8 administration, however, the survival time was significantly prolonged to 395.2 +/- 64.4 minutes (p = 6.0 x 10-(10) versus control). Failure in the vital signs (mean arterial pressure, heart rate, and body temperature) was significantly ameliorated by injection of Urge-8. The plasma amylase level was elevated after pancreatitis was induced and peaked at 3 hours (4915 +/- 1966 IU/L in mean +/- SD). This level was suppressed during the first 3 hours by injection of Urge-8 (3372 +/- 1223 IU/L); however, the amylase level increased thereafter, becoming comparable with the peak in the control group, and then death occurred. Arterial blood gas and plasma electrolyte analyses showed that pH, base excess, and plasma potassium levels in the group treated with monoclonal antibody were significantly improved. CONCLUSIONS: It was suggested that neutrophils play some critical role in the exacerbation of acute necrotizing pancreatitis and its related symptoms. Although not capable of preventing death in our model, treatment with the antineutrophil monoclonal antibody Urge-8 after the onset of acute pancreatitis prolonged the survival time significantly.

P-glycoprotein-mediated acquired multidrug resistance of human lung cancer cells in vivo.

We examined whether the increased expression of P-glycoprotein (P-gp) encoded by the human multidrug resistance gene MDR1 is related to the acquired multidrug resistance of lung cancer in vivo. We estimated the chemosensitivity of lung cancer xenografts (LC-6, adenocarcinoma; Lu-24, small-cell cancer) by calculation of relative tumour growth (T/C%, treated/control) in vivo, based on statistical significance determined by the Mann-Whitney U test (P < 0.01, one-sided). MDR1 gene expression levels were evaluated by reverse transcription-polymerase chain reaction (RT-PCR) assay. P-gp production and P-gp localisation were examined by Western blotting and by immunohistochemical analysis respectively. LC-6 and Lu-24 were initially sensitive to both vincristine (VCR, 1.6 mg kg-1: LC-6, 45%; Lu-24, 39%) and doxorubicin (DOX, 12 mg kg-1: LC-6, 26%; Lu-24, 27%) in vivo. VCR-resistant variants (LC-6R, 66% and Lu-24R, 68%) selected with VCR (0.4 mg kg-1, x 9) significantly acquired cross-resistance to DOX (LC-6R, 55% and Lu-24R, 55% respectively). RT-PCR assay showed increased levels of MDR1 expression in LC-6R and Lu-24R with stable MDR1 expression levels. P-gp expression levels were elevated, and the percentage of P-gp-positive tumour cells increased in both LC-6R and Lu-24R. These results suggest that P-gp/MDR1 overexpression is related to acquired multidrug resistance in lung cancer in vivo.

Vasodilation induced by vasopressin V2 receptor stimulation in afferent arterioles.

We have previously reported that vasopressin (AVP) V2 receptor stimulation increased renal blood flow in dogs anesthetized with pentobarbital. In this study, we examined the direct effects of AVP on afferent arterioles to clarify the role played by V2 receptors in regulating afferent arteriolar tone. We microdissected a superficial afferent arteriole with glomerulus from the kidney of a New Zealand White rabbit. Each afferent arteriole was cannulated with a pipette system and microperfused in vitro at 60 mm Hg. The effects of vasoactive substances were evaluated by changes in the lumen diameter of afferent arterioles. We found that AVP decreased the lumen diameter of microperfused afferent arterioles dose-dependently and that a V1 antagonist, OPC21268, inhibited the vasoconstrictor action of AVP. However, AVP 10(-8) M increased the lumen diameter of norepinephrine (NE)-constricted afferent arterioles pretreated with OPC21268 (OPC + NE, 8.2 +/- 0.7 microns; OPC + NE + AVP, 9.9 +/- 0.9 microns*; *P < 0.05, N = 13). This vasodilatory effect of AVP was abolished by pretreatment with a V2 antagonist, OPC31260. Desmopressin (dDAVP), a V2 agonist, increased the lumen diameter of the NE-constricted afferent arterioles (NE, 7.4 +/- 0.9 microns; NE + dDAVP, 10.1 +/- 0.7 microns*; *P < 0.05, N = 9). These results suggest that AVP V2 receptors are present in rabbit afferent arterioles and that V2 receptor stimulation induces vasodilation in rabbit afferent arterioles.

Microwave coagulation therapy for hepatocellular carcinoma.

BACKGROUND & AIMS: Surgical resection is not always feasible in patients with hepatocellular carcinoma. Microwave coagulation therapy has been used as an alternative to resection, and its efficacy has been evaluated. METHODS: Nineteen patients with unresectable hepatocellular carcinoma underwent microwave coagulation therapy through laparotomy (n = 12), laparoscopy (n = 5), or thoracotomy (n = 2) because of advanced liver cirrhosis and/or intrahepatic metastases. One nodule was treated in 13 patients, tumor size ranged from 5 to 90 mm. Patient outcomes were studied. RESULTS: Microwave coagulation therapy created a reproducible regional necrosis. Fourteen patients underwent potentially curative treatment; the remaining 5 patients underwent palliative treatment (n = 4) or incomplete tumor coagulation (n = 1). Of the 31 nodules treated, 28 underwent complete tumor ablation. Only 2 patients undergoing laparoscopic microwave coagulation therapy developed local recurrence. The coagulated area subsequently shrank. Patients showed rapid recovery without hepatic dysfunction. Thirteen patients, including 2 long-term survivors, are alive either without tumor (n = 10; 14-64 months) or with tumor (n = 3; 17-22 months). Six patients died of hepatocellular carcinoma (n = 4) or liver insufficiency (n = 2). CONCLUSIONS: This preliminary study suggests the efficacy of microwave coagulation therapy, including safety and potential curability, in patients with hepatocellular carcinoma with advanced liver cirrhosis and multifocal or central tumors.

G2 arrest following fractionated irradiation in the mouse jejunal crypt.

A G2 block is a well-known response to irradiation, showing a delay time that depends on the given single dose. Our question was whether the division delay time is the same after multiple doses as after a single dose. We therefore studied mitosis in the mouse jejunal crypt using a quantitative approach. Three radiation schedules with a fraction size of 2 Gy were compared: a single dose and twice-daily irradiation at a 4-h interval, for 1 or 3 days. With the single dose, mitotic figures disappeared at 0.5 h and reappeared after 2 h. At the end of each multiple-dose regimen, significant numbers of mitoses could be observed at 0.5 and 1 h but a trough was seen at 2 h, with numbers then increasing again. Calculated average division delays after the single dose and repeated doses for 1 and 3 days were 1.5, 1.3 and 1.7 h respectively. In summary, most crypt cells treated with multiple fractions remain susceptible to G2 arrest, showing the same delay time as with the single dose.

Preceding PTGBD decreases complications of laparoscopic cholecystectomy for patients with acute suppurative cholecystitis.

Laparoscopic cholecystectomy (LC) has become one of the options for the treatment of acute cholecystitis as surgeons gain facility with this procedure. However, acute suppurative cholecystitis is still a severe condition, because a high mortality rate still exists. In the early years (1991 to 1992), 4 patients were operated on without a preceding percutaneous transhepatic gallbladder drainage (PTGBD) at our hospital, however, one patient died of septic shock after a laparoscopic cholecystectomy. Conversion to open surgery was performed on two patients. However, in later years (1992 to 1995), 14 patients were operated on with preceding PTGBDs safely. Here, we report the safeness and significance of the combination therapy of PTGBD and LC for patients with severe acute suppurative cholecystitis.

Sympathetic overactivity of intraocular muscles evaluated by accommodation in patients with hyperthyroidism.

Sympathetic overactivity occurs in Graves' disease, but little is known about autonomic nervous function in the eyes of subjects with this disease. We examined this function of the intraocular muscles in 12 patients with hyperthyroid Graves' disease and 12 healthy controls. Pupil size, pupillary unrest, and accommodation were measured with a computer-assisted infrared optometer and pupillometer. The mean and the coefficient of variation of the areas of the pupils were used to express pupil size and the degree of pupillary unrest, respectively. Accommodation was measured with the target light beam moving slowly and steadily, or instantaneously, and the results are expressed as the change in the eye's refractive power in response to these movements. The mean pupil size of the patients was not different from that of the controls. Pupillary unrest in the patients was smaller than in the controls. Accommodation in the patients was lower than that of the controls. Five patients were examined again 3 months later when they became euthyroid; pupillary unrest and accommodation had improved in all five patients. There were no significant differences in the activity of sympathetic nerves governing intraocular muscles in patients with or without eyelid retraction. These results indicate that intraocular muscles are sympathetically overactive in patients with hyperthyroidism, and suggest that eyelid retraction is not caused by sympathetic overactivity alone, but by another factor or factors, in addition.

Identification of five embryonic hemoglobins of rat and ontogeny of their constituent globins during fetal development.

Hemoglobins of rats switch from an embryonic to an adult type during fetal development. However, very little is known about the structures and molecular species of hemoglobins occurring in the fetal life of rats. In the present study we isolated five embryonic hemoglobins, designated E1, E2, E3, E4, and E5, from the blood of rat fetuses on day 14 of gestation by ion exchange chromatography. Reverse-phase high performance liquid chromatography revealed that these hemoglobins each consist of two kinds of globins: E1(11 alpha:epsilon 1), E2(1 alpha: epsilon 1), E3(zetta: epsilon 1), E4(1 alpha: epsilon 3), and E5(zetta: epsilon 3), respectively. The complete amino acid sequences of the zetta, epsilon 1, and epsilon 3 globins were determined. The zetta globin showed characteristic features common in alpha-type embryonic globins of known species in that the N-terminus is blocked and the amino acid at position 38 is Gln. epsilon 1 and epsilon 3 are beta-type embryonic globins, sharing 73.7% amino acid homology. Interestingly, they are more similar to the corresponding mouse beta-type embryonic globins, y and z, respectively, than to each other, implying that these globins have evolved orthologously from common ancestral proteins. It was also shown that the zetta, epsilon 1, and epsilon 3 globins are almost completely replaced by the adult type alpha and beta globins in the blood of rat fetuses by day 18 of gestation.

Situation of monomethoxypolyethylene glycol covalently attached to lysozyme.

We selectively introduced monomethoxypolyethylene glycol (mPEG) 5000, 2000, and 550 into Asp119 in lysozyme. To examine how the mPEGs were present on the surface of the modified lysozyme, the activities, binding abilities to the Fab fragment of anti-lysozyme monoclonal antibody, net charges and nuclear magnetic resonance (NMR) spectra of mPEG lysozymes were examined. With the increase in molecular weight of mPEG, the activities and binding abilities to the Fab of mPEG lysozyme decreased. However, introduced mPEG5000 did not cause complete inhibition of the activities and binding abilities to the Fab, while the maximum length of mPEG5000 was so great that it largely covered the surface of the lysozyme molecule. Analyses of the net charges and NMR suggested that the introduced mPEG preferentially assumed a folded conformation on the surface rather than spread all over the surface. Based on the structure of mPEG lysozyme, the mechanism of the reduced immunogenicity of mPEG lysozyme was discussed.

Inhibition of growth and metastasis of ovarian carcinoma by administering a drug capable of interfering with vascular endothelial growth factor activity.

The present study investigates the relationship between in vivo growth/metastasis of tumor cells and their capacity to produce the vascular endothelial growth factor (VEGF), as well as the regulation of tumor growth/metastasis using an angiogenesis-inhibitory drug. Two cloned tumor cell lines designated OV-LM and OV-HM were isolated from a murine ovarian carcinoma OV2944. OV-LM and OV-HM cells grew in cultures at comparable rates. However, when transplanted s.c. into syngeneic mice, OV-HM exhibited a faster growth rate and a much higher incidence of metastasis to lymph nodes and lung. Histologically, intense neovascularization was detected in sections of OV-HM but not of OV-LM tumor. OV-HM and OV-LM tumor cells obtained from in vitro cultures expressed high and low levels of VEGF mRNA, respectively. A difference in VEGF mRNA expression was much more clearly observed between RNAs prepared from fresh OV-HM and OV-LM tumor masses: RNA from OV-HM contained larger amounts of VEGF mRNA, whereas RNA from OV-LM exhibited only marginal levels of VEGF mRNA. An angiogenesis-inhibitory drug, FR118487 inhibited the VEGF-mediated in vitro growth of endothelial cells but did not affect the expression in vitro of VEGF mRNA by OV-HM tumor cells. Intraperitoneal injections of FR118487 into mice bearing OV-HM tumors resulted in: (i) a subsequent growth inhibition of primary tumors; (ii) a marked decrease in neovascularization inside tumor masses expressing comparable levels of VEGF mRNA to those detected in control OV-HM masses; and (iii) almost complete inhibition of metastasis to lymph nodes and lung. These results indicate that growth/metastasis of tumor cells correlates with their VEGF-producing capacity and that an angiogenesis inhibitor, FR118487, inhibits tumor growth and metastasis through mechanism(s) including the suppression of VEGF function in vivo.

Prolongation of rat liver graft survival by splenectomy combined with low dose FK506 therapy.

The effects of low dose FK506 therapy (0.1 mg/ kg/day x 1 day) on graft survivals were analyzed, and the feasibility of splenectomy was assessed. ACI strain liver grafts were orthotopically implanted into LEW male rat recipients. In the control group, the survival period was 10.4 +/- 1.4 days. In the group treated with splenectomy, the survival period was 13.4 +/- 2.0 days. In the groups with low dose FK506 therapy, the survival periods were 22.7 +/- 6.7 and 39.7 +/- 6.3 days with or without splenectomy, respectively. Rats in the group with average dose FK506 (1.0 mg/kg/day x 7 days) survived more than 100 days. In summary, the effect of low dose FK506 therapy was relatively limited. Splenectomy by itself was marginally effective; however, this effect was enhanced when combined with low dose FK506 therapy.

A percutaneously accessible pulsatile left ventricular assist device: modified assist device type 5.

To provide percutaneous access, a new circulatory assist system was developed. We call this newly developed system the modified assist device (MAD). The system is composed of a sac-type blood pump and cannula. Inflow and outflow valves are mounted in the apex and at the side wall 10 cm from the apex of the cannula, respectively. During systole, the blood is sucked from the left ventricle through the inflow valve of the cannula connected to the blood pump, and during diastole, the blood is ejected to the root of the aorta through the outflow valve. In vitro and in vivo evaluations of the pump performance were performed. The maximum flow rate of 1.9 L/min was obtained in the mock circulatory system. In our animal experiment, effective systolic unloading and diastolic augmentation were observed by activation of this system during regular sinus rhythm. In conclusion, the MAD-5 is thought to be percutaneously accessible and increases systemic and coronary flow.

Isolation of alkaline phosphatase-positive gingival fibroblasts from patients with chronic inflammatory periodontal disease.

We have reported recently that increased expression of membrane alkaline phosphatase (ALP) activity is a phenotypical characteristic of gingival fibroblasts located in chronic inflammatory periodontal lesions. To understand the cellular properties of these cells, we isolated ALP-positive gingival fibroblasts from patients with adult periodontitis and evaluated their proliferative potential. Using an enzymatic digestion procedure, we prepared gingival cell suspensions containing ALP-positive fibroblasts without affecting their ALP activities. These cell suspensions were then subjected to 1 g sedimentation, followed by allowing cells to adhere to substrata. Using this procedure, 71.9% of isolated cells were ALP-positive. Dissociation of ALP-positive fibroblasts and contamination by non-fibroblastic cells were examined by cytochemical and immunocytochemical analyses. The proliferative capacity of ALP-positive fibroblasts in culture was assessed by monitoring the proportion of ALP-positive cells after repeated subculture passages and by labelling DNA-synthesizing cells with bromodeoxyuridine (BrdU). The proportion of ALP-positive fibroblasts decreased during cell culture passages without an apparent change in the ALP-positive phenotype. The percentage of BrdU-positive cells was significantly lower among ALP-positive than among ALP-negative fibroblasts. These results indicate that ALP-positive fibroblasts in chronic inflammatory periodontal lesions have low growth potential. We suggest that their reduced capacity to grow in vitro reflects a more differentiated state induced under inflammatory conditions in vivo.

Cavitary tuberculosis produced in rabbits by aerosolized virulent tubercle bacilli.

Liquefaction of solid caseous tuberculous lesions and the subsequent cavity formation are probably the most dangerous processes in the pathogenesis of human pulmonary tuberculosis. In liquefied caseum, the tubercle bacilli grow extracellularly for the first time since the onset of the disease and can reach such large numbers that mutants with antimicrobial resistance may develop. From a cavity, the bacilli enter the bronchial tree and spread to other parts of the lung and also to other people. Of the commonly used laboratory animals, the rabbit is the only one in which cavitary tuberculosis can be readily produced. This report is the first to describe and analyze the complete course of cavitary tuberculosis, produced by aerosolized virulent bovine-type tubercle bacilli in commercially available New Zealand white rabbits. After the inhalation of 220 to 880 bacillary units, all of the rabbits were overtly well until they were sacrificed at 33 weeks. After the inhalation of 3,900 to 5,800 bacillary units, half of the rabbits died of progressive tuberculosis between 5 and 9 weeks and the other half lived until they were sacrificed at 18 weeks. Pulmonary cavities developed in both low- and high-dose groups, some beginning as early as 6 weeks. Bacilli from primary cavities sometimes caused nearby secondary cavities, but more frequently, they ascended the bronchial escalator, were swallowed, and caused secondary tubercles in the lymphoid tissue of the appendix and ileocecal junction. Histologically, and by culture, the number of bacilli found in the liquefied caseum varied from many to comparatively few. Strong tuberculin reactions at 4 weeks after infection were associated with fewer primary lesions, while strong tuberculin reactions at 33 weeks were associated with more cavitary lesions. In the tuberculous granulation tissue surrounding caseous and liquefied pulmonary foci and cavities, we found many mature epithelioid macrophages that contained high levels of the proteinase cathepsin D. Therefore, cathepsin D probably plays a major role in the liquefaction of solid caseous material and in the subsequent cavity formation.

Gadolinium-enhanced breath-hold three-dimensional MR angiography of the portal vein: value of the magnetization-prepared rapid acquisition gradient-echo sequence.

The authors evaluated magnetic resonance (MR) angiography of the portal vein performed with a breath-hold, gadolinium-enhanced, magnetization-prepared rapid acquisition gradient-echo sequence in five volunteers and 24 patients. The main portal vein and its branches and the liver were depicted clearly in all images. The hepatic vein was clearly depicted in 21 of the 24 patients. MR angiography performed with this sequence allowed high-quality imaging of the portal and hepatic veins.
