[Aorto-profunda femoris bypass grafting in the treatment of aorto-ilio-femoral atherosclerotic occlusive disease].

Fourteen patients with aorto-ilio-femoral or ilio-femoral atherosclerotic occlusive disease were treated with aorto-profunda femoris bypass grafting. Excellent results were confirmed after a mean follow-up period of 15 months. Ankle-brachial index raised from 0.1 +/- 0.12 before operation to 0.64 +/- 0.26 months after operation. The authors considered that the profunda femoral artery plays an important role in aorto-femoral arterial reconstruction. It is a good outflow providing arterial blood to severely ischemic legs with occlusion of aorto-ilio-superficial femoral or ilio-superficial femoral arteries.

[Ondansetron in the prophylaxis of acute emesis induced by supra-high single dose total body irradiation (TBI)].

One hundred sixteen cases of leukemia patients received supra-high single dose TBI for bone marrow transplantation (BMT) with total a radiation dosage of 700-770 Gy at about 5cGy/min. Seventy patients were given ondanstron (8mg) plus dexamethasone (DXM, 10mg) for prophylaxis of TBI-induced acute nausea and vomiting, 46 patients were given paspertin (10mg) plus dexamethasone (10mg) as controls. The clinical results showed that ondansetron plus DXM achieved complete or major control of vomiting in 59/70 (84%) of cases and there was little or no nausea in 61/70 (87%), while paspertin plus DXM achieved complete or major control of vomiting only in 9/46 (20%) of cases and little or no nausea in 5/46 (11%). It is concluded that ondansetron has significantly greater advantage over paspertin in the control of acute emesis induced by TBI.

Morphology and morphogenesis of hepatitis E virus (strain 87A).

The morphology and morphogenesis of isolated hepatitis E virus (HEV, strain 87A) were observed by electron microscopy (EM) and immune electron microscopy (IEM). Progressively developing local vesicles, virions accumulation in crystalline arrays and viroplasmic focus were seen in cytoplasm of infected cells. Replication and assembly of the new generation viruses were closely associated with rough endoplasmic reticulum (RER), inclusion body (IB) and microfibrils. Condensation and margination of chromatin, dispersion of nucleolar material, nuclear membrane alteration and masses of threads, granular material, and fibrillar component of the nucleus were frequently found. These changes revealed that this strain virus was confirmed as a RNA virus. The shape of the virus particles appeared approximately spherical whether the specimens were from the tissue culture crude suspension or purified highly concentrated preparations. The size of the virion was about 30 nm in diameter. The viral particles appeared unsmooth and irregular in outline. The spike-like structures may be occasionally observed on the surface of some viral capsides. The diameter of the strain 87 A virus is larger than the picornavirus and smaller than the calicivirus. This strain virus is different from classical calicivirus in without the cup-shaped surface depressions. The new genus, heparnavirus genus of caliciviridae family should be proposed for HEV.

[Effects of isorhynchophylline on physiological characteristics of isolated guinea pig atrium].

Isorhynchophylline (Iso) inhibits the automaticity and contractile force of isolated guinea pig atrium in a concentration-dependent manner, 30 mumol/L markedly depresses adrenaline-induced automaticity, 10 mumol/L prolongs functional refractory period and decreases excitability. Furthermore, 10 mumol/L of Iso reduces the effect of ouabain on contractile force in left atrium and 0.3 mmol/L markedly inhibits the response to paired stimulation.

Radiologic manifestations of osteopetrosis.

The clinical radiologic manifestations of 8 patients with osteopetrosis (4 males and 4 females) are analyzed. Six of the cases are benign. The pathologic basis for the radiologic manifestations is discussed, and a classification is suggested. "Broom-like" metaphyses, metaphyses containing three or four dense layers of bone, and ossification of soft tissues around joints were found and differential diagnosis was made.

Clinical significance of complete left bundle branch block in dilated cardiomyopathy.

Clinical, electrocardiographic and echocardiographic findings in 64 patients with dilated cardiomyopathy were restrospectively studied. Compared with 51 patients without complete left bundle branch block (CLBBB), 13 patients with CLBBB had higher New York Heart Association (NYHA) functional class (P < 0.05), increased left ventricular end-diastolic and end-systolic diameters (P < 0.002) and myocardial mass (P < 0.02), severe mitral regurgitation (P < 0.01) and higher mortality rate (P < 0.04). Multivariate stepwise regression analysis revealed that the presence of CLBBB was an independent prognostic factor for patients with dilated cardiomyopathy.

[Adhesion of erythrocytes of patients with cerebral thrombosis to injured endothelial cells].

Using flow chamber system, we quantitatively studied the adhesion of erythrocytes (RBCs) of patients with cerebral thrombosis (CTh) to cultured human umbilical vein endothelial cells (HUVECs) treated with H2O2. The results showed that injured HUVECs enhanced adhesion of RBCs to HUVECs in patients with CTh. There is injury of endothelial cells in the pathogenetic and pathologic process of CTh, and the adhesion of RBCs to ECs promotes vaso-occlusive and vascular damage; thus the adhesion of RBCs to injured ECs might play an important role in the pathogenesis and pathologic process of CTh.

[Step increase in shear stress effects on releasing prostacyclin of human umbilical vein endothelial cell].

Adopting the flow chamber, we studied the effect of step increase in shear stress on prostacyclin releasing of human umbilical vein endothelial cell (HUVEC). HUVEC monolayer was exposed to step increase in shear stress levels of 1, 10 and 20 dyne/cm2 for 100 minutes. By radioimmunoassay, 6-keto-PGF1 alpha in flowing out liquid from the flow chamber was measured. The results displayed: when HUVEC was not exposed to shear stress, the PGI2 released from HUVEC was not detectable; when HUVEC was exposed to shear stress, the flow onset led to an initial burst increase of PGI2 production rate, which went down subsequently within several minutes. We noticed that the larger the step increase of shear stress became, the higher the peak value of PGI2 production rate ascended, the quicker the peak value subsequently declined, and the higher the average value of PGI2 releasing rate in 80-100 minutes stood. The results demonstrated that in vivo hemorheological factor-the shear stress of blood flowing stimulated the release of prostacyclin from HUVEC.

High-performance liquid chromatographic determination of free ferulic acid in serum of rabbits with blood stasis.

A Simple, rapid and sensitive reversed phase high performance liquid chromatographic (RP-HPLC) method for the analysis of free ferulic acid in serum of rabbits with blood stasis has been developed. Coumarin was used as internal standard. 0.2 ml of serum containing ferulic acid was deproteinized with acetonitrile which contained the internal standard (coumarin). The supernatant was evaporated to dryness. The residue was dissolved in 60 microliters of acetonitrile. A Du Pont Zorbox C18 column was used. A mixture of acetonitrile and 0.10 mol.L-1 phosphoric acid (3:7, v/v, pH 2.5) was used as the mobile phase, and UV detection was performed at 320 nm. The detection limit (signal-to-noise ratio = 3) and the mean recovery from serum were 1 ng and 97.0 +/- 1.8%, respectively. The method was linear within the range of 20-800 ng.ml-1 (r = 0.9986). The relative standard deviation were all less than 7%. The method was applied to pharmacokinetic studies in healthy rabbits and rabbits with blood stasis. Through this experiment we have found significant difference in the pharmacokinetic parameters between healthy rabbits and rabbits with blood stasis. The results obtained are coincident with syndrome and pharmacokinetics.

[Effects of nasal continuous positive airway pressure on breathing pattern and respiratory drive in patients with obstructive sleep apnea syndrome].

To examine the influence of continuous positive airway pressure (CPAP) therapy on respiratory center drive in patients with obstructive sleep apnea syndrome (OSAS), 20 normocapnic OSAS patients (group 0) and 20 simple snoring patients were studied. In the first night, diagnostic polysomnography (PSG) was performed. Before and after PSG monitoring, mouth occlusion pressure (P0.1), tidal volume (VT), minute ventilation (VE), respiratory rate (RR), inspiratory time (Ti), expiratory time (Te), total cycle duration (Ttot), inspiratory duty cycle (Ti/Ttot), mean inspiratory flow (VT/Ti) and effective inspiratory impedance (P0.1/VT/Ti, Ieff) were measured while they were breathing room air. In the following night the OSAS patients were treated with nasal CPAP and PSG monitoring and the above mentioned measurements were repeated. The results showed that pre-PSG values of P0.1, RR and P0.1/VT/Ti in the OSAS patients were significantly higher than those in the snoring patients, while VT, Ti, Te and Ttot values were lower. In the first night, the post-PSG P0.1 value in the OSAS patients increased markedly as compared with the pre-PSG. After overnight nasal CPAP therapy, the respiratory disorder index in the OSAS patients decreased markedly, the nadir SaO2 increased markedly, but the post-PSG P0.1 value did not increase significantly. It is concluded that, before sleep, OSAS patients exhibit a higher respiratory drive and a shallow and frequent breathing pattern as compared with simple snoring patients. After nocturnal sleep, the respiratory drive of OSAS patients increases significantly, the breathing pattern becomes more shallow and frequent. Nasal CPAP may effectively relieve the sleep apnea and hypopnea as well as the resulting hypoxemia and therefore correct the changes in breathing pattern and respiratory drive through nocturnal sleep in patients with OSAS.

[The effects of Salvia miltiorrhiza and polysaccharide sulphate on the adhesion of erythrocytes of the patients with cerebral thrombosis to cultured endothelial cells].

Salvia miltiorrhiza and polysaccharide sulphate are commonly prescribed for curing cerebral vascular diseases. In this study, we found that the adhesion of erythrocytes of the patients with cerebral thrombosis to cultured human umbilical vein endothelial cells was decreased in number and intensity after the erythrocytes were treated with salvia miltiorrhiza and polysaccharide sulphate in flow field. Moreover we found that polysaccharide sulphate was more effective than salvia miltiorrhiza in preventing adherence in common doses. The two drugs' effects of preventing adherence might be an important mechanism for curing the patients with cerebral thrombosis.

[Clinical analysis of anaerobic septicemia in 26 patients with extensive burn].

To understand the incidence and importance of anaerobic infection, the aerobic and anaerobic blood culture were carried out simultaneously in 127 patients with extensive burns (TBSA > 50%). Among 39 patients with positive culture, 26 had anaerob growth in blood culture. The total incidence rate of anaerobic septicemia was 20.4%. 61 strains (9 species) of anaerobes were isolated from blood specimens. The predominant anaerobes were Peptococcus (37.7%) and B. fragilis (36.1%). 20 (76.9%) were mixed infection of aerobes and anaerobes. The others thers suffered from combined infection of B. Fragilis and peptococcus combined infection. Finally, 19 patients survived and 7 died (26.9%). The sensitive tests showed that the effective drugs were metronidazot, chloramycin, and lincomycin. These data suggested that the septicemia of extensive burn patients is often caused by anaerobes. Therefore, anaerobe plays an important role in burn infection.
