[The clinical study on urinary albumin and calcium output in 24 hours to serve as early markers for pregnancy induced hypertension].

OBJECTIVE: To assess the efficacy of urinary calcium and albumin output in 24 hours for prediction of the occurrence of pregnancy induced hypertension (PIH). METHODS: 24-hour urine samples were collected at 24-32 weeks' gestation in 98 health women for the measurement of calcium and albumin, at the same time plasma calcium and creatinine were also determined. All patients were followed up until delivery and were classified subsequently according to the occurrence of PIH. Microalbumin was measured by RIA and calcium by automatic biochemistry analysis. RESULTS: The patients in whom PIH developed later (n = 14) had significant lower urinary calcium excretion [(1.43 +/- 0.37) mmol/24 h VS (3.26 +/- 0.75) mmol/24 h] and higher microalbuminuria [(12.68 +/- 6.81) micrograms/24 h VS (6.08 +/- 3.48) micrograms/24 h] as compared with the group which remained normal (n = 84). CONCLUSIONS: The decrease of calcium and increase of albumin per 24 hours of urine were observed 4-8 weeks before the onset of PIH. The above observation may serve as early markers for PIH.

[Fentanyl reduces the MAC of sevoflurane].

OBJECTIVE: To determine the interaction between fentanyl and sevoflurane following a bolus injection of fentanyl after induction iwth sevoflurane inhalation only. METHODS: Sixty children aged 5-10 years who were diagnosed as having VSD or ASD were randomly allocated into three groups. The first group received sevoflurane in oxygen without any other drugs. The second group received sevoflurane in oxygen with fentanyl 3 ng/kg, and the third group received sevoflurane in oxygen with fentanyl 6 ng/kg. MAC determination, in response to the stimulus of skin incision, was made using the "up-down" method. RESULTS: The MAC of sevoflurane in oxygen was (2.72 +/- 0.08)%. Fentanyl 3 ng/kg bolus injection produced a fentanyl plasma concentration of (0.64 +/- 0.13) microgram/L at skin incision and resulted in a MAC for sevoflurane of (1.03 +/- 0.07)%. Fentanyl 6 ng/kg bolus injection produced a fentanyl plasma concentration of (1.51 +/- 0.18) micrograms/L at skin incision and resulted in a MAC for sevoflurane of (0.64 +/- 0.01)%. By the stimulation of skin incision, the Hr, MAP of group I increased 39%, 29%, compared with group II 14%, 15%, group III 20%, 14%, respectively. These results were significantly different (P < 0.01). CONCLUSION: The MAC of sevoflurane is significantly reduced 20 minutes after a single dose of 3 ng/kg of fentanyl and that increasing the fentanyl dose to 6 ng/kg produces a little further decrease in MAC of sevoflurane.

[Effects of sleep stage on blood pressure and heart rate in patients with obstructive sleep apnea syndrome].

OBJECTIVE: To determine the effects of sleep stage on blood pressure(BP) and heart rate(HR) and to explore the relationship between BP and HR in patients with obstructive sleep apnea syndrome (OSAS). METHODS: Manometric tube was retained in the radial artery, and whole-night polysomnography (PSG) was done synchronously in 13 patients with OSAS. RESULTS: (1) The increase in BP during apnea was higher in REM sleep than in NREM sleep, but the difference was not significant (P > 0.05). When BP in awake state was regared as baseline, the increases in SBP pre-apnea and post-apnea and the increases in DBP post-apnea were significantly higher in REM sleep(-1.9 +/- 11.9 mm Hg, 32.6 +/- 20.6 mm Hg, 18.2 +/- 11.6 mm Hg, respectively) than in NREM sleep (-8.4 +/- 8.0 mm Hg, 20.4 +/- 6.9 mm Hg, 13.2 +/- 5.8 mm Hg, P < 0.05, respectively). (2) In different sleep stages, HR was markedly higher at the end of apnea than before apnea(NREM: 76 +/- 8 bpm vs 70 +/- 8 bpm, P < 0.05; REM: 85 +/- 11 bpm vs 71 +/- 8 bpm, P < 0.01). HR in post-apnea (P < 0.01) and the increase in HR during apnea (NREM: 6.9 +/- 2.2 bpm, REM: 14.1 +/- 10.1 bpm, P < 0.05) were remarkably higher in REM sleep than in NREM sleep, but not for pre-apneic HR. (3) In NREM sleep, HR in pre-apnea was significantly correlated with the increase in SBP and DBP during apnea(r = -0.699, P < 0.01; r = -0.602, P < 0.05, respectively); The increase in SBP during apnea in NREM sleep was also correlated with post-apnea HR (r = -0.669, P < 0.01). CONCLUSION: Sleep stage has effects on BP and HR and the changes in BP are correlated with the changes in HR during apnea in patients with OSAS.

[Clinical observation of the treatment of infantile viral myocarditis with puerarin].

OBJECTIVE: To observe the clinical effect of the treatment of infantile viral myocarditis with puerarin. METHODS: Puerarin was administered intravenously together with conventional treatment in the treated group, while that of conventional treatment solely was given in the control group. Creatine kinase isoenzyme, lactate dehydrogenase isoenzyme, cardiac function and clinical manifestations before and after treatment were observed. RESULTS: Puerarin could significantly relieve the symptoms of patients of infantile viral myocarditis, enhance the metabolism of myocardium and improve the cardiac function. The total effective rate of the treated group was 87.04%, significantly higher than that of the control group, the difference between them was significant (P < 0.05). CONCLUSION: Puerarin can be used to treat patients with infantile viral myocarditis with a satisfactory result.

[Ailing No. I in treating 62 cases of acute promyelocytic leukemia].

OBJECTIVE: To obtain further knowledge on the effect, mechanism and toxic side-effect of Ailing No. I (AL), a preparation of As2O3, in treating acute promyelocytic leukemia (APL). METHODS: AL was used in induction and consolidation treatment of 62 cases of APL, the effect of treatment and the changes of clinical symptoms, peripheral blood and bone marrow pictures, T-lymphocyte subset, and immunofunctions of patients after treatment were observed. RESULTS: Twenty-seven cases out of the 31 patients who received induction treatment got complete remission (CR), the CR rate being 87.1%. Of the 31 cases who received consolidation treatment, the longest remission period was 37 months, and 7 cases relapsed within one month to 2 years. Peripheral white blood cell count raised after one week but lowered after 4 weeks of induction treatment, but in receiving consolidation treatment, it started to lower after one week and raised after 4 weeks of treatment. No obvious change on hemoglobin level was observed. Platelet count significantly increased after treatment. The treatment showed no obvious inhibition on bone marrow. The T-lymphocyte subsets and immunoglobulin level were changed insignificantly after treatment. Various side-effects were shown clinically, particularly those digestive tract involved symptoms were prominent, and liver damage was shown in a certain degree. CONCLUSION: AL has good and protracted effect in treating APL with high CR rate, the mechanism may be related with its cytotoxicity and action of differentiation induction.

[Sex hormone receptor of hemangioma and vascular malformation in children].

OBJECTIVE: To investigate the level and difference of estrogen receptor (ER), progestogen receptor (PR) and androgen receptor (AR) in hemangioma tissues of Children. METHODS: 31 specimens of hemangioma and 36 specimens of vascular malformation were compound stained with hematoxylin-eosin and enzyme-linking affinitive immunohistochemistry. The prepared tissue slices were examined microscopically to find the ER, PR and AR in all the cells. RESULTS: The ER, PR and AR were detected in the two kinds of the tissue. However, their levels in the hemangioma were higher than those in the vascular malformation (P < 0.01) and the levels in the hemangioma of proliferating phase were higher than those in the hemangioma of regressive phase (P < 0.01). CONCLUSIONS: Hemangioma is one of the target tissues of estrogen. ER, PR and AR play an important role in the growth and development of hemangiomas. The binding site of the receptor may be occupied by extrogenous glucocorticoid so that hemangiomas gradually degenerates.

[Use of ultrasonography to evaluate carotid atherosclerosis and hemodynamics].

One hundred and one cases of carotid atherosclerosis(CAS) were diagnosed by ultrasonography. The intermediate(IMT) of common carotid endomembrane in CAS group was much thicker than that in the healthy group(P < 0.01). There was no significant difference between the two groups in the peak velocity in systolic stage(MAX), the velocity in diastolic stage(MIN), resistance index(RI) and vessel size. The atheromatous plagues were usually flat plaques, seen at the bifurcation of common carotid artery. They belong to early pathological changes. It seems that the plaques don't affect MAX, MIN and RI of the common carotid artery.

[Antihypertensive study of xingnao qingxuan recipe].

OBJECTIVE: To observe the effect of decreasing blood pressure of Xingnao Qingxuan Casuples. METHOD: The blood pressure in normal and anesthetised cats was examined with direct method, the blood pressure in the conscious SHR was obtained by indirect method. RESULTS: Duodenuos administration(id) of XQC of 2.8 g/kg produced a decrease of blood pressure in normal cats; XQC of 1.4, 2.8 and 5.6 g/kg once a day for 14 days produced a dose-dependent reduction of blood pressure in SHR, 3-4 days after administration the blood pressure returned to the level before but the change was not significant. With gastric administration of XQC of 2.8 and 5.6 g/kg for 3 times, the incubation period of eyeball tremor induced by chloroform by dropping into the ear was prolonged by 14.4% and 13.0%, and the keeping time shortened by about 33.3% and 23.3% respectively. Brain basic arterial spasm induced by KCl or 5-HT in dog was relaxed obviously by XQC in vitro experiment. CONCLUSION: XQC has the effect of reducing blood pressure resisting dizziness, which is related to the diastole of brain blood vessel.

[Free radicals on cataracts induced by sodium selenite studied by ESR and LPO].

We made a electron spin resonance (ESR) study on rat cataracts induced by Na2SeO3. A signal was detected at about g = 2.0026, and the content of electron spins (CES) in lens of rat administrated Na2SeO3 for a short time is higher than that of control significantly. At the late stage of cataracts, the content of CES was lower than normal level. The results of kinetics analysis showed that the CES grow up with the administration of Na2SeO3 and decays in index-law after administration. The results suggested that free radical scavengers or antioxidants should be useful for preventing cataracts at initial stage of the oxidation damage to lens, but they could aggravate cataracts at late stage of cataracts. This can explain the controversy about the curative effect of antioxidant to cataracts.

[Chiral drug separation by supercritical fluid chromatography].

Because of the high-efficiency, fast separation, low-temperature analysis and applicable to a wide variety of detectors, supercritical fluid chromatography(SFC) is an attractive alternative for chiral drug separation. The development and application of this technique in enantiomer separation are reviewed with 48 references. The separation modes and the choices of experimental condition are discussed.

[Determination of enantiomeric purity for epinephrine by high performance liquid chromatography].

A procedure for the resolution of D,L-epinephrine and the determination of D-isomer ratio in a pharmaceutical formulation is described. The optical isomers of epinephrine were resolved by RP-HPLC with heptakis(2,6-di-O-methyl-beta-cyclodextrin) as a chiral mobile phase additives. The results showed that a correction factor must be introduced into the equation used for the calculation of the percentage of D-epinephrine, because the UV absorbance may not be the same when DM-beta-CD is present. Quantitation was achieved with external and internal standard method, HVA as an internal standard. This method can be used for the determination of the D-isomer in the L-epinephrine pharmaceutical formulation. The results showed that the mass fraction of D-isomer in the formulation increased during the period of storage.

[Phase variation in Salmonella typhimurium phagetype].

The phase variation of phagetype was observed in cultures of Salmonella typhimurium collected from Xinjiang Uygur Autonomous Region during 1980-1993. A few cultures of phagetype 4774 or 4776(full type) may change into another phagetype, i.e. 4000(phase 1) or 0774 and 0776 (phase 2). In reverse, more strains of these phagetype 4000 and 0774(0776) easily back into their original phagetype 4774 or 4776. But some 4000(phase 1) can change to 0776(phase 2), and some 0774(phase 2) can also change to 4000(phase 1). Similarly, a few cultures of phagetype 7776(full type) can change to 7000(phase 1) or 0776(phase 2). More strains of these phagetype 7000(or 7002) and 0776 reverse easily to their original phagetype 7776 or 7774. The variability is 15.6% from full type to phase 1 or phase 2, and is 53.2% from phase 1 or phase 2 to full type. Understanding of phase variation of Salmonella typhimurium phagetype will be helpful to Salmonella phagetyping, and will be significance on epidemiological analysis of Salmonella infection.

[Analysis of kinetoplast DNA of Leishmania isolates in China by PCR-SSCP].

AIM: To analyse the kDNA of the pathogens of leishmaniasis in China. METHODS: Based on leishmaniasis specific primers 13A, 13B and a set of oligonucleotide primers I and II with Leishmania donovani (L. d.) Sichuan isolate specificity, PCR were conducted to amplity minicircle kDNA fragments (297 bp and 120 bp) in the pathogens of leishmaniasis from different epidemiologic foci in China. The products were analyzed by single strand conformation polymorphism technology (SSCP). RESULTS: PCR amplified 297 bp product occurred in L. d. isolates from hill and desert foci, but no product was found in L. d. isolates from plain foci in China. SSCP of these 297 bp kDNA fragments showed that there was no difference in the mobility of ssDNA between isolates from hill foci, but there was apparent difference in the mobility of ssDNA between L. d. isolates from hill and desert foci. PCR amplified 120 bp products occurred in L. d. Sichuan isolate, L. d. Wenchuan isolate, L. d. Gansu isolate from hill foci and L. d. Shandong isolate and L. d. Jiangsu isolate from plain foci. SSCP of the 120 bp products showed that no difference in the mobility of ssDNA was found between two isolates from plain foci. There was also no difference in the mobility of ssDNA between L. d. Wenchuan isolate and L. d. Gansu isolates from hill foci. But there was apparent difference in the mobility of ssDNA between L. in fantum and L. d. isolates from different foci. CONCLUSION: Heterogeneity does exist between the kDNA of L. d. isolates from different foci of leishmaniasis of China.

[Studies on immunoprotection in mice after immunization with Schistosoma japonicum 22.6 kDa recombinant protein].

AIM: To evaluate the immunoprotective effect of Schistosoma japonicum recombinant 22.6 kDa (rSj22.6) and Sj22.6/Sj26 GST fusion protein. METHODS: The Sj22.6/Sj26 GST fusion protein was prepared by affinity chromatography using glutathione Sepharose 4B. The purified rSj22.6 could be cleaved easily from the fusion protein with Thrombin. 17 and 12 mice immunized with rSj22.6 and Sj22.6/Sj26 GST separately were each challenged with 40 +/- 1 S. japonicum cercariae. RESULTS: In BALB/c mice, the rSj22.6 and Sj22.6/Sj26 GST could induce 32.1 (P < 0.005) and 34.9% (P < 0.02) worm reduction, respectively, as well as 28.4% (P < 0.02) and 45.1% (P < 0.005) total egg reduction, respectively. CONCLUSION: Bpth rSj22.6 and Sj22.6/Sj26 GST fusion protein are partially effective against S. japonicum.

[Development of healthy beverage from Eucommla ulmoides foliage].

Effective compositions of Eucommla ulmoides foliage, sampled from the south side west section of the Qinling Mountain area, are analyzed qualiatively and quantitatively. It is shown that the foliage contains plenty of mineral elements, seventeen amino acids, proteins, sugars, alkaloids, flavonoids, aucubin, chlorogenic acid, pinoresinol diglucoside, coffeic acid juice and Jingniping glucoside. Based on the analysis and the main material of Eucommla ulmoides foliage, a healthy beverage functioning especially on resisting tire actions is formulated and produced. This work has provided a dependable new idea for further exploitation and utilization of the natural resources.

[Significance of polypeptide antigen of HCVE2/NS1 relative conservation region in detection of anti-HCV].

OBJECTIVE: In order to study the significance of HCVE2/NS1 relative conservation region antigen. METHODS: EIA was established with HCVE2/NS1 relative conservation polypeptide antigen, as well as serum HCV RNA and ALT in patients with HCV infection. RESULTS: The positive rate of 96 cases patients who were infected by HCV is 41.67%, acute 13.55% and chronic 25.04%, and we found that there were 3 cases weak positive in 40 healths; Agreement rate is 62.5% in 40 cases of the positive of anti-E2/NS1 in sera with HCVRNA, and they have relation. In ALT level in sera, acute infection of HCV is higher than chronic (P < 0.05). CONCLUSION: There are virus necleic acid in sera when anti-HCVE2/NS1 appearence positive; however, antibody of anti-E2/NS1 relative conservation region has the some neutralive actions, and this humor immunity may have the actions in clearing virus, but wether anti-body of anti-E2/NS1 relative conservation of HCV is or not neutralive antibody well be investigated further.

[Direct determination of raw plant drugs by FTIR].

Twenty-five common raw plant drugs were determined quickly and directly with diffuse reflectance drawing sample of FTIR for the first time. The results show that the drugs with different chemical compounds are characteristics of different peaks. The category of plant drugs may be firmly identified through their respective IR characteristic absorption spectrum. Furthermore, the approach can be achieved quickly and accurately without extraction and separation of samples.
