Epinephrine absorption in children with a history of anaphylaxis.

BACKGROUND: Prompt injection of epinephrine is the cornerstone of systemic anaphylaxis treatment. The rate of epinephrine absorption has not been reported previously in allergic children. OBJECTIVE: Our objective was to study the clinical pharmacology of epinephrine in this population. METHODS: We performed a prospective, randomized, blinded, parallel-group study in 17 children with a history of anaphylaxis to food, Hymenoptera venom, or other substances. We injected 0.01 ml/kg epinephrine solution (maximum 0.3 ml [0.3 mg]) subcutaneously, or 0.3 mg epinephrine intramuscularly from an autoinjector. Plasma epinephrine concentrations, heart rate, blood pressure, and adverse effects were monitored. RESULTS: In nine children who received epinephrine subcutaneously, the mean maximum plasma epinephrine concentration (+/- SEM) was 1802 +/- 214 pg/ml, achieved at a mean time of 34 +/- 14 minutes (range, 5 to 120 minutes). Only two of the nine children achieved maximum plasma concentrations by 5 minutes. In eight children who received epinephrine intramuscularly, the mean maximum plasma concentration was 2136 +/- 351 pg/ml, achieved at a mean time of 8 +/- 2 minutes, which was significantly faster than the mean time at which maximum plasma concentrations were achieved after subcutaneous epinephrine injection (p < 0.05). Six of the eight children achieved maximum plasma concentrations by 5 minutes. The terminal elimination half-life was 43 +/- 15 minutes. No serious adverse effects were noted in any child. CONCLUSIONS: In children, recommendations for subcutaneous epinephrine injection are based on anecdotal experience, and should be reevaluated in view of our finding of delayed epinephrine absorption when this route is used. This delay might have important clinical implications during an episode of systemic anaphylaxis. The intramuscular route of injection is preferable.

Modulation of VEGF production by pH and glucose in retinal Müller cells.

PURPOSE: To investigate the influence of pH and glucose concentration, both of which represent significant biochemical variables in tissue ischemia, on the production of VEGF protein by retinal Müller cells and C6 glioma cells, under normoxic and hypoxic conditions. METHODS: Rat retinal Müller cells and C6 glioma cells grown in tissue culture monolayers were studied. The effect of pH (range 7.0-8.0) and glucose concentration (0.6-25 mmol/L) on VEGF protein production, under both normoxic and hypoxic conditions, were evaluated by ELISA analysis of the conditioned media. Establishment of significant cell hypoxia was verified by measurement of lactate release into the conditioned media. RESULTS: Hypoxia caused a 7.9-fold increase in VEGF production in C6 cells at 24 h, and a 3.4-fold increase in Müller cells after 48 h. Under hypoxic conditions, VEGF protein production was increased further by increasing pH and increasing glucose, and decreased by low pH and low glucose. Varying the glucose concentration or pH of the medium did not result in significant induction of VEGF protein production by either cell type under normoxic conditions. CONCLUSIONS: Both glucose and pH significantly affected VEGF production induced by low oxygen. However, neither exerted a measurable stimulatory effect on VEGF production in normoxic conditions. Coexisting hypoxia and acidosis or hypoglycemia, as might occur in severe tissue ischemia, may render glial cells incapable of effectively upregulating VEGF synthesis, while alkalosis or hyperglycemia may augment hypoxia-induced VEGF production.

Glycine-valine dimorphism at the 86th amino acid of HLA-DRB1 influenced the prognosis of postschistosomal hepatic fibrosis.

Chinese patients (n = 113) with schistosomal hepatic fibrosis diagnosed by ultrasonography (grade I, II, or III) and 184 age- and sex-matched persons with no clinical information of schistosomal infection were typed for their HLA-DRB1 alleles by DNA typing. There was no single allele that conferred susceptibility or resistance to fibrosis. However, there were three groups of alleles that showed decreased (resistant), increased (susceptible), or neutral frequency in the patients with fibrosis. The susceptible alleles, DRBI *1202, DRB1 *1404, and DRBI *1405, shared a valine at amino acid residue 86, whereas the resistant alleles, DRB1*11011, DRB1*0409, and DRB1*0701, all had glycine at position 86. Therefore, this study focused on the glycine-valine dimorphism at aa 86, which influences the depth of the P1 pocket in the antigen binding groove, and found that the 86th valine allele was significantly increased in the patients with fibrosis (odds ratio = 2.2; 95% CI = 1.34-3.61, corrected P < .05).

Correlation between the substitution rate and rate variation among sites in protein evolution.

It is well known that the rate of amino acid substitution varies among different proteins and among different sites of a protein. It is, however, unclear whether the extent of rate variation among sites of a protein and the mean substitution rate of the protein are correlated. We used two approaches to analyze orthologous protein sequences of 51 nuclear genes of vertebrates and 13 mitochondrial genes of mammals. In the first approach, no assumptions of the distribution of the rate variation among sites were made, and in the second approach, the gamma distribution was assumed. Through both approaches, we found a negative correlation between the extent of among-site rate variation and the average substitution rate of a protein. That is, slowly evolving proteins tend to have a high level of rate variation among sites, and vice versa. We found this observation consistent with a simple model of the neutral theory where most sites are either invariable or neutral. We conclude that the correlation is a general feature of protein evolution and discuss its implications in statistical tests of positive Darwinian selection and molecular time estimation of deep divergences.

Sequence variation in ZFX introns in human populations.

DNA variation in human populations was studied by examining the last intron of the ZFX gene (about 1, 151 bp) with a worldwide sample of 29 individuals. Only one polymorphic site was found, which is located in an Alu sequence. This polymorphism is present at an intermediate frequency in all populations studied, and could be a shared polymorphism or due to migration among populations in Asia, Europe, and Africa. The nucleotide diversity is 0.04%, supporting the view that the level of nucleotide variation in nuclear DNA is very low in humans. From the sequence data, the age (T) of the most recent common ancestor of the sampled sequences is estimated: the mode of T is about 306,000 years, and the 95% confidence interval of T is 162,000-952,000 years. This mode estimate is considerably older than the estimates from Y-linked sequences.

Structure of double-shelled rice dwarf virus.

Rice dwarf virus (RDV), a member of the Reoviridae family, is a double-stranded RNA virus. Infection of rice plants with RDV reduces crop production significantly and can pose a major economic threat to Southeast Asia. A 25-A three-dimensional structure of the 700-A-diameter RDV capsid has been determined by 400-kV electron cryomicroscopy and computer reconstruction. The structure revealed two distinctive icosahedral shells: a T=13l outer icosahedral shell composed of 260 trimeric clusters of P8 (46 kDa) and an inner T=1 icosahedral shell of 60 dimers of P3 (114 kDa). Sequence and structural comparisons were made between the RDV outer shell trimer and the two crystal conformations (REF and HEX) of the VP7 trimer of bluetongue virus, an animal analog of RDV. The low-resolution structural match of the RDV outer shell trimer to the HEX conformation of VP7 trimer has led to the proposal that P8 consists of an upper domain of beta-sandwich motif and a lower domain of alpha helices. The less well fit REF conformation of VP7 to the RDV trimer may be due to the differences between VP7 and P8 in the sequence of the hinge region that connects the two domains. The additional mass density and the absence of a known signaling peptide on the surface of the RDV outer shell trimer may be responsible for the different interactions between plants and animal reoviruses.

Experimental study on firearm wound in maxillofacial region.

OBJECTIVE: To make clear the range of firearm wound in the maxillofacial region, the optical repair time and the characteristics of accompanied indirect brain damage, and to offer the principle of emergency treatment and the early repair of war wound. METHODS: With the aid of the standard Sweden model, 200 dogs were used in the experiment. Varies tissues around the primary canal were harvested chronologically, in different zone and different tissue, for histopathological examination. RESULTS: The necrotic range of various tissues in the maxillofacial region was less than that in the extremities. In the maxillofacial region, there was a significant temporary cavity following the passing of bullet, which caused indirect brain damages. CONCLUSION: These findings are helpful to the treatment of war wound in the maxillofacial region. Early bone transplantation using microvascular anastomosis in the treatment of gunshot wound in the maxillofacial region is recommendable.

[Extraction and purification of mouse H-2 antigen].

This study was designed to purify the mouse major histocompatibility complex antigen (H-2Ag). The detergent was used to extract the crude H-2Ag, and monoclonal antibodies affinity chromatography was applied to purify H-2 antigen. A 45 kd heavy chain and a 12 kd light chain from the purified proteins were shown by electrophoresis. Besides, the pure H-2 antigen was found to have immunological and biological activity. This method should be useful in purifying large quantities of H-2Ag for the researches in transplantation and functional analysis of H-2 antigen.

[The effect of chitosan in prevention of fibrous scar tissue formation after laminectomy].

OBJECTIVE: To study the effect of chitosan in the prevention of fibrous scar formation in the epidural space after laminectomy. METHOD: Fourty-eight SD rats were used as the animal model. Laminectomy was performed in lumber 1, and the exposed dura was covered with hyaluronate (group A), free fat (group B), chitosan (group C), and saline solution (group D) respectively. At 4, 8 and 12 weeks postoperation, scar formation was observed. RESULT: The results revealed that scar formed in the group D at the fourth week reached a peak at the eighth week and began to soften and constrict at the twelfth week. Sodium hyaluronate and chitosan had a significant efficacy in the prevention of scar formation, but the effect of chitosan was superior to that of sodium hyaluronate. CONCLUSION: The findings of this study indicate that chitosan has a preventive effect on scar tissue formation.

[The detection of breast carcinoma micrometastases in axillary lymph nodes by reverse transcriptase-polymerase chain reaction].

OBJECTIVE: To detect tumor micrometastases in axillary lymph nodes of breast carcinoma so as to obtain more reliable information about nodes status and its clinical significance. METHOD: The expression of CK19 mRNA was measured in 61 axillary lymph nodes (LNs) of 15 breast carcinomas by using reverse transcriptase-polymerase chain reaction (RT-PCR). RESULT: CK19 mRNA expression was detected in all 15 breast carcinomas, but not in 5 control LNs. Of 61 axillary LNs analysed by HE staining and CK19 RT-PCR, 7 LNs were found to have metastases by histological staining and CK19 RT-PCR. Of the remaining 54 axillary nodes, 12 were found to have metastases by using CK19 RT-PCR method. CONCLUSION: The CK19 RT-PCR method is more sensitive than histological study for the detection of micrometastases in LNs (P < 0.01). This new method is of value in differentiating breast cancer patients at high risk for relapse from those with negative lymph node.

[Long-term outcome of selective posterior rhizotomy for spastic cerebral palsy].

OBJECTIVE: To investigate the long term outcome and complication of selective posterior rhizotomy for spastic cerebral palsy. METHOD: 26 patients with cerebral palsy who had received SPR were followed up for four years. RESULT: In the 9 patients who could walk and stand up from squatting position independently, 7 could stand steadily on one foot after operation. In the 12 who patients could walk and stand up from squatting position dependently, 7 could walk independently in the room after operation. In 5 patients couldn't walk and stand up from squatting position, 3 patients could walk dependently in the room after operation. Complications occurred in 3 patients. CONCLUSION: Selective posterior rhizotomy is effective in the treatment of spastic cerebral palsy. Strictly selecting candidates for the operation and good rehabilitation training before and after operation should be emphasized for good results.

[Pathological studies of 23 cases of thrombus formation in cardiac cavity].

23 cases of thrombus formation in cardiac cavity were studied. 9 were cased by extensive trauma in soft tissue, 13 by various cardiac diseases. There was no significant difference between trauma and heart disease (p > 0.05). The white thrombus formation was mainly caused by trauma and was often found in the right cardiac cavity (P < 0.001). The mixed thrombus was mainly caused by heart disease and no difference was found between in fight and left cardiac cavity (p > 0.05). The types, positions and causes of thrombus were significantly related (p < 0.001). The causes, pathologic mechanism and forensic identification of thrombus formation in cardiac cavity were discussed.

[The degeneration and regeneration of skeletal muscle after tenotomy].

In order to investigate the degeneration and regeneration of skeletal muscle fiber after tenotomy, the tenotomy were carried out on gastrocnemius muscles in 15 adult BALB/C mice. The tenotomized muscles were removed on 1, 2 and 4 weeks postoperatively. Specimens were processed for histological study with light microscope and TEM, and skeletal muscle specific protein expression analysis with myoactin (MA) and myoglobin (MG) immunohistochemical staining. The results showed that the regenerating muscle cells and the degenerating muscle fibre coexisted in tenotomized muscle. The internal structure and histochemical pattern of muscle fiber changed after tenotomy. The regenerating muscle cells fused with degenerating fibres.

[An epidemiologic retrospective study of 1693 maxillofacial injuries].

A 10-year review of 1693 maxillofacial injuries was presented with respect to age, sex, cause of injury, pattern of fracture, and associated systemic injuries. The majority of fractures in maxillofacial region were found in males, the most prevalent age ranged from 21 to 40 years. The leading cause of facial trauma was traffic accidents. Mandible fracture outranked zygomatic and maxillary fracture. Multiple fractures were more common than single fracture. The average number of fractures for each patient was 1.7. Accompanied injuries to brain and skull happened in almost half patients, which were more often than other part of the body. There was a higher incidence that midfacial fractures coincided with brain and skull damage.

Directional mutational pressure affects the amino acid composition and hydrophobicity of proteins in bacteria.

The relationship between change in genomic GC content and protein evolution in bacteria was studied by simple correlational analysis (at the genus level) and by Felsenstein's (1985) independent contrast test. We first used the dnaA gene in bacteria as an example to show (1) that the amino acid composition of a protein can be dramatically affected by mutational pressure (the genomic GC content), (2) that surprisingly, deleting relatively closely-related genera may increase rather than decrease the correlation between genomic GC content and amino acid composition, and (3) that most unexpectedly, as the genomic GC content increases, both strongly hydrophobic and strongly hydrophilic amino acids tend to change to ambivalent amino acids, suggesting that the majority of these amino acid substitutions are not caused by positive Darwinian selection. These patterns were then also shown to hold for the 14 other genes studied, indicating their generality for the evolution of bacterial proteins. As directional mutation pressure can affect the amino acid composition of proteins, it may mislead phylogenetic inference, even if protein instead of DNA sequences are used.

Distribution and putative roles of fibroblast growth factor-2 isoforms in corneal endothelial modulation.

PURPOSE: Corneal endothelial modulation factor (CEMF) released by inflammatory cells induces de novo synthesis of fibroblast growth factor (FGF)-2, which is a morphogen and a potent mitogen of corneal endothelial cells (CECs). Four isoforms of FGF-2 have been found in the nucleus, cytoplasm, or extracellular matrix (ECM) in different cell lines. In the present study, the profiles of the isoforms of FGF-2 that are induced by CEMF were investigated, and whether the differential localization of the isoforms of FGF-2 plays a role in CECs proliferation and subsequent modulation was examined. METHODS: Nuclear, cytoplasmic, and ECM fractions of normal and modulated CECs were separated, and FGF-2 isoforms were further purified by heparin-Sepharose column. The molecular sizes of the isoforms were determined by immunoblot analysis, using a specific antibody directed against FGF-2. Cell proliferation was determined by cell counting. Cellular localization of FGF-2 was determined by immunofluorescence staining during different stages of cell growth. RESULTS: To confirm that CEMF modulated CECs under the conditions used in this study, its effect on cell proliferation and cell shape was determined: CEMF-treated cells showed enhanced cell proliferation profiles and fibroblastlike morphology. In rapidly growing normal CECs, FGF-2 was predominantly present in the nucleus. As the cells reached confluence, the staining potential in the nucleus was markedly reduced. Cytoplasmic staining of FGF-2 was barely detectable, regardless of cell stages. In CEMF-modulated cells, the rapidly growing cells showed strong staining of FGF-2 in the nucleus, whereas cytoplasmic and ECM staining was weak. When modulated cells reached confluence, the staining of FGF-2 in the nuclei remained strong, whereas ECM staining was significantly increased. Immunoblot analysis of the subcellular fraction showed that the 24-kDa FGF-2 was predominantly present in the nucleus, whereas the 18-kDa form was the major molecule in cytoplasmic and ECM fractions in normal and modulated cells. CONCLUSIONS: These findings indicate that 24-kDa nuclear FGF-2 may be involved in cell proliferation in growing CECs. The persistent nuclear localization and simultaneous ECM localization of FGF-2 are induced by CEMF, and these FGF-2 isoforms seem to play a role in cell proliferation and modulation.

Integrin alpha v beta 6 enhances coxsackievirus B1 lytic infection of human colon cancer cells.

Viral entry into host cells depends upon specific interactions between virus attachment proteins and cell surface receptors that enable virus binding and internalization of virus and/or the virus-receptor complex. We have recently reported that the ubiquitous cell surface molecule, decay-accelerating factor (DAF), is a major cell attachment receptor for Coxsackieviruses B1, B3, and B5. However, DAF permits only virus binding and not virus internalization, invoking the presence of secondary or accessory receptors. Among the known receptors for enteroviruses are members of the cell adhesion molecule family known as integrins. In the present study, we found that expression of the epithelial-restricted integrin, alpha v beta 6, on colonic epithelial cells significantly enhanced Coxsackievirus B1-mediated cell lysis. Importantly, the viral-mediated cell killing required the presence of the 11-amino-acid C-terminal cytoplasmic extension unique to the beta 6 subunit, providing the first evidence of regulation of viral infectivity by integrin cytoplasmic domains. These results indicate that alpha v beta 6 expression on intestinal epithelial cells critically affects Coxsackievirus B1 infectivity. This may be essential in the conversion of asymptomatic enterovirus infection into clinically apparent disease.

Involvement of T-cell subsets and natural killer (NK) cells in the growth suppression of murine fibrosarcoma cells transfected with interleukin-12 (IL-12) genes.

A 3-methylcholanthrene-induced fibrosarcoma cell line of BALB/c origin, CMS5j, was co-transfected with cDNA for the p40 and p35 subunits of interleukin-12 (IL-12). Injection of transfected cells producing 5 x 10(3) U IL-12/10(6) cells/ml/day in nude mice with an established fibrosarcoma at a contralateral site efficiently eliminated tumor growth in the early phase (injection on day 0 or 4) but not later (day 8). This effect could be abrogated by simultaneous i.v. injection of antibodies against NK1.1 or ASGM1 (asialoGM1 = ganglio-N-tetraosyl-ceramide), which indicates that natural killer (NK) cells play a major role in tumor eradication or suppression when stimulated by IL-12. In wild-type mice, application of IL-12-secreting CMS5j cells abrogated growth of tumors established 8 days before but not earlier. Based on our experiments with antibody blocking in vivo, all CD4+, CD8+ and ASGM1+ cells are involved in tumor rejection. However, in our system, CD4+ cells or CD8+ cells alone, but not ASGM1+ cells alone, also could lead to tumor rejection. IL-12-engineered fibrosarcoma cells may constitute an efficient and safe system for immunotherapy of cancer.
