[Diagnosis of sex chromosome abnormality by fluorescence in-situ hybridization].

OBJECTIVE: To evaluate the diagnostic value of fluorescence in-situ hybridization (FISH) in sex chromosome abnormality. METHOD: alpha-satellits DNA probes of X, Y chromosomes were used to FISH with blood samples from 19 patients with primary anemia or of gonad dysplasia. Five infertile men who were detected abnormal by G-banding analysis during metaphase and interphase. Samples from healthy men and women were used as positive controls and reactions with hybridization fluid without probes as negative controls. RESULTS: The karyotypes, 45, X; 45, X/46, XX/47, XXX; 45, X/46, XY; 47, XXY; 45, X/46, XXX were detected by FISH. Others were not detected by G banding such as 45, X/46, X,r? 46, X,r? but were found to be 45, X/46, X,r(X), 46, X, r(X), by FISH technique, confirming to be X ring chromosome. A female patient whose karyotype was 45, X/46, X, mar. by G banding. But G banding technique could not analyze the property of the marker and FISH revealed the marker was dici(yq), that is dicentric chromosome Y with two long arms of equal length. The real karyotype was 45, X/46, X, dici(yq) mosaicism. It belonged to Y chromosome abnormal and gonadal dysplasia syndrome. CONCLUSION: FISH can help to detect those chromosome abnormalities which can not be confirmed by traditional cytogenetics. It is of value in studying the complex chromosome mutation such as sex chromosome abnormality, unknown little marker, ring chromosome, mosaiciasm and translocation.

Relationship between pathogenesis of Guillain-Barre syndrome and Penner's serotypes of Campylobacter jejuni.

OBJECTIVE: To investigate the relationship between the pathogenesis of Guillain-Barre syndrome (GBS) and Campylobacter jejuni (C. jejuni) in China. METHODS: C. jejuni strains were isolated from fresh stools of 47 GBS patients in Beijing area from 1995 to 1997 by modified Skirrow's method. Serotyping of C. jejuni was performed with Penner's method in 47 GBS patients and 171 patients with C. jejuni enteritis in our hospital during the same period. The stools from which no C. jejuni strains could be isolated were typed by PCR-RFLP (restriction fragment length polymorphism) method. RESULTS: Six C. jejuni strains were isolated by Skirrow's method, including 3 strains of Penner serotype 2, 1 Penner serotype 4, 1 Penner serotype 19 and 1 Penner serotype 26. Three strains of DNA-I, which was equal to the same type of Penner serotype 19, were found by PCR-RFLP method. CONCLUSION: C. jejuni isolated from Beijing area are similar to those reported in literature, which once more confirms the molecular mimicry pathogenetic theory of GBS caused by C. jejuni infection in China. The similarities of serotypes between C. jejuni strains from GBS patients and those from C. jejuni enteritis patients may explain the high incidence of GBS following C. jejuni infection in China.

[Respiratory effects of beta-endorphin in chronic hypoxic rats].

OBJECTIVE: To assess the role of beta-EP in the physiological and pathophysiological process of respiratory regulation in chronic hypoxic rats. METHODS: The chronic hypoxic rat model was established by intravenous injections of papain 6 times, once a week. The concentration of beta-endorphin in medulla, pons, hypothalamus, central gray and plasma of chronic hypoxic rats were measured by radioimmunoassay. All animals were pretreated with sodium pentobartital (35 mg/kg) before experiment. 102 rats were randomly divided into three groups. Group 1 (n = 36). The respiratory rate (RR) and tidal volume (VT) were measured after intravenous injection of naloxone (NLX, 2 mg/kg), beta-EP (40 micrograms/kg, 160 micrograms/kg) or normal saline, Group 2 (n = 48). By the intracerebroventricular administration of NLX and beta-EP to the models, RR, VT and PaCO2 were observed after microinjection 5, 15, 30, 45, 60 min. Group 3 (n = 18). The respiratory effects of beta-EP after directly into the nucleus tractus solitari of the anaesthetised rats were investigated. RESULTS: The beta-EP contents in medulla, pons, hypothalamus, central gray and plasma of chronic hypoxic rats were significantly increased compared with control subjects (P < 0.01). It suggested that the pathophysiology of chronic hypoxic process infleuced the contents of beta-EP in the CNS and plasma in rats. Intracerebroventricular microinjection of beta-EP in normal rats, resulted in a significant decrease in RR (P < 0.05). No significant difference in RR and VT was observved after intravenous injection of naloxone (2 mg/kg) and beta-EP (40 micrograms/kg, 160 micrograms/kg) in treatment group compared with the control group (P > 0.05), Intracerebroventricular microinjection of NLX in chronic hypoxic rats, resulted in a marked increase in RR (P < 0.05) and central hypercapnic-sensitivity (t = 2.76, P < 0.05), Intracerebroventriculalr microinjection of beta-EP in chronic hypoxic models, resulted in severe respiratory depression after injection 15, 30, 45, 60 min (RR t = 3.41, 6.54, 6.97, 7.87, P < 0.01; VT t = 3.07, 7.27, 6.14, 6.08). CONCLUSIONS: These results indicate the beta-endorphin may be involved in central respiratory control of chronic hypoxic rats.

[The relationship between Guillain-Barre syndrome and serotypes of Campylobacter jejuni isolated from the patients].

OBJECTIVE: To investigate the relationship between Campylobacter jejuni (CJ) serotype and the high incidence of Guillain-Barre syndrome (GBS) following CJ enteritis in China. METHODS: CJ was isolated from fresh stool of 47 GBS patients in Beijing area from 1995-1997 with a combination of Skirrow method and micro-filtration method. Serotyping of the CJ strains was performed with passive hemagglutination method with standard serum from CDC. 171 CJ strains from CJ enteritis in our hospital in the same period were serotyped with the same method. The stool from which no CJ could be isolated was typed with PCR-RFLP method. RESULTS: 3 Penner 2, 1 Penner 4, 1 Penner 19 and 1 Penner 26 strains were isolated and serotyped. Three strains which could not be isolated were found by PCR-RFLP method to be DNA-I, which is equivalent to the type of Penner 19. There were some CJ strains which could be typed with serum from CDC. CONCLUSION: CJ from GBS patients in Beijing area are similar to those reported in the world; it is suggested that CJ may cause GBS by molecular mimicry mechanism in China. The most common serotype of CJ from GBS was reported to be Penner 19 in Japan, while it was rare in CJ enteritis. However, the similarity of the serotypes between CJ from GBS patients and CJ from enteritis patients may explain the high incidence of GBS following CJ infection in China. Penner 26 is reported to be isolated from GBS patients for the first time.

[Piezoelectric, electrohydraulic and electromagnetic shock wave lithotripsy for lower ureteral stone].

OBJECTIVE: To treat patients with lower ureteral stones by extracorporeal shock wave lithotripsy (ESWL) using piezoelectric, electrohydraulic and electromagnetic second lithotriptors. METHODS: Six hundred and three patients were divided into 3 groups according to the modes of lithotriptors. There were 179 patients in EDAP LTO1 piezoelectric group, 294 in JDPN IV electrohydraulic group and 130 in Dornier Compact S electromagnetic group. Every group was further divided into 3 subgroups with 10 mm, 10 - 15 mm and 15 mm in diameters of stones. RESULTS: The stone-free rate in single session of ESWL was higher in the electromagnetic group than in the other two groups. The efficiency quotient was lower in piezoelectric group than in the electrohydraulic and electromagnetic groups. The stone-free rate in single session was achieved in more than 70% of stones less than 15 mm in diameter for electromagnetic and electrohydraulic ESWL. CONCLUSIONS: The overall results of electromagnetic and electrohydraulic ESWL on lower ureteral stones are better than those of piezoelectric ESWL, and ESWL is suitable for stone less than 15 mm in diameter.

[Extracorporeal shock wave lithotripsy in situ for middle ureteral stones].

OBJECTIVE: To report the experience with in situ ESWL for patients with middle ureteral stones. METHODS: 52 patients suffering from stones with an average 11 mm (range 5 - 20 mm) in dimension were treated using modified Puigvert technique in prone position via the entry route of lateral abdomen. The patients received 3,000 - 5,000 shocks per session at the power setting ranged from 2 to 7. RESULTS: The stone free rate at 3 months was achieved in 96% patients retreatment was performed in 30% patients. The therapeutic results were calculated as efficiency quotient being 74%. No severe complications occurred in all patients. CONCLUSIONS: In situ ESWL for middle ureteral stone is safe and effective and may serve as the first line therapy.

[Experimental studies on the effect of the immunosuppressant FK-506 on penetrating keratoplasty rejection model in rats].

OBJECTIVE: To study the immunosuppressive effects of FK-506 on allogeneic transplantation in a rat model. METHODS: Inbred strain Lou rats were used as recipients, and F344 rats were used as donors. Subconjunctival injection of FK-506 0.1 mg/kg and cyclosporine A (CsA) 3 mg/kg were administered respectively for 2 weeks, and the grafts were inspected by clinical evaluation. RESULTS: The group receiving FK-506 had a significant delay in allograft rejection with mean survival time (22.1 +/- 5.17) days vs. (18.4 +/- 1.4) days for the CsA group and (12.1 +/- 2.13) days for the control group (P < 0.01). CONCLUSION: It is indicated that FK-506 would be a useful drug to suppress corneal graft rejection.

[Extra-trabeculotomy and trabeculectomy in treatment of primary infantile glaucoma].

OBJECTIVE: To compare the effectiveness and complication of extra-trabeculotomy and trabeculectomy in the treatment of primary infantile glaucoma. METHOD: Patients with primary infantile glaucoma and having undergone either of the two procedures from Jan, 1992 - June, 1996 were selected. Among them, group one (30 patients, 51 eyes) underwent extra-trabeculotomy, while group two (26 patients, 46 eyes) underwent trabeculectomy. RESULTS: The success rates in group one were 92.2% at the time of discharge and 87.5% during follow-up (mean 26.3 months); the rates in group two were 73.9% and 62.9% (mean 28.1 months) respectively, the differences being significant (P < 0.05). In group one, there was no severe complication, while in group two, two cases had vitreous prolapse. CONCLUSION: Extra-trabeculotomy is more efficacious and safer than trabeculectomy in the treatment of primary infantile glaucoma. It should be the first choice in the treatment of primary infantile glaucoma.

[Histopathology studies on 8,673 cases of ocular adnexal hyperplastic lesions and tumors].

OBJECTIVE: To study the histopathologic classification of ocular adnexal hyperplastic lesions and tumors according to the histogenesis and the anatomic sites. METHOD: To analyze retrospectively the pathologic specimens of 8,673 cases of ocular adnexal hyperplastic lesions and tumors between 1961 - 1997. RESULTS: There were 7,265 benign cases (83.77%), and 1,408 malignant cases (16.23%). Classification by anatomic sites: eye lid 5,313 cases (61.26%), orbit 1,484 cases (17.11%) and epibulbar 1,876 cases (21.63%). Classification by histogenesis: (1) Primary tumor 6,679 cases: epitheliogenic 2,481 cases (37.15%), mesenchymal 753 cases (11.27%), lympho-hematopoietic 102 cases (1.53%), neurogenic 1,542 cases (23.09%), congenital abnormal embryogenetic 1,767 cases (26.46%), uncertain resource tumor 34 cases (0.50%). (2) Secondary tumor 13 cases; (3) Metastatic tumor 9 cases; (4) Non-tumor lesion 1,972 cases. CONCLUSION: The classification is helpful to the histopathologic diagnosis and the study about the detailed distribution of common ocular adnexal hyperplastic lesions and rare tumors, and it also increases the pathologic diagnostic data in North China.

[Confocal scanning laser Doppler retinal flowmetry in normal eyes].

OBJECTIVE: To introduce a new noninvasive equipment to measure the blood flow of optic nerve head and retina. METHODS: The scanning laser Doppler retinal flowmeter (Heidelberg Engineering, Heidelberg, Germany) was used to measure the blood flow of optic nerve head and retina of 82 eyes of 48 normal individuals. RESULTS: The average blood flow volume, blood flow velocity and the erythrocyte flow velocity of the vessel in optic nerve head were 203.14 +/- 74.77, 6, 272.43 +/- 2,260.07, 13.14 +/- 2.58 respectively; they were 26.36 +/- 14.74, 467.99 +/- 270.15, 1.93 +/- 1.07 in lamina cribrosa; 22.90 +/- 11.32, 514.53 +/- 370.21, 1.69 +/- 1.12 in the temporal rim of papilla; 21.77 +/- 9.83, 493.21 +/- 290.29, 1.65 +/- 0.90 in nasal rim of papilla; 32.55 +/- 13.00, 558.63 +/- 293.45, 1.78 +/- 0.83 in the temporal juxtapapillary retina; 20.61 +/- 8.92, 358.64 +/- 212.66, 1.20 +/- 0.66 in the nasal juxtapapillary retina; 28.89 +/- 11.52, 497.97 +/- 278.70, 1.64 +/- 0.83 in the avascular area of temporal retina; 25.00 +/- 8.04, 455.87 +/- 203.65, 1.55 +/- 0.64 in the avascular area of nasal retina. CONCLUSIONS: Confocal laser scanning Doppler retinal flowmetry, which is based on confocal laser scanning technology and Doppler flowmetry, is an effective clinical method to determine the blood flow in optic nerve head and retina.

[Relationship between the prognosis of retinal detachment in emmetropic eyes and vitreous changes].

OBJECTIVE: To study the relationship between surgical prognosis of retinal detachment (RD) in emmetropic or myopic eyes and vitreous changes. METHODS: Postoperatively, 1,294 eyes of 1,262 patients with rhegmatogenous RD including 243 emmetropic and 1,031 myopic eyes were retrospectively studied. The affects of proliferative vitreoretinopathy (PVR), posterior vitreous detachment (PVD), vitreous liquefaction and concentration on the surgical prognosis were evaluated. RESULTS: The cure rate in emmetropic group (88.5%) was lower than that in myopic group (93.8%, P < 0.01). Also, the cure rate by initial surgery in emmetropic group (65.4%) was lower than that in myopic group (76.4%, P < 0.01). In addition, the occurrences of PVD (39.6%) and liquefaction (76.1%) in emmetropic group were lower than that in myopic group (51.7%, and 81.8% respectively, P < 0.05). However, focal vitreous concentration in emmetropic group (56.8%) was much higher than that in myopic group (19.6%, P < 0.01). CONCLUSIONS: The surgical prognosis of RD especially by initial operation is worse in emmetropic group than that in myopic group. The lower occurrences of PVD and liquefaction but higher occurrence of focal vitreous concentration in emmetropic eyes may have impact on the prognosis. It is possible that the vitreous characteristics in emmetropic eyes with RD have relationship to the occurrence and development of PVR that has the influence on the prognosis to some extent.

[Technique of enzyme digestion adding brushing for isolating bronchial epithelial cells].

This study improved the previous techniques of harvesting rabbit bronchial epithelial cells. With 0.05% of trypsin on the epithelial side of trachea and bronchus, a mild digestion was used before a brushing protocol. The cells were identified with immunocytochemistry and electron microscope, both the cell viability and the membrane integrity of cells were evaluated. A comparison analysis of this method with the simple mechanic brushing and the simple digestion method were done. The results showed that, with this method, cells in a high purity and high viability could be obtained and the recovered cell number could be enough for experiments. It can be concluded that the method is useful and effective for isolating bronchial epithelial cells especially for small animals.

[Modulation of endothelin-1 on pulmonary surfactant synthesis of cultured alveolar type II cells].

The effects of endothelin-1(ET-1) on pulmonary surfactant(PS) synthesis of cultured AT II cells were observed. The results showed that: 1. ET-1(10(-11)-10(-8) mol.L-1) enhanced PS synthesis of cultured AT II cells in a dose-dependent manner. 2. The minimum effective concentration of ET-1 enhanced PS synthesis of cultured AT II cells was higher than that of lung explants. 3. ETA antagonist BQ123 decreased the effect of ET-1 on PS synthesis, but ETB antagonist BQ788 did not change the effect. 4. ET-1 (10(-12) and 10(-10) mol.L-1) had no effect on the proliferation of AT II cells. These results confirmed that ET-1 can enhance directly and indirectly on the PS synthesis of AT II cells mediated via ETA receptor. The effect of ET-1 on PS synthesis was not induced by changing the number of AT II cells.

[Affection of epidermal growth factor on VIP secretion and VIPR expression in airway epithelial cells].

In order to confirm the essential role of vasoactive intestinal peptide(VIP) on protection of local airway epithelium and investigate the modulation of the protective effect of VIP, secretion of VIP and expression of VIP receptor(VIPR) were studied in bronchial epithelial cells(BEC), and alveolar macrophages (AM). The radioimmunoassay was used for determining VIP secretion and the radio-ligand affinity analysis for VIPR expression. The effect of epidermal growth factor(EGF) was observed. The results showed that: 1. Both BEC and AM secreted VIP basically without remarkable stimulus, and some vesicles with electron-dense core which usually exist specifically in neuropeptide secreting cells were found in plasma of BEC; 2. In a dose-dependent manner, EGF promoted BEC to secrete VIP(r = 0.84, P < 0.05), and it was found that VIP secretion from AM was also elevated in EGF pretreated group(P < 0.05); 3. EGF exhibited a dose-dependent up-regulatory effect on the expression of VIPR in BEC(r = 0.95, P < 0.01). These results indicate that VIP may play an essential role in the protection at local airway by autocrine or paracrine, and the protective effect can be modulated by growth factors.

[Comparison of constituents extracted from prepared Licorice by semi-bionic extraction with those by water extraction].

OBJECTIVE: To study the granulation of prepared licorice. METHOD: Semi-bionic extraction(SBE) and water extraction(WE) were used in the extraction of prepared licorice, and then compared with glycyrrhetinic acid, total flavone and extract with which were taken as the indexes. RESULT: SBE is superior to WE. CONCLUSION: It is better to use SBE in the preparation of oral licorice granules.

[Studies on chemical constituents of Euphorbia ebracteolata Ilayata].

OBJECTIVE: To search for the effective anti-epilepsiy components from the Chinese folk drug Langdu, the root of Euphorbia ebracteolata. METHOD: Chromatography and spectroscopic analysis were used to isolate and elucidate the chemical constituents in the plant. RESULT: Four compounds, namely 10-nonacosanol, 24-methylenecycloartanol, stigmasterol and dacosterol have been isolated from the precipitate of the plant's methanol extract. CONCLUSION: 10-nonacosarnol and 24-methylenecycloartanol were obtained from the root of E. ebracteolate for the first time.

[The immunohistochemistry and clinic study on surgical margin of laryngeal cancer].

OBJECTIVE: To probe into the safe incisal range of removing laryngectomy cancer determined from the albumen balance of cells. METHOD: LSAB or SABC of immunohistochemistry was applied, and three groups of samples with cancer tissue, nearby cancer tissue and distal cancer tissue of 36 primary laryngectomy cancer were tested by antibodies p16, p21, TGF-beta 1, PCNA and EGFR. RESULT: The difference of the positive rate with the five antibodies between the cancer group and the two side incisal group were very notable with P < 0.01 or P < 0.05, but which between nearby cancer tissue and distal cancer tissue were not notable with P > 0.05. The differences of the positive rates with 5 antibodied between high differentiation group and low differentiation group were notable with P < 0.05, and P < 0.05 with PCNA in the group whether the lymphonodi cervicales transmitted or not. The incisals for all the cases are beyond 5 mm, the recurerence rate is 3%. CONCLUSION: The 5 mm incisal for laryngectomy cancer is appropriate on the albumen cevel of cells.

[The normal parameters in people with static posturography].

OBJECTIVE: To get the normal parameters of static posturography in people. METHOD: 279 normal people from 5 to 80 year old were measured when their eyes opened and closed separately by G-5500 gravity sway apparatus. RESULT: The normal parameters of static posturography were acquired. CONCLUSION: The result can be taken as normal in clinical examination.
