OBJECTIVE: The prevalence of albuminuria and the risk factors associated with albuminuria were evaluated among the Chinese patients diagnosed with type 2 diabetes aged over 30 in the Shanghai downtown. We also evaluated the variability of urinary albumin-to-creatinine ratio (ACR) among the three measurements and the relationship between diabetic retinopathy (DR) and albuminuria. METHODS: The 1039 Chinese patients diagnosed with type 2 diabetes aged over 30 were investigated by randomized cluster sampling in the Shanghai downtown and 1018 patients were analyzed in this study. Body mass measurements including height, weight, waist circumference and hip circumference, resting blood pressure, fasting blood measures, urinary ACR and the digitally stored fundus images were investigated. The prevalence of albuminuria was calculated and the risk factors associated with albuminuria were evaluated by stepwise logistic regression. The concordance of urinary ACR was evaluated by observed agreement. The relationship between albuminuria and DR was also evaluated. RESULTS: (1) The mean age of all patients was 66.10+/-11.54 years and the duration of diabetes was 7.89+/-7.16 years. (2) The prevalence of albuminuria was 49.6% among the Chinese patients diagnosed with type 2 diabetes aged over 30 in the Shanghai downtown, 41.4% with microalbuminuria and 8.2% with macroalbuminuria. (3) Microalbuminuria was significantly associated with systolic blood pressure, gender and waist circumference. Macroalbuminuria was significantly associated with systolic blood pressure and duration of diabetes. (4) Observed agreement among the three urinary ACR measurement for albuminuria staging was 73.3% (first versus second), 64.5% (first versus third) and 77.5% (second versus third). Observed agreement in the albuminuria staging between the single urinary ACR measurement and all three urinary ACR measurements was 85.8% (first versus all three), 87.6% (second versus all three) and 81.9% (third versus all three). (5) The percentage of DR in the macroalbuminuric group (59.2%) was significantly higher than that in the normalbuminuria group (16.1%) and microalbuminuria group (24.6%). (6) The macroalbuminuric patients with DR had significantly increased fasting blood glucose and HbA1c compared with the macroalbuminuric patients without DR. CONCLUSION: The prevalence of microalbuminuria observed in the Chinese patients diagnosed with type 2 diabetes aged over 30 in the Shanghai downtown reached up to 41.4% though the observations in our study might be representative of the diabetic patients of the Shanghai downtown. We agreed that at least two of the three urinary collections were done in a 3- to 6-month period because of the day-to-day variability in albumin excretion. The percentage of DR among the patients with macroalbuminuria was 59.2%, and the macroalbuminuric patients with the significantly high plasma glucose and DR were prone to diagnose DN.
The aggregation in conjugated polyelectrolytes (CPs) can be effectively reduced by the formation of CP/nanoparticle assemblies. The photophysical properties of various nanoassemblies were studied by means of UV-visible and fluorescence spectroscopy in solution and as thin films. The dissociation of the polymer chains is caused by favorable electrostatic interactions between the cationic substituents of the CPs and the anionic charges present on the surface of the nanoparticles. Such an efficient displacement of pi-stacking by competitive positive interactions constitutes the first example of positive aggregation modulation.
The SO4(2-)-induced micellization of poly(ethylene glycol)-block-poly(4-vinylpyridium) (PEG110-b-P(4-VPH+)35) and the thermoresponsiveness of these hybrid micelles are studied by dynamic and static light scattering. When the concentration of H2SO4 is high enough, PEG110-b-P(4-VPH+)35 forms stable hybrid micelles with an ionic core of P(4-VPH+)35/SO4(2-) and a PEG corona at 25 degrees C. The formation of the hybrid micelles is reversible. A thermodynamic equilibrium exists between the hybrid micelles and PEG110-b-P(4-VPH+)35 unimers. The shifts of the equilibrium are mainly attributed to the variation of the electrostatic energy and entropic energy of the system. Therefore, the temperature can determine the states of the equilibrium, which means that the dissociation or the formation of the hybrid micelles can be triggered by just varying the temperature.

Bile acids in the family of steroid compounds can be chemically modified for biochemical and other applications. Derivatives of cholic acid with multiple methacrylate groups can be prepared by the use of methacrylic acid, methacryloyl chloride and methacryloyl anhydride as the acylating agents. The hydroxyl groups of cholic acid methyl ester and cholic acid ethylene glycol ester have been selectively acylated by changing the acylating agents and the number of substitutions have been varied by changing the amount of the acylating agents used. In the acylation reactions with methacryloyl chloride, the reactivity of secondary hydroxyl groups on the steroid skeleton of cholic acid derivatives follows the order of C3>C12>C7.
The degree of conversion (DC) of methacrylate dental resins is typically determined by spectroscopically measuring the decrease of the vinyl (C=C) stretching band at 1640 cm(-1), ratioed before and after polymerization to an internal standard (aromatic ring quadrant stretching vibration (Ph)) at around 1609 cm(-1). While standard methods exist for measuring the intensity of the C=C and Ph peaks from the FTIR or Raman spectrum, these methods either fail under certain circumstances, or lack a physical basis, being purely based on spectral features. In this study, we present a rigorous method (named rotational isomerism method) for determining the intensity of the vinyl and aromatic bands from the FTIR and Raman spectra of dental monomer mixtures, and compare it to a standard baseline method (SBM) and a standard curve fitting method. Model triethyleneglycol dimethacrylate (TEGDMA)/2,2-bis(4-hydroxyphenyl)propane (Bisphenol-A) and TEGDMA/2,2-bis(4-(2-hydroxy-3-methacryloxypropoxy)phenyl)propane (Bis-GMA) mixtures with a series of known C=C/Ph molar ratios were prepared in order to simulate the effect of curing. The accuracy of the new method for measuring the DC was found to be as good as the commonly used SBM. The standard curve fitting method was shown to be inappropriate for measuring C=C/Ph ratios by FTIR spectroscopy due to its inability to realistically simulate the features of the spectra. The insight gained through the use of this new method may be useful for the characterization of other methacrylate biomaterials.
Oxygen inhibits free radical polymerization and yields polymers with uncured surfaces. This is a concern when thin layers of resin are being polymerized, or in circumstances where conventional means of eliminating inhibition are inappropriate. In this study, we tested the hypothesis that viscosity, filler content, and polymerization temperature modify oxygen diffusion in the resin or the reactivity of radical species, and affect the degree of conversion near the surface. Confocal Raman micro-spectroscopy was used to measure monomer conversion from the surface to the bulk of cured resins. Increased viscosity was shown to limit oxygen diffusion and increase conversion near the surface, without necessarily modifying the depth of inhibition. The filler material was shown to increase, simultaneously, oxygen diffusivity and the viscosity of the resin, which have opposite effects on conversion. Polymerization at a temperature above approximately 110 degrees C was shown to eliminate oxygen inhibition.
Polymer microspheres can be employed to deliver medication in a rate-controlled and sometimes targeted manner. Medication is released from a microsphere by drug leaching from the polymer or by degradation of the polymer matrix. Since the rate of drug release is controlled by these two factors, it is important to understand the physical and chemical properties of the releasing medium. This review presents the methods used in the preparation of microspheres from monomers or from linear polymers and discusses the physio-chemical properties that affect the formation, structure, and morphology of the spheres. Topics including the effects of molecular weight, blended spheres, crystallinity, drug distribution, porosity, and sphere size are discussed in relation to the characteristics of the release process. Added control over release profiles can be obtained by the employment of core-shell systems and pH-sensitive spheres; the enhancements presented by such systems are discussed through literature examples.
UNLABELLED: The aim of this study was to describe the glycaemic and metabolic control and diabetes-related complications in type 1 and type 2 Asian patients. METHODS: Data of diabetes patients from 230 diabetes centres in 12 Asian regions were collected on a retrospective-prospective basis through review of medical records, interview and laboratory assessments. Analysis of glycated haemoglobin (HbA1c) was carried out in central laboratories appointed by Bio-Rad. The data collection case record forms were scanned electronically. RESULTS: 22177 patients with valid data made up the analysis population. Among patents with type 1 and type 2 diabetes, there was a higher proportion of women than men (53% vs. 47% for type 1 patients and 56% vs. 44% for type 2 diabetes). Hypertension (61%) and overweight (40% with BMI > or = 25 kg/m2 were common in type 2 patients. Dyslipidaemia was also present in at least half of both types of patients. Control of glycaemia (mean HbA,1c and fasting blood glucose [FBG]) was poor in type 1 (9.9 +/- 2.5%; 10.2 +/- 5.2 mmol/l) and type 2 patients (8.5 +/- 2.0%; 8.9 +/- 3.4 mmol/l). Glycaemia in the majority of both types of patients fell short of those stipulated by various guidelines. In type 2 patients, glycaemia deteriorated (HbA1c > 7.5%, FBG > or = 7.0 mmol/l) with duration of diabetes > 7 years. Both types of diabetes appear to share a similar high prevalence of complications of cataract, retinopathy and neuropathy, although the prevalence of cataract (27%) and neuropathy (35%) was higher in type 2 diabetes. Screening for microalbuminuria was not common. CONCLUSIONS: The Inadequate metabolic and hypertension control, especially in type 2 patients, needs to be addressed.
The complex hydrogen-bonding arrangement in the biologically important molecule bilirubin IXalpha is probed by using 1H double-quantum (DQ) magic-angle spinning (MAS) NMR spectroscopy. Employing fast MAS (30 kHz) and a high magnetic field (16.4 T), three low-field resonances corresponding to the different hydrogen-bonding protons are resolved in a 1H MAS NMR spectrum of bilirubin. These resonances are assigned on the basis of the proton-proton proximities identified from a two-dimensional rotor-synchronized 1H DQ MAS NMR spectrum. An analysis of 1H DQ MAS spinning-sideband patterns for the NH protons in bilirubin allows the quantitative determination of proton-proton distances and the geometry. The validity of this procedure is proven by simulated spectra for a model three-spin system, which show that the shortest distance can be determined to a very high degree of accuracy. The distance between the lactam and pyrrole NH protons in bilirubin is determined to be 0.186 +/- 0.002 nm (corresponding to a dominant dipolar coupling constant of 18.5 +/- 0.5 kHz). The analysis also yields a distance between the lactam NH and carboxylic acid OH protons of 0.230 +/- 0.008 nm (corresponding to a perturbing dipolar coupling constant of 9.9 +/- 1.0 kHz) and an H-H-H angle of 122 +/- 4 degrees. Finally, a comparison of 1H DQ MAS spinning-sideband patterns for bilirubin and its dimethyl ester reveals a significantly longer distance between the two NH protons in the latter case.
The human TAX-1 gene encodes a Mr 135,000 glycoprotein that is transiently expressed on the surface of a subset of neurons during development and is involved in neurite outgrowth. The TAX-1 gene has been mapped to a region on chromosome 1 that has been implicated in microcephaly and the Van der Woude syndrome. Using restriction landmark genome scanning to search for amplified genes in gliomas, we found TAX-1 to be amplified in 2 high-grade gliomas among a group of 26 gliomas investigated. Real-time reverse transcription-quantitative PCR analysis detected high levels of TAX-1 mRNA in glial tumors, even in the absence of TAX-1 gene amplification. Immunohistochemical analysis revealed abundant levels of TAX-1 in neoplastic glial cells of glioblastoma multiforme tumors. Because glial tumors are highly invasive and in view of the role of TAX-1 in neurite outgrowth, we investigated the potential role of TAX-1 in glioma cell migration. Using an in vitro assay, we found that the migration of glioma tumor cells is profoundly reduced in the presence of either an anti-TAX-1 antibody or a TAX-1 antisense oligonucleotide. Our findings suggest that TAX-1 plays a role in glial tumorigenesis and may provide a potential target for therapeutic intervention.
BACKGROUND: Amplification of the oncogene MYCN in neuroblastoma has been found to correlate with aggressive tumour growth and is used as a predictor of clinical outcome. The MYCN amplicon is known to involve coamplification of extensive DNA regions. Therefore it is possible that other genes are coamplified in this amplicon and that they may play a role in the poor outcome of MYCN amplified tumours. PROCEDURE: We have implemented an approach for the two-dimensional separation of human genomic restriction fragments to detect and isolate as yet unknown amplified sequences in the MYCN amplicon in neuroblastoma. Using this approach we have recently cloned a novel gene referred to as NAG that is frequently coamplified with MYCN in neuroblastoma. RESULTS AND CONCLUSIONS: We report here the identification and cloning of two additional CpG islands that are amplified in neuroblastoma. One contains a sequence that is identical to the first intron of DDX1. The other represents a novel CpG island that is associated with an as yet unidentified gene. We show that the novel CpG island is located in close proximity to the MYCN locus on chromosome 2 and is as frequently coamplified with MYCN in neuroblastoma as NAG and DDX1.
Cationic dextran hydrogel microspheres with pendant quaternary ammonium groups having alkyl substituents (C(2)-C(12)) at quaternary nitrogen were synthesized. The in vitro sorption of sodium salts of four bile acids (glycocholic, cholic, taurocholic, and deoxycholic acids) with these hydrogels was studied as a function of substituent alkyl chain length and bile acid hydrophobicity. Sorption experiments were performed in phosphate buffer solutions (pH 7.4) containing one bile salt (individual sorption) or mixtures of several bile salts (competitive sorption). Parameters for individual sorption were calculated taking into consideration the stoichiometric and cooperative binding of bile salts to oppositely charged polymer hydrogels. The results show that the increase in the length of the alkyl chain of the substituent leads to an increase in both ionization constant K(0) and overall stability constant of binding K, but decreases the cooperativity parameter u. The competitive sorption studies indicate that the hydrogels display a good affinity for both dihydroxylic and trihydroxylic bile salts. The molar ratio of maximum amounts bound for the two types of bile acid is 2 to 1, which is much lower than those reported for other cationic polymers recommended as bile acid sequestrants. The binding constants for the sorption of bile salts by some dextran hydrogels are 20-30 times higher than those obtained for cholestyramine under similar sorption conditions.
Using immunocytochemistry based on a monoclonal antibody against Diploptera punctata allatostatin I and horseradish peroxidase-diaminobenzidine reaction, the presence of allatostatin-like immunoreactivity is demonstrated in the synganglion of Dermacentor variabilis females. The immunoreactive cells are located in the protocerebral, cheliceral, palpal, stomodeal, postesophageal, and opisthosomal regions of the synganglion. Strongly immunoreactive granules accumulate in the boundary area of the subganglia in the preesophageal part of the synganglion. This suggests that the immunoreactive materials may be released directly from there. In addition, a putative neurohemal area is found in the anterior area of the opisthosomal ganglion, where abundant immunoreactive materials are stored. Weak immunoreactivity and fewer immunoreactive cells are seen in newly molted females compared with one month old, unfed females. Thus, the immunoreactive products may be depleted during molting and synthesized in females before feeding.
A highly cross-linked porous polymer resin based on styrene-divinylbenzene matrix with pores created by the use of micellar imprinting technique was used as chromatographic packing material. Its performance as a column packing material in inverse size-exclusion chromatography was compared with a non-imprinted resin of the same polymer matrix. The porous structures (the pore size and the porosity) of the resins in the dry and wet states and their relationships with the elution volume of probe solutes (alkanes and polystyrene standards) were established. Characteristic properties of the resins such as specific pore volume, specific surface area and porosity are compared with results obtained by other methods of characterization such as mercury intrusion porosimetry, solvent regain and nitrogen sorption. The results show that the new porous resin can be used in the separation of small molecules. The separation is based on the size of the molecules, and the larger pores (meso- and macropores) in the porous resin can provide a much easier access to the smaller pores (micropores) which are useful in the chromatographic separations.
OBJECTIVE: To investigate relationships between dietary macronutrient intakes and glucose tolerance in pregnancy RESEARCH DESIGN AND METHODS: Nulliparous pregnant Chinese women diagnosed with gestational diabetes mellitus (GDM) (n = 56) were compared to age-, gestational age-, height-, and parity-matched groups with normal glucose tolerance (n = 77) and glucose intolerance (IGT) (n = 38) based on the results of an oral glucose tolerance test (National Diabetes Data Group criteria), performed between 24 and 28 weeks of pregnancy. A 24-h recall dietary assessment was also obtained at the time of screening. RESULTS: Subjects with IGT and GDM were significantly heavier (66.1 +/- 1.4 and 68.6 +/- 1.2 kg, respectively, mean +/- SEM) (P < 0.0001) than the normal group (61.2 +/- 1.8 kg) and had a higher BMI. Overall energy intake was similar between groups, as were the intakes of each macronutrient (%kcal). However, there was a highly significant reduction in polyunsaturated fat intake in the IGT and GDM groups whether expressed as %kcal, % of total fat, or fat kcal. This effect was independent of body weight or BMI whether assessed by ordinal logistic regression or by analysis of a weight- and BMI-matched subgroup of the subjects (P = 0.002 for %kcal; n = 47 normal, 26 IGT, and 43 GDM subjects). In logistic regression analysis of the complete data set, increased body weight (P < 0.0001) and decreased polyunsaturated fat intake (P = 0.0014) were both independent predictors of glucose intolerance (IGT and GDM), as were increased body weight and a low dietary polyunsaturated to saturated fat ratio. CONCLUSIONS: Increased polyunsaturated fat intake is associated with a reduced incidence of glucose intolerance during pregnancy. This finding may have major implications for dietary management of women with or at risk of developing GDM.
Substantial evidence implicates amplification of the N-myc gene with aggressive tumor growth and poor outcome in neuroblastoma. However some evidence suggests that this gene alone is not the sole determinant of outcome in N-myc amplified tumors. We have searched for genes that co-amplify with N-myc in neuroblastoma by means of two-dimensional analysis of genomic restriction digests. Using this approach, we have identified and cloned a novel genomic fragment which is co-amplified with N-myc in neuroblastomas. This fragment was mapped in close vicinity to N-myc on chromosome arm 2p24. It was amplified in 5/8 N-myc amplified neuroblastoma cell lines and in 9/13 N-myc amplified tumors. Using a PCR-based approach we isolated a 4.5 kb c-DNA sequence that is partly contained in the genomic fragment. The open reading frame of the cDNA encodes a predicted protein of 1353 amino acids (aa). The homology of the predicted protein, which we designated NAG (neuroblastoma amplified gene), to a C. elegans protein of as yet unknown function, and its ubiquitous expression suggest that NAG may serve an essential function. By Northern blot analysis we showed that amplification of the cloned gene correlates with over-expression in neuroblastoma cell lines. Amplification and consequent over-expression of NAG may, therefore, contribute to the phenotype of a subset of neuroblastomas.
