[Observations of predominant methanogens on biomass from AF, ABR, and UASB reactors treating different wastewaters].

The biomass taken from AF, ABR, and UASB digesters treating municipal wastewater, swine waste and molassese stillage wastewater, municipal and molassese stillage wastewater respectively, were observed by light, epifluorescence microscope and scanning electron microscope. The prevalent methanogens in these digesters were Methanothrix. Exception for ABR carried out to treat swine waste, Methanosarcina with three forms of aggregations, cysts, and granules were presented in other digesters. A suggestion can be made that reactors with packing materials, such as, anaerobic filters, anaerobic contact beds and two-phase methanation digesters can be used to accumulate Methanosarcina due to their morphological, mu max, and Ks characteristics, in order to increase the wastewater treatment efficiency further.

[Fructose-1,6-diphosphate in the treatment of chronic heart failure].

Fructose-1,6-diphosphate (FDP) was given to 30 patients with chronic heart failure (CHF) caused by various kinds of heart diseases with the purpose to evaluate the effects of FDP on CHF patients. Definite hemodynamic and clinical improvement has been found in this group. CO increased by 1.61 +/- 0.31 L/Min (35%) (P less than 0.01) PCWP decreased by 5.5 +/- 1.08 mmHg (31%); mean PAP decreased by 5.8 +/- 2.07 mmHg (P less than 0.05). EF increased by 6.9 +/- 1.5 (15.1%) as shown by echocardiography and the peak effect of the drug appeared at 2 hours after administration. The results showed that FDP is effective in the treatment of heart failure, especially in patients with dysfunction of other organs.

Effects of betaxolol on heart rate in patients with a recent transmural myocardial infarction.

A randomized double-blind trial was performed with betaxolol, a beta 1-selective and long-acting beta-blocker, in patients with a recent first uncomplicated acute myocardial infarction (AMI). Patients were treated between the 3rd and 14th day after the onset of AMI with either a single oral dosage of 20 mg betaxolol or placebo. The effects on heart rate, ventricular and supraventricular arrhythmias were studied by continuous 24 hours ECG recordings on the 7th and between the 9th to 12th after the onset of AMI and by a submaximal exercise test on the 12th day. Hourly mean, peak and minimal heart rate were during the whole day significantly lower in the active treatment group. In betaxolol treated patients diurnal variations in heart rate were definitely changed. No differences in the occurrence of ventricular arrhythmias were found between both groups; auricular fibrillation was more common in placebo treated patients. Heart rate and pressure rate product were significantly higher during exercise in the placebo group. During exercise ventricular arrhythmias were infrequent in both groups. The clinical tolerance of betaxolol was excellent.

Acute myocardial infarction in the very elderly. A comparison with younger age groups.

In a consecutive series of 2312 patients with acute myocardial infarction (AMI) admitted from 1973 till 1979, 188 were 80 years or older (group III). They were compared with 1167 patients younger than 65 years (group I) and 957 aged 65 to 79 years (group II). The sex ratio (males/females) fell from 5.46 in group I to 0.9 in group III. Group III patients had more frequently a history of previous heart failure and more often atypical or no chest pain before admission. Less group III patients were admitted within 4 hours after onset of symptoms, but the incidence of heart failure, pulmonary edema and cardiogenic shock on admission and during CCU stay was definitely higher than in younger patients. Atrial arrhythmias, 2nd and 3rd degree atrioventricular block, complete bundle branch block and intraventricular conduction disturbances occurred more frequently in group III. The electrocardiographic extent and location of the infarction and peak enzyme levels were similar in the three groups. Mortality in group III was 43.6% at the 28th day and 76.6% at one year after AMI. At different intervals after the onset of AMI mortality increased progressively from group I to III. Age by itself, probably on the basis of definite structural changes of the heart and of other organs occurring during aging, leads to higher early and late mortality in very elderly people.

Cardiac free wall rupture after acute myocardial infarction. Clinical and pathological analysis.

Clinical and pathological findings in 15 autopsy cases, 13 males and 2 females, confirming cardiac free wall rupture after AMI were reported. The incidence is 30.6% of all autopsy cases of AMI in Chinese PLA General Hospital from 1958 to 1979. The ages ranged from 46 to 79 years, 10 being above 60 years. For 73.3% it was the first AMI and 66.7% of the patients had a history of hypertension. Thirteen of the 15 patients died within 5 days after the onset of AMI and another 2 within 7 days. When the cardiac rupture occurred, the ECG generally showed bradycardia, AV-junctional rhythm, III degrees AV block or isorhythmic ventricular rhythm and cardiac arrest. Both the gross and microscopic AMI were examined in 13 cases. All of them had a septal infarct, but only 2 had an ECG diagnosis. Of the 6 patients with inferior MI on ECG, 5 had right and left coronary lesions worse than grade III. The effective prevention of cardiac rupture consists of early diagnosis, control of chest pain and vomiting, prevention or treatment of hypertension or hypotension and 1 to 2 weeks of bed rest after the onset of AMI.

The value of radioimmunoassay of myoglobin in the diagnosis of acute myocardial infarction.

Thirty-one patients with acute myocardial infarction (AMI) or doubtful AMI, admitted to the coronary care unit (CCU), were selected for study of serum myoglobin (Mb). In 22 cases with AMI a sequential study of serum Mb shows that it begins to rise 2-3 hours after the onset of the AMI. The average value for the first blood sample taken within 12 hours after the onset was 268.7 +/- 57 ng/ml (ranging from 86-800), and the elevation within 24 hours averaged 2.4-5.0 times the normal value. They were distinctly higher than those of SGOT (0.6-2.1), CPK (2.0-3.4) and LDH (0.6-1.9). Taking into account the peak values determined within 12 hours, Mb showed a 100% correlation with the diagnosis, CPK 85.7% (12/17), GOT 35.9% (6/17) and LDH showed a 13.3% (2/15) correlation. In patients with angina pectoris and old MI, serum Mb was within the normal range, suggesting that the determination of serum Mb is of practical value in the differential diagnosis of angina pectoris and AMI.
