Trandolapril in left ventricular dysfunction after myocardial infarction: focus on the TRACE study.

The new angiotensin-converting enzyme (ACE) inhibitor trandolapril has been the subject of a broad pharmacological and clinical development program. The active metabolite, trandolaprilat, has been found to have a high level of affinity for angiotensin-converting enzyme. Trandolapril has demonstrated potent ACE inhibition, a long plasma half-life and a high degree of lipophilicity. The drug has been used to treat patients with mild-to-moderate hypertension and congestive heart failure following myocardial infarction. In the former patients, a once-daily dosage of trandolapril has produced significant and long-lasting reductions in blood pressure and has reduced left ventricular hypertrophy. Trandolapril is one of a few ACE inhibitors with a mean trough:peak ratio of blood pressure reduction with once-daily administration of over 50%. In addition, studies in hypertensive patients have revealed still greater reductions in blood pressure when trandolapril is combined with the calcium antagonist verapamil. In patients with left ventricular dysfunction after myocardial infarction, the large TRACE trial showed that mortality was reduced and life expectancy increased with trandolapril treatment.

The antiangiogenic and therapeutic implications of endostatin.

Angiogenesis plays a vital role in the pathology of cancer, ischemic diseases and chronic inflammation, among other conditions. Endostatin, a newly found protein that is distributed in some parts of the human body, has been demonstrated to have a strong inhibitory role in angiogenesis. It specifically inhibits the proliferation of endothelial cells and induces their apoptosis both in vitro and in vivo. Preclinical research has proven its effective role in the treatment of various experimental tumors in rodents. Although endostatin therapy has entered phase II clinical trials in the USA, the exact mechanism and its effects on antiangiogenesis, especially the action on the suppression of endothelial cell proliferation and induction of apoptosis, remain unclear. The treatment modality for malignancies and other angiogenesis-related diseases still requires further analysis.

Ramosetron, a 5-HT3 receptor antagonist for the control of nausea and vomiting.

Ramosetron is a selective serotonin 5-HT3 receptor antagonist with an affinity higher than that of the previously available drugs ondansetron, granisetron and tropisetron. Ramosetron was shown in pharmacological assays to inhibit activities mediated by 5-HT3 receptors, such as emesis caused by cisplatin. In clinical trials, ramostreon was at least as effective as the reference "setrons" against nausea and vomiting induced by chemotherapy or surgical interventions, but the efficacy was well maintained during a 48-h period, so that efficacy was significantly higher with ramosetron in terms of nausea and vomiting 6-48 h after treatment. Hence, ramosetron represents a good alternative over previously available drugs for the prophylaxis and treatment of chemotherapy-induced and postoperative nausea and vomiting. Data in children also support the efficacy of ramosetron, which is well tolerated by both child and adult patients.

Recombinant human endostatin is beneficial to endothelial cell growth exposed to mildly oxidized low-density lipoproteins.

Endostatin significantly reduced atherosclerosis in genetically susceptible mice. One of the main factors associated with atherogenesis is oxidized low-density lipoproteins (LDL), which also causes apoptosis of endothelial cells. Therefore, we proposed that the antiatherogenic effect of endostatin was partly associated with its protective effect on the endothelial injury induced by oxidized LDL. To confirm such a hypothesis, we studied the effects of recombinant human endostatin (rhEndo) on the proliferation of cultured endothelial cells exposed to mildly oxidized LDL (mox-LDL), rhEndo did not show an obvious inhibitory effect on the proliferation of rabbit aorta endothelial cells (RAEC) (p > 0.05), while mox-LDL inhibited their proliferation (p < 0.01 or p < 0.05). Interestingly, rhEndo seemed to antagonize the role of mox-LDL in inhibiting the proliferation of RAEC. rhEndo seemed, thus, to be beneficial to the proliferating endothelial cells, suggesting that it protects RAECs from the injury caused by mox-LDL. The activity of rhEndo in endothelial cells may possibly result from the interaction of different factors in cell signaling, which remains to be further elucidated.

Lornoxicam, a new potent NSAID with an improved tolerability profile.

Lornoxicam is a member of the oxicam group of nonsteroidal antiinflammatory drugs (NSAIDs). Oxicams have potent antiinflammatory and analgesic effects, but their use is associated with a high risk of gastrointestinal adverse effects. Lornoxicam has been shown to be at least as effective as comparative NSAIDs and more effective than 10 mg morphine when used at doses > or = 8 mg to control pain after oral surgery. In addition, oral doses of lornoxicam of 16-24 mg daily have been more effective than tramadol 300 mg daily in pain following knee surgery. Lornoxicam combines the high therapeutic potency of oxicams with an improved gastrointestinal toxicity profile as compared to naproxen, for example. This is probably due to the short half-life of lornoxicam as compared to the other oxicams. The clinical trials published so far, mostly comparative, clearly do- cument the efficacy of lornoxicam as a potent analgesic with excellent antiinflammatory properties in a range of painful and/or inflammatory conditions, including postoperative pain and rheumatoid arthritis.

Alosetron.

Alosetron, a 5-HT(3)-receptor antagonist that is very closely related to ondansetron in terms of both chemistry and pharmacology, is the first compound of this type to be developed for irritable bowel syndrome. Clinical data for up to 3 months of treatment indicate that alosetron is orally bioavailable in tablet form, is well tolerated and is significantly superior to both placebo and the smooth muscle relaxant, mebeverine, in improving perception of visceral pain, spasms and diarrhea in female diarrhea-predominant irritable bowel syndrome. In males, symptoms were not alleviated to a statistically significant extent.

Perspectives in the treatment of Parkinson's disease: COMT inhibitors open up new treatment strategies.

Catechol O-methyltransferase (COMT) inhibition has been proposed as a means to increase levodopa bioavailability in patients with Parkinson's disease, which may be useful in reducing the total dose of levodopa, diminishing its frequency of administration and, more importantly, improving fluctuations in the response to each individual dose of levodopa, which usually develop after chronic treatment. Entacapone is a potent, selective peripheral catechol O-methyltransferase inhibitor which effectively inhibits the O-methylation of levodopa, thus increasing its central bioavailability and potentiating its behavioral effects. In clinical trials in patients with Parkinson's disease, entacapone has prolonged and improved the therapeutic effect of levodopa combined with a dopa decarboxylase inhibitor. The new drug has increased the "on" time duration, decreased the "off" time duration and has reduced the total dose requirements of levodopa. Entacapone has shown optimal tolerability at the currently recommended doses, with the added benefit of a lower required dose of levodopa. This, in turn, improves the tolerability of levodopa and thus the overall tolerability of the treatment of advanced Parkinson's disease.
