[Telomerase activity of common malignant tumors].

OBJECTIVE: To study the usefulness of detection of telomerase activity in the diagnosis of malignant tumors. METHODS: Modified telomeric repeat amplification protocol (TRAP) was employed to examine telomerase activity in 283 malignant tumor tissues, 42 adjacent tissues and 61 benign tumor tissues. RESULTS: Of the 283 malignant tumors and 42 adjacent tissues, telomerase was positive in 264 (93.3%) and 11 (26.1%), respectively. Only 4 of the 62 (6.6%) benign tumors were telomerase positive. CONCLUSION: Telomerase activity may be used as tumor marker in the diagnosis of malignant tumors.

[Relationship among telomerase activity, telomerase RNA and telomerase catalytic subunit gene expression and their significance in non small-cell lung cancer].

OBJECTIVE: To study telomerase activity (TA), telomerase RNA (hTR) and telomerase catalystic subunit (hTRT/hEST2) code gene expression in non-small cell lung cancer (NSCLC), their relationship and significance. METHODS: Telomerase repeat amplification protocol PCR (TRAP-PCR) for TA, RT-PCR for hTR and in situ hybridization for hTRT/hEST2 mRNA expression were performed. RESULTS: Positive rates of TA, hTR and hTRT/hEST2 were 68.4%(26/38), 51.7%(32/58) and 74.2%(43/58) in NSCLC. But, the adjacent and benign lung tissues were negative for expression of TA, hTR and hTRT/hEST2. A statistically significant positive correlation between TA and hTRT/hEST2 was observed(r = 0.84, P = 0.01), though there was no statistically significant positive association between TA and hTR (r = 0.16, P = 0.23). The median survivals of TA and hTRT/hEST2 positive groups (10.4 months, 7.5 months) were shorter than the negative ones (13.5 months, 14.7 months). However, only the hTRT/hEST2 group was found to show statistical significance to be an independent prognostic factor by multivariate Cox regression analysis. CONCLUSION: Telomerase activity, hTR expression and hTRT/hEST2 are frequently detected at higher levels in NSCLC than the pericancerous normal tissues. There is a positive correlation between TA and hTRT/hEST2, both of which are markers of poor magnificent phenotype of NSCLC. The hTRT/hEST2 has an independent prognostic significance.

Myocardial protection with pinacidil induced hyperpolarized arrest during cardiopulmonary bypass.

OBJECTIVE: To investigate the myocardial protective effects of pinacidil-induced hyperpolarized arrest and compare with those afforded by conventional depolarized hyperkalemic arrest. METHODS: Eighteen dogs were equally divided into three groups: normothermic hyperpolarized group (Group A), hypothermic hyperpolarized group (Group B), and hyperkalemic group (Group C). Pinacidil (50 mumol/L) containing 37 degrees C St. Thomas solution (K+ 5 mmol/L, 10 ml/kg), pinacidil (50 mumol/L, Sigma, USA) containing 4 degrees C St. Thomas solution (K+ 5 mmol/L, 10 ml/kg) and 4 degrees C standard St. Thomas solution (K+ 16 mmol/L, 10 ml/kg) were infused respectively through the aortic root after aortic-clamping. Heart arrest and its recovery, ultrastructure of the myocardium, the level of serum myocardial enzymes, and lipid peroxide and adenine nucleotide of the myocardium were measured. Hemodynamics during ischemia and after reperfusion were observed. RESULTS: The percentages of normal mitochondria and glycogen did not change much during ischemia (except at 60 min) and after reperfusion in B Group, but declined markedly in Group C 30 min and 60 min after ischemia and 20 min after reperfusion (P < 0.01). In Group A, they were lower than those of Group B before ischemia, but higher than those of Group C. The recoveries of CO, SV, CI, LVSW, RVSW and MAP in Group B were significantly better than those in other two groups 15 min and 30 min after reperfusion (P < 0.05 and 0.01, respectively). However, they were still better in Group A than those in Group C (P < 0.05 and 0.01, respectively). The onset of heart arrest was faster in Groups C and B than that in Group A. Highly elevated serum myocardial enzymes were observed 60 min after ischemia and 20 min after reperfusion in Group C, while they were only mild in the hyperpolarized groups, especially in Group B, and their recoveries were rapid. Adenine nucleotides of the myocardium were better preserved in Group B than in other two groups 30 min, 60 min after ischemia, and 20 min after reperfusion (P < 0.05 and 0.01, respectively). They were also much better in Group A than in Group C (P < 0.05 and 0.01, respectively). Lipid peroxide of the myocardium were significantly lower in Group B than in other groups 20 min after reperfusion (P < 0.01), and they were lower in Group A than in Group C (P < 0.05). CONCLUSIONS: Myocardial protection for global ischemia during cardiopulmonary bypass (CPB) could be achieved with hyperpolarized heart arrest induced by pinacidil, an ATP-sensitive potassium channel opener, especially in the hypothermic state. The protection is weaker in normothermia but is still superior to that with traditional depolarized hyperkalemic arrest.

[Correlation of coronary angiographic morphology with clinical presentation in unstable angina].

OBJECTIVE: To seek the relation between clinical presentation (Braunwald classification) or electrocardiogram(ECG) and angiographic morphology in patients with unstable angina (UA) and to determine which clinical markers could reliably predict unstable lesions and in-hospital cardiac events. METHODS: 120 hospitalized patients with a clinical diagnosis of UA and an abnormal angiograms(>or=50% diameter stenosis) were selected. Patients were classified according to Braunwald criteria before cardiac catheterization was performed. Coronary arteriograms were reviewed. The lesion morphology (including simple lesion, complex lesion and intracoronary thrombus [ICT]) and in-hospital events (including acute myocardial infarction,cardiac death and prompt revascularization) were observed. Logistic regression was used to evaluate the ability of clinical markers (Braunwald classification or an abnormal ST segment of ECG) to predict unstable lesions and cardiac events. RESULTS: There were more complex lesions in class III group(61%, or 20 of 33) than in class I group(30%, or 13 of 43, P < 0.05) and more ICT in class III group(18%, or 6 of 33) than in class II group (2%, or 1 of 44, P < 0.05). The rate of cardiac events was higher in class III patients (58%, or 19 of 33) than in class I patients (19%, or 8 of 43, P = 0.01 ) or class II patients (25%, or 11 of 44, P < 0.01). Logistic regression analysis demonstrated that an abnormal ST segment of ECG or Braunwald class III were highly predictive of the presence of complex lesion morphology (P < 0.01, OR 4.9; P < 0.01, OR 3.3, respectively). An abnormal ECG was the single predictive clinical indicator of triple vessel disease (P < 0.01, OR 3.9) and cardiac events (P < 0.001, OR 4.8). CONCLUSION: Complex lesion can be best identified by the clinical feature of recent onset refractory angina at rest and abnormal ST segment of ECG in UA patients. Patients with abnormal ST segment represents a high risk of triple vessel disease and cardiac events.

[Prospective randomized trial on the efficacy of adjuvant endocrine therapy for ER-positive breast cancer patients after radical mastectomy].

OBJECTIVE: To asses the efficacy of adjuvant endocrine therapy in ER-positive breast cancer patients after radical mastectomy. METHODS: 369 estrogen receptor (ER)-positive breast cancer patients were divided into two groups after radical mastectomy: 1. Endocrine group (N-194). The postmenopausal patients in this group received oral tamoxifen (TAM) for five years and the premenopausal patients were first treated with bilateral ovary ablation and, then, given TAM for five years. 2. The chemotherapy group patients (N-175) received CTX + MTX + 5-Fu + VCR + DXM) (CMFVP) or CTX + MTX + 5-Fu (CMF) multi-drug chemotherapy. RESULTS: Postmenopausal patients in the endocrine and chemotherapy groups gave disease-free survival rates (DFSR) of 78.4% and 45.4% (P < 0.01). Their overall survival rates were 83.3% and 52.9% (P < 0.05). The premenopausal patients gave DFSR of 72.8% and 35.7% (P < 0.01), and their overall survival rates were 80.7% and 60.6% (P < 0.05). But, no significant difference was observed in the stage I patients and those having > or = 8 axillary lymph node metastases (P > 0.05). CONCLUSION: For ER-positive breast cancer patients, the adjuvant endocrine therapy gives results superior or equal to multi-drug chemotherapy after radical mastectomy.

[Aging changes of alpha-amylase and lysozyme in normal parotid gland].

OBJECTIVE: To investigate the aging changes, the distribution and the content of alpha-amylase, lysozyme in the normal parotid glands. METHODS: Fifty-one specimens of parotid glands with 4 different age groups were studied by immunohistochemical method. The intensity of staining of alpha-amylase, lysozyme was recorded in different age groups. RESULTS: The staining positivity rate of alpha-amylase, lysozyme had striking differences in 4 different age groups and it gradually decreased along with the age increasing. CONCLUSIONS: There are differences among seous acinus which had similar shape and staining in HE slice. The characters of aging changes of alpha-amylase, lysozyme secreted by acinar and intercalated duct cells are conformity with the results of quantitative studies in histology.

[Expression of rat amelogenin gene in Escherichia coli].

OBJECTIVE: To detect the expression of amelogenin (Am) gene in Escherichia coli (E. coli). METHODS: The cDNA fragmain of Am gene was obtained with EcoRI and Xhol I from the plasmid PUC18/Am. The fragment was inserted into prokaryotic gene fusion vector PGEX-4T-2 and an expression plasmid PGEX-4T-2/Am was constructed. PGEX-4T-2/Am was induced by IPTG for 4 h. RESULTS: 15% SDS-PAGE revealed a new forein protein band near 27,000. CONCLUSION: The constructed plasmid expresses Am gene in E. coli.

[Discussion on some basic problems in design of wetland ecosystem].

The design of wetland ecosystem is the key and fundament in wetland restoration and mitigation. In recent years, it has been frequently applied to the proper construction of wetlands. The paper first reviews the concept of wetland ecosystem design, as well as the basis of ecological engineering, and then presented the basic principles in the design of wetland ecosystem. Some major ecological indicators including hydrology, chemical, substrate, soil and biotic indicators were also discussed, and some reference values for ecological indicators were presented. On the base of the designing purposes, three major types in the wetland design were analysed. They include constructing wetlands for wastewater treatment, for mitigation wetlands, and for controlling flooding and non-point source pollution.

The temperature sensitive mutant p53-143ala extends in vitro life span, promotes errors in DNA replication and impairs DNA repair in normal human oral keratinocytes.

Many human cancers contain a hemizygous point missense mutation in p53, allowing expression of both wild-type and mutant p53. To understand the relationship between wild-type and mutant p53 in cells, we investigated the influence of a naturally occurring temperature-sensitive mutant p53 (valine to alanine substitution at codon 143: mp53-143ala) on the life span of normal human oral keratinocytes (NHOK) and the expression of wild-type p53. We also investigated the effect of the mutant p53 on the genetic stability of NHOK. The mp53-143ala extended the in vitro life span of NHOK by four-fold, but failed to overcome the M2 crisis stage for immortalization. The mp53-143ala notably suppressed wild-type p53 in NHOK at post-transcriptional levels. Moreover, the mp53-143ala notably increased both spontaneous and genotoxic agent-induced mutation frequency of a shuttle vector in NHOK. These data indicate that mutant p53 induces genetic instability by, in part, inhibiting the expression of wild-type p53 through a dominant negative role in cells expressing both mutant and wild-type p53.

[A field evaluation of the epidemiological efficacy of an attenuated live hepatitis A vaccine (H2 strain)].

OBJECTIVE: To evaluate the epidemiological efficacy of an attenuated live hepatitis A vaccine (H2 strain). METHODS: A randomized and controlled field trial was carried out in part of Zhengding County and the Jiaoqu District of Shijiazhuang city. There were about 37,000 children in these areas involved in the study and they were randomly divided into a vaccine group and a control group according to the months they were born. Children in the vaccine group were given a dose of 10(6.5) TCID50/ml attenuated live hepatitis A vaccine while those in the control group were given nothing. As soon as the children in the vaccine group had been vaccinated, all the subjects were carefully followed up for hepatitis A cases. RESULTS: In the 3.5 years of follow-up, there were 20 hepatitis A cases in the control group and the incidence rate was 136.4 per 100,000 population. There was only one hepatitis A case in the vaccine group and the incidence rate was 8.1 per 100,000 population. The difference of the incidence rates between the two groups was highly significant (P < 0.001) and the protective efficacy of the vaccine was 94.4% (95% CI: 67.1%-100%). CONCLUSION: The protective efficacy of the vaccine was excellent and the vaccine is suitable to be widely used.

[Nested case-control study on the risk factors of colorectal cancer].

OBJECTIVE: To investigate the risk factors of colorectal cancer. METHODS: A nested case-control study was conducted based on a cohort of 64,693 subjects in Jiashan county, Zhejiang, China. One hundred and ninety-six cases of colorectal cancer were detected during a follow-up program for 10 years. Nine hundred and eighty non-colorectal cancer subjects were randomly selected from the cohort as controls. Univariate analysis and mutivariate non-conditional logistic regression were used to analyse associations between the exposure factors and colorectal cancer. RESULTS: The age of case group was significantly older than that of the control group. Moreover, the age at diagnose of colon cancer was significantly older than that of the rectal cancer. Other than age, the final regression model showed that mucous blood stool history and colon polyp history were significantly associated with colon cancer (OR = 2.961, 95% CI:1.202-7.298 and 8.941, 95% CI:1.820-43.926, respectively) while mixed sources of drinking water was found to be associated with rectal cancer with a OR value of 1.823 (95% CI:1.024-3.247). CONCLUSION: The risk factors of colon cancer were different from that of the rectal cancer. Age was an important factor influencing the incidence of both colon cancer and rectal cancer. Mucous blood stool history and colon polyp history were associated with colon cancer, while drinking mixed water was closely associated with rectal cancer.

[The influence of HOXB2 anti-sense oligodeoxynucleotides on the proliferation and expression of human umbilical vein endothelial cells].

OBJECTIVE: To explore the effects of HOXB2 anti-sense oligodeoxynucleotides (asodn) on the proliferation and the expression of human umbilical vein endothelial cells (HUVECs). METHODS: Various concentrations of HOXB2 ASODN modified by thiophosphate were transfected into HUVECs by liposome mediation. MTT and RT-PCR methods were employed to determine the influence of different concentrations of ASODN on endothelial proliferation and the expression level of HOXB2 mRNA. RESULTS: After the transfection of HOXB2 ASODN, the endothelial proliferation was inhibited in dose-dependent manner. Simultaneously, the expression level of HOXB2 mRNA decreased significantly. CONCLUSION: HOXB2 might play important roles in the proliferation of endothelial cells.

[Apoptosis of retinal capillary cells in diabetic rats and expression of apoptosis-related genes].

OBJECTIVE: To determine the apoptosis of retinal capillary cells in early diabetic rats and observe the expression of apoptosis-related genes (bcl-2, Bax) in retinas of early diabetic rats. METHODS: 40 healthy adult Wistar rats were chosen and randomly divided into four groups: normal control (CON), diabetes mellitus one month (DM1), three months (DM3) and six months (DM6). Diabetic mellitus in rats was induced by streptozotocin (STZ) intraperitoneally. Retinal preparations of blood vessels and paraffin sections were stained by TdT-mediated dUTP nick end labeling (TUNEL) and immunohistochemical (ABC) method respectively and analyzed by computer-picture analytic system on the results of ABC method. RESULTS: Positive pericyte nuclei labeled by TUNEL were emerged in DM3 and DM6, but positive endothelial cell nuclei only in DM6. No positive reaction was seen in CON and DM1. Chromatin of TUNEL positive nuclei was distributed unequally, showing apoptotic characteristics of ring-like and crescent nuclei. In ABC staining method, two genes' (bcl-2, Bax) proteins were expressed in retinal vessels. Along with the progression of diabetes, the intensity of positive reaction was increased in degrees. Moreover, pigment epithelial cells in DM3 showed Bcl-2 positive reaction that again extended further to inner rod segments and ganglion cells in DM6, while in DM6, Bax was expressed in ganglion cells. CONCLUSIONS: The nature of pericyte dropout in early diabetic rats is apoptosis that also exists in endothelial cells; the expression of Bax and bcl-2 in retinas of early diabetic rats is enhancing.

[Reversed trabeculectomy for treatment of glaucoma].

OBJECTIVE: To reduce the postoperative blocking of the filtration canal in routine trabeculectomy with modification. METHODS: The modification of the procedure of routine trabeculectomy was that one end of the 4.0 mm x 1.5 mm deep layer scleral tissue including part of trabecular meshwork was not cut. Instead, the free end of the strip of deep scleral tissue was vertically rotated and sutured on the deep layer of the scleral bed posterior to it. The projection of the strip was utilized to prevent postoperative blocking of filtration canal. RESULTS: The mean period of postoperative follow-up was 23.5 months (range, 12 -- 36 months). In the treatment group (38 eyes of 33 cases), the intraocular pressure (IOP) was lowered from preoperative (25.41 plus minus 2.41) mm Hg to postoperative (14.74 plus minus 0.90) mm Hg. The IOP was lowered from preoperative (24.96 plus minus 1.35) mm Hg to postoperative (19.40 plus minus 1.43) mm Hg in the control group (40 eyes of 33 cases). The success rates were 94.7% and 77.5% in the treatment and control group respectively. CONCLUSIONS: The modified trabeculectomy is safe, easy to perform, and can effectively relieve the postoperative blocking of the filtration canal, control postoperative IOP and reduce postoperative recurrence.

[A gene study on multidrug resistance of retinoblastoma].

OBJECTIVE: To investigate the expression of multidrug resistance gene (MDR1) and multidrug associated protein (MRP) gene in the retinoblastoma (Rb) cell line HXO-RB(44), and study the mechanism of multidrug resistance (MDR) phenotype occurring in retinoblastoma. METHODS: Reverse transcriptase polymerase chain reaction (RT-PCR) was employed to detect the transcription of MDR1 and MRP gene in the HXO-RB(44) cell line, and immunohistochemical technique was used to detect their protein products P-glycoprotein and multidrug resistance associated protein (P190). RESULT: Both MDR1 and MRP genes were expressed in HXO-RB(44) cell line, as well as P-glycoprotein and P190 were over-expressed (96% - 97%) in this cell line. CONCLUSION: Over-expression of MDR1 and MRP genes is significantly involved in the mechanism of MDR phenotype in retinoblastoma.

[Surgical treatment of congenital nuclear cataract].

OBJECTIVE: To study the characteristics of congenital nuclear cataract and evaluate the therapeutic effects of extracapsular cataract extraction (ECCE) with or without intraocular lens (IOL) implantation in different age groups. METHOD: ECCE with or without IOL implantation was performed on 76 eyes of 38 cases with congenital nuclear cataract in different age groups. Under operative microscope, the characteristics of the opacity of the posterior capsule were described. The visual acuity and possible complications were followed up for a mean period of 6 months. RESULTS: The opacity of the posterior capsule was progressively developing, its diameter was similar to that of the opaque nucleus. The difference of the post-operative corrected visual acuity in different age groups was significant (chi(2) = 27.84, P < 0.005). The rates of the secondary cataract were negatively correlated to the age groups (r = -0.969 2, P < 0.05). In 2 eyes, slight iris capture occurred in 3 - 12 year group. CONCLUSIONS: Congenital nuclear cataract can combine with posterior capsular cataract. The etiology remains to be studied. The therapeutic effects of the congenital nuclear cataract mainly depend on the time of operation. The early treatment of post-operative complications in children is of primary importance.

[An experimental study of the influence of burn on the expressions of IFN-gamma and IL-4 in T lymphocytes and on the expression of IL-12 in macrophages in mice].

OBJECTIVE: To investigate the influence of burn on the expressions of IFN-gamma and IL-4 in T lymphocytes and on the expression of IL-12 in macrophages in mice. METHODS: The mice inflicted by 12% TBSA burn on the back were employed as the model. The peritoneal macrophages (PMphis) were isolated and counted under light microscope (LM). The gene expression level of IL-12 P35 and P40 subunits was determined with RT-PCR. The ratio of spleen to body and splenic histology were also determined. Splenic T lymphocytes were isolated and the gene expression levels of IFN-gamma and IL-4 in the T lymphocytes were examined by RT-PCR. RESULTS: (1) PMphis decreased significantly on 3 postburn day (PBD) and recovered quickly thereafter. (2) The gene expressions of IL-12 P35 and P40 subunits in PMphis were enhanced obviously. (3) Splenic index increased evidently and the karyokinesis phase increased in splenic lymphocytes. (4) The expressions of IFN-gamma and IL-4 in splenic T lymphocytes increased postburn. CONCLUSION: The expressions of IFN-gamma and IL-4 in splenic T lymphocytes and the expressions of IL-12 in PMphis could be enhanced by burn injury.

[Dynamic monitoring of Ph positive cells during early post-transplant period in chronic granulocytic leukemia by interphase FISH].

OBJECTIVE: To assess the effectiveness of transplantation and observe the kinetics of residual leukemic cells during early post-transplantation period in chronic granulocytic leukemia (CGL). METHODS: Ten normal controls, seven untreated CGL patients and eleven CGL patients after allogeneic bone marrow transplantation (allo-BMT) or peripheral blood stem cell transplantation (PBSCT) were studied by interphase fluorescence in situ hybridization (I-FISH) with the FITC fluorescein labeled BCR probe. Conventional cytogenetic analysis (CCA) and reverse transcriptase-polymerase chain reaction (RT-PCR) were also employed. RESULTS: The normal cutoff was defined as less than 6.68% (x +/- 3s) from the analysis of normal controls. The reproduciblity of FISH method was confirmed by serial dilutions by mixing Ph positive cells and normal cells. Forty specimens from 11 CGL patients at 1 week to 6 years post-BMT were investigated after PBSCT and the results showed that (1) FISH and RT-PCR could accurately evaluate Ph (+) cells during early transplantation period (within one month), but RT-PCR was more sensitive in six months after BMT. (2) Residual Ph (+) cells tended to be linear decreasing with the time after transplantation and fell in normal range in most samples three months later. The mean time of normaligation was 57 days (20 to 170 days). (3) One case relapsed and received a second transplantation. FISH presented more precise data for predicting relapse as compared to CCA and RT-PCR. CONCLUSIONS: I-FISH as a useful tool can not only offer more precise assessment of residual leukemic cells compared with CCA and RT-PCR, but also provide dynamic monitoring during early post-BMT period to evaluate the transplantation outcome and predict relapse.
