Primary Sjögren's syndrome and its lymphoid malignancy: a report of four cases.

OBJECTIVE: To evaluate the incidence and spectrum of malignancy of primary Sjögren's syndrome (pSS). METHODS: 250 pSS who were followed-up in Peking Union Medical College (PUMC) Hospital were analyzed. RESULTS: Four of them were diagnosed with histopathological findings of 2 non-Hodgkin Lymphoma, 1 AILD, 1 multiple myeloma. Two died of secondary infection while receiving chemotherapy for lymphoma, 2 remained remitted. CONCLUSIONS: The risk factors were persistent enlargement of major salivary glands, appearance of monoclonal serum lg, and disappearance of auto antibodies.

Clinicopathological study of renal involvement in patients with systemic sclerosis.

OBJECTIVE: To assess the incidence of renal involvement in patients with systemic sclerosis (SSc) as well as its clinical and pathological changes. METHODS: The renal involvement was studied clinicopathologically in 93 patients who were compatible with the diagnosis of SSc retrospectively. RESULTS: Eighteen patients (19.4%) were diagnosed as renal involvement by one or more of the following: proteinuria, renal hypertension, elevated levels of blood urea nitrogen (BUN) and/or serum creatinine (sCr). Renal impairment was observed in 5 patients (5.4%). The mortality rate was 12.9%, and 5 patients died of renal failure. Histological study was performed in 5 patients. The thickening of interlobular arterioles with intimal proliferation was found in 4 of the patients who also showed mild nonspecific glomerular changes. Two had no clinical features of renal involvement, 1 had renal hypertension and 1 died of renal failure. Another patient with a 22-year disease duration showed chronic glomerulonephritis with nephrosclerosis. CONCLUSIONS: SSc patients should be followed-up clinically and renal biopsy performed if necessary in order to discover early renal involvement and to insert rational therapy to improve its prognosis.

[Identification of antigens shared between Schistosoma japonicum and Trichinella spiralis].

Cross-reactivity was analysed between the antigen of T. spiralis muscular larvae and antigens of cercariae, liver stage schistosomula, 30 day male and female adult worms of S. japonicum by means of EITB technique. The result showed that the antigens of cercariae schistosomula, female worms except male worms, were recognized by sera from rabbits immunized with T. spiralis antigens or by sera from T. spiralis infected rabbits. The molecular weight of recognized antigens ranges from 15 to 100 kD, mainly from 50 to 70 kD. The immunized rabbit sera and the infected rabbit sera reacted with the same cross-antigens of S. japonicum. Among these schistosome stages, the cercariae showed the most cross-reactive bands, the second were the liver stage schistosomula, and 30 day female worms only showed a weak band which located at 97 kD position. Meanwhile, the anti-S. japonicum sera recognized several T. spiralis antigens which located at 34 kD and above, especially 35-38 kD. The results suggest that there are many antigens shared between T. spiralis and S. japonicum and the main cross-reactive antigens are found in the larva stages of S. japonicum and T. spiralis muscular larva. The cross-reactive antigens are different between various stages of S. japonicum and muscular larva of T. spiralis.

[Schistosoma japonicum ferritin: cloning, nucleotide sequencing, expression, and purification].

Our previous work showed that immunization of mice with Schistosoma japonicum (Sj) immature eggs induced significant immunity against fecundity and embryonation of the parasite. The Sj adult cDNA library was screened by sera from rabbits against Sj immature egg antigen (RASjIEA). The genes encoding molecules which may induce immunity against fecundity/embryonation were chosen for further cloning and expression. First of all, RASjIEA was absorbed with E. coli lysate to remove cross reactive antibodies. The cDNA library was then immunoscreened using the routine method. The resulted positive plaques were rescreened till individual clones were confirmed. Phagemids were obtained using in vivo excision. The positive clones were amplified using PCR. The sizes of the genes were determined by agarose gel electrophoresis. After DNA sequencing of the genes cloned, Gene bank was searched and six different genes were identified from a total of 102 positive clones. One of six identified genes, Sj ferritin (SjFer) was chosen to subclone into pGMC vector. According to DNA sequences of Sj Fer and MCS (multiple cloning site) of the vector, forward primer (Fer/GMC1) and reverse primer (Fer/GMC2) were designed and used to amplify Sj Fer by PCR. The Sj Fer cDNA and expression vector pGMC were digested with BamHI and XhoI. The digested cDNA and pGMC were ligased by T4 DNA ligase to construct a recombinant which was then used to transform E. coli strain ER2566. The fusion protein GMCSF-Sj Ferritin was expressed in insoluble form, the inclusion body. Pellets were harvested and resolved in Tris-HCl buffer containing 8M urea. GMCSF-Sj Ferritin was purified by affinity chromatography using Ni-NTA resin. The molecular weight was determined by SDS-PAGE. This study first reports the gene encoding S. japonicum ferritin as a new candidate for schistosome vaccine.

[The effect of a vaccine made from 39kd hydrophobic outer membrane protein of Leptospira interrogans on neurohumoral and red cell immunity function of the guinea pigs].

A randomized control trial was conducted to determine the immunoprotective efficacy of OmpL39. 36 guinea pigs were divided into OmpL39 group, whole leptospiral cell vaccine (WLCV) group, other proteins of Leptospira group, and negative control group (normal saline, NS). The results showed that all the guinea pigs of infected OmpL39 and WLCV still survived, but all the control guinea pigs died. Immunoprotective efficacy was 100% for OmpL39 and WLCV. OmpL39 levels were similar to WLCV levels and higher than controls (P < 0.05). These suggested that OmpL39 could be used as important immunoprotective antigen to develop the genetic vaccine of the targeting delivery system. Also it was observed that OmpL39 could produce 100% immunoprotective efficacy, have higher MAT level (> 1:3200) and regulate 5-HT, 5-HIAA, DA, NE. The 5-HT, DA, NE concentration was lower and 5-HIAA was higher than that of controls after stimulating the guinea pigs with OmpL39 (P < 0.05). The results suggested that OmpL39 genetic vaccine could produce immunity function and have an active effect on absorbing inflammation and protecting organs and tissues. Besides, the red cell immunity efficacy of the guinea pigs was changed during immunity response and the RBC-C3b RR and the RBC-ICR were increased. The RFER was increased; the RFIR was decreased. RFER/RFIR was remarkably higher than that of control (P < 0.05). These suggested that OmpL39 could increase red cell immunity function.

[Schistosoma japonicum: purification and diagnostic application of recombinant 32 kD antigen].

Separation and purification of rSj32 were carried out with SDS-PAGE and electroelution technique. The purified rSj32, AWA and SEA were used to examine sera of patients with schistosomiasis, healthy individuals and patients with other parasitic diseases by ELISA. The results showed that the molecule weight of rSj32 was 37 kD. The sensitivity and specificity of rSj32 as a coating antigen in ELISA were as good as those of AWA and SEA. It is suggested that rSj32 antigen is a potential antigen for immunodiagnosis of schistosomiasis japonica.

[Determination of IL-6 in serum and perilymph during inner ear immune response].

During secondary inner ear immune responses against keyhole limpet hemocyanin (KLH), to understand the change of interleukin-6 (IL-6) levels in perilymph and serum, IL-6 levels in perilymph and serum were investigated using an immunoassay. No IL-6 levels were detectable at Day 0 in perilymph. The earliest perilymph IL-6 levels were observed after 6 h, peaking at 24 h, and then decreasing gradually. IL-6 level remnants were detected at 7 days in perilymph. In contrast, during this observation period, low IL-6 levels were only detectable in perilymph from control ears, disappearing after 72 h. IL-6 levels don't change in serum. Previous study has identified the endolymphatic sac (ES) lags behind the earlier appearance of intercellular adhesion molecule-1 (ICAM-1) in the spiral modiolar vein and spiral ligament during immune response. The present study not only provides further support for the existence of an inner ear immune response which is regulated by cytokines, but also supports that cytokines controlling expression of adhesion molecules are released probably by cells which located out of ES.

[The general survey for chondromalacia of 2,743 Chinese population].

OBJECTIVE: To evaluate the distribution of chondromalacia patella in Chinese population. METHODS: A random cluster sampling survey was performed covering 2,743 subjects varied in age, sex and occupation in 1995. RESULTS: The prevalence rate is 36.2%. The occurrence in women was higher than that in men (P < 0.01), while in the age group, 30 to 39 years was the highest being 55.8%. The prevalence rate in soldier being 47.5% was the highest among varied occupations. CONCLUSIONS: Our study is the first survey to be performed in a large number of Chinese population. This investigation may reflect the prevalence rate of chondromalacia patella in China.

[Long-term efficacy of plasma-derived hepatitis B vaccine among Chinese children--a 12-year follow-up].

OBJECTIVE: To evaluate protective efficacy of a plasma-derived hepatitis B vaccine during 12 years after vaccination. METHODS: A 12-year follow-up of 350 children who participated a randomized, double blind, placebo-controlled trial in 1982. RESULTS: During the 12 years after vaccination, more vaccinated children had anti-HBs of 10 S/N ratios or more than did the controls. At the 12 years 46.1% of the vaccinated and 33.9% of the controls retained anti-HBs levels of S/N ratios > or = 10 (P < 0.05). However, at 12 years median S/N ratio of anti-HBs of the vaccinated children with detectable anti-HBs was lower than that of the controls (15 vs 36, P < 0.0001). Protective efficacy against HBsAg carriage status at 5-10 years after vaccination varied from 83% to 100%. At 12 years, 14 chronic HBsAg carriers were identified among the controls, whereas only 1 case was noted in the vaccinated group (7.6% vs 0.6%, P < 0.01), account for an efficacy of 92%. CONCLUSIONS: Protective efficacy against chronic HBsAg carriage was satisfactory at 12 years after vaccination. No evidence was showed to support a booster of hepatitis B vaccine at this time.

[Persistence of HB vaccine immune protection and response to hepatitis B booster immunization].

OBJECTIVE: To determine the persistence of immune protection of China-made, plasma-derived HBV vaccine in infants and the time table of booster immunization. METHODS: Serological effect of 7-year follow-up study and antibody response of an experimental study on booster were detected using radioimmunoassay. RESULTS: The protective anti-HBs positive rates of 1,018 children, who were vaccinated according to the regimen of three doses of 10 micrograms HBV vaccine, declined from 75.0% during the first two years to 48.2% at 7 years after the first dosages, however, the positive rates for HBsAg and anti-HBc always fluctuated at low frequency. 144 children aged 6 or 7 years old, who received the HBV vaccine in their infancy and were negative for both HBsAg and anti-HBc before booster, were selected from 1,018 children of the follow-up study and boosted with 1 microgram (intradermally) or 2 micrograms (hypodemically) HBV vaccine. Their anti-HBs GMT and anti-HBs positive rate were 190.6 mIU/ml and 89.6% at one month after booster, significantly higher than 14.7 mIU/ml and 54.9% before booster (P < 0.01), and declined back to 25.3 mIU/ml and 75.5% at 12th month; among 65 children with the anti-HBs negative before booster, 40 of them had the level of anti-HBs > or = 100 mIU/ml one month after booster, suggesting the retention of immunological memory in most of them. CONCLUSIONS: No need for revaccination against hepatitis B at 7th year after the initial immunization due to better persistence of immune protection of the vaccine.

[Discernment of the sensitized inner ear by peripheral immunocompetent lymphocytes].

OBJECTIVE: To understand the role of peripheral immunocompetent lymphocytes during an immune response in the inner ear. METHODS: Sensitized lymphocytes from peripheral blood, neck lymph nodes and spleen from three groups of donor animals (group A was immunized intradermally with KLH and both inner ears were challenged with KLH; group B received only KLH intradermally without an inner ear challenge, and groups C were naive animals) were labelled with 51Cr and injected intravenously into recipients (all of which were immunized intradermally with right inner ears challenged with KLH). Twenty to twenty-four hours later, the temporal bones of recipients were assayed for radioactivity to detect the infiltration of labelled cells. RESULTS: In group A and B, the right temporal bones showed higher radioactivity than the left temporal bones (P < 0.05). The highest labelling was detected in the right temporal bones of group A. The lowest migration of 51Cr-labelled cells to the inner ear was found in group C animals. The highest ratios between right (challenged) and left (unchallenged) were found in group A animals compared with those of group B and group C. The value was significantly higher in group A than in group B and C (P < 0.05). CONCLUSION: The results showed that the inner ear comes under the immunosurveillance of the peripheral circulation in response to antigenic stimulation.

[Expression of intercellular adhesion molecule-1 in immune response of the inner ear].

OBJECTIVE: To understand the role of intercellular adhesion molecule-1(ICAM-1) in the immune response of the inner ear. METHODS: Inner ear immune response was induced in rats by inoculation of key-hole limpet hemocyanine(KLH) into the scala tympani of animals who had been systemically sensitized. Then the expression of ICAM-1 in the inner ear was examined by immunohistochemistry. RESULTS: ICAM-1 was found in the epithelium of the spiral modiolar vein (SMV) with its collecting venules (CV) as early as 6 hours postchallenge. Expression of ICAM-1 was observed on the epithelium of the endolymphatic sac(ES) and perisaccular region at 12 hours. The intensity of ICAM-1 staining reached a maximum by 24 or 48 hours in these sites of the inner ear. By day 28, most specimens were devoid of significant staining for ICAM-1. CONCLUSION: The study elucidates the important role of adhesion molecules in extravasation of inflammatory cells from the systemic circulation during an inner ear immune response. It also shows that cytokines controlling expression of adhesion molecules may be released by cells located outside of ES besides those cells in the ES.

[The study on the correlation between the effect of dental appliances on OSAS and the morphology of upper airway and craniofacial structures].

OBJECTIVE: The purpose of this present study was to evaluate the effectiveness of dental appliances on OSAS by combining the use of overnight polysomnography and cephalometry. METHODS: 22 OSAS adult patients wore the dental appliances designed by authors. The association between the improvement of respiratory variables and the cephalometric variables were investigated on the basis of linear correlation analysis. We also classified the subjects by AI of post-treatment to analyze the morphological disparity of upper airway and craniofacial structures among the groups. RESULTS: There was no significant correlation between the improvement of AI and the morphology of the upper airway and its surrounding soft structures, but there existed significant linear correlation between the improvement of AI and the morphology of the craniofacial skeletal structure. The improvement of the lowest SaO2 was only correlated significantly with the body length of the mandible and the anteroposterior position of the base of tongue. However, the improvement of the RDI wasn't associated significantly with any morphological variables. In addition, there were morphological differences among the different groups. CONCLUSION: The above findings suggested that the effectiveness of dental appliance on OSAS is mainly associated with craniofacial skeletal structures, but no correlation existed between the sagittal dimension of upper airway and the effectiveness.

[Gliosis: follow-up and pathological study of 34 cases].

OBJECTIVE: To discuss the etiology, clinical characteristics, pathology and canceration in gliosis. METHOD: 34 cases of gliosis were analyzed retrospectively follow-up for a long period. Their pathologic slices were restudied. RESULT: The causes of gliosis included cranial infection, specially infection caused by virus without distinct symptoms cerebral ischemia, brain injury, radiological treatment of brain. most cases showed symptoms of intracranial hypertension, 55.9% of the patients accompanied with epilepsy. Pathological examination revealed proliferation of small gliosis in all cases, lligodendrogliosis in 79.4% of the cases. The cases with proliferation had better rehabilitation. Canceration was noted in 11.8% of all cases. CONCLUSION: Gliosis is a nonmalignant disease, however, it has the possibility of canceration.

[Cross-protection against Schistosoma japonicum infection in mice immunized with Trichinella spiralis muscle larva antigen].

AIM: To explore the cross-protection against S. japonicum cercaria challenge in mice immunized with T. spiralis larva antigen. METHODS: Groups of BALB/c mice were immunized with 4 preparations of T. spiralis larva antigen, respectively. Vaccinated mice were then challenged with 30 and 100 S. japonicum cercariae, respectively. Forty-five days later, all mice were sacrificed and examined for adult worm burden, liver and stool egg count. RESULTS: All groups of mice immunized with 4 preparations of T. spiralis larvae antigen without adjuvant showed significant protection against S. japonicum. Among these antigen preparations, the soluble antigen (TsLSA) presented highest protection with reductions in worm burden and liver and stool egg counts of 21.3%, 48.0% and 58.5%, respectively. When mice were immunized with TsLSA plus Freunds complete adjuvant (FCA) or with double-dose of the antigen, the worm reduction rate was increased to 29.3% and 39.6%, respectively. CONCLUSION: Different preparations of T. spiralis muscle larva antigen could induce protection against S. japonicum in mice.
