[The correlation between tumor angiogenesis and lymph node metastasis in primary breast carcinoma].

To study the relationship between tumor angiogenesis and lymph node metastasis in primary breast carcinoma. Agiogenesis was assessed by the microvessel density (MVD) and expression of vascular endothelial growth factor (VEGF) using immunohistochemical staining. Paraffin-embedded specimens from 70 patients with primary breast cancer who had undergone radical mastectomy from 1984 to 1985 were studied. Thirty-one patients had histologically proven positive axillary lymph node (N+). The axillary node negative (N-) group was composed of thirty-nine patients. Microvessels per 200x (as MVD) and VEGF positively stained cancer cells per 400x were counted with light microscope. MVD and VEGF expression were higher in tumors with N+ showed than those with N-, MVD and VEGF expression were higher in N+ than in N-. MVD and the expression of VEGF are highly correlated with metastasis in primary breast cancer, which may serve as a parameter for determining tumor biological, metastatic potential and prognosis.

[Medicine S inhibits class II MHC expression of intragraft renal in rats].

The Purpose of this study was to address the mechanism of medicine S which has anti-rejection effects of renal allografts in rats. Kidney transplantations were performed from SD to Wistar strain (allogeneic) and from Wistar to Wistar (Isograft) using the same modified technigue described by Fabre and kamada. Experimental rats were divided into five groups. Group I (Isograft group) and group II (allograft group) as controls were not treated with medicine. The others were allograft groups which received medicine S, Cyclosporine A, and low-dose Cyclosporine combined with medicine S, respectively. Renal function and resultant morphology changes were assessed 2, 4 weeks after transplantation. All sections of kidney grafts were stained with monoclonal antibody class II MHC (OX6), and then the surface densities of positive staining were quantified by computer image analysis. The level of molecular expression in group II was significantly increased (7.61 +/- 0.57 vs 0.51 +/- 0.2 of group I, P < 0.01). In groups I and IV, the molecule of expression was reduced, compared with the groups II, III and V (P < 0.05). The results suggest that medicine S decreases the level of class II MHC expression and medicines combined with lowe-dose cyclosporine is more effective than cyclosporine alone.

[Studies on the antilipid peroxidation of nine sorts of Chinese herbal medicines with the function of protecting liver].

The antilipid peroxidation of nine sorts of Chinese herbal medicines with the function of protecting liver, including Salivia miltiorrhiza, Hypericum japonicum, Scutellaria baicalensis, Callicarpa cathayana, Chrysanthemum indicum, Paeonia latiflora, Lysimachia christinae, Ligustrum lucidum (L1), Patrinia villosa (Pv) on hepatic homogenate of rat were tested. It was found that all tested medicines showed inhibition with dose-effect relationship, the inhibitory rate of L1 and Pv were lower than the other's. All results showed that these Chinese herbal medicines have strong antilipid peroxidation and are natural oxidation inhibitor. It was suggested that their function of protecting liver and others have relationship with the antioxidation.

Mutational analysis of the PEX gene in patients with X-linked hypophosphatemic rickets.

X-linked hypophosphatemic rickets (HYP) is a dominant disorder characterized by renal phosphate wasting and abnormal vitamin D metabolism. PEX, the gene that is defective in HYP and is located on Xp22.1, is homologous to members of the neutral endopeptidase family. However, the complete coding sequence of the PEX cDNA, the structure of the PEX gene, and the role that PEX plays in phosphate transport remain unknown. We determined the genomic structure of the published PEX gene, which was found to be composed of 18 short exons, and demonstrated that the genomic organization of PEX shares homology to members of the family of neutral endopeptidases. Primer sets were designed from the intron sequence, to amplify each PEX exon from genomic DNA of HYP patients. Mutations in PEX were identified in 9/22 unrelated HYP patients, confirming that defects in PEX are responsible for HYP. The mutations detected included three nonsense mutations, a 1-bp deletion leading to a frameshift, a donor splice-site mutation, and missense mutations in four patients. Although the entire PEX gene has not been identified and some mutations may have been missed, the lack of detection of mutations in the remaining 13 patients, especially in 1 patient who has an apparently balanced, de novo 9;13 translocation, implies that there may be other loci involved in the generation of the HYP phenotype.

Na+/glucose cotransport in the colonic adenocarcinoma cell line HT29 cl.19A: effect of cAMP.

The aim of the present study was to investigate the earlier finding that cAMP stimulation activates Na+/glucose cotransport in intestinal epithelia. Western blotting demonstrated the existence of Na+/glucose cotransporters in the colonic adenocarcinoma cell line HT29 cl.19A. Monolayers of this cell type showed a glucose transport, which was inhibited by 0.5 mM phlorizin (specific inhibitor of Na+/glucose cotransport). Brush border membrane vesicles of HT29 cl.19A cells exhibited a Na(+)-gradient dependent glucose transport, which was stimulated by DbcAMP-pretreatment (dibutyryladenosine 3',5'-cyclic monophosphate) of the cells. In the Ussing chamber, glucose (10 mM) unexpectedly lacked stimulatory effect on short circuit current (Isc) in HT29 cl.19A monolayers in the control situation. In DbcAMP-stimulated monolayers, glucose induced a complex Isc-response consisting of both stimulatory and inhibitory components, usually leading to a 'net' stimulation of lsc. Phlorizin (0.5 mM) did not prevent the stimulatory effect of glucose. Mannitol, alanine, fructose, ethanol (solvent for phlorizin) and the non-metabolizable glucose analogue 3-o-methyl-alpha-glucopyranoside inhibited Isc in a similar fashion as did phlorizin. Glucose transport in human colon biopsies were studied both in [14C]glucose accumulation experiments and in a specially designed Ussing chamber. There were no indications of glucose absorption in neither of these experiments. We conclude: (1) The human colon lacks Na+/glucose transport, (2) HT29 cl.19A cells exhibit Na+/glucose cotransport, which is stimulated by cAMP, (3) but this mechanism seem to be of a different type from the Na+/glucose cotransport of the 'normal' small intestine.

Apolipoproteins of HDL can directly mediate binding to the scavenger receptor SR-BI, an HDL receptor that mediates selective lipid uptake.

The class B type I scavenger receptor, SR-BI, binds HDL, mediates selective uptake of HDL cholesteryl esters by cultured cells, and its expression is coordinately regulated with steroidogenesis in several endocrine tissues (adrenal, ovary, testes). SR-BI can also bind LDL and anionic phospholipids, which raised the possibility that HDL apolipoproteins might not participate directly in HDL binding. We have examined the ability of individual human HDL apolipoproteins (apoA-I, apoA-II, and apoC-III) reconstituted into phospholipid/unesterified cholesterol complexes to bind to murine SR-BI (mSR-BI) expressed in stably transfected cultured cells. All three apolipoprotein/phospholipid/unesterified cholesterol complexes specifically associated with mSR-BI expressing cells with high affinity and competed for the binding of HDL, while apolipoprotein-free complexes did not. Furthermore, lipid-free forms of these soluble apolipoproteins also competed for HDL and apolipoprotein/ phospholipid/cholesterol complex association with mSR-BI, but locust high density lipophorin and bovine serum albumin were not effective competitors. Thus, all three of the HDL apolipoproteins (apoA-I, apoA-II, and apoC-III) tested can directly mediate binding to mSR-BI, and this multiligand apolipoprotein receptor may be responsible for at least some of the multilipoprotein and apolipoprotein binding activity previously observed in cells and tissues.

Nitinol alloy endotracheal stent for treatment of tracheal stenosis.

OBJECTIVE: To treat inoperable tracheal stenosis by using nitinol endotracheal stent and to solve some problems facing endotracheal stenting with other kinds of stents. METHODS: The stent was made of nitinol wire. It can be deformed to smaller size in iced water. With a self-made introducer, the stent was placed in the location of stenosis. After hot water was injected into the introducer, the stent resumed its original form and anchored there itself. RESULTS: Altogether, 5 patients with severe tracheal stenosis received this treatment. Dyspnea was relieved immediately after implantation of stents in all patients. Long-term follow-up also showed satisfactory results. CONCLUSION: Nitinol stent has some remarkable advantages as compared with other tracheal stents. It can be used as a new endoprosthesis in treating narrowed trachea.

[Expression of Cath-D, P-gp and Her-2 in carcinoma of the cardia].

OBJECTIVE: To investigate if Cath-D, P-gp and Her-2 are correlated with prognosis in patients with cardiac carcinoma. METHODS: Histological sections of 43 patients with cardiac cancer were tested by immunohistochemistry and analyzed for pathological type and lymphatic metastasis. RESULTS: The research show and that P-gp and Her-2 were not significant in predicting lymphatic metastasis (P > 0.05), but Cath-D positive was significant in lymphatic metastasis (P < 0.01). The P-gp and Cath-D were not significant in differential level of cardiac carcinoma and expression of Her-2 was significant (P < 0.05). CONCLUSION: The results suggest that positive Cath-D is correlated with metastases and positive Her-2 indicates poor survival. Special treatment plans for patients with carcinoma of the cardia should be designed depending on the above marker tests.

[In vivo induction of antitumor immune response by tumor cells fused with GM-CSF gene-modified dendritic cells].

OBJECTIVE: To investigate the induction of antitumor immune response and therapeutic effects of the vaccination with the fused cells between tumor cells and GM-CSF gene -modified dendritic cells. METHODS: The dendritic cells were isolated from the spleens of C57BL/6 mice, infected with GM-CSF adenovirus, and then fused with B16 murine melanoma cells. The positive fused cells were enriched by magnetic cell sorting with anti-Ia antibody and adherent culture. The protective and therapeutic effects of the vaccination with the fused cells were investigated. RESULTS: Vaccination with the inactivated fused cells induced more potent CTL activity against parental B16.F10 cells, and led to the resistance against the rechallenge of alive parental B16.F10 cells. After treatment with the inactivatedly fused cells, the survival time of the tumor-bearing was mice greatly extended and the metastatic tumor nodules on the pulmonary surfaces reduced. CONCLUSIONS: Vaccination with the fused cells between tumor cells and dendritic cells could elicit potent antitumor responses, and combined with GM-CSF gene transfection, more potent results can be achieved.

Complications of laparoscopic cholecystectomy in China: an analysis of 39,238 cases.

OBJECTIVE: To investigate the complications of laparoscopic cholecystectomy in China. METHODS: All Chinese articles about laparoscopic cholecystectomy published between April 1994 and November 1995 were identified through CMCC (Chinese Medical Computerized Contents). From more than 600 titles, 105 articles were screened for analysis. Another 21 articles from the 6th Biliary Surgical Congress and 300 cases from the General Hospital of PLA were added. A total of 39,238 cases of laparoscopic cholecystectomy from 91 hospitals were studied. RESULTS: Severe complications of laparoscopic cholecystectomy were identified in 409 (1.04%) patients, including bile duct injury (in 0.32% of patients), postoperative cystic duct leak (0.11%), postoperative bile leak (0.20%), peritoneal abscess (0.07%), bowel injury (0.06%) and postoperative hemorrhage (0.1%). Fourteen postoperative deaths (0.04%) resulted from operative injury. CONCLUSIONS: The data demonstrate that laparoscopic cholecystectomy is an operation associated with low morbidity and mortality rate, but bile duct injury is still a major problem. Complications of laparoscopic cholecystectomy can be minimized by improving operative procedure.

[The characteristics of labour course and perinatal prognosis in cases of fetal persistent occiput-transverse position and persistent occiput-posterior position].

OBJECTIVES: To study the characteristics of the labour course and perinatal prognosis in cases of fetal persistent occiput-transverse position (POTP) and persistent occiput-posterior position (POPP). METHODS: All the cases, delivered with POTP and POPP in or hospital from Nov, 1995 to July 1996 were analyzed retrospectively. RESULTS: In abnormal fetal occipital position group, the following indices were significantly higher than those of normal group: fetal macrosomia, uterine inertia, and duration of each labour course. The speed of descending of presentation was obviously slower. The incidence of abnormal labour course was markedly increased. The rate of operative delivery was significantly higher. The total rate of operative delivery was 82.81% in the POTP group, and 92.31% in the POPP group. The incidence fetal hypoxia and neonatal asphyxia was markedly high in the abnormal occiput group. CONCLUSIONS: The POTP and POPP are major cases of dystocia. If management is unsuitable, perinatal prognosis will be poor.

Alterations in renal function in patients with obstructive sleep apnea syndrome and effects of continuous positive airway pressure.

OBJECTIVE: To assess renal function in patients with obstructive sleep apnea syndrome (OSAS) during two nights when they were untreated and treated with continuous positive airway pressure (CPAP). METHODS: Tubular function was estimated on the basis of the renal lithium clearance and its derived formulae, urinary osmolality, osmolal clearance and free water clearance; while glomerular function was evaluated by endogenous creatinine clearance. Twenty patients with OSAS and sixteen normal controls were studied. RESULTS: The untreated patients with OSAS had significantly lower fractional proximal tubular sodium reabsorption (59.1% +/- 7.1%, P < 0.001 vs controls), lower fractional distal tubular sodium reabsorption (93.8% +/- 1.7%, P < 0.05 vs controls) and urinary osmolality (573.0 +/- 107.9 mosm.kg-1, P < 0.05 vs controls). The absolute distal tubular reabsorption rate of sodium and osmolal clearance in the untreated patients were higher 47.0 +/- 26.0 mmol.L-1 and 25.0 +/- 4.1 ml.L-1; P < 0.01 and P < 0.05 respectively vs controls). During CPAP-treated nights all abnormal renal function indexes in the OSAS patients restored to normal control values (P > 0.05). CONCLUSIONS: The natriuresis and diuresis of OSAS patients were due to the decrease in proximal and distal tubular sodium reabsorption and in tubular concentration ability during their nocturnal sleep and were returned to normal by CPAP therapy.
