Occult systemic infection and persistent simian immunodeficiency virus (SIV)-specific CD4(+)-T-cell proliferative responses in rhesus macaques that were transiently viremic after intravaginal inoculation of SIV.

The intact cervicovaginal mucosa is a relative barrier to the sexual transmission of human immunodeficiency virus type 1 (HIV-1). In the simian immunodeficiency virus (SIV) macaque model of HIV infection, seronegative transient viremia (STV; virus isolation positive followed by repeated negative cultures) occurs after intravaginal inoculation of a low dose of pathogenic SIVmac251 (C. J. Miller, M. Marthas, J. Torten, N. Alexander, J. Moore, G. Doncel, and A. Hendrickx, J. Virol. 68:6391-6400, 1994). Thirty-one adult female macaques that had been inoculated intravaginally with pathogenic SIVmac251 became transiently viremic. One monkey that had been culture negative for a year after SIV inoculation became persistently viremic and developed simian AIDS. No other STV monkey developed persistent viremia or disease. Results of very sensitive assays showed that 6 of 31 monkeys had weak SIV-specific antibody responses. SIV-specific antibodies were not detected in the cervicovaginal secretions of 10 STV monkeys examined. Twenty of 26 monkeys had lymphocyte proliferative responses to p55(gag) and/or gp130(env) antigens; 3 of 6 animals, including the monkey that became persistently viremic, had detectable cytotoxic T-lymphocyte (CTL) responses to SIV. At necropsy, lymphoid tissues and vaginal mucosa were virus culture negative, but in 10 of 10 animals, SIV provirus was detected by PCR using gag-specific primer pairs. Fifty percent of the PCR-positive tissue samples were also positive for SIV gag RNA by reverse transcriptase PCR. Thus, transient viremia following intravaginal inoculation of pathogenic SIV is associated with persistent, systemic infection, either latent or very low level productive. Atypical immune responses, characterized by lymphocyte proliferation and some CTL responses in the absence of conventionally detectable antibodies, develop in transiently viremic monkeys.

Role of monkey cerebellar nuclei in skill for sequential movement.

To examine whether the cerebellum is involved in learning and memory of visuomotor sequences, we trained two monkeys on a sequential button press task and inactivated different portions of the cerebellar nuclei by injecting a small amount of muscimol (gamma-aminobutyric acid agonist). Before the injection experiments started, the monkeys had learned a set of sequences (n = 21 and 12) extensively. After each injection, we had the monkeys perform the learned sequences and, in addition, learn new sequences. We found deficits in learning/memory by the injections into the dorsal and central part of the dentate nucleus. The number of errors increased significantly for the learned sequences but not for the new sequences. This effect was present only when the hand ipsilateral to the muscimol injection was used. Consistent with this result, anticipatory saccades, the occurrence of which is correlated closely with motor skill, also became less frequent particularly when the ipsilateral hand was used. No effect on learning/memory was observed after injections into the ventral or lateral parts of the dentate nucleus, interpositus nucleus, or fastigial nucleus. In contrast, hand movements became slower after ipsilateral injections at all of the injection sites. These results suggest that, among the cerebellar nuclei, the dentate nucleus, especially its dorsal and central regions, is related to the storage and/or retrieval of long-term memory for motor skill.

[Sequence analysis on HIV1 env V3 rigion from HIV1 infected intravenous drug users in Ruili city, Yunnan].

To study the molecular-epidemiologic features of HIV1 epidemic strains from intravenous drug users (IVDUs) in Yunnan, nested primers polymerase chain reaction was developed for the amplification of env gene V3 fragment of HIV1 strains. DNA sequencing and analysis were performed with these fragments. Results showed that the variation of HIV1 gp120 V3 amino acids sequences among 16 HIV1, strains was averaged 7% (0%-23%), when comparing their consensus sequence. The amino acids consensus sequences of gp120 V3 region showed that more than 90% of homology compared with that of HIV1 SF2 strain and HIV1 American-European strain. However, lower homology with that of HIV1 strains from Haiti, Japan and Africa was revealed. The results suggested that there was a close relation among these 16 HIV1 epidemic strains isolated in Yunnan Ruili in terms of virus evolution. The epidemic HIV1 strains in the area were predominately American-European, SF2 and their derivatives in this period.

[Influence of portal triad clamping on the intestine in pigs].

A pig model of liver ischemia and intestinal congestion was made by portal triad clamping (PTC) for 45 minutes to investigate the influence of PTC on intestine. The results were as follows: 1. The level of cathepsin D was increased significantly after PTC as compared with the value before clamping (P < 0.05); 2. The endotoxin and lactic acid were significantly increased after PTC comparing with those of pre-clamping (P < 0.05, P < 0.01) and the control group (P < 0.05, P < 0.01); 3. The intestinal mucosa had a significant damage (P < 0.05) after PTC. The results indicate that PTC can result in intestinal mucosal lesion and enterotoxin absorption. Therefore, the protection of intestinal functions is important in hepatic surgery and preoperative bowel preparation should be done.

[The use of pediculated super-long randomized flap in repairing soft tissue defects of the head and neck].

A double-pediculated randomized flap of the shoulder and thorax was designed. The flap was transferred to cover the wound on the recipient area. The wound on the donor site was closed directly or covered by split skin graft. From January 1990 to January 1997, this method was used in 10 patients. After operation, a six month to three year following up indicated that the flaps were completely survived and the soft tissue defects of the head and neck were repaired. As a consequence, the pediculated super-long randomized flap was effective in repairing soft tissue defect.

[Secretive activity of tumor necrosis factor alpha (TNF alpha) of marrow stromal cells in acute leukemia and its clinical significance].

OBJECTIVE: To investigate the TNF alpha secreting activity of marrow stromal cells in acute leukemia. METHODS: TNF alpha in vitro culture supernate of marrow stromal cells from 20 acute leukemia patients and 10 normal controls was detected. TNF alpha activities in patients and control serum and in patients leukemic cell culture supernate were also detected. RESULTS: TNF alpha activity in leukemic stromal cell supernate was significantly higher than that of the control (P < 0.05). Some AML cell could spontaneously secret TNF alpha. Serum TNF alpha activity of the patient group is also evidently higher than that of the control group (P < 0.001). CONCLUSION: There were qualitative and quantitative defects of marrow stromal cells in acute leukemia patients. Increased secretion of TNF alpha is probably an important factor leading to the abnormal proliferation of acute leukemia cells.

[Polymerase chain reaction associated with dig-labelled DNA hybridization of mip gene to identify new isolated Legionella pneumophila strains].

OBJECTIVE: By testing mip gene, one highly conservative virulence gene of Legionella pneumophila (Lp), as targets of polymerase chain reaction (PCR) and Digoxin-labelled probe hybridization to establish a rapid, specific and sensitive gene analysis method that can identify new isolated suspected bacteria strains as Lp. METHOD: Abstracting all the CDC reference Lp strains' and none-Lp Legionella strains' DNA, as well as the domestic isolated Lp strains' and some non-Legionella control bacteria's DNA as templates, then were processed. All the positive PCR products(if no PCR amplicons were available, the original whole DNA should be detected) were hybridized with Digoxingen-labelled mip gene probe in dot-blot procedure. RESULT: All the tested Lp strains had positive PCR and hybridization results, at the same time, all none-Legionella bacteria and none-Lp Legionella strains got negative results. 6 of 26 isolates were identified as Lp strains by this method. CONCLUSION: Such a Lp strain identification procedure shows high specificity and sensitivity (nearly 100%, in this study), and mean-while can be completed in a relative short time. Surely, this method can be largely available and has a potential value in diagnosis of clinical Legionellosis cases and pursuing the pathogen during Legionnaires' disease outbreaks.

[Analysis and application of polypeptide antigens of 13 mycobacterium strains].

OBJECTIVE: To analyze and compare polypeptide antigens of 13 Mycobacterium strains and try to find out tuberculosis associated antigens. METHOD: Polypeptide antigens were analyzed by using SDS-polyacrylamide gel electrophoresis (SDS-PAGE) and immunoblots techniques. Specific antibodies in serum or CSF samples obtained from 575 tuberculosis patients, 450 healthy individuals and 250 non-tuberculosis patients were detected by using immunoblots technique with purified specific antigen. RESULTS: The results showed that polypeptide of H37Rv, H37Ra, BCG, Bovis strains had the similar polypeptide bands and the similar immunogenicity in major polypeptide antigens. Molecular weight of major polypeptide antigens were 76,000, 65,000, 34,000-40,000, 28,000 and 25,000. The 76,000, 65,000 and 25,000 polypeptides were found in all the 13 Mycobacterium strains and they were common antigens probably. Purified specific antigen was used in serodiagnosis of tuberculosis and the sensitivity was 89.7%, while specificity was 95.7%. CONCLUSION: Polypeptide bands of Mycobacterium were found different and Mycobacterium strains could be identified by SDS-PAGE technique. The results suggested that specific antigen of H37Rv strain for antibody detection was a useful supplementary tool for diagnosis of extrapulmonary tuberculosis and bacteria-negative pulmonary tuberculosis.

[Comparative study on results of pulmonary tuberculosis surveillance before and during implementation of tuberculosis control project in Ningxia Hui autonomous region].

OBJECTIVE: To understand the effect of the World Bank-loaned tuberculosis control project in Ningxia Hui autonomous region. METHOD: Five counties which can reflect the situation of tuberculosis control in Ningxia were selected for observation, and the same method of surveillance was carried out. The subjects investigated were required to fill in the surveillance and the project forms at the same time, and the data of the tuberculosis control before (1990, 1991) and during (1995, 1996) implementation of the project were compared. RESULT: The registration rate of new smear-positive patients was 20.35 per 100,000 population during the implementation of the project, with a 3.06 times increase compared with that before implementation of the project. The proportion of new smear-positive patients increased from 21.37% to 61.70%, the smear-positive rate of newly registered patients from 22.80% to 35.32%, and the finding rate of new smear-positive patients from 7.97% to 31.11%. The mortality rate of pulmonary tuberculosis decreased by 58.36%. The total impact of finding and treatment increased from 7.60% to 31.90%. CONCLUSION: Satisfactory results have been achieved during the implementation of the project in Ningxia.

[Clinical study on relationship among Heart-Qi Deficiency, left ventricular function and myocardial ischemia in patients with coronary heart disease].

OBJECTIVE: To study the relationship among Heart-Qi Deficiency (HQD), left ventricular function and myocardial ischemia in patients with coronary heart disease (CHD). METHODS: Doppler echocardiogram, electrocardiogram were used to observe 103 patients of CHD clinically. Patients of stable angina pectoris treated with Radix Codonopsitis Pilosulae (50 cases) or Nifedipine (20 cases) were observed as well. RESULTS: Occurrence of left ventricular function insufficiency, both systolic and diastolic, was higher in CHD patients of HQD type as compared with that of non-HQD type. The left ventricular diastolic function showed high susceptibility (87%) and the left ventricular systolic function showed high specificity (88%) for HQD evaluation. Radix Codonopsitis Pilosulae could protect the left ventricular function and improve myocardial ischemia. CONCLUSIONS: Myocardial ischemia and blood stasis cause HQD, while the Chinese herbal medicine treatment on HQD could improve the blood stasis to alleviate myocardial ischemia effectively. The mechanism of which might be to enhance the positive diastolic and systolic function of cardiac muscles, reduce the resistance pressure and keep pressure of coronary perfusion so as to improve the blood supply of heart.

[Quantitative analysis of velopharyngeal function by nasopharyngeal fibroscope (NPF)].

OBJECTIVE: To estimate the reliability of the nasopharyngeal fibroscope (NPF) for assessing velopharyngeal function. METHODS: 16 well-pronounced males were examined by NPF which was positioned in different depth. Same procedures were repeated after a week. RESULTS: The results indicated that the rate of velopharyngeal incomptence (RVPI) of NPF had no significant difference when checked in different time and various depths. CONCLUSION: RVPI can be used as a parameter of quantitative analysis for velopharyngeal function, and our results suggested that it is reliable and reproduciable.

[Three cases of Dieulafoy's disease].

OBJECTIVE: To increase the knowledge of Dieulafoy's disease and decrease the misdiagnosis rate. METHOD: Three cases of Dieulafoy's disease were analyzed clinically and their therapeutic methods were discussed. RESULT: Subtotal gastrectomy including the lesion is better than other methods. CONCLUSION: It is necessary to understand the clinical pattern and pathological characteristics in order to increase the diagnosis rate and decrease misdiagnosis rate. Subtotal gastrectomy or wide wedge resection of the lesion is recommended.

[Construction of the prokaryotic expression plasmid pYNenv encoding the gp120 env gene of human immunodeficiency virus type 1 of yunnan strain].

The recombinant envelope protein of human immunodeficiency virus type 1(HIV-1) of Chinese Yunan epidemic strain was expressed in E. coli for confirming the antigenic domains of the protein. The recombinant prokaryotic expressive plasmid pYNenv was constructed by inserting the env DNA fragment which encode the gp120 of HIV-1 Yunnan strain into pBV220. The env DNA fragment was obtained by nested polymerase chain reaction with the HIV-1 infected patients peripheral blood macrophage and monocyte genomes from Yunan HIV-1 epidemic area as template. The recombinant plasmid pYNenv was confirmed by restriction enzyme analysis. The recombinant protein was expressed in E. coli DH10b by pYNenv after vibration culture at 30 degrees C for 20 hours then at 42 degrees C for 5 hours and confirmed by SDS-PAGE electrophoresis. The protein can specifically react with the serum of HIV-1 infected patient from Yunan epidemic area by Western blot assay. The result showed that the recombinant protein can be used as antigen for detection of HIV-1 membrane glycoprotein antibody as well as for the further study of the role of envelope protein in pathology.

[Detection of proviral gene of human immunodeficiency virus type 1 from dried blood spots].

A method for detection of proviral human immunodeficiency virus type 1 DNA fragment encoded pol gene in dried blood spots on filter paper by nested primers polymerase chain reaction (nPCR) has been developed. The filter paper was pre-saturated and dried by EDTA proteineas K. About 50 microliters whole blood from HIV1 infected individuals was collected and onto the piece of filter paper, and the paper was air dried stored sealed in a plastic bag at 4 degrees C or room temperature for 1 to 64 weeks. The PCR with outer primers was performed in 0.5% ml tube with the piece of filter paper. Following the PCR with the inner primers was finished. The results showed that template DNA could detected in piece of filter papers which stored at 4 degrees C for 40 weeks as well as stored at room temperature for 24 weeks. The results could directly determined by ethidium bromide staining of the PCR products when it compared with the PCR products of standard positive control and DNA molecular weight marker in electrophoresis analysis on agarose gel. The method is rapid, specific, and sensitive and has the ability to detect as little as 10 proviral template DNA. The collected samples could mail from basic laboratory to center laboratory and the method could be applied in confirming and screening of HIV1 infection.

[Mitral valve tissue characterization using acoustic microscopy].

Mitral valve hardness in the rough and clear zones was evaluated by measuring the propagation velocity (m/sec) of ultrasound through the valve, based on the hypothesis that harder tissues will have larger ultrasonic velocity values passing through the tissue. Mitral valve specimens were obtained from 16 normal subjects (age, 15-72 years) at autopsy. An acoustic microscope, operating at 450 MHz, was used to measure the ultrasonic velocity through the three layers of the valve: the atrialis; the spongiosa; and the fibrosa. Furthermore, the mean ultrasonic velocity through the three layers was measured. These measurements were conducted in the rough and clear zones of the valve. In the rough zone, the ultrasonic velocities were 1,634 +/- 71 in the atrialis, 1,574 +/- 37 in the spongiosa, and 1,726 +/- 97 m/sec in the fibrosa. In the clear zone, the ultrasonic velocities were 1,691 +/- 117 in the atrialis, 1,575 +/- 44 in the spongiosa, and 1,909 +/- 131 m/sec in the fibrosa. There were significant differences between velocities in the three layers in both the rough and clear zones. Furthermore, the mean ultrasonic velocity in the three layers in the clear zone (1,887 +/- 138 m/sec) was higher than that in the rough zone (1,642 +/- 53 m/sec; p < 0.001). These findings show that fibrosa, which is rich in collagen fibers, is harder than atrialis, which is rich in elastic fibers, and the spongiosa, which is poor in connective tissues, is the softest. The higher mean ultrasonic velocity in the clear zone than in the rough zone indicates that the clear zone is harder than the rough zone, although the clear zone is thinner than the rough zone.
