[Expression of the genes FHIT, Bcl-2 and Bax in breast infiltrating ductal carcinoma and clinicopathological significance thereof].

OBJECTIVE: To investigate the expression of the genes fragile histidine triad (FHIT), Bcl-2, and Bax, biological markers of breast infiltrating ductal carcinoma in this carcinoma and clinicopathological significance thereof. METHODS: The clinical data of 100 patients with breast infiltrating ductal carcinoma, all females, aged 435 (28 - 92), were collected. Immunohistochemistry was used to detect the protein expression of FHIT, Bcl-2 and Bax in the carcinoma tissues resected during operation. RESULTS: The protein expression rates of FHIT, Bcl-2 and Bax in the tumor tissues were 73%, 50%, and 34% respectively. The disease free survival (DFS) and overall survival (OS) of the FHIT positive patients were 81.8 months and 123.6 months, both significantly longer than those of the FHIT-negative patients (27.7 months and 74 months, both P < 0.05). The DFS of the Bcl-2-positive patients was 83 months, significantly longer than that of the Bcl-2-negative patients (45 months, P < 0.05). The mean DFS of the Bcl-2-positive patients who received postoperative adjuvant chemotherapy was 54.8 months, significantly longer than that of the Bcl-2-negative patients who received postoperative adjuvant chemotherapy (41.6 months). The mean DFS and OS of the Bcl-2-negative patients receiving CAF regimen were 55 months and 58.8 months respectively, both longer than those of the Bcl-2-negative patients receiving other regimens (27 months and 36 months respectively). However, the expression of Bax failed to show correlation with the prognosis of breast infiltrating ductal carcinoma. CONCLUSION: Expression of FHIT and expression of Bcl-2 are positively correlated to the DFS and OS of the breast infiltrating ductal carcinoma. Bax is not predictive to the prognosis of breast infiltrating ductal carcinoma.

[To inquiry into clinical-pathological factors influencing cytological accuracy of pancreatic head lesions].

OBJECTIVE: To inquiry into clinical-pathological factors influencing cytological accuracy of pancreatic head lesions. METHODS: Cytology was retrospectively evaluated in 94 inpatients with a mass in head of pancreas existing with chronic pancreatitis in the past decade, the results of cytology were compared with clinical pathology or clinical follow-up to estimate the value and accuracy of cytology in detecting pancreatic cancer. Pancreatic clinical-pathology includes size of mass and component of mass which was composed of cancerous mode of development including shape of conglomeration, nest and pervasion, and pancreatic ductal epithelium inside the vicinity of 1 cm around the mass. Pancreatic ductal epithelium were divided into PanIN1, PanIN2 and PanIN3 three types according to classified criterion of pancreatic intraepithelial neoplasia (PanIN). RESULT: Forty-six patients were pathologically diagnosed as pancreatic cancer and five patients as chronic pancreatitis, accordingly, 29 malignant, 5 suspicious, 10 atypical hyperplasia among of them 5 malignant, 3 hyperplastic ductal epithelium, 1 nondiagnostic results due to interfered by blood and 3 insufficient specimens. 43 patients were clinically diagnosed as chronic pancreatitis. Cytologic evaluation of pancreatic cancer has an 84.2% accuracy less than or equal to 2.5 cm and 71.9% larger than 2.5 cm. Shape of conglomeration, and nest have more accurate than pervasion in cytological diagnosis. CONCLUSIONS: Cancerous mode of development is a vital factor influencing accuracy of cytology, cytological estimation of atypical hyperplasia and is still waiting for further investigation.

[Clinicopathological significance of expression of c-kit protein in neuroendocrine lung carcinoma].

OBJECTIVE: To investigate the expression of c-kit protein in neuroendocrine lung carcinoma and evaluate its clinicopathological significance. METHODS: The expression of c-kit protein in the 90 specimens of neuroendocrine lung carcinoma obtained during operation was detected by immunohistochemistry. The relationship between the expression of c-kit protein and the clinicopathological indices was analyzed statistically. RESULTS: The positive staining of c-kit protein was mainly localized on the membrane and in the cytoplasm of the tumor cells. The positive expression rates of c-kit protein were 21.4% (3/14), 90.0% (18/20), 38.7% (12/31), and 76.0% (19/25) in the carcinoid, atypical carcinoid, large cell neuroendocrine carcinoma, and small cell lung cancer respectively with significant differences between any 2 histological types (P = 0.000). The c-kit protein expression rate was 26.3% (5/19) in the stage I carcinoma, 62.5% (15/24) in the stage II carcinoma, 67.4% (29/43) in the stage III carcinoma, and 75.0% (3/4) in the stage IV carcinoma with significant differences between any 2 stages (P = 0.014); was 43.1% (22/51) in the tumors less than 5 cm in diameter, and 76.9% (30/39) in the cancers equal to or more than 5 cm in diameter (P = 0.002); 66.1% (47/71) in the tumors with lymphatic metastasis and 26.3% (5/19) in those without lymphatic metastasis (P = 0.003); and 64.8% (46/71) in the males patients and 31.6% (6/19) in the female patients (P = 0.017). Statistical analysis showed that the expression of c-kit protein was not related to age (P = 0.394) and localization (P = 1.000). The median survival in patients with c-kit negative tumors was 97 months, whereas that in those with c-kit positive carcinomas was 16 months (Log rank = 22.85, P = 0.0000). CONCLUSION: c-kit protein expression appears to play an important biological or clinical role in the development and evolution of neuroendocrine lung carcinoma, and may become a useful molecular marker to predict the prognosis of patients. Thus targeted therapies interacting with c-kit-signaling pathway should be taken into account for possible future clinical trials.

Apoptosis initiated by carbon tetrachloride in mitochondria of rat primary cultured hepatocytes.

AIM: To investigate the mitochondria-initiated apoptosis pathway involved in Carbon tetrachloride (CCl4) hepatotoxicity in vitro. METHODS: Several cytotoxicity endpoints, including WST-8 metabolism, lactate dehydrogenase leakage and morphological changes, were examined. The 5,5'-dithio-bis(2-nitrobenzoic acid) reaction was used to measure reduced glutathione level, and the malondialdehyde level was determined using the thiobarbituric acid assay. The release of cytochrome c and Bcl-X(L) was detected by Western blot. Caspase-3 activity was measured using the fluorogenic substrate Ac-DEVD-AMC. DNA fragmentation was used to evaluate cell apoptosis. RESULTS: A time- and dose-dependent decrease in cellular glutathione content was observed, along with a concomitant increase in malondialdehyde levels following the application of CCl4. Caspase 3 activity was stimulated at all doses of CCl4, with the most significant activation at 3 mmol/L. Cytochrome c was released obviously after CCl4 treatment. A time-dependent decrease in Bcl-X(L) expression was observed. DNA fragmentation results revealed apoptosis and necrosis following CCl4 treatment. CONCLUSION: Oxidative damage is one of the essential mechanisms of CCl4 hepatotoxicity, which triggers apoptosis via the mitochondria-initiated pathway.

Effect of Feitai on bleomycin-induced pulmonary fibrosis in rats.

The current therapeutic approaches for pulmonary fibrosis, which is characterized by fibroblast proliferation and extracellular matrix remodeling, are unsatisfactory. Feitai, consisting of several herbs, is a folk formula for pulmonary tuberculosis therapy in China. To investigate the effects of Feitai on pulmonary fibrosis, Feitai was administered orally to bleomycin (BLM)-treated rats, and the lung toxicity effects were evaluated according to inflammatory cell count, protein concentration, and lactate dehydrogenase (LDH) activity in the bronchoalveolar lavage fluid (BALF), malondialdehyde level and hydroxyproline content in lung tissue 28 days post-BLM. Serial sections of the lung were stained with hematoxylin and eosin (HE) and Masson trichrome, respectively. The degree of fibrosis was assessed quantitatively using LEICA QWin image analyzer. Results showed that Feitai inhibited BLM-induced lung fibrotic lesions in a dose-dependent manner as reflected by decreased the lung hydroxyproline content and lung fibrosis fraction 28 days after BLM instillation. Treatment with Feitai also significantly ameliorated the BLM-induced lung toxicity effects detected in BALF and lung tissue. The effects in vitro on WI-38 human lung fibroblast cell line showed that Feitai significantly reduced the cell proliferation and transforming growth factor (TGF)-beta stimulated type I collagen synthesis. These results strongly demonstrate that Feitai may be useful in the treatment of pulmonary fibrosis.

[Glial abnormalities in progressive supranuclear palsy and corticobasal degeneration].

OBJECTIVE: To study pathologic features of glial cells in progressive supranuclear palsy (PSP) and corticobasal degeneration (CBD) and to explore their pathologic significance. METHODS: Brain tissues from 2 cases with PSP and 3 cases with CBD, all confirmed by autopsies, were examined by routine neuropathologic methods, Gallyas-Braak staining and tau immunostaining. Brain tissues from 6 Alzheimer's disease cases, 4 cases with Parkinson's disease and 6 elderly with no neurologic abnormality were used as controls. RESULTS: Gallyas-Braak staining demonstrated tuft-shaped astrocytes and coiled-body oligodendroglial cells in the brain tissues of 2 cases with PSP and 3 cases with CBD. The tuft-shaped astrocytes appeared prominently in the frontal and parietal cortex, basal ganglia and grey matter of the brainstem. The coiled-body oligodendroglial cells were distributed widely in the white matter of the frontal and parietal lobes, basal ganglia, brainstem and cerebellum. However, astrocytic plaques, composed of degenerative stubby processes with radiating arrangement, only appeared in the frontal, parietal and cingular cortex, as well as in the striatum of 3 cases with CBD. The astrocytic plaques and tuft-shaped astrocytes coexisted in the same areas, including parietal and cingular cortex and striatum, in CBD. All these glial abnormalities showed tau-positive immunoreaction not found in control cases. CONCLUSIONS: The tuft-shaped astrocytes and coiled-body oligodendroglial cells are common glial morphologic features of both PSP and CBD. Astrocytic plaques are also characteristically seen in CBD.

[Pathologic diagnoses of core needle biopsies of the mediastinum].

OBJECTIVES: To assess the diagnostic accuracy and to study the histologic typing of mediastinal lesions using core needle biopsies. METHODS: The histopathology and immunophenotype of 65 mediastinal core needle biopsy specimens were studied retrospectively by light microscopy and immunohistochemical staining (ABC method). Gene rearrangement studies were performed in some of the non-Hodgkin's lymphomas cases using PCR. Follow-up records were also analyzed. RESULTS: Morphologically, all specimens showed a combination of epithelioid cells, lymphoid cells and fibrous tissue in different proportions. The pathologic diagnoses included lymphoma (21 cases), pulmonary carcinoma (20 cases), thymoma (14 cases), thymic carcinoma (4 cases), seminoma (3 cases) and chronic inflammation (1 case). Definitive diagnosis was not possible in 2 cases due to insufficient material. The tumor cells in lymphoma (21 cases) expressed CD20, CD3, TDT, CD30, CD15 or EMA, depending on their histologic subtypes. Tumor cells in the 17 pulmonary carcinoma cases expressed cytokeratin (CK), except 3 cases of small cell carcinoma of lung. Synaptophysin, chromogranin A and neuron-specific enolase were all positive in the 10 cases of small cell carcinoma of lung and 1 case of thymic small cell carcinoma (which was also CD5 negative). The 3 cases of adenocarcinoma of lung showed positivity for thyroid transcription factor-1 (TTF-1) and they were negative for CD5. The 14 thymoma cases expressed CK, CD3 or CD20. The 3 thymic carcinoma cases expressed CK and CD5. Placental-like alkaline phosphatase (PLAP) was positive in 3 seminoma cases which were CK-negative. Immunoglobulin heavy chain gene was rearranged in the 3 cases of diffuse large B-cell lymphoma and 1 B-cell anaplastic large cell lymphoma case. T-cell receptor beta gene was rearranged in 5 T-cell lymphoblastic lymphoma cases. CONCLUSIONS: Microscopic assessment of tissue samples from mediastinal core needle biopsies should be made in combination with clinical and radiologic information. Ancillary investigations, including immunohistochemical staining and/or gene rearrangement studie, are needed in both non-lymphoma and lymphoma cases of mediastinum.

[Pathologic diagnosis of non-Alzheimer type dementia].

OBJECTIVE: To characterize histopathologic features of non-Alzheimer type dementia. METHODS: Bodian, Gallyas-Braak silver staining, tau and ubiquitin immunohistochemistry were applied in an analysis of 22 cases of autopsy-proven neurodegenerative dementia. Appearance, distribution and immunoreactivity of neuronal and glial inclusions in the brain were observed. The final histological diagnoses were made according to the pathological criteria for several types of common non-Alzheimer type dementia. RESULTS: Among the 22 cases of neurodegenerative dementia, 12 cases were identified as non-Alzheimer type dementia, including Pick's disease (2 cases), progressive supranuclear palsy (3 cases) and corticobasal degeneration (3 cases), dementia with Lewy bodies (1 case), and Parkinson's disease (3 cases). Another 10 cases consisted of pure Alzheimer's disease (AD, 9 cases) and AD combined with argyrophilic grain disease (1 case). Characteristic neuronal and glial inclusions, such as classical and cortical Lewy body, Pick body, Globous NFTs, astrocytic plaque and tufted astrocyte, argyrophilic grain were found in the brains of non-Alzheimer type dementia. Classical and cortical Lewy bodies were not argyrophilic but were immunoreactive to ubiquitin. Pick bodies, Globous NFTs, astrocytic plaques, tufted astrocytes and argyrophilic grains were all argyrophilic. Pick bodies showed tau and ubiquitin immunoreactivity. However, Globous NFTs, astrocytic plaques, tufted astrocytes, and argyrophilic grains were reactive only to tau immunohistochemistry. CONCLUSIONS: Findings of characteristic neuronal and glial inclusions may help to differentiate non-Alzheimer type dementia from AD, and in conjunction with Gallyas-Braak staining and immunohistochemistry for tau and ubiquitin, to further define histopathologic subcategories of non-Alzheimer type dementia.

[Differences in morphology and immunohistochemistry between hepatic angiomyolipoma and renal angiomyolipoma].

OBJECTIVE: To investigate the clinicopathological and immunohistochemical characteristics of hepatic angiomyolipoma (AML) and renal AML, and to compare the similarities and differences between the two. METHODS: Hematoxylin-eosin staining and SP immunohistochemical staining were performed on the specimens resected during operation of 12 cases of hepatic AML, 2 males and 10 females, aged 41, and 38 renal AML cases, 14 males and 24 females, aged 43. Wilcoxon rank test was made on the proportions of the 3 kinds of myoid cells and fat tissues. RESULTS: All tumors were composed of three basic components: blood vessels, fatty tissue, and myoid cells epithelioid, spindle, or intermediate in shape. Epithelioid cells accounted for more than 50% of the cells in 6 out of the 12 hepatic AML cases often with apparent cellular and nuclear atypia (12/12), and accounted for less than 25% in all renal AML cases. Spindle-shaped cells were scarcely observed in hepatic AML and were common in renal AML. Nuclear atypia was observed in 20 out of the 38 renal AML cases, 18 cases being mild and 1 being moderate. Fat cells accounted for less than 50% in hepatic AML, and more than 50% in renal AML (P = 0.001). Extramedullary hematopoietic tissue was often seen in hepatic AML but never observed in renal AML. Strong positivity of HMB45 and focal positivity of SMA were common in hepatic AML and strong positivity of SMA and sporadic positivity of HMB45 were observed in the renal AML specimens. CONCLUSION: The histological composition of AML is highly variable in the proportion and distribution of the 3 component cells between hepatic AML and renal AML.

[A study on the effects of rAd-gfp-bcl-X(L) on retinal degeneration in mice].

OBJECTIVE: To observe the therapeutic effect of rAd-gfp-bcl-X(L) on retinal degeneration (RD) in mice. METHODS: rAd-gfp-bcl-X(L) granules were injected into the subretinal space of right eyes of RD mice in Group A (postnatal days 10, p10) and Group B (postnatal days 22, p22) (n = 20). The morphological changes were observed by light microscopy and transmission electron microscopy at 15 - 30 days post-injection. Frozen section was used to observe the expression of GFP. The level of bcl-X(L) mRNA and bcl-X(L) protein were assayed by RT-PCR and immunohistochemistry assay. RESULTS: In frozen section, GFP was observed in the treated eyes under fluorescence microscopy, indicating that rAd-gfp-bcl-X(L) had transfected into the retinal cells successfully and that bcl-X(L) was expressed. In Group A, an increased expression of bcl-X(L) mRNA and Bcl-X(L) protein were observed in the treated eyes. The outer nuclear layer of the treated eyes in Group A was thicker than that of the control eyes at 30 days post-injection. The inner segment of treated eyes in Group A was healthier than that of control eyes, as indicated by transmission electron microscopy. No difference was found in the thickness of retina between treated eyes and control eyes in Group B. CONCLUSION: The bcl-X(L) can be expressed and have an anti-apoptosis effect when the rAd-gfp-bcl-X(L) particles are injected into the subretinal space of RD mice's eyes at an early stage. But the expression and protective effects were not observed in RD mice at a later stage. The therapy of rAd-gfp-bcl-X(L) for retinal degenerative diseases still needs further study.

[An analysis of the causes of dementia in 383 elderly autopsied cases].

OBJECTIVE: To survey the causes of dementia confirmed by autopsy in the elderly and to have a better understanding of various causes of senile dementia. METHODS: A retrospective study on clinical and pathological diagnosis in 383 cases aged 60 and over with autopsy performed was carried out clinical diagnosis of dementia was based on DSM-IVR, NINCDS-ADRDA and NINCDS-AIREN criteria. Gallyas-Braak staining, Tau and Ubiquitin Immunohistochemistry staining were adopted for diagnosing neurodegenerative dementia and distinguishing various degenerative diseases. RESULTS: Among the 383 aged cases with consecutive autopsy, 78 cases were found to have clinical features and brain pathologic change related to dementia. The incidence of dementia in the aged patients with autopsy was 20.4%. Of all the cases, vascular dementia accounted for 38.5% (30 cases), being the most frequently on countered. Next in order were degenerative dementias including Alzheimer's disease (11 cases) and non-Alzheimer's degenerative dementia (9 cases) including dementia with Lewy bodies, corticobasal degeneration, progressive supranuclear palsy, Pick's disease and idiopathic Parkinson'disease. Degenerative dementias accounted for 25.6% (20 cases). Various medical disorders including hepatic encephalopathy, pulmonary encephalopathy, hypoglycemia with cognitive impairment were also relatively common, they accounted for 20.5% (16 cases). The other less common causes included brain tumor, normal pressure hydrocephalus and subdural hematoma, 12 cases accounted 15.4%. The total accordance rate of clinical and pathologic diagnosis of senile dementia was 64.5%, the accordance rates of medical disorders such as hepatic encephalopathy, pulmonary encephalopathy and brain metastatic tumors were all 100%, the accordance rate of vascular dementia and degenerative dementia was 66.7% and 40% respectively. CONCLUSION: Senile dementia is a clinical syndrome caused by multiple factors. Physician should think of various possible causes of dementia, basing on their clinical history and laboratory results. Promoting autopsy and adopting new pathological techniques would increase the diagnostic accuracy of degenerative dementia.

[Determination of DNA by fluorimetry with flow injection].

A highly sensitive fluorimetry determination of DNA is developed in this paper. The maximum fluorescence intensity was produced in the pH 3.05, with the maximum excitation and emission wavelength of 362 and 531 nm, respectively. Under the optimum conditions, the fluorescence intensity was proportional to the concentration of DNA. The linear range for calf thymus DNA is of 0-120 mg.L-1. The limit of determination was 0.07 mg.L-1. When the proposed method was used to determine DNA in the presence of some coexisting substances, a satisfactory result was obtained.
