[Observation of the long-term serum insulin level in women with gestational diabetes history].

OBJECTIVE: To determine beta-cell's function by measuring blood glucose level and serum insulin level in women who had history of gestational diabetes mellitus (GDM), but had no long-term diabetes. METHODS: 30 cases in GDM history group, 29 cases in abnormal oral glucose tolerance test(OGTT) history group, and 38 cases in control Group were followed up to undergo fasting blood sugar test and 75 g glucose load test, and to measure their insulin levels. All the cases had no long-term diabetes. RESULT: (1) the blood glucose levels at 2 hour in GDM history group and abnormal OGTT history group were higher than that in control group especially the former, which was very significantly higher (P < 0.001). (2) The insulin at 2 hours in GDM history group was higher than that in control group (P = 0.00067). CONCLUSION: Though women with GDM history do not have long-term diabetes, insulin resistance still exists, postings a possible sign for future diabetes attack.

Immunological studies on the cellular phenotype involved in corneal allograft rejection.

OBJECTIVE: To investigate the cellular phenotype involved in corneal allograft rejection using wholemounts analysis. METHODS: Corneal transplantation was performed between Sprague Dawley (SD) and Wistar rats. Corneal wholemounts were prepared from control rats and those after corneal transplantation on day 7 and 12. Immunohistochemical stain was performed on these wholemounts using monoclonal antibodies to transforming growth factor beta 1(TGF-beta 1), CD3, CD4, CD8, B lymphocytes, macrophages, dendritic cells and major histocompatibility complex (MHC) class II antigen. RESULTS: Corneal allograft rejection started on day 7 and reached its maximum from 10 to 14 days after corneal transplantation. Presence of TGF-beta 1-, CD3-, CD4-, CD8-, MHC class II-positive cells, macrophages and dendritic cells were noted at the limbus of both SD rats and Wistar rats. No positive cell was present in the central cornea of normal rats. All positive cells but B lymphocyte were noted in large numbers in the cornea after corneal allograft transplantation. Marked staining for TGF-beta 1 was noted during graft rejection. CONCLUSION: The corneal wholemounts technique provides a good visualization for the cellular phenotype involved in corneal allograft rejection. A variety of cells including TGF-beta 1, CD3, CD4, CD8, MHC class II antigen positive cells, macrophages and dendritic cells are involved in corneal allograft rejection. TGF-beta 1-positive cell might be an important immunosuppressive factor after corneal transplantation and also involved in the induction of fibrosis.

[Regulation of arsenic trioxide-inducing apoptosis].

OBJECTIVE: To explore the relationship among intercellular -SH, caspase, retinoic acid (RA) and arsenic trioxide(As2O3)-induced apoptosis. METHODS: The in vitro effect of different thiols compounds, RA and caspase inhibitors on As2O3-induced apoptosis in NB4 and HL-60 cells was studied. RESULTS: 1. NAC completely blocked, BSO potentiated while MTG, BAL had no effect on As2O3-induced apoptosis. 2. Z-VAD.fmk blocked while Y-VAD.fmk had no effect on As2O3-induced apoptosis. 3. RA and As2O3 showed synergism in HL-60 cells, while showed antagonism in NB4 cell. CONCLUSIONS: 1. As2O3 binds with intracellular -SH, changes signal transduction, selectively activates caspase and causes apoptosis. 2. The regulating effect of RA on As2O3-induced apoptosis depends on cell types.

Angiotensin I-converting enzyme gene polymorphism in Chinese patients with obstructive sleep apnea syndrome.

OBJECTIVE: To investigate the relationship of an insertion/deletion (I/D) polymorphism of the angiotension-converting enzyme (ACE) gene to obstructive sleep apnea syndrome (OSAS) patients and the control subjects. METHODS: Genomic DNA was extracted from blood samples and amplified by polymerase chain reaction (PCR). PCR primers flanked the polymorphic region in intro 16 of the ACE gene. RESULTS: OSAS patients had significantly higher frequencies of I/I genotype and insertion allele of the ACE gene as compared with the control subjects in Chinese population. The OSAS patients with I/I genotype had significantly longer apnea time, lower minimum SaO2 and greater AHI than the OSAS patients with I/D genotype. CONCLUSION: These results indicate that the I/I genotype and I allele are a risk factor for OSAS in Chinese.

Classification and digital subtraction angiography evaluation of carotid cavernous fistulas.

OBJECTIVE: To investigate the role of digital subtraction angiography (DSA) in evaluation of carotid cavernous fistulas, and to initiate a new classification for carotid cavernous fistula. METHODS: Cerebral angiography and selective external and internal carotid angiography were carried out in 56 patients who were classified into 5 types according to their hemodynamic features. RESULTS: The types were A (38 patients), B (4), C (10), D (2), and E (2). One of the 38 direct fistulas was located at C1 of the cavernous segment. Fistulas at C2 were observed in 10 patients, at C3 in 4, at C4 in 10 and at C5 in 5. The anterior communicating arteries were patent and well compensated in 33 of the 56 patients. The posterior communicating arteries were patent and well compensated in 39 patients. Both communicating arteries were well compensated in 31 patients. Fifty of the 56 patients were drained by ophthalmic veins, 47 by inferior petrous sinuses, 18 by cortical veins, and 8 to the posterior fossa region. CONCLUSIONS: Digital subtraction angiography was helpful in localization of the rents in carotid cavernous fistulas, in evaluation of function of Willis circles and provided valuable information for treatment of carotid cavernous fistulas. Carotid cavernous fistulas were divided into 5 categories: type A, B, C, D and E, among which type E was not previously described elsewhere.

[Chronic obstructive pulmonary disease and pulmonary structural remodeling].

OBJECTIVE: To understand the pathological characteristics of the lungs in 20 autopsy cases of chronic obstructive pulmonary disease(COPD) complicated with cor pulmonale and its relationship with structural remodeling of the lungs. METHODS: The lungs of 20 autopsy cases of COPD complicated with cor pulmonale were fixed in 10% formalin or by low pressure 18.4-33.4 mm Hg perfusion with 10% formalin. The lungs were cut sagitally into large slices with 2 cm in thickness, and the percentage of the emphysematous areas was calculated by point counting. The pathological characteristics of bronchioles and the stromal fibrosis area were examined microscopically and quantitatively using a latticed micrometer (Zeiss). The distribution and the ratio of type I to type III collagen in the lung tissue were determined using a sirius red-polarizing microscopy-morphmetry method. RESULTS: Of the 20 cases of COPD, irregular and saccular emphysema were the most frequent (17/20). 10 cases had moderate and high degree of emphysema, and 6 cases showed hypertrophy of the right ventricle (heart weight > 400 g). The mean area of stromal fibrosis auounted for total area of the examined lungs was (27.1 +/- 2.9)% and control group (12.0 +/- 1.8)%. The ratio of the area of the type I collagen to type III collagen was 5.6:1 (control group 1.3:1). All 1504 bronchioles showed chronic inflammation, 38% of which revealed acute exacerbation, and 53% of which showed lumen stenosis and fibrotic obstruction. CONCLUSIONS: Acute and chronic bronchiolitis and peri-bronchiolitis are the essential pathological changes in COPD, and also are the key link of the structural remodeling of the pulmonary tissue and vessels.

[Direct determination of serum content of ferulic acid treated with boiling water in HPLC in volunteers after oral Administration of Coronary Heart No. II decoction].

OBJECTIVE: To develop a new method for clinical pharmacokinetic detection of chemical component of Coronary Heart NO. II in serum after oral administration in healthy person. METHODS: Content of ferulic acid (FA) in serum, which had been treated with boiling water bath, was determined directly by high performance liquid chromotography (HPLC) and spectrophotometer. After qualitative detection performed by using Bi- and tri-dimensional HPLC and ultraviolet spectrophotometer, the quantitative detection was determined with the internal standard (coumarin). RESULTS: When eluted by mobile phase of a mixture of methanol, acetic acid and water (38:0.3:61.7 in ratio), and stationary phase of C18(ODS2), column (150 mm x 4.6 mm, 5 microns), the minimal detectable limit of free methanol FA was 6 ng (N/S = 3), and the minimal detectable serum FA concentration, after 10 min of boiling water bath, was 20.2 micrograms/L, with a linearity of 33.7-2156.8 micrograms/L, r = 0.9993. The mean recovery of serum FA was (93.59 +/- 2.36) micrograms/L. The RSD within day and day-to-day were all less than 8.44%. CONCLUSION: In comparing with sample Preparation of acetonitrile, this method is simple, rapid, sensitive, cheap, accurate, specific, reproducible and no toxic.

[The relationship between obstructive sleep apnea and gastroesophageal reflux and the effect of antireflux therapy].

OBJECTIVE: To evaluate the relationship between gastroesophageal reflux (GER) and obstructive sleep apnea (OSA) and the effect of combined therapy with cisapride and omeprazole. METHODS: 24-hour esophageal pH and manometric monitoring and polysomnography (PSG) were performed in 18 patients with snoring, daytime sleepiness and acid reflux, heartburn and regurgitation. Cisapride 10 mg qid and omeprazole 20 mg q12h were given to patients with GER and OSA. these tests were repeated on the 7th day. RESULTS: 7 of the 18 patients with OSA had severe GER. All except one had daytime GER as well as nocturnal GER. Nocturnal recordings were analyzed and it was found that esophageal pH below 4.0 was frequently followed by swallow action (58.6%, control = 20.6%, P < 0.05), gross body movement (18.6%, control = 4.8%, P < 0.05) and arousal (26.8%, control = 16.8%, P < 0.05). The mean lowest esophageal pressure [(-15.2 +/- 9.6) mmHg] before and [(-18.6 +/- 10.1) mmHg] after the drops of pH were significantly lower than that during control periods [(-10.6 +/- 8.1) mmHg, P < 0.005]. After antireflux therapy, there was dramatically improvement in total apnea time [from (52.7 +/- 27.1) to (21.6 +/- 21.2) minutes, P < 0.002)] and apnea hypopnea index (AHI) [from (38.9 +/- 17.6) to (15.2 +/- 12.3) No./h, P < 0.002]. CONCLUSION: GER is a predisposing factor for OSA, they may coexist and aggravate each other. Combined therapy with cisapride and omeprazole is a new, simple and effective method for GER with OSA.

[Extensive HLA class II studies in Chinese narcoleptic patients].

OBJECTIVE: Narcolepsy is a debilitating, lifelong sleep disorder. Its familial occurrence suggests that genetic factors may be of importance in the etiology. Narcolepsy is a very rare disease among Chinese, thus it was of interest to study the association of narcolepsy with the HLA system in Chinese narcoleptic patients. METHODS: To explore the role of HLA-DRB genes in the development of narcolepsy, we studied 10 narcoleptic patients and 50 race matched controls in whom HLA-DR typing was performed by the method of DNA amplification with sequence-specific primers (PCR-SSP). RESULTS: All narcoleptic patients were found to be of DRB1 * 1501 and DRB5 * 0101 genotype. CONCLUSION: These results indicate that HLA-DRB1 * 1501 is a better primary candidate susceptibility gene for narcolepsy in Chinese.

[Liver metastasis in colorectal cancer and tumor angiogenesis].

OBJECTIVE: To investigate the association between tumor angiogenetic activity and metastatic potential of colorectal cancer (CRC) retrospectively. METHODS: The microvessel density (MVD) and the level of vascular endothelial growth factor (VEGF) expression were examined immunohistochemically from 61 specimens obtained by surgical resection of CRC. RESULTS: MVD was (27.0 +/- 8.4) and the expression of VEGF was (59.75 +/- 12.36)%. Both were significantly higher in the group of liver metastasis, and in the group of lymph node metastasis (P < 0.01, P < 0.05). Survival analysis indicated a significant difference between increasing MVD group and the control group (P < 0.01). CONCLUSIONS: VEGF-mediated angiogenesis plays an important role in liver metastasis of CRC, and MVD appears to be a new prognostic factor for the prediction of outcome in patients with CRC.
