Extension of a numerical algorithm to proton dose calculations. I. Comparisons with Monte Carlo simulations.

A numerical algorithm originally developed for electron dose calculations [Med. Phys. 21, 1591 (1994)] has been modified for use with proton beams. The algorithm recursively propagates the proton distribution in energy, angle, and space from one level in an absorbing medium to another at slightly greater depth until all protons stop. Vavilov's theory is used to predict, at any point in the absorber, the broadening of the primary proton energy-spectrum. Moliere's theory is applied to describe the angular distribution, and it is shown that the Gaussian first term of Moliere's series expansion is of sufficient accuracy for dose calculations. These multiple scattering and energy loss distributions are sampled using equal probability spacing to optimize computational speed while maintaining calculational accuracy. Inelastic nuclear collisions along the proton trajectories are modeled by a simple exponential extinction. Predictions of the algorithm for absolute dose deposition by a 160 MeV initially monoenergetic proton beam are compared with the results of Monte Carlo simulations performed with the PTRAN code. The excellent level of agreement between the results of these two methods of dose calculation (< 5% dose and < 3 mm spatial deviations) demonstrate that dose deposition from proton beams may be computed to high accuracy using this algorithm without the need for extensive empirical measurement as input.

Bovine aortic endothelial cells contain an angiotensin-(1-7) receptor.

Angiotensin-(1-7) is a novel peptide of the renin-angiotensin system that counteracts the pressor and proliferative responses to angiotensin II. We now report that cultured bovine aortic endothelial cells contain a saturable, high-affinity [125I]angiotensin-(1-7) binding site with an affinity of 19.3 +/- 10.7 nmol/L and a density of 1351 +/- 710 fmol/mg protein. Angiotensin-(1-7) competed at a second lower-affinity site, with an IC50 of 2.9 mumol/L. The high-affinity angiotensin II receptor antagonist sarcosine1-isoleucine8-angiotensin II blocked [125I]angiotensin-(1-7) binding to bovine aortic endothelial cells at both a high- (IC50 = 1.3 nmol/L) and a low-affinity (IC50 = 6.2 mumol/L) binding site. In contrast, D-alanine7-angiotensin-(1-7) completely blocked [125I]angiotensin-(1-7) binding, with an IC50 of 19.8 nmol/L, suggesting that D-alanine7-angiotensin-(1-7) may selectively block responses to angiotensin-(1-7) in endothelial cells. Neither the AT1 antagonist losartan nor the AT2 antagonist PD 123319 exhibited significant competition for [125I]angiotensin-(1-7) binding to endothelial cells isolated from bovine aorta, in agreement with the absence of detectable mRNAs encoding typical angiotensin receptor subtypes 1 or 2 (AT1 or AT2). Angiotensin II also competed for [125I]angiotensin-(1-7) binding to bovine aortic endothelial cells; however, the relative affinity was 13-fold lower than angiotensin-(1-7), suggesting a preference for angiotensin-(1-7) over angiotensin II. These results demonstrate that bovine aortic endothelial cells contain a unique non-AT1, non-AT2 angiotensin receptor that preferentially binds angiotensin-(1-7).

[Effects of dexamethasone on cAMP production induced by prostaglandin E2 in aortic smooth muscle cells].

OBJECTIVE: To study the effects of dexamethasone on cAMP production induced by prostaglandin E2 (PGE2) in cultured rabbit aortic smooth muscle cells. METHODS: cAMP concentration was determined by protein-competition-contact assay. RESULTS: Dexamethasone significantly inhibited cAMP accumulation induced by PGE2 in a dose dependent manner in the range between 10 pmol/L and 10 nmol/L. The inhibitory effect grew stronger as the time of pretreatment extended. Adenylate cyclase activity induced by PGE2 was significantly reduced in the cells pretreated with dexamethasone. CONCLUSION: The results strongly suggest that dexamethasone inhibits cAMP production induced by PGE2 in cultured aortic smooth muscle cells and the inhibitory effect is exerted at the level of adenylate cyclase. In addition, the inhibitory effect of PGE2 on cultured smooth muscle cell proliferation was resisted by dexamethasone, anyhow no direct enhancing effect on cell proliferation by dexamethasone was detected.

[The protective effect of SOD on injury of bone marrow hematopoietic progenitor cells of mice under 4 degrees C storage].

The protective effect of Cu, Zn, SOD on bone marrow hematopoietic progenitor cells of mice, under 4 degrees C storage was studied. The results showed, if 1.65 U or 0.165 U/ml SOD was added to the cell suspension for 3 days at 4 degrees C, the production rate of CFU-GM, CFU-E, BFU-E, CFU-Meg and CFU-Mix were 6.2, 2.6, 2.9, 4.0 and 5.1 times that of control group (without SOD) respectively. The survival rate of hematopoietic progenitor cells increased obviously. The mechanism of SOD effect is supposed to prevent hematopoietic progenitor cells from death, instead of the regulating proliferation. It may be closely related to the antioxidation of SOD which scavenges the superoxide free radicals.

[The dosage of hyperbaric oxygen therapy in patients with sudden hearing loss].

Free radical metabolism substances in different stages of hyperbaric oxygen (HBO) therapy in 33 patients with sudden hearing loss were studied. The time of treatment is 40 min each day and the pressure of HBO is 0.2 MPa, with treatment lasting for 20 days. The result indicated that after ten times of HBO therapy, the level of lipid peroxidation (LPO) in plasma increased slightly and the activities of superoxide dismutase (SOD) and glutathione peroxidase (GSHPx) in erythrocytes decrease was not significant (P > 0.05). Only the levels of glutathione (GSH) in erythrocytes decreased significantly (P < 0.05). After 20 days, HBO therapy, the plasma LPO value increased significantly (P < 0.05), the activities of SOD, GSHPx and the values of GSH in erythrocytes decreased significantly (P < 0.05). The results indicated that continuous therapy for patients with HBO should not be carried out for more than 20 days.

[Treatment of lingual angiocavernoma by selective arteriocatheterization with urea injection].

Selective arteriocatheterization with urea injection was administered to 36 cases of lingual angiocavernoma. There were 20 males and 16 females, ranging in age from 6 to 18 years with an average of 12 years. Two to five mililiters of 40% urea were injected once a day for 10-15 days as a course of treatment, 29 cases (80.6%) were cured in one course of treatment. The other 7 cases (19.4%) were cured in the 2nd course of treatment. The results suggest that selective arteriocatheterization with urea injection is a safe, convenient and effective technique in the treatment of ingual angiocavernoma.

Proposal for a New Staging Scheme for Intraocular and Extraocular Retinoblastoma Based on an Analysis of 103 Globes.

Management of retinoblastoma is highly individualized and depends heavily on grouping or staging. In view of evolving methods of imaging and treating retinoblastoma, we have developed and evaluated a revised staging scheme. We analyzed the survival data of 73 patients treated at St. Jude Children's Research Hospital to compare the ability of the Reese-Ellsworth grouping system, the original St. Jude, and the modified St. Jude staging schemes to predict progression-free survival. None of the staging schemes significantly correlated with progression-free survival. This modified staging scheme provides an instrument for assessing the natural history of retinoblastoma based on ophthalmolgic, other clinical, and imaging findings. Because it can identify patients at higher risk, the modified St. Jude scheme may be useful in selecting appropriate therapy regimens for children with retinoblastoma.

[Detection and differentiation of Helicobacter pylori from gastric biopsy and saliva by PCR-SSCP].

In this study we detected the species-specific antigen gene of Helicobacter pylori (Hp) from gastric biopsy specimens and saliva of 48 patients by polymerase chain reaction (PCR), then adopted single strand conformation polymorphism (SSCP) to differentiate Hp strains from population and different sites of the same individual. Among patients of positive gastric biopsy results, the detection rate of saliva was 72.2% (26/36). In 12 patients of negative gastric biopsy results, 2 saliva specimens were positive. Further SSCP analysis demonstrated that in population the infected Hp strains showed 6 different band patterns. SSCP analysis of PCR products from the gastric biopsy specimen and saliva of the same individual showed no significant difference. The results showed that Hp at different sites of one individual probably belongs to the same strain.

[The primarily study on the correlation between the cephalometric characteristics and the results of ploysomnography].

A study on correlation between temporomandibular joint ankylosis and Obstructive Sleep Apnea Syndrome (OSAS) was conducted. The statistic results demonstrate that there is significant correlation between (TA + SPA)/OPA and AI, and there is negative correlation between PAS and AI, (TA + SPA)/OPA and SaO2min by correlation and step regression between the cephalometric analysis and polysomnogram of fourteen cases of OSAS patients.

[CT analysis of hepatic metastases].

OBJECTIVE: To describe the manifestations of the hepatic metastases. To investigate the value of CT examination in diagnosis of hepatic metastases. METHOD: CT radiographs of 90 patients with hepatic metastases were studied retrospectively. The tumor number, size, density, shape, spreading in the hepatic parenchyma were analyzed. The size of liver and spleen was measured. RESULTS: Three or more metastatic nodules spreading in the hepatic parenchyma were found in 65 (72%) cases. Two nodules were shown in 6 (7%) cases. 19(21%) cases had only single nodule. 29 of the metastatic deposits located in the hepatic parenchyma near the liver edge. The size of metastatic nodules varied from 0.5 cm to 14 cm. In 87 cases, the density of metastatic nodules was lower than the surround liver parenchyma. In 3 cases of diffuse fatty liver, the metastatic deposits appeared as higher CT density nodules. Central necrosis of the tumor foci was seen in 44 cases. Striking hepatomegaly was noted in 31 cases with widespread nodules. In 26 cases which had one and two nodules, the shape of the liver was normal. In 45 cases, the primary tumour was found by CT at the same time. CONCLUSION: The CT findings of hepatic metastases differ. Multiple nodules disseminating in liver parenchyma is the general sign. Tiny nodules without other clinical signs can be detected by CT.

[Detection of occult tumor cells in resected lymph nodes of patients with stage I carcinoma and its clinico-pathological significance].

OBJECTIVE: Detection of tumor micrometastases in resected lymph nodes of cancer patients to obtain more reliable information concerning nodal status and its clinicopathological importance. METHODS: Paraffin blocks of 3,715 resected lymph nodes from 350 Stage I cancer patients, suffering from NSCLC (n = 94), breast cancer (n = 112), esophageal cancer (n = 115), and vulvar cancer (n = 29), were re-examined by immunohisto-chemical staining, using monoclonal anti-cyokeratins (AE1/AE3), anti-EMA, and polyclonal anti-keratins antibodies. RESULTS: Occult nodal metastases was observed in 113 of 350 cancer patients (32.5%), and in 203 of 3,715 lymph nodes examined. The positive rates both in patients and in lymph nodes were higher in NSCLC than in others. Occult nodal metastasis was seen in 58% of pulmonary squamous-cell carcinomas and 53.8% of adenocarcinomas, while it was seen in 22.5% of esophageal and 10.3% of vulval squamous-cell carcinomas, and in 27.7% of breast adenocarcinomas (P < 0.05). Follow up of a fraction of breast cancer patients showed that the prognosis of patients with positive nodes was worse than that in patients with negative nodes (P < 0.05). CONCLUSION: The data suggested that the immunohistochemical technique can significantly facilitate the detection of micrometastatic tumor cells in lymph nodes. The frequency of occult lymph node metastasis may have significant impact on the prognosis of cancer patients.

[Laboratory study on pharmacology of Chinese medication qizhi weitong powder].

The Chinese medication Qizhi Weitong Powder has been proved very helpful in depressing gastric evacuation and small intestinal propel reducing gastric secretion, lowering acidity of gastric liquid and activity of pepsin as well as enhancing peristalsis of domestic rabbits. Other effects such as distinct protection to ligated ulcer of pylorus and painkilling have also been confirmed.

[Chemical studies on Campylotropis hirtella (Franch. Schindl.)].

From the roots of Campylotropis hirtella 6 compounds were isolated. Based on the extensive spectrae analysis their chemical structures were identified as beta-sitosterol, oleanolic acid, ursolic acid, euscaphic acid, tomentic acid and rosamultin.

[ECG changes during [correction of dring] high G centrifuge training].

ECG of 47 healthy subjects, during G-training on the human centrifuge were recorded and analyzed. Training consisted of 4 G profiles and carried out in 6 d. Subjects were divided into 3 groups according to different anti-G equipment used. The results showed that: high incidence of arrhythmia (87%) was observed, and it increased with the increase of G level; subjects with R/S < 1 under +Gz environment, or HR sharply dropped immediately after the +Gz load have lower G-tolerance; incidence of arrhythmia is lower when advanced anti-G equipment was used.

[Effect of different Anti-G suits on lower limb blood flow].

To study effects of different Anti-G suits (AGS) and different inflation pressure on human cardiac system, changes in lower limb arterial blood flow (BF), heart rate and blood pressure in 5 male subjects with three bladder coverage AGS and a capstain AGS were observed and recorded until inflation pressure reached to its maximum. The percent of bladder coverage was about 30%, 65% and 90% of lower body for three bladder AGS. All AGS were inflated to maximum pressure in +1Gz seating position. The results showed that calf BF, could be blocked by bladder AGS, and the relationship between the mean pressure for BF block and the percentage of bladder coverage was y = 29.66 - 16.35x. (P < 0.0001, r = -0.9229). While the thigh and calf BF was not blocked in subjects with the capsule AGS. The results indicate that the effect of "the higher bladder coverage AGS + lower IP" is probably equal to that of "the lower bladder coverage AGS + higher IP" in holding calf BF.
