[Comparative pharmacokinetic study of two different ways of using L-asparaginase as the main drug in combination chemotherapy].

OBJECTIVE: To search for a safer and more effective way of using L-asparaginase as the main drug in combination protocol. METHOD: Twenty children with lymphoid malignancies were randomly divided into two groups: Group A and Group B, for L-asparaginase therapy. Group A: Leunase 6,000 KU/m2, continuous intravenous injection every day for 8 times; Group B: Leunase 6,000 KU/m2, continuous intravenous injection every other day for 8 times. Blood samples were collected every day for group A and every other day for group B and serum L-asparaginase and L-asparagine levels were tested. RESULTS: 1. The pre-treatment mean value of serum L-asparaginase in group A was significantly higher than that in group B (P < 0.01). The levels of L-asparaginase in group A tended to increase day by day during the therapeutic course while those in group B without increasing tendency. 2. The L-asparagine levels and their dynamic changes were not significantly different between the two groups (P > 0.05). CONCLUSION: The administration of Leunase by continuous intravenous injection every other day is a much safer and effective protocol and worthy of recommendation.

[Experimental study on the osteoradionerosis of jaw model of dogs with vascular corrosion casting].

OBJECTIVE: To study pathologic alterations of mandibular microvascular system in osteoradionecrosis of jaws (ORNJ) in order to disclose its pathologic mechanism. METHODS: The right maxillofacial regions of 10 dogs were exposed to cobalt-60 radiation with a total dose of 60-80 Gy and dental extraction were performed at 3, 6 months after radiation. The ORNJ was diagnosed by gross observation, X-ray film and histopathologic examination. Then, the vascular corrosion castings of jaws were implemented and investigated. RESULTS: At 12 months after radiation, 7 cases of ORNJ occured. It was discovered on the casting specimen that the continuity of inferior alveolar arteries remained intact except one case where the artery was obliterated due to lesion involvement. There were defects of vascular networks in the necrotic area with irregular capillaries proliferating in the periphery of lesions and around the inferior alveolar arteries, where x-ray examination showed new bone formation. CONCLUSION: Based on the experimental evidences, a new concept of pathogenesis of ORNJ is put forward: following the formation of "3-hypo" structures, local trauma-repairing process might induce capillary proliferation which bring about a large amount of mitotic death of endotheliocytes, the microcirculation failure takes place leading to ORNJ eventually.

[Alteration of blood glucose, insulin and lipid in the offsprings of patients with essential hypertension].

We prospectively investigated the serum insulin, glucose and serum lipoproteins in 40 normotensive offsprings(test group) of parents with essential hypertension compared with 35 normotensive offsprings (control group) of the normotensive families. Compared with control group, the test group was significantly elevated in fasting serum insulin levels(6.82 +/- 2 vs 4.02 +/- 1.7 microU.ml-1) (P < 0.05), fasting serum glucose(4.32 +/- 0.4 vs 3.84 +/- 0.5 mmol.L-1), serum total triglycerides(1.97 +/- 0.8 vs 1.32 +/- 0.5 mmol.L-1), total cholesterol(Tch) (5.13 +/- 1.0 vs 4.24 +/- 0.6 mmol.L-1), low density lipoprotein cholesterol (2.82 +/- 1.0 vs 2.24 +/- 0.7 mmol.L-1), very low density lipoprotein cholesterol (1.02 +/- 0.4 vs 0.66 +/- 0.27 mmol.L-1), high density lipoprotein cholesterol/Tch ratio (0.27 +/- 0.08 vs 0.33 +/- 0.07 mmol.L-1). While post-glucose load the serum insulin and glucose level after glucose load were much higher (P < 0.01) in the test group than that in controls. The findings demonstrated that young normotensive offsprings of parents with hypertension had excellent health, they had an impairment of insulin-mediated glucose disposal, hyperinsulinemia, and dyslipidemia. These findings supported that a common hereditary defect exits in familial trait for essential hypertension and disturbance of carbohydrate and lipoprotein metabolism may be detected before or at least at very early stage of the development of essential hypertension.

[Significance of the changes of plasma endothelin, norepinephrine and atrial natriuretic peptide in treatment of heart failure with beta-blocker].

Thirty patients with chronic heart failure after conventional treatment for two weeks with cardiac diuretic and vasodilator were divided into two groups. There was addition of betaloc into Group A, but Group B kept the original treatment plan for 8 weeks. Group C consisted of 16 normal controls. The results showed that: 1. The level of plasma endothelin(ET), norepinephrine(NE), atrial natriuretic peptide(ANP) in Group A and B were higher than those in Group C. 2. The decreasing degrees of ET, NE, ANP, heart function score and the increasing degrees of CI and LVEF in Group A were more than those in Group B. 3. The change of ET was related to the changes of the others. Therefore, the beta-blocker may be associated with the decrease of the plasma concentration of ET, NE and ANP and improve heart function. It is considered that the degree of change of plasma ET may be the simple and reliable index which evaluate the degree of improving heart function.

[Changes in trace elements in epithelial cells of oral submucous fibrosis].

OBJECTIVES: To assay the concentrations of the trace elements in intranuclei and extranuclei between oral submucous fibrosis (OSF) and normal buccal membrane, to observe the relationship between the trace elements and OSF, and finally to understand the mechanism of OSF. METHODS: The concentrations of the trace elements of internuclei and extranuclei in OSF epithelial cells were assayed in computer automatic process by electron probe micro-analysis. RESULTS: The concentrations of Cu, Zn, S, and As in OSF were significantly lower than those in the normal buccal membrane (P < 0.01). The concentrations of Se and Mo were significantly higher than those in the normal buccal membrane(P < 0.01). CONCLUSION: The epithelial cells of OSF are seriously damaged by the low level of Cu and Zn and high level of Se in intracell. These changes are the element bases that OSF has canceration trend.

[Acute flare-up of senile cardiopulmonary disease and multi-system organ failure].

In this paper we report 163 cases of senile cardiopulmonary disease. Among them, 60 sustained multi-system organ failure(36.9%). Of those 60, 19 patients died(death rate 31.7%). We believe that most of the acute attacks of senile cardiopulmonary disease were induced by pneumonia. Therefore it is very important to control infection while treating respiratory-circulation failure, pulmonary encephalopathia, shock, acid-base imbalance, and other complications.

Transcriptional control of the pref-1 gene in 3T3-L1 adipocyte differentiation. Sequence requirement for differentiation-dependent suppression.

Preadipocyte factor-1 (Pref-1) is a transmembrane epidermal growth factor-like domain-containing protein highly expressed in 3T3-L1 preadipocytes, but is undetectable in mature fat cells; this down-regulation is required for adipocyte differentiation. We show here that pref-1 transcription is markedly suppressed during adipose conversion and results in decreased Pref-1 RNA levels. Using 3T3-L1 cells stably transfected with Pref-1 5'-deletion constructs truncated at -6000, -2100, -1300, -692, -300, -235, -193, -183, -170, -93, and -45 base pairs, we determined that the -183 to -170 region is responsible for the suppression of the pref-1 gene during adipogenesis. This is distinct from the -93 to -45 sequence important for pref-1 promoter activity in preadipocytes. The placement of a 40-base pair -193 to -154 pref-1 sequence containing the putative SAD (suppression in adipocyte differentiation) element upstream of the SV40 promoter decreased promoter activity by 85% upon adipocyte differentiation, compared with 40% observed with the SV40 promoter alone. The SAD element is therefore sufficient for adipocyte differentiation-dependent down-regulation of a heterologous promoter. A DNA-protein complex was observed when the -193 to -174 sequence was used with 3T3-L1 nuclear extracts in gel mobility shift assays. Competition with oligonucleotides harboring base substitution mutations identified a core sequence of -183AAAGA-179 as crucial for DNA-protein complex formation. UV cross-linking predicts that an approximately 63-kDa protein specifically binds the SAD element.

Crystal structure of JNK3: a kinase implicated in neuronal apoptosis.

BACKGROUND: The c-Jun N-terminal kinases (JNKs) are members of the mitogen-activated protein (MAP) kinase family, and regulate signal transduction in response to environmental stress. Activation and nuclear localization of JNK3, a neuronal-specific isoform of JNK, has been associated with hypoxic and ischemic damage of CA1 neurons in the hippocampus. Knockout mice lacking JNK3 showed reduced apoptosis of hippocampal neurons and reduced seizure induced by kainic acid, a glutamate-receptor agonist. Thus, JNK3 may be important in the pathology of neurological disorders and is of significant medical interest. RESULTS: We report here the structure of unphosphorylated JNK3 in complex with adenylyl imidodiphosphate, an ATP analog. JNK3 has a typical kinase fold, with the ATP-binding site situated within a cleft between the N- and C-terminal domains. In contrast to other known MAP kinase structures, the ATP-binding site of JNK3 is well ordered; the glycine-rich nucleotide-binding sequence forms a beta-strand-turn-beta-strand structure over the nucleotide. Unphosphorylated JNK3 assumes an open conformation, in which the N- and C-terminal domains are twisted apart relative to their positions in cAMP-dependent protein kinase. The rotation leads to the misalignment of some of the catalytic residues. The phosphorylation lip of JNK3 partially blocks the substrate-binding site. CONCLUSIONS: This is the first JNK structure to be determined, providing a unique opportunity to compare structures from the three MAP kinase subfamilies. The structure reveals atomic-level details of the shape of JNK3 and the interactions between the kinase and the nucleotide. The misalignment of catalytic residues and occlusion of the active site by the phosphorylation lip may account for the low activity of unphosphorylated JNK3. The structure provides a framework for understanding the substrate specificity of different JNK isoforms, and should aid the design of selective JNK3 inhibitors.

beta1 integrins are critically involved in neutrophil locomotion in extravascular tissue In vivo.

Recruitment of leukocytes from blood to tissue in inflammation requires the function of specific cell surface adhesion molecules. The objective of this study was to identify adhesion molecules that are involved in polymorphonuclear leukocyte (PMN) locomotion in extravascular tissue in vivo. Extravasation and interstitial tissue migration of PMNs was induced in the rat mesentery by chemotactic stimulation with platelet-activating factor (PAF; 10(-7) M). Intravital time-lapse videomicroscopy was used to analyze migration velocity of the activated PMNs, and the modulatory influence on locomotion of locally administered antibodies or peptides recognizing various integrin molecules was examined. Immunofluorescence flow cytometry revealed increased expression of alpha4, beta1, and beta2 integrins on extravasated PMNs compared with blood PMNs. Median migration velocity in response to PAF stimulation was 15.5 +/- 4.5 micron/min (mean +/- SD). Marked reduction (67 +/- 7%) in motility was observed after treatment with mAb blocking beta1 integrin function (VLA integrins), whereas there was little, although significant, reduction (22 +/- 13%) with beta2 integrin mAb. Antibodies or integrin-binding peptides recognizing alpha4beta1, alpha5beta1, or alphavbeta3 were ineffective in modulating migration velocity. Our data demonstrate that cell surface expression of beta1 integrins, although limited on blood PMNs, is induced in extravasated PMNs, and that members of the beta1 integrin family other than alpha4beta1 and alpha5beta1 are critically involved in the chemokinetic movement of PMNs in rat extravascular tissue in vivo.

[Cytogenetic analysis of 600 cases with chronic myelogenous leukemia].

OBJECTIVE: This was a retrospective analysis of cytogenetic data from 600 cases with chronic myelogenous leukemia (CML) to investigate the features of Ph chromosome and its significance. METHODS: Bone marrow direct method and/or short-term culture were used to prepare the chromosomes and karyotype analysis was performed with R-banding technique. RESULTS: 30 cases (5%) were Ph negative; 570 cases (95%) were Ph positive. 535 cases (93.8%) had standard Ph translocation;34 cases(5.9%) had variant translocation, including 13 cases (2.2%) with simple variant translocation, 13 cases (2.2%) with complex variant translocation and 8 cases (1.4%) with masked Ph chromosome. 526 cases (92.2%) had 100% of Ph positive cells; 44 cases (7.7%) had normal karyotype in partial or all metaphases after treatment such as allogeneic bone marrow transplantation, interferon and pulse hydroxyurea therapy, but conventional chemotherapy had no effect on the percentage of Ph positive cells. 50.6% of Ph positive CML with blast crisis had extra chromosomal abnormalities, of which, the most common ones were +8(46.1%),2 Ph(33.9%) and i(17q) (23%) in descending order. CONCLUSION: These facts indicate that chromosome examinations not only help diagnose and differentiate CML,but also help predict the blast crisis, evaluate the therapeutic effect, and make a cytogenetic classification for CML.

The economic costs of illicit drug use in Ontario, 1992.

The use of illicit drugs causes health and social problems which imply economic costs to society. This paper uses the cost-of-illness method, in particular, the human-capital approach to estimate the prevalence-based economic costs of illicit drug use in Ontario in 1992. This methodology is consistent with international guidelines formulated at the 1994 International Symposium on Economic and Social Costs of Substance Abuse. The economic cost of illicit drug use is estimated at $489.29 million (Canadian dollars) in 1992. Associated with these costs are health-related harms: 211 deaths, half of which occur before the age of 35; and 20 690 days stay in public hospitals.

A single amino acid substitution makes ERK2 susceptible to pyridinyl imidazole inhibitors of p38 MAP kinase.

Mitogen-activated protein (MAP) kinases are serine/threonine kinases that mediate intracellular signal transduction pathways. Pyridinyl imidazole compounds block pro-inflammatory cytokine production and are specific p38 kinase inhibitors. ERK2 is related to p38 in sequence and structure, but is not inhibited by pyridinyl imidazole inhibitors. Crystal structures of two pyridinyl imidazoles complexed with p38 revealed these compounds bind in the ATP site. Mutagenesis data suggested a single residue difference at threonine 106 between p38 and other MAP kinases is sufficient to confer selectivity of pyridinyl imidazoles. We have changed the equivalent residue in human ERK2, Q105, into threonine and alanine, and substituted four additional ATP binding site residues. The single residue change Q105A in ERK2 enhances the binding of SB202190 at least 25,000-fold compared to wild-type ERK2. We report enzymatic analyses of wild-type ERK2 and the mutant proteins, and the crystal structure of a pyridinyl imidazole, SB203580, bound to an ERK2 pentamutant, I103L, Q105T, D106H, E109G. T110A. These ATP binding site substitutions induce low nanomolar sensitivity to pyridinyl imidazoles. Furthermore, we identified 5-iodotubercidin as a potent ERK2 inhibitor, which may help reveal the role of ERK2 in cell proliferation.

The economic costs of alcohol abuse in Ontario.

The abuse of alcohol causes health and social problems, such as sickness, death, injury, pain, suffering and crime. These harms impose an economic burden on society. Resources are used or foregone as a consequence of alcohol abuse. This article provides an estimate of the economic cost of alcohol abuse in Ontario in 1992. It uses the cost-of-illness method, in particular, the human-capital approach to estimate the prevalence-based economic costs of alcohol abuse. This methodology is consistent with international guidelines formulated at the 1994 International Symposium on Economic and Social Costs of Substance Abuse. The direct and indirect economic costs of alcohol abuse are identified and estimated. The total economic cost of alcohol abuse, from a societal perspective, is estimated to be US$2261.10 million in Ontario in 1992.

A rare variant translocation t(3;8)(q29;q22) without AML1/ETO fusion transcript in a case of oligoblastic leukemia.

A 43-year-old man with oligoblastic leukemia and t(3;8) variant translocation is reported. At first he was classified as refractory anemia with excess of blasts in transformation according to the FAB criteria for myelodysplastic syndrome. Remission was obtained after intensive chemotherapy. After 8 months, a relapse occurred as overt M2 AML. At presentation chromosome study of bone marrow cells using R- and G-bandings revealed 45,X, -Y,t(3;8)(q29;q22) in 35 of the 42 metaphases analyzed and 46,XY,t(3;8) in one metaphase in addition to normal karyotype in the other six metaphases. However, RT-PCR assay showed no AML1/ETO fusion transcript. At relapse, a karyotype of 46, XY,t(3;8), deletion(4)(p14), add(7)(q32) was observed in all abnormal cells indicating a clonal karyotypic evolution. We believed that this case should be diagnosed as an early form of M2 AML initially. It may be the first case of oligoblastic leukemia with t(3;8) variant translocation. Further study is needed to elucidate its molecular entity.

A case of acute megakaryocytic leukemia presenting as peripheral acute leukemia with complex karyotypic abnormalities.

Peripheral acute leukemia (PAL) is extremely rare. We report on the first case of acute megakaryocytic leukemia presenting with PAL and complex chromosomal abnormalities. At diagnosis, the patient had 10.5-21% marrow blasts and 55-60% peripheral blasts which expressed CD42b and CD61. Two related clones with the karyotype of 48,X,-X,+3,+9,del(9)(q12q31) x 2,+17,i(17)(q10)/ 49,idem, +18 were revealed by conventional cytogenetics, but no AML-1/ETO fusion transcript was detected by RT-PCR assay. The significance of this case is discussed concisely.

Bax expression has prognostic significance that is enhanced when combined with AgNOR counts in glottic carcinomas.

Using nucleolar organizer regions (NORs) as a proliferative marker and Bax expression as a marker for apoptosis, we have studied the individual and combined prognostic significance of these markers. Successive sections of diagnostic, formalin-fixed and paraffin-embedded specimens from 69 patients with T1-4 tumours were stained with a rabbit anti-human Bax polyclonal antibody and silver nitrate for visualization of NORs (AgNORs). After classification for staining intensity and the percentage of Bax expression, a final score resulting in four classes of increasing Bax expression was obtained. AgNOR counts were expressed as mean counts (mAgNOR) and the percentage of tumour nuclei with more than one AgNOR (pAgNOR>1). Both AgNOR parameters were grouped in three classes with increasing values. Low Bax scores correlated significantly with poor prognosis (P = 0.0106). For mAgNOR and pAgNOR>1, high values correlated with poor prognosis (P = 0.0185 and P = 0.0003 respectively). A combined parameter, for which the Bax score was subtracted from the AgNOR scores, appeared to be statistically stronger than the individual parameters (P < 0.0001). Both Bax expression and AgNOR scores, and in particular the combination of these parameters, appear to be strong prognostic markers in glottic squamous cell carcinomas.

Characteristics of histamine-induced leukocyte rolling in the undisturbed microcirculation of the rat mesentery.

1. The main objective of this study was to analyse the role and mode of action of the mast cell mediator histamine in leukocyte-endothelium interactions in small venules in vivo. For this purpose, we used a histological approach (combined with intravital microscopy) that allows studies of rapid mediator-induced venular leukocyte accumulation, reflecting leukocyte rolling, in the undisturbed microcirculation of the rat mesentery where rolling is normally absent. 2. We first examined the relative importance of histamine and 5-hydroxytryptamine (5-HT) in acute mast cell-dependent leukocyte recruitment. The mast cell secretagogue compound 48/80 (i.p. for 15 min) induced a marked venular accumulation of polymorphonuclear leukocytes (PMNL) which was almost abolished by combined histamine1 (H1)- and histamine2 (H2)-receptor blockade. In contrast, the 5-HT-receptor antagonist methysergide was inactive in this regard. Moreover, exogenous 5-HT was less active than exogenous histamine in evoking venular PMNL accumulation (histamine response dose-dependent; 5-HT response bell shaped). Prostaglandin D2 did not cause PMNL accumulation. 3. The venular PMNL response to exogenous histamine peaked between 15 min and 1 h, was still significantly elevated at 2 h, and then returned to prechallenge values after 3 h. At all time points, the histamine-induced PMNL accumulation was nearly abolished by i.v. treatment with the polysaccharide fucoidin (which blocks rolling but not firm adhesion per se), suggesting that the PMNL response to histamine was due to rolling rather than firm adhesion over the entire 3 h period. At no time point did histamine trigger accumulation of mononuclear leukocytes (MNL). 4. To examine the role of histamine-receptors in the histamine-induced PMNL accumulation (i.e. rolling), the animals were pretreated with diphenhydramine (H1-receptor antagonist), cimetidine, or ranitidine (H2-receptor antagonists). Diphenhydramine alone inhibited the venular PMNL response to histamine by 52%, while both H2-receptor antagonists were completely inactive. However, the combination of cimetidine and diphenhydramine reduced the histamine-induced PMNL rolling by 82%. Furthermore, in contrast to an H3-receptor agonist, challenge with either the H1-receptor agonist 2-thiazolylethylamine or two different H2-receptor agonists (impromidine, dimaprit) was sufficient to provoke significant venular PMNL accumulation. 5. Treatment with the nitric oxide-synthase inhibitor L-NAME did not affect the histamine-induced PMNL rolling. On the other hand, 3 h pretreatment with dexamethasone reduced the PMNL response to histamine by 73%, and flow cytometric analysis showed that the dexamethasone treatment almost completely inhibited binding of soluble P-selectin to rat isolated PMNLs. 6. We conclude that initial leukocyte recruitment after mast cell activation in the rat mesentery is critically dependent on histamine release. The cellular response to histamine was specifically due to PMNL rolling, involved activation of both H1- and H2-receptors, and lasted for 2 3 h. Moreover, the histamine-induced PMNL rolling was not dependent on nitric oxide synthesis, but was sensitive to glucocorticoid treatment, possibly via inhibition of expression or function of leukocytic P-selectin ligand(s).
