Liver T2-weighted MR imaging: comparison of fast and conventional half-Fourier single-shot turbo spin-echo, breath-hold turbo spin-echo, and respiratory-triggered turbo spin-echo sequences.

PURPOSE: T2-weighted spin-echo (SE) magnetic resonance imaging in the liver was compared with four sequences: fast and conventional half-Fourier single-shot turbo SE (HASTE) and breath-hold and respiratory-triggered turbo SE. MATERIALS AND METHODS: In 58 patients with focal liver lesions, imaging was performed with a 1.5-T magnet and a phased-array coil. Images obtained with fast HASTE conventional HASTE, breath-hold turbo SE, and respiratory-triggered turbo SE were compared. Results of quantitative, qualitative, and receiver operating characteristic analyses were compared. RESULTS: Lesion-to-liver contrast for solid tumors was significantly higher with the fast HASTE and respiratory-triggered turbo SE sequences than with the conventional HASTE and breath-hold turbo SE sequences (P < .01) and for cystic lesions was significantly higher with the conventional and fast HASTE sequences than with the breath-hold or respiratory-triggered turbo SE sequences (P < .01). Artifacts were negligible on all fast and conventional HASTE images. In receiver operating characteristic analyses, diagnostic performance was significantly higher with the fast HASTE and respiratory-triggered turbo SE sequences than with the conventional HASTE and breath-hold turbo SE sequences (P < .01). CONCLUSION: Images obtained with the fast HASTE sequence were free from motion artifacts, and observer diagnostic performance was similar to that with the respiratory-triggered turbo SE sequence.

MCI-154, a Ca2+ sensitizer, decreases the oxygen cost of contractility in isolated canine hearts.

An increase in the responsiveness of the contractile machinery to Ca2+ could theoretically enhance the mechanoenergetics of the heart. To clarify this unresolved issue, we studied the effects of MCI-154, a Ca2+ sensitizer, on the mechanoenergetics in terms of the left ventricular contractility index [slope of end-systolic pressure-volume relationship (Emax)] and the relationship between myocardial oxygen consumption (VO2) and left ventricular pressure-volume area in excised cross-circulated canine hearts. MCI-154 increased Emax by 42 +/- 31% (SD), although the slope of the VO2-PVA relationship (an indicator of contractile efficiency) was unchanged by MCI-154. Despite equal increases in Emax, the relative increase in unloaded VO2 (delta VO2/delta Emax) during infusion of MCI-154 was, however, significantly less than that during CaCl2 infusion (0.0016 +/- 0.0018 vs. 0.0059 +/- 0.0054; P < 0.05). By contrast, delta VO2/delta Emax for milrinone was the same as that for CaCl2 (0.0043 +/- 0.0041 vs. 0.0039 +/- 0.0045; P > 0.05). Basal metabolism in KCl-arrested hearts was unchanged by MCI-154, indicating that MCI-154 consumes less energy than CaCl2 for excitation-contraction coupling. These findings suggest that MCI-154 acts energetically as a Ca2+ sensitizer in beating canine whole hearts.

Regional blood flow in Dahl-Iwai salt-sensitive rats and the effects of dietary L-arginine supplementation.

The purpose of the present study was to determine 1) whether different organs undergo similar increase in vascular resistance in Dahl-Iwai salt-sensitive (S) rats, and 2) the effects of chronic oral L-arginine supplementation on the regional hemodynamics in S rats. Male 6-wk-old S rats and salt-resistant (R) rats were maintained on an 8% NaCl chow for 4 wk. One group (S or R rats) was maintained on tap water and the other group (S/Arg or R/Arg rats) received tap water containing L-arginine at a concentration of 1.5%. Organ blood flow and cardiac output were measured with microspheres in the conscious condition. Mean blood pressure in S, S/Arg, R, and R/Arg rats was 159 +/- 5, 138 +/- 3, 111 +/- 4, and 112 +/- 4 mmHg, respectively. Urinary excretion of protein and albumin in S/Arg rats was significantly suppressed compared with S rats. Concerning regional hemodynamics, the flow rate of the kidney was lower in S rats than in R rats, but there were no differences between S and R rats in the flow rates of the brain, heart, lung, liver, spleen, intestine, skeletal muscle, and skin. Thus the renal blood flow was solely reduced in S rats on a high-salt diet. The flow rate of the kidney in S/Arg rats was maintained at a higher level compared with that of S rats. L-Arginine treatment tended to produce a recovery in the urinary excretion of guanosine 3',5'-cyclic monophosphate in S rats, but had no effect in R rats. Thus the supplementation of L-arginine prevented the increase in blood pressure in S rats on a high-salt diet and normalized the abnormality of renal hemodynamics accompanying salt-induced hypertension.

Immunohistochemical expression of retinoid X receptor isoforms in human pituitaries and pituitary adenomas.

Retinoid X receptors (RXRs) are transcriptional factors that belong to the steroid/thyroid hormone receptor superfamily. There are 3 RXR isoforms-alpha, beta, gamma-known to bind 9-cis-retinoic acid as their ligand. The expression of RXRs in human pituitary glands and pituitary adenomas has not been extensively investigated. To determine whether specific RXR isoforms may play roles in the state of differentiation of pituitary adenomas, we have investigated the immunohistochemical expression of RXR alpha and RXR gamma in 6 nontumorous pituitaries and in 60 different pituitary adenomas using isoform-specific antibodies. In the nontumorous pituitaries. RXR alpha was expressed in the nuclei of almost all cells, while RXR gamma was only expressed in thyrotropin (TSH) cells and in some cells positive for growth hormone (GH) and glycoprotein alpha-subunit (alpha SU) but not in luteinizing hormone (LH) beta-subunit, follicle-stimulating hormone (FSH) beta-subunit, prolactin (PRL) or adrenocorticotropin (ACTH) cells by double immunostaining. All 60 adenomas were RXR alpha positive, and 39 of 60 adenomas (65%) were positive for RXR gamma. The incidence of RXR gamma immunoreactivity in the different adenoma types was: 13 of 16 GH-producing adenomas (81.3%), 9 of 14 PRL-secreting adenomas (64.3%), 6 of 6 TSH-secreting adenomas (100%), 2 of 5 ACTH-secreting adenomas (40%) and 9 of 19 nonfunctioning adenomas (47.4%) including immunohistochemically gonadotropin-subunit-positive adenomas. The colocalization of RXR gamma with the TSH beta subunit, GH and alpha SU in the same adenoma cells was frequently observed, and sometimes RXR gamma was colocalized with PRL, ACTH, FSH beta or LH beta as shown by double immunostaining. We conclude that RXR alpha is expressed in both human pituitaries and pituitary adenomas. In contrast, RXR gamma is expressed more broadly in pituitary adenomas than in normal pituitaries and thus may play a role in the differentiation-specific cell types in the human pituitary both under physiological and pathological conditions.

Effects of KW-3902, a selective and potent adenosine A1 receptor antagonist, on renal hemodynamics and urine formation in anesthetized dogs.

The purpose of the present investigation was to examine the effects of KW-3902 [8-(noradamantan-3-yl)-1,3-dipropyl-xanthine], a selective and potent adenosine A1 receptor antagonist, in order to clarify the role of adenosine in the control of renal hemodynamics and urine formation in anesthetized dogs. KW-3902 was directly infused into the renal artery to eliminate the systemic effects of the drug. KW-3902 (10 micrograms/kg/min) almost completely inhibited the renal vasoconstriction induced by adenosine via A1 receptors. Intrarenal infusion of KW-3902 did not affect mean arterial pressure, renal blood flow, creatinine clearance, or arterial plasma renin activity, but drastically increased urine flow, urinary excretion of sodium, and osmolar clearance. Inhibition of the renin-angiotensin system using CV-11974 [2-ethoxy-1-((2'-(1-H-tetrazole-5-yl)biphenyl-4-yl) methyl)-1-H-benzimidazole-7-carboxylic acid], a selective AT1 antagonist, did not affect the diuretic action of KW-3902. These data suggest that endogenous adenosine does not play a significant role in the control of renal hemodynamics in whole kidney, but that it plays an important role in preserving body fluid via the A1 receptor independent of the renin-angiotensin system in anesthetized dogs.

Paradoxical action of activin A on folliculogenesis in immature and adult mice.

Activin A is a gonadal protein originally isolated from follicular fluid and is recognized as a local regulator of granulosa cell differentiation. Whether activin A promotes folliculogenesis, however, still remains unclarified. The present study was designed to elucidate the effect of activin A on follicular growth in in vitro follicle culture systems. Preantral follicles, 100-120 microm in diameter, were mechanically isolated from BDF1 hybrid immature mice (11 days old) and adult mice (8 weeks old), then cultured for 4 days in a serum-free medium supplemented with activin A (100 ng/ml), FSH (100 mIU/ml), and a combination of both. Follicular diameter was measured daily, and the amount of estradiol and inhibin released at day 4 was determined by RIA. Preantral follicles collected from immature mice showed a significant increase in diameter when cultured with activin A or both activin A and FSH. FSH alone showed no significant effect on the diameter of follicles from immature mice. In contrast, the diameter of preantral follicles from adult mice significantly increased in response to FSH. Activin A did not stimulate growth of follicles from adult mice, and more interestingly, blocked the effect of FSH. The inhibitory action of activin A was in part restored by co-culture with follistatin (100 ng/ml). These results indicate that activin A is folliculogenetic in the prepubertal mouse, but not in adults.

The role of basally synthesized nitric oxide in modulating the renal vasoconstrictor action of angiotensin II.

Interactions between nitric oxide (NO) and angiotensin (ANG) II in renal vascular beds were examined in anesthetized dogs. The renal blood flow (RBF) response to an intrarenal arterial injection of ANG II was significantly augmented by intrarenal infusion of the NO synthase inhibitor N(G)-nitro-L-arginine (LNA, 50 microg/kg/min). The simultaneous intrarenal infusion of L-arginine (1 mg/kg/min) prevented the potentiating action of LNA. Similar potentiation was also seen in phenylephrine-induced renal vasoconstriction. Moreover, during simultaneous intrarenal infusion of sodium nitroprusside (SNP), an NO donor, the potentiating action of LNA on the renal vasoconstrictor action of ANG II disappeared. Under these conditions, the released NO stimulated by ANG II was still inhibited by LNA, if present, but basally released NO was resupplied by SNP, as indicated by the return of the RBF. During an infusion of phenylephrine, which produced an increase in renal vascular tone similar to that observed during the infusion of LNA, the renal vasoconstrictor action of ANG II was not augmented. These data suggest that basally released NO plays an important role in the regulation of renal hemodynamics by modulating the renal vasoconstrictor actions of ANG II and phenylephrine.

Role of nitric oxide in desmopressin-induced vasodilation of microperfused rabbit afferent arterioles.

We have previously reported that desmopressin (dDAVP) increased the lumen diameter of norepinephrine (NE)-constricted isolated microperfused rabbit afferent arterioles. In this study, we examined the role of nitric oxide in dDAVP-induced vasodilation of afferent arterioles. We microdissected a superficial afferent arteriole from the kidney of a New Zealand white rabbit. Each afferent arteriole was cannulated with a pipette system and microperfused in vitro at 60 mmHg. dDAVP increased the lumen diameter of NE-preconstricted rabbit afferent arterioles dose-dependently. dDAVP-induced vasodilation was abolished by pretreatment with NG-nitro-L-arginine (L-NNA, 10(-4)M) (L-NNA + NE, 6.7 +/- 1.1 microns; L-NNA + NE + dDAVP, 7.3 +/- 1.4 microns, n = 8). dDAVP increased the lumen diameter of NE-preconstricted afferent arterioles pretreated with L-NNA and L-arginine (10(-2)M) (L-NNA + L-arginine + NE, 6.1 +/- 1.1 microns; L-NNA + L-arginine + NE + dDAVP, 8.7 +/- 0.9 microns*; *p < 0.05, n = 6). Aspirin-DL-lysine (10(-4)M) did not influence dDAVP-induced afferent arteriolar vasodilation (aspirin + NE, 6.4 +/- 0.8 microns; aspirin + NE + dDAVP, 9.6 +/- 1.3 microns *; *p < 0.05, n = 5). These results suggest that nitric oxide may be responsible for dDAVP-induced afferent arteriolar vasodilation.

Circadian locomotor rhythms in the cricket, Gryllodes sigillatus. I. Localization of the pacemaker and the photoreceptor.

Circadian locomotor rhythm and its underlying mechanism were investigated in the cricket, Gryllodes sigillatus. Adult male crickets showed a nocturnal locomotor rhythm peaking early in the dark phase of a light to dark cycle. This rhythm persisted under constant darkness (DD) with a free-running period averaging 23.1 +/- 0.3 hr. Although constant bright light made most animals arrhythmic, about 40% of the animals showed free-running rhythms with a period longer than 24 hr under constant dim light condition. On transfer to DD, all arrhythmic animals restored the locomotor rhythm. Bilateral optic nerve severance resulted in free-running of the rhythm even under light-dark cycles. The free-running period of the optic nerve severed animals was significantly longer than sham operated crickets in DD, suggesting that the compound eye plays some role in determining the free-running period. Removal of bilateral lamina-medulla portion of the optic lobe abolished the rhythm under DD. These results demonstrate that the photoreceptor for entrainment is the compound eye and the optic lobe is indispensable for persistence of the rhythm. However, 75% and 54% of the optic lobeless animals showed aberrant rhythms with a period very close to 24 hr under light and temperature cycles, respectively, suggesting that there are neural and/or humoral mechanisms for the aberrant rhythms outside of the optic lobe. Since ocelli removal did not affect the photoperiodically induced rhythm, it is likely that the photoreception for the rhythm is performed through an extraretinal photoreceptor.

Circadian locomotor rhythms in the cricket, Gryllodes sigillatus. II. Interactions between bilaterally paired circadian pacemakers.

The optic lobe is essential for circadian locomotor rhythms in the cricket, Gryllodes sigillatus. We examined potential interactions between the bilaterally paired optic lobes in circadian rhythm generation. When one optic lobe was removed, the free-running period of the locomotor rhythm slightly but significantly lengthened. When exposed to light-dark cycles (LD) with 26 hr period, intact and sham operated animals were clearly entrained to the light cycle, but a large number of animals receiving unilateral optic nerve severance showed rhythm dissociation. In the dissociation, two rhythmic components appeared; one was readily entrained to the given LD and the other free-ran with a period shorter than 24 hr, and activity was expressed only when they were inphase. The period of the free-running component was significantly longer than that of the animals with a single blinded pacemaker kept in LD13:13, suggesting that the pacemaker on the intact side had some influence on the blinded pacemaker even in the dissociated state. The ratio of animals with rhythm dissociation was greater with the lower light intensity of the LD. The results suggest that the bilaterally distributed pacemakers are only weakly coupled to one another but strongly suppress the activity driven by the partner pacemaker during their subjective day. The strong suppression of activity would be advantageous to keep a stable nocturnality for this cricket living indoors.

Pressure palsy as the initial presentation in a case of late-onset Charcot-Marie-Tooth disease type 1A.

We report siblings of late-onset Charcot-Marie-Tooth disease type 1A (CMT1A) of whom the elder brother presented with drop foot. DNA analysis confirmed the 17p11.2-p12 duplication. Estimation of the age of onset is difficult in CMT1A because of the subtle and slow progress of the symptoms. These cases are unique in that the initial clinical manifestation was a pressure palsy in the brother and both siblings were quite delayed in their clinical presentations. The peripheral myelin protein-22 gene duplication is uniformly found in CMT1A patients. However, other factors influencing the disease manifestations are yet to be determined.

Multiple hemorrhagic gastric ulcers due to polyarteritis nodosa.

A 73-year-old man developed refractory multiple hemorrhagic gastric ulcers. Emergent gastrectomy was performed, and the diagnosis of polyarteritis nodosa was made by histologic evaluation of the resected stomach. Although gastric ulceration is a relatively rare complication of polyarteritis nodosa, it should be considered in the differential diagnosis of refractory hemorrhagic gastric ulcers. Emergent surgery may be indicated in such cases.

MR imaging of the fetus by a HASTE sequence.

OBJECTIVE: The value of a half-Fourier acquisition single-shot turbo spin-echo (HASTE) sequence, in which high-resolution heavily T2-weighted images can be obtained within 2 sec, was evaluated in the imaging of the fetus during the second or third trimester. MATERIALS AND METHODS: Eighteen women with complicated pregnancies as revealed on a sonogram during the second and third trimesters (16-36 weeks' gestation) were studied with a 1.5-T superconductive MR imaging unit that used a body phased-array coil. After informed consent, T1-weighted fast low-angle shot images, T2-weighted turbo spin-echo images, and HASTE images were obtained without any premedication. Images were analyzed with regard to image quality, degree of blurring, visualization of the normal fetal organs, and visualization of fetal and maternal abnormalities. RESULTS: On HASTE sequences, visualization of the fetal brain, visceral organs (lung, heart, liver, kidney, and bladder), extremities, and umbilical cord were significantly better than on fast low-angle shot or turbo spin-echo sequences (p < .01). In the brain, the white matter-gray matter distinction, gyrus formation, and myelination of the brain were clearly revealed by the HASTE sequence. Pathologic processes including fetal abnormalities (anomalies of the central nervous systems [n = 5], placenta previa [n = 1], and transverse lie in the third trimester [n = 1]) and maternal abnormalities (leiomyoma [n = 5], ovarian tumors [n = 3], and hydronephrosis [n = 1]) were clearly seen on HASTE imaging. The peak specific absorption rate for RF exposure in these studies was less than 1.5 W/kg. CONCLUSION: In situations when sonography is suggestive but not definitive, MR imaging with a HASTE sequence allows clear fetal imaging with high T2-weighted contrast.

In vitro and clinical studies of image acquisition in breath-hold MR cholangiopancreatography: single-shot projection technique versus multislice technique.

OBJECTIVE: This study was undertaken to compare the in vitro and clinical value of two-dimensional multislice breath-hold MR cholangiopancreatography (MRCP) with a single-shot projection technique using a half-Fourier acquisition single-shot turbo spin-echo sequence. SUBJECTS AND METHODS: We examined 108 patients with pancreaticobiliary diseases, using breath-hold MRCP and a half-Fourier acquisition single-shot turbo spin-echo sequence on a 1.5-T MR unit with a body phased-array coil. Two data acquisition techniques were employed: multislice acquisition postprocessed by maximum intensity projection (MIP) (multislice technique) and single-shot projection with a thick slice (projection technique). In the multislice technique, nine contiguous slices were obtained with a thickness of 5 mm (acquisition time. 18 sec). In the projection technique, a single slice was obtained with a thickness of 30, 50, or 70 mm (acquisition time. 2 sec). Contrast-to-noise ratio (CNR) between the common bile duct and the liver as well as detectability of normal structures and diseases were compared for these two acquisition techniques. In the multislice technique, source images were also evaluated. ERCP or percutaneous transhepatic cholangiography images were used as the gold standard. RESULTS: Most of the pancreatic duct and common bile duct was revealed on 54% and 100% of the projection images, respectively, and on 35% and 98% of the MIP images, respectively, CNR was significantly higher with the multislice technique than with the projection technique (p < .01). With the projection technique, CNR decreased as slice thickness increased. Dilatation and occlusion of the pancreaticobiliary tree were equally well revealed by the two imaging techniques. However, abnormalities in the periampullary region and anomalies in the pancreaticobiliary tree were more clearly seen on projection images than on MIP images (p < .05). Stones in the common bile duct, gallbladder, or intrahepatic bile duct were best seen on source images acquired by the multislice technique (83% sensitivity). CONCLUSION: Because of the absence of misregistration and the speed of image acquisition, breath-hold single-shot MRCP using the projection technique with a slice thickness of 30 or 50 mm consistently revealed the pancreaticobiliary tree and periampullary region with an acceptable CNR. Stones in the bile duct were best seen on the source images acquired by the MIP technique.

Biphenotypic blast crisis of chronic myelogenous leukemia: abnormalities of p53 and retinoblastoma genes.

The molecular mechanisms responsible for progression of chronic myelogenous leukemia (CML) to blast crisis have not been well defined. Blast crisis may be partially related to inactivation of tumor suppressor genes/such as p53 or retinoblastoma (Rb) gene. There is evidence for an association of blast cell phenotypes in CML with alterations of these genes: a strong association of myeloid phenotypes with abnormalities of the p53 gene and a weaker association of lymphoid phenotypes with abnormalities of the Rb system. We found a marked decrease in Rb gene product and rearrangements of the p53 gene simultaneously in two cases of biphenotypic blast crisis of CML (myeloid and B-lymphoid). These results support the association of blast cell phenotypes with alterations in tumor suppressor genes in CML blast crisis.

Use of a catecholamine sensor in the control of an artificial heart system.

An electrochemical sensor system to allow real-time measurement and feedback of catecholamine concentrations was developed for use in the control of artificial hearts. Electrochemical analyses were carried out using a carbon fiber working electrode, an Ag-AgCl reference electrode, and a potentiostat. The operating parameters of the pneumatically-driven artificial heart system were altered in accordance with the algorithm for changes in the catecholamine concentration. The minimum detectable concentrations of both adrenaline and noradrenaline in a mock circulatory system using a phosphate-buffered solution were approximately 1-2 ng/ml (10(-8) mol/L). An artificial heart control system utilizing this set-up performed satisfactorily without delay, although sensor sensitivity decreased when placed in goat plasma instead of a phosphate-buffered solution, due to the adsorption of various substances such as plasma proteins onto the electrodes. This study demonstrated the future feasibility of a feedback control system for artificial hearts using catecholamine concentrations.

Leucocytoclastic vasculitis associated with mixed cryoglobulinaemia and hepatitis C virus infection.

Mixed cryoglobulinaemia is frequently associated with chronic hepatitis. We report a patient with mixed cryoglobulinaemia, hepatitis C virus (HCV) infection and palpable purpura. The skin manifestations were diagnosed as leucocytoclastic vasculitis in view of both the clinical appearance and the histological findings. In this study, we demonstrated the presence of IgG-class anti-HCV-antibody, HCV-RNA and IgA-class rheumatoid factor in the cryoprecipitate. These results suggest that the cryoglobulinaemia in this case was caused by aggregation of an immune complex comprised of HCV and anti-HCV antibody with IgA-type-rheumatoid factor, and that this led to a cutaneous vasculitis.

Serial assessment of sympathetic reinnervation in a patient with myocardial infarction.

Resting [123I]MIBG and 201TI imaging were performed at 2 wk and 4 and 12 mo after successfully reperfused myocardial infarction. Although [123I]MIBG uptake of the infarcted segments revealed significant improvement in the early image at 4 mo, delayed image displayed decreased [123I]MIBG uptake. However, decreased [123I]MIBG uptake of the delayed image became almost uniform at 12 mo. These observations suggest that reinnervation initially occurs in norepinephrine uptake and then in retention ability. On the other hand, a 201TI defect remained in the infarcted segments at 12 mo. Thus, reinnervation can occur not only in the peri-infarct area but also in the infarcted area.
