The immune microenvironment of human fracture/soft-tissue hematomas and its relationship to systemic immunity.

The immune environment of human soft-tissue injury is unstudied. We studied fracture soft-tissue hematomas (FxSTH) in 56 patients with high-energy bony fractures. FxSTH serum and mononuclear cells (MNC) as well as fracture patient plasma and blood MNC were studied. Twenty healthy controls donated plasma and MNC. Soluble tumor necrosis factor (TNF)-alpha, interleukin (IL-1 beta, IL-2, 6, 8, 10, 12, and interferon-gamma were studied by enzyme linked immunosorbent assay. Cells were studied by flow cytometry after cell-membrane stains for CD-14, TNF-alpha (mTNF), and human leukocyte antigen-DR, or intracellular stains for TNF (icTNF) and IL-10. Thirty-six patients with Injury Severity Score < 15 were analyzed further to evaluate the effects of isolated fracture on systemic immunity. Cytokines were rarely detectable in control plasma. TNF-alpha, IL-1 beta, IL-2, and interferon-gamma were rarely found in FxSTH serum or fracture patient plasma. All FxSTH sera were rich in IL-6, peaking before 48 hours (12,538 +/- 4,153 vs. 3,494 +/- 909 pg/mL, p = 0.02, U test). In Injury Severity Score < 15, IL-6 was not detectable in most early fracture patient plasma, but rose after 48 hours (p = 0.028). FxSTH serum IL-8 peaked after 48 hours (440 +/- 289 vs. 4,542 +/- 1,219 pg/mL, p = 0.006) and circulating IL-8 appeared after 72 hours. IL-6 and IL-8 showed gradients from FxSTH serum to paired PtS (p < 0.05, Wilcoxon). IL-10 was abundant (884 +/- 229 pg/mL) in FxSTH serum < 24 hours old. FxSTH serum IL-12 peaked late (3,323 +/- 799 pg/mL, day 4-7) then fell (p < 0.001, analysis of variance). Only IL-12 was higher in fracture patient plasma (1,279 +/- 602 pg/mL) than FxSTH serum (591 +/- 327 pg/mL) during the first 48 hours (p = 0.032, U test). On flow cytometry, control monocytes expressed 201 +/- 31 mTNF sites/cell, but icTNF was absent. mTNF was up-regulated after injury more in FxSTH monocytes (3,202 +/- 870 sites/cell) than peripheral blood monocytes (584 +/- 186 sites/cell) (p < 0.05 vs. peripheral blood monocytes by Wilcoxon, p < 0.001 vs. control monocytes by U test). Intracellular IL-10 was abundant in all MNC, but varied widely after injury. Fracture and peripheral blood monocytes expressed far less human leukocyte antigen-DR than control monocytes. Fractures create an inflammatory local environment. Proximal mediators are cell-associated and relatively confined to the wound, but soluble IL-6, IL-8, and IL-10 are abundant and probably exported. Systemic MNC have complex responses to local injuries. These may reflect the combined impact of multiple soluble cytokines initially generated within the wound. FxSTH appear to be a potentially important source of immunomodulatory cytokines in trauma.

Evidence that DNA-A of a geminivirus associated with severe cassava mosaic disease in Uganda has arisen by interspecific recombination.

Geminivirus isolates associated with the epidemic of severe cassava mosaic disease in Uganda were studied and compared with virus isolates from the part of Uganda outside the epidemic area, and with African cassava mosaic virus (ACMV) and East African cassava mosaic virus (EACMV). Isolates of a novel type [the Uganda variant (UgV)] were detected in severely affected plants from the epidemic area, whereas those from plants outside the epidemic area were typical of ACMV. The complete nucleotide sequences of DNA-A of UgV (2799 nt) and of a Tanzanian isolate of EACMV (2801 nt) were determined and are extremely similar, except for the coat protein (CP) gene. The CP gene of UgV has three distinct regions: the 5' 219 nt are 99% identical to EACMV (only 79% to ACMV); the following 459 nt are 99% identical to ACMV (75% to EACMV); and the 3' 93 nt are 98% identical to EACMV (76% to ACMV). UgV DNA-A therefore is considered to have arisen by interspecific recombination of EACMV and ACMV. Despite the hybrid nature of their CP, UgV isolates were indistinguishable from ACMV in tests with 20 monoclonal antibodies (MAbs), including seven which reacted with ACMV but not EACMV. The discontinuous epitopes detected by these seven MAbs must involve amino acids which lie in the central part of the CP (residues 74-226) and which differ in ACMV and EACMV. UgV isolates were detected in severely mosaic-affected plants from all 11 widely separated locations sampled. The probable role of recombination in geminivirus evolution in the short to medium term is discussed.

On the mechanism of ventricular defibrillation.

The mechanism of ventricular defibrillation can be considered at many different levels. The highest level is considered at strength of the shock given through the defibrillation electrodes. At the next level, the mechanism of defibrillation can be examined in terms of the electrical field that the shock produces throughout the ventricles. Other levels include the effects this electric field has on the activation sequences and on the cellular action potentials that either initiate or inhibit the early sites of activation following the shock. Yet another level considers the mechanism by which the shock field initiates new action potentials or prolongs the action potential by changing the transmembrane potential during the shock. Finally, the subcellular level is considered, which involves the response of the individual ion channels to the shock. This review gives a brief overview of some salient features of defibrillation at each of these mechanistic levels.

Prevention of action potentials during extracellular electrical stimulation of long duration.

INTRODUCTION: This study investigated if action potentials can be prevented by electrical field stimuli of long duration. METHODS AND RESULTS: The transmembrane potential was recorded by a double-barrel micro-electrode during field stimulation given across a papillary muscle from 10 guinea pigs. After 10 stimuli (S) with a 200-msec S-S interval, a 400-msec square wave shock was given just before or after the end of the effective refractory period following the 10th stimulus through electrodes 1 cm on either side of the papillary muscles. Another two stimuli (S' and S") having the same 200-msec S-S interval were given during the shock pulse to test if the action potentials induced by these two stimuli could be prevented by the shock. The shock strength was increased until the shock field prevented the action potentials induced by the S' and S" stimuli. The resting membrane potential was -85.5 +/- 2.9 mV. For shocks causing depolarization at the recording site, the field strength required to prevent S'- and S"-induced action potentials was 1.5 +/- 0.4 V/cm, which depolarized the transmembrane potential to -55.3 +/- 8.9 mV and -58.1 +/- 7.2 mV from the resting membrane potential at the time of the S' and S" stimuli, respectively. The strength of shocks causing hyperpolarization required to prevent S'- and S" -induced action potentials was 5.0 +/- 0.8 V/cm, which hyperpolarized the transmembrane potential to -105 +/- 6.5 mV and -115.6 +/- 6.9 mV from the resting membrane potential at the time of the S' and S" stimuli, respectively. CONCLUSION: Both depolarization and hyperpolarization caused by an electrical field can prevent action potentials.

Comparison of PCR and plaque assay for detection and enumeration of coliphage in polluted marine waters.

A total of 68 marine samples from various sites impacted by sewage and storm waters were analyzed by both the plaque assay and a reverse transcriptase (RT) PCR technique for F(sup+)-specific coliphage. The coliphage levels detected by the plaque assay averaged 1.90 x 10(sup4) PFU/100.0 ml. Using a most probable number (MPN) PCR approach, the levels averaged 2.40 x 10(sup6) MPN-PCR units/100.0 ml. Two samples were positive by RT-PCR but negative by plaque assay, and 12 samples were positive by plaque assay but negative by RT-PCR (levels lower than 11.00 PFU/100.0 ml). The host system used for the plaque assay may detect somatic coliphage in addition to the F(sup+)-specific coliphage. When it is used as an indicator of pollution, contamination may be missed with more restrictive systems. The difference in results may be due to the sensitivity, specificity, or inhibition of RT-PCR in marine samples. This study provides information on quantifying PCR results by an MPN method and insights into interpretation of PCR data for detection of viruses in marine environments.

Isolation and characterization of an attenuated strain of Pseudomonas aeruginosa AC869, a 3,5-dichlorobenzoate degrader.

Pseudomonas aeruginosa AC869, a 3,5-dichlorobenzoate degrader, is a mouse pathogen and has a reported 50% lethal dose (LD50) of 1.05 x 10(7) CFU when given intranasally to C3H/HeJ mice (S.E. George, M.J. Kohan, M.I. Gilmour, M.S. Taylor, H.G. Brooks, J.P. Creason, and L.D. Claxton, Appl. Environ, Microbiol. 59:3585-3591, 1993). AC869 was serotyped as O6 when grown in CD-1 mouse cecal and lung mucus but could not be assigned an O serotype when grown in Luria broth (LB). After growth in mouse cecal mucus, a less virulent mutant, AC869-11, was isolated from AC869 by using bacteriophage E79, which adsorbs to the O side chain of lipopolysaccharide (LPS). AC869-11 produced significantly less O antigen on its LPS than AC869 when grown in mouse lung and cecal mucus. The mutant also produced half the amount of exoenzyme S and 16-fold less extracellular protease than AC869 and was more sensitive than its parent to a number of antibiotics when grown either in LB or in mouse lung mucus. AC869-11 had ninefold higher LD50 than AC869 in CD-1 mice when administered intranasally. AC869-11 was found in the lungs, small intestine, cecum, and large intestine in numbers at least 100-fold below AC869, 3 h after intranasal exposure of mice to a sublethal dose of the two strains. Moreover, AC869-11 induced a decreased pulmonary inflammatory response relative to AC869. In contrast to AC869, AC869-11 did not translocate to the mesenteric lymph nodes, liver, and spleen following a sublethal dose. Despite attenuation, AC869-11 grew as well as AC869 with 3,5-dichlorobenzoate as the sole carbon and energy source. However, although AC869-11 survived in 3,5-dichlorobenzoate-contaminated soil as well as AC869 for 1 week, it failed to survive as well thereafter. These results suggest the possibility that mutations that lead to pulmonary attenuation of P. aeruginosa in mice also lead to weakness in the environment, despite such mutants maintaining the ability to degrade toxic substances under laboratory conditions.

Plasma membrane calcium ATPase isoform expression in cultured rat mesangial cells.

The maintenance of intracellular ionized calcium (iCa2+) in the submicromolar range is important for mesangial cell (MC) function, and, as in most mammalian cells, plasma membrane Ca(2+)-ATPases (PMCA) play an important role in the homeostatic process. Molecular studies have demonstrated four PMCA isoforms, each with multiple splice variants. The present study examines the expression of PMCA isoforms and calmodulin-binding region splice variants in cultured MC from Sprague-Dawley rats and from spontaneously hypertensive rats (SHR) and Wistar-Kyoto (WKY) rats before and after the onset of hypertension in SHR. Using reverse transcription-polymerase chain reaction (RT-PCR) and Southern blot analyses, we have demonstrated PMCA1, -3, and -4, but not PMCA2, to be present in MC from these rat strains. Splice variant analysis revealed PMCA1a and -1b, PMCA3a, -3b, and -3c, and PMCA4a and -4b to be expressed in MC from all three strains. The relative quantities of PMCA1 and PMCA4 mRNA were not different in age-matched SHR vs. WKY rats, correlating with similar iCa2+ measurements. The expression of all three isoforms declined with age in SHR and WKY.

Expression of insulin-like growth factor binding protein-2 during postnatal development of the rat neurointermediate pituitary.

The expression of insulin-like growth factor binding protein-2 (IGFBP-2) during postnatal development of the rat neurointermediate pituitary was characterized using immunocytochemistry and in situ hybridization. Glial cells in the neural lobe (NL) showed robust expression of IGFBP-2 throughout the postnatal period and continuing into adulthood. These were identified as pituicytes by the presence of S-100 protein; IGFBP-2 was not present in microglia in the NL. IGFBP-2 immunoreactivity was more punctate in mature pituicytes, suggesting a possible association with the cell membrane. No expression of IGFBP-2 by brain astrocytes was observed at any age examined, even in regions such as the median eminence and subfornical organ that lack a blood-brain barrier. IGFBP-2 was also localized immunocytochemically in melanotropes and glial-like cells of the intermediate lobe (IL). Positive cells were most numerous in neonates and declined thereafter, with immunoreactivity undetectable by 65 days of age. IGFBP-2 mRNA was detected in the IL only at 1 day of age. These findings are consistent with a potential role for IGFBP-2 in modulating effects of the insulin-like growth factors during development of the neurointermediate lobe. The constitutive expression of high levels of IGFBP-2 by mature pituicytes also suggests that this protein could be secreted and/or influence a variety of processes in the mature NL, such as glial proliferation, axonal growth, or morphological plasticity of pituicytes.

A home electrocardiography and blood pressure telemonitoring system.

A home electrocardiography (ECG) and blood pressure telemonitoring system for cardiac patients was installed in the Macau region. The monitoring centre was established in the emergency unit at the Government Hospital of Macau. The first users were 10 cardiovascular patients selected by a physician. The average age of these users was 61 years (range 30-78). The results of a three-month trial showed that the system was easy to operate and technically reliable. It was found to be helpful for cardiac patients. The most significant problem during the trial was electrical noise from the ECG electrodes.

[A study on expression of fibronectin receptor alpha 5 beta 1 of Ito cells in experimental liver fibrosis in rats].

OBJECTIVE: The study was designed to study the role of fibronectin (FN) receptor alpha 5 beta 1 in liver fibrogenesis. METHODS: Northern blot analysis and immunohistochemical techniques were used to observe the in vivo and in vitro changes in the expression of FN and its receptor alpha 5 beta 1 in CCl4 induced rat liver fibrosis in Ito cells. RESULTS: (1) alpha 5 beta 1 was mainly detected in the endothelia and some of the desmin (DM) positive cells (Ito cells) of the sinusoids in normal rat liver. The expression of alpha 5 beta 1 of DM positive cells in the experimental groups was enhanced and reached their peaks by the 10th week. The changes in FN was similar to that of alpha 5 beta 1; (2) The expression of FN, alpha 5 and beta 1 mRNA of the liver in the experimental groups detected by Northern blot analysis was increased and reached their peaks by the 6th week. The content of these mRNAs of the cultured Ito cells isolated from the experimental group was increased more than that from normal liver specimens. CONCLUSION: Ito cells expressed FN receptor alpha 5 beta 1. The activation of Ito cells resulted in the increase of the expression of the three mRNAs during fibrogenesis. It is suggested that the detection of gene transcription of FN and its receptor mRNAs by Northern blot analysis could reveal the activation and proliferation of Ito cells and thereby provides a sensitive signal of liver fibrosis.

[Role of endothelial-derived nitric oxide and its synthase in the development of hypoxic pulmonary hypertension in rat].

To clarify the role of endothelial-derived nitric oxide (EDNO) and its synthase (NOS) in the normal and hypertensive pulmonary vasculature, activity of endothelial NOS in the lungs, ENDO-dependent vasodilating response induced by bradykinin (BK), and cGMP content of lung tissue in normoxic and hypoxic rats were investigated. We also studied the effects of NOS inhibitor-L-NAME on the activity of NOS, cGMP content, mean pulmonary arterial pressure (mPAP) and carotid systolic arterial pressure (CAPs) in both rats. The results were as follows (1) In normoxic rats there was no NOS activity in the endothelium of small vessels (phi < or = 80 microns) and no relaxing response to BK. Long-term administration of L-NAME obviously inhibited the activity of ecNOS and cGMP content in the lungs of normoxic rats, therefore it led to the increment of CAPs but failed to elevate mPAP. (2) After hypoxic exposure for 10 days, NADPH-diaphorase (NADPH-d and ecNOS immunoreactivity turned to be positive in the endothelium of small vessels with diameter less than 80 microns. BK-induced EDNO-dependent vasodilation, the enzyme activity of cNOS and cGMP content in the lungs of hypoxic rats were significantly enhanced as compared with normoxic rats. Long-term administration of L-NAME in hypoxic rats markedly inhibited the enhancement of cNOS enzyme activity, the production of EDNO and cGMP content in rat lungs, consequently it significantly decreased mPAP but elevated CAPs obviously. These results suggest that the role of EDNO in maintaining the low basal tone of normal adult pulmonary circulation remain to be studied more precisely. The increased activity of ecNOS and the enhancement of EDNO synthesis might act to moderate the hypertension. The excess synthesis of EDNO might be toxic to the endothelium of pulmonary vessels, therefore potentiating the development of pulmonary hypertension.

[Alterations of MDM2 and p53 genes in bone tumors].

OBJECTIVE: To determine the frequency of MDM2 and p53 genes and their possible role in the pathogenesis and development in bone tumors. METHODS: Digoxigenin labeling in situ hybridization technique was used to investigate the expression of MDM2 and p53 in 38 cases of bone tumors, including 12 osteosarcomas, 10 chondrosarcomas, 14 giant cell tumors of bone and 2 chondroblastomas. The relationship between MDM2 and p53 expressions was analyzed. RESULTS: The positive rates for MDM2 in osteosarcoma, chondrosarcoma and giant cell tumor of bone were 41.7%, 50.0% and 35.7%, while those for p53 were 58.3%, 40.0% and 21.4%, respectively. The two cases of chondroblastoma showed both MDM2 and p53 overexpression. There was a striking association between MDM2 and p53 overexpressions. CONCLUSION: MDM2 and p53 alterations are frequent events in bone tumors and may be involved in tumorigenesis or tumor progression in bone tumors.

[Effect of L-arg and SNP on pulmonary arterial pressure and vascular structural changes of chronically hypoxic rats].

The effects of L-arginine (L-arg) and sodium nitroprusside (SNP) on pulmonary arterial pressure, the percentage of muscularization of intra-acinar vessels and ultrastructural changes of extra pulmonary artery and pulmonary arteriole of chronically hypoxic rats were studied. The results showed that: (1) Both L-arg and SNP decreased mean pulmonary arterial pressure of chronically hypoxic rats significantly. (2) Both L-arg and SNP reduced the percentage of muscularization of intra-acinar vessels of chronically hypoxic rats significantly. (3) Both L-arg and SNP protected pulmonary artery from the damages of endothelium and the changes in smooth muscle cell phenotype by hypoxia. These results suggested that exogenous nitric oxide might play a role in the protection of pulmonary arterial function and structure which alleviate the development of pulmonary hypertension induced by chronic hypoxia.

[The setting up of human serum calcitonin radioimmunoassay and its clinical application].

This study deals with the setting up of human serum calcitonin radioimmunoassay (hCT-RIA) using the high titer and specific antibody which was prepared by our own laboratory. The final dilution of the antibody titer was 1:1,200,000 when the binding rate of Bo/T was 30%. The antibody's affinity constant (Kd) was 2.25 x 10(11) L/M. No cross reaction was found between the antibody and the following eight different peptide hormones and proteins, viz.: TSH, T3, T4, ACTH, PTH, BGP, Insulin and Gastrin. The coefficients variation of intra and inter assay were 3.2% and 9.0%, respectively. The mean recovery rate of CT was 99.8%. The sensitivity of this assay was 9.4 pg/ml. The examination of this methodology showed that all its indices met the demands of RIA. There was no difference in the mean value of serum CT between the two sexes in 232 normal subjects, but the serum CT concentration in those aged of sixty and over of men and women, were much lower than those aged of fourty group. Mean CT concentration was low in the patients with primary osteoporosis, but were significantly higher in those suffering from medullary thyroid carcinoma, primary hyperparathyroidism crisis and chronic renal failure.

[The frequencies of the vitamin D receptor gene alleles in adults of Han nationality in Beijing area].

Vitamin D receptor (VDR) gene is considered as one of candidate genes related to the genetic basis of bone and mineral metabolism. In order to determine VDR genotypes, we analysed the polymorphisms, or genotypes of Bsm I, Apa I, and Taq I restriction enzymes, by PCR technique in 223 unrelated healthy adults of Han nationality in Beijing area. Some of the VDR genotypes were also confirmed by Southern blotting analysis. We discovered that Chinese had high frequencies of b, a, T alleles, as high as 95%, 75%, and 95% respectively, which were different from those in Caucasians and Japanese, but similar to those in Koreans (P > 0.05). This difference gave us further insight into the different pathogeny of osteoporosis in various ethnics.
