High frequency loss of heterozygosity in the region of the DBH locus in bladder cancer.

OBJECTIVE: To determine the location of a putative tumor suppressor gene (TSG), and evaluate the frequency loss of heterozygosity (LOH) of the long arm of chromosome 9 (9q) in bladder cancer. METHODS: We analyzed 25 patients with bladder cancer for LOH of 9q using 25 high-density microsatellite markers. RESULTS: Twenty-three samples (92%) showed LOH at least at one locus on 9q. We identified that the commonly deleted region were at 9q12-q21, 9q22, and 9q34. The rate of LOH was 44.0%, 22.7%, 22.7%, 16.0%, 12.0% on DBH, D9S15, D9S1815, D9S1831, D9S176 locus, respectively, and was not significantly related with grades and stages of tumor. CONCLUSION: These data suggest that alteration of a TSG at DBH of 9q may play an important role in the development of bladder cancer.

Chinese-made electrolytic detachable coil in the embolization of a dog aneurysmal model.

OBJECTIVE: To develop a Chinese-made electrolytic detachable coil (CEDC) that would allow patients to enjoy the benefit of electrolytic detachable coil without having to rely on the expensive Guglielmi detachable coil (GDC), and to test the safety and efficiency of CEDC. METHODS: Radiopacity of the marker of the CEDC was tested. The anti-strain force of coils and connection points were measured. In vitro and in vivo electrolyses were conducted. We produced 21 aneurysm models including lateral, bifurcation, and terminal aneurysm models, and embolized 18 models with CEDC. RESULTS: Radiopacity of the marker was satisfactory. Anti-strain of the coil and the connection point have shown the safety and efficiency of CEDC. Electrolysis in vitro and vivo, was similar to that of GDC. 83% of the aneurysm models were completely occluded. Coils were found to induce thrombosis in the aneurysm cavity, and could lead to a new endothelium over the entrance of the aneurysm, with organized tissue replacing thrombosis in 2 months. CONCLUSIONS: CEDC is as efficient and safe as GDC. The marker, the ability to induce thrombosis inside the sac and the growth of new endothelium over the orifice are very satisfactory.

[Chemoprotection of transfer of multidrug resistance gene into human hematopoietic progenitor cell].

OBJECTIVE: To observe the effect of the transfer of multidrug resistance gene (mdr1) into human hematopoietic progenitor cells (HPC) on the chemoprotection. METHODS: Human CD34+ cells served as a target of mdr1 gene transfer. Retroviral vector SF-mdr containing human total length mdr1cDNA was introduced into packing cells GP-envAM12 by liposome-mediated transfection. The mdr1 gene was transduced into human CD34+ cells by retroviral supernatants of packing cells. The integration and expression of the mdr1 gene and its protein (P170) in transduced cells were determined by PCR, RT-PCR, and flow cytometry. The drug resistance of chemotherapy in transduced HPC was determined by culturing colonies. RESULTS: The mdr1 gene was integrated and expressed in transduced CD34+ cells. The efficiency of mdr1 gene transfer was 10%-14%. Compared with untransduced controls, within a certain range of drug concentration, the number of drug-resistant colony in transduced HPC for taxol, doxorubicin, VCR and VP16 were increased by 3.6 +/- 2.1 fold, 2.9 +/- 0.3 fold, 1.9 +/- 0.4 fold, and 3.5 +/- 0.5 fold, respectively. CONCLUSION: The transfer of the mdr1 gene into human HPC can increase the drug resistance of the transduced cells to corresponding chemotherapeutic drugs that may provide some degree of chemoprotection for HPC.

[The effect of Nifedipine on postpartum blood loss in patients with pregnancy induced hypertension].

OBJECTIVE: To determine the efficacy of Nifedipine on postpartum blood Loss in patients with Pregnancy Induced Hypertension (PIH). METHODS: 64 cases of PIH were randomly divided into study group (n = 32) and control group (n = 32). Before labor Nifedipine 10 mg-20 mg was given three time daily for all cases. In study group Nifedipine was taken orally every 6 hours during the labor course until the end of second stage. In control group Nifedipine was not given in the labor stages. The postpartum blood loss was collected and measured carefully within 2 hours after delivery. RESULTS: The amount of pastpartum hemorrhage was (359.1 +/- 136.6) ml in study group which was significantly higher than that of control group (268.5 +/- 110.7) ml P < 0.01. The postpartum hemorrhage rate in study and control group was 43.75% and 18.75% respectively. The difference was statistically significant (P < 0.05). CONCLUSION: During labor using Nifedipine for PIH cases can increase the amount of postpartum blood loss and rate of postpartum hemorrnage.

[Evaluation on the compliance of livial therapy in peri and postmenopausal women].

OBJECTIVE: To evaluate the compliance of livial therapy in peri and postmenopausal women. METHODS: One hundred thirty four peri and postmenopausal women advised to take Livial were studied for persistency, efficacy and side effect of treatment. RESULTS: The mean age is 55.2 years old and post menopausal period is averaged 6.7 years. Their chief complaints were mainly climacteric symptoms(97.0%). Ninety percent of women took livial after prescription. 41.0% of women ceased the therapy after (0.7 +/- 0.8) years of treatment, while 26.2% of them restored after (0.3 +/- 0.2) years due to recurrence of symptoms. The major reason of cease was fear of cancer. The climacteric symptoms were well relieved while body mass index remained unchanged. The occurrence of vaginal bleeding, breast tenderness, edema and gastrointestinal symptoms was 5.6%, 19.7%, 8.2% and 9.0% respectively. The side effect was dose dependent. CONCLUSION: Postmenopausal women had well compliance to Livial despite of mild side effects and ceasing ratio.

[Detecting Epstein-Barr virus in nasopharyngeal carcinoma tissue using in situ polymerase chain reaction].

OBJECTIVE: To develop a sensitive non-isotopic method for in situ detection of Epstein-Barr virus (EBV) in paraffin-embedded section of nasopharyngeal carcinoma (NPC) tissue. METHODS: The EBV BamHI-W fragment labeled with digoxigenin as probe was used to perform in situ hybridization and PCR in situ hybridization (in situ PCR). RESULTS: The method of in situ PCR for the detection of EBV in NPC tissue was proved to be much more sensitive than in situ hybridization. CONCLUSION: Hybridization in situ of PCR product is useful to detect EBV in NPC tissues.

[Prognostic value of serum CA125 level in patients with advanced non-small cell lung cancer].

OBJECTIVE: To judge the prognostic value of serum CA125 level in patients with advanced non-small cell lung cancer. METHODS: Sixty-six untreated patients with advanced non-small cell lung cancer (NSCLC) confirmed histologically were studied. They received two to four cycles of chemotherapy, some of them combined with radiotherapy. All these patients were assayed for serum CA125 before treatment. The cut-off value of serum CA125 level was 35 U/ml. RESULTS: Increased serum CA125 levels were observed in 24 out of 66 patients (36.4%). Patients with increased serum CA125 levels had an average survival rate of 12.5% at 1 year and 0 at 2 years, whereas that of patients with normal serum CA125 levels was of 57.1% at 1 years, 14.3% at 2 years and 7.1% at 3 years. Cox proportion hazard multivariate analysis showed that the prognosis of patients was related to serum CA125 levels (P = 0.000) and effects of treatment (P = 0.046). CONCLUSION: CA125 can be used as an independent prognostic parameter in advanced NSCLC.

[Correction of dento-maxillofacial deformities by using both of orthodontics and orthognathic surgery].

OBJECTIVE: To study the clinical results and stability after correction of dento-maxillofacial deformities by using both of orthodontics and orthognathic surgery. METHODS: A group of 45 patients with dento-maxillofacial deformities were corrected by preoperative orthodontics-orthognathic surgery-postoperative orthodontics procedure. As a control, another group of 64 patients with the deformities were corrected by orthognathic surgery only. The facial appearance, occlusion and stability of two groups after correction were investigated and compared. RESULTS: In orthodontics and orthognathic surgery group, 62.2% of patients declared that their mastication were improved. Relapse rate of deformities was 4.4%. In orthognathic surgery group, 42.2% of patients declared that their mastication were improved. Relapse rate of deformities was 9.4%. CONCLUSIONS: Using both of orthodontics and orthognathic surgery to correct dento-maxillofacial deformities is a good approach to improve results of correction, keep balance of occlusion and reduce the relapse of the deformities.

[Comparison of effect of the mandible setbacked by SSRO and IVRO on gnathostomatic system].

OBJECTIVE: To compare the effect of the mandible setbacked by sagittal split ramus osteotomy (SSRO) and intraoral vertical ramus osteotomy (IVRO) on gnathostomatic system. METHODS: 27 cases with mandibular prognathism were treated, 16 received SSRO and 11 IVRO. Their occlusal force and masticatory efficiency were measured preoperatively and at 3 and 6 months after operation. The condylar positions were assessed by radiography before surgery and at 6 months postoperatively. RESULTS: In three months postoperatively, the occlusal force and masticatory efficiency of IVRO group was decreased more than that of SSRO group. At six months postoperatively, the occlusal force and masticatory efficiency of SSRO group recovered quicker than IVRO group and better than that of before operation. In IVRO group, the occlusal force approached the preoperative level, the masticatory efficiency was better than preoperative value. 26/32 (81.2%) condyles were relatively retropositioned in SSRO group, 14/22 (63.6%) condyles were relatively forward positioned in IVRO group. CONCLUSIONS: The function recovery time after surgery was shorter in SSRO group. The masticator muscles were detached in varying degree and the bone segments healed slowly, the effect on gnathostomatic system was greater and recovery time after surgery was longer in the IVRO group.

[Experimental study on effect of ligustrazine on peritoneal ultrafiltrative function in rats].

OBJECTIVE: To study the effect of ligustrazine on peritoneal transport in various dosages. METHODS: Forty SD rats received intraperitoneal injection of 25 ml 4.25% glucose dialysis solution (GDS) or GDS plus 40 mg/L ligustrazine (GDS-LL) or GDS plus 160 mg/L ligustrazine (GDS-HL) daily, for 45 days. Then a 4-hour dwell study was performed in each rat to evaluate the peritoneal transportation. RESULTS: The drainage volume at 4 hours was significantly lower in all the dialysis groups as compared to the control group, especially in the GDS-HL group. However, drainage volume was significantly higher in the GDS-LL group as compared to the GDS and GGS-HL groups. There were no significant differences in the peritoneal fluid absorption rate among the 3 dialysis groups. The fluid absorption rate and the direct lymphatic absorption rate were all significantly higher in the 3 dialysis groups as compared to the control group. In the GDS-LL group, the ratio of urea level in dialysis solution and in plasma (D/Purea) was not different in comparing with that in the GDS group, but the ratio of glucose concentration in dialysis solution and that in unused dialysis solution (D/D0), and urea clearance were significantly higher than those in the GDS group and the GDS-HL group. D/Purea increased in the GDS-HL group significantly. CONCLUSION: Low dose of ligustrazine adding to the dialysis solution could reduce the peritoneal damage caused by the hypertonic dialysis solution, increase the ultrafiltration function and dialysis function of peritoneum. However, high dose of ligustrazine does not display any beneficial effect, it increases the permeability of peritoneum.

[Clinical and experimental study on effect of rhubarb on gastrointestinal blood flow perfusion].

OBJECTIVE: To study the effect of rhubarb on gastrointestinal (GI) perfusion in critical illness and hemorrhagic shocked rats. METHODS: Clinical study: Sixty-four septic patients suffered from stress ulcer were treated with rhubarb at a dose of 25 mg/kg, 25 non-septic patients were taken as control. The GI perfusion was evaluated by intramural pH. Animal study: SD rats were anesthetized with intraperitoneal sodium pentobarbital at a dose of 20 mg/kg. Then the animals were blood-letting, the blood pressure reduced to 5.32 kPa and maintained for 120 minutes. They were resuscitated at the end of shock by reperfusing all the shed blood. The rats were divided into four groups: normal control, shock group, treated group (shocked rats were treated with 50 mg/kg rhubarb by the end of shock), and rhubarb group (normal rats were treated with rhubarb). Laser Doppler determination was applied to estimate the GI perfusion. RESULTS: Clinical study: The pH in septic patients was much lower than that in the control, whereas rhubarb could obviously elevate GI pH (P < 0.01). In addition, rhubarb also had a good effect on gastric hemorrhage caused by stress ulcer, the effectiveness was 73.4%. Animal study: Although the shocked rats were resuscitated completely, their GI perfusion was much lower than that in the control. Rhubarb could significantly improve the mucous membrane of GI and mesenterium perfusion (P < 0.01). Furthermore, rhubarb also increased the GI perfusion in normal rats. CONCLUSION: The rhubarb could improve the GI blood perfusion in critical illness and shocked rats.

[Exercise induces increased CLUT4 gene expression and protein content in diabetic rats].

OBJECTIVE: To investigate the effects of exercise on insulin receptor, glucose transporter 4 (GLUT4) gene expression and protein content in skeletal muscle cells of streptozotocin-induced diabetic rats. METHODS: All rats were randomized into three groups: normal, diabetic and diabetic exercise groups. Diabetic exercise rats were swim-trained for 6 weeks. The GLUT4 mRNA of skeletal muscle was determined with dot blot and GLUT4 protein was detected by Western blotting. RESULTS: After 6 weeks of exercise training, plasma glucose level of diabetic rats decreased from 18.5 mmol/L +/- 1.9 mmol/L to 14.0 mmol/L +/- 3.3 mmol/L (P < 0.01). The dot blot revealed that GLUT4 mRNA decreased by 54.9% in skeletal muscle cells of diabetic rats compared with the normal rats. The Western blotting showed that GLUT4 protein of skeletal muscle cell in diabetic rats decreased by 30.7% compared with the control rats. GLUT4 mRNA and the protein of skeletal muscle in diabetic exercise group increased by 56% and 60.8% respectively as compared with the diabetic rats. However, the maximum binding of insulin receptor of skeletal muscle cells in diabetic exercise group decreased significantly compared with diabetic group. CONCLUSION: The weaker expression of GLUT4 gene and the lower level of GLUT4 protein may be one of the causes of hyperglycemia in diabetic rats. Exercise training may raise the GLUT4 gene expression and the amount of GLUT4 protein in diabetic rats, contributing to the main mechanism of improved post-receptor insulin resistance and decreased hyperglycemia in diabetic rats.

[Construction and expression of single-chain Fv antibody against human VEGF].

OBJECTIVE: Binding specificity of a monoclonal antibody (MAb) (gamma. k 2b) E11 which can react with the peptide of human VEGF was determined by ELISA and immunohistochemistry. The E11 primarily proved to be potential activity of suppressing tumor growth in vivo. The construction and expression of engineering antibody (ScFv) could widen application of this MAb in both diagnosis and treatment of human cancer. METHODS: From the hybridoma cell line secreting MAb against VEGF, total RNA was prepared and used as a template for cDNA synthesis and cloning. Reverse transcription and polymerase chain reaction (RT-PCR) were used to clone the immunoglobulin variable region gene (VH, VL). The cloned VH and VL were linked by well documented flexible peptide bridging sequence (GGGGS) 3 in the VH-linker-VL orientation. The constructed ScFv gene was inserted into pET-15b and expressed in E.coli BL21 (DE3). Biding activity of ScFv was evaluated by ELISA and immunohistochemical methods after its denature and renature treatment. RESULTS: The sequence analysis revealed that the full length of VH was 369 bp, and VL was 333 bp. According to Kabat classification, VH and VL are the member of mouse Ig variable gene heavy chain subgroup II (A) and light chain subgroup III, respectively. The ScFv was highly expressed in the form of inclusion bodies which activity was recovered after being renatured. Immunohistochemical results showed that ScFv retained almost the same antigen affinity and specificity as its parent monoclonal antibody. CONCLUSIONS: Recombinant antibody ScFv is potential in both diagnostic and therapeutic application of human malignant tumors.

[Separation of thoracopagus conjoined twins].

OBJECTIVE: To sam up the experience in diagnosis and separation of a thoracopagus conjoined twins. METHODS: Thoracopagus twin boys were admitted for evaluating possible separation and repair at age of 21 days. X-rays, CT, MRI, and ultrasound scan of torso showed separate and normal gastrointestinal and biliary tracts, little shared vasculature, but shared pericardium, diaphragm and liver parenchyma. The chest junctions were at the sternums and the most inferior two pairs of the ribs. One twin (twin A) had a protruding heart with VSD and ASD. RESULTS: The infants were separated successfully at age of 45 days. The chest defect of twin A was closed with aid of a polyester pericardial patch graft and a silicon supporter. Managing anesthesia met some difficulty in understanding of the anatomical and physiological consequences of the complex anomaly. They are had brain edema and multiple system organ failures after separation, and were cured under intensive care. They are alive and well 6 months after surgery. CONCLUSIONS: Careful preoperative evaluation proper anesthesia and surgical technique, and postoperative care are essential to the successful separation of the conjoined twins.

[Microcirculation of thoraco-acromial flaps: monitoring and clinical application].

OBJECTIVE: To monitor the microcirculation of thoraco-acromial flaps. METHODS: The microcirculation in 12 thoraco-acromial flaps was monitored continuously by laser Doppler flowmetry (LDF). RESULTS: The microcirculatory changes in thoraco-acromial flaps were divided into two phases: rapid rising (2 - 4 days after operation) and plateaurising (4 - 7 days after operation). PU values were 7.4 +/- 2.0, 7.6 +/- 2.3 and 7.3 +/- 1.6. CONCLUSIONS: In division of the thoraco-acromial flaps within the plateau, the value (PU) returned to or exceeded 50% of that before application of the tourniquet. All flaps survived. The time of division was decreased to 6 - 8 after operation days.

[Simultaneous nasal reconstruction and facial defect repair using expanded forehead flap].

OBJECTIVE: To improve the method for the repair of facial defect using expanded forehead flap. METHODS: 10 patients were treated with the expanded forehead flap for total nose reconstruction and repair of facial defects. The expanded flap was divided two parts: the supratrochlear vessels for nasal reconstruction and the frontal branch of the superficial temporal vessels for facial defects. The periorbital or zygomatic area, the upper or lower lip. The defect of the forehead donor site was directly closed. RESULTS: Nasal reconstruction and repair of facial defects were satisfactory in all patients. CONCLUSION: Nasal reconstruction and simultaneous repair of facial defects extend the scope of use of expanded forehead flap.

[Hyperpolarized cardiac arrest with ATP-sensitive potassium channel opener on myocardial protection during CPB].

OBJECTIVE: To investigate the myocardial protective effects of pinacidil-induced hyperpolarized arrest and compare them with those induced with depolarized hyperkalemic arrest. METHODS: 18 dogs were equally divided into three groups. In the hypothermic hyperpolarization group (LH group), after aortic cross-clamping, a single dose of 4 degrees C pinacidil containing St. Thomas cardioplegic was infused through the aortic root. Temperature during CPB was kept between 26 - 28 degrees C and warmed to 37 degrees C before aortic declamping. Global ischemia lasted 60 min and then reperfusion started for 30 min. In the normothermic hyperpolarized group (WH group), the same procedure was set as in the LH group, except maintaining temperature of 35 - 37 degrees C for CPB and pinacidil solution. In the control group (group C), no pinacidil in St. Thomas solution was the only difference to the other 2 groups. Cardiac arrest and its recovery, the ultrastructure of the myocardium and hemodynamic during ischemia and after reperfusion were observed in the 3 groups. RESULTS: (1) The percentages of normal mitochondria and glycogen were not changed significantly during ischemia and after reperfusion in the LH group, but declined markedly in the group C at ischemic 30, 60 min, and reperfusion for 20 min (P < 0.01). In the WH group, they were lower than those of the group LH, but higher than those of the group C before ischemia. (2) The recoveries of CO, SV, CI, LVSW, RVSW, MAP in the LH group were significantly better than those in the other two groups after reperfusion for 15 minutes and 30 minutes (P < 0.05 or 0.01). However, they were much better in the WH group than in the group C (P < 0.05 or 0.01). (3) The time from cardioplegic infusion to cardiac arrest was shorter in the group C and the group LH than in the group WH. CONCLUSIONS: Myocardial protection for global ischemia during CPB could be well achieved with hyperpolarized cardiac arrest induced by ATP-sensitive potassium channel opener, pinacidil, especially in the hypothermic state. The protection is weaker in normothermia but is still stronger than that with traditional depolarized arrest.

[A serological investigation of Legionella infection in eight-species of poultry and domestic animals in Sichuan province].

OBJECTIVE: To understand the prevalence rates of Legionella infection in poultries and domestic animals in Sichuan province. METHODS: Serological investigation of antibodies against Legionella pneumophila serogroups 1 - 14 and Legionella micdadei was carried out, using microagglutination test (MAT) in eight species of poultries and domestic animals. RESULTS: Infections with multiple serogroups were identified. However, each species had its own major serogroup with positive rates of different serogroups ranging from 0.83% to 59.18%. Positive rates of the poultry and domestic animals were accounted for 7.73% and 4.25% respectively. CONCLUSION: It was suggested that poultries and domestic animals were widely infected with Legionella. It is important to carry out epidemiological surveillance in these domestic animals, poultries and environment for a better control program of this disease.
