Subgingival microbiota in adult Chinese: prevalence and relation to periodontal disease progression.

The "checkerboard" Dna-Dna hybridization technology was used to study the epidemiology of 18 microbial species associated with various states of periodontal health and disease, in a sample of 148 Chinese subjects never exposed to systematic dental therapeutic intervention, aged 30 to 39 and 50 to 59 years. Our aims were to: 1) describe the prevalence of these microorganisms; 2) correlate the microbiological and clinical profiles of the subjects; and 3) examine the association between the microbiological variables and the longitudinal changes of periodontal status that occurred over a preceding 10-year period. A maximum of 14 subgingival samples were obtained from each subject-1,864 in all. The frequency of occurrence of the 18 species examined was high in this Chinese population, on both the subject and the tooth site level. However, all species were not found equally capable of reaching high numbers in the subgingival samples and, as a rule, colonized heavily only limited proportions of tooth sites within each mouth. There was a profound increase of certain species such as Porphyromonas gingivalis, Treponema denticola, and Bacteroides forsythus in deep pockets or progressing sites. Multivariate techniques using the subgingival profile could effectively discriminate between deep/shallow pockets and progressing/ stable tooth sites. The microbiological variables showed an enhanced discriminating potential when classifications were performed on the individual subject level. Colonization by P. gingivalis, B. forsythus, Campylobacter rectus, and T. denticola at levels exceeding certain thresholds entailed a significantly increased probability (odds ratios > 4) for an individual subject to harbor deep pockets or progressing tooth sites.

Longitudinal study of carbohydrate metabolism in healthy obese pregnant women.

OBJECTIVE: To longitudinally characterize changes in insulin sensitivity in obese women during and after pregnancy. RESEARCH DESIGN AND METHODS: Six glucose-tolerant obese women underwent a 4-h euglycemic-hyperinsulinemic (500-600 pmol/l) clamping during the second (22.5 +/- 2 weeks [mean +/- SD]) and third trimester (36.8 +/- 0.9) of pregnancy and again 15.6 +/- 1.4 weeks after delivery. Rates of total body glucose turnover (with [6.6-2H2]glucose) and oxidation (with indirect calorimetry) were measured. RESULTS: There were no significant changes with respect to the action of insulin on rates of glucose disappearance (GRd), carbohydrate oxidation, or endogenous glucose production (EGP), comparing the second trimester of pregnancy with the nonpregnant (postpartum) state. The third trimester, however, was characterized 1) by reductions in insulin-stimulated GRd (-28%, P < 0.05, compared with the second trimester and -40%, P < 0.05, compared with postpartum); 2) by even larger reductions in insulin-stimulated carbohydrate oxidation (-46%, P < 0.05, compared with the second trimester and -54%, P < 0.02, compared with postpartum); and 3) by reduction of insulin suppression of EGP (-39% compared with -79% at the second trimester and -77% postpartum, P < 0.01). CONCLUSIONS: Glucose-tolerant obese women developed peripheral was well as hepatic insulin resistance during the third trimester of pregnancy. These alterations were reversed after delivery and appeared to be adaptive mechanisms to cope with the increased demand for glucose of the growing fetus.

Molecular cloning of the cDNA for rat hepatic, bile salt-dependent cholesteryl ester/retinyl ester hydrolase demonstrates identity with pancreatic carboxylester lipase.

Rat liver homogenates contain a neutral lipid ester hydrolase that requires millimolar concentrations of bile salts for maximal activity in catalyzing the hydrolysis of cholesteryl esters and retinyl esters in vitro. Previous studies have demonstrated that this hepatic hydrolase resembles rat pancreatic carboxylester lipase because it reacts with a specific pancreatic carboxylester lipase antibody and the eight N-terminal amino acids of the hepatic protein are identical to those of the pancreatic enzyme. Nonetheless, the exact molecular relationship between the hepatic and pancreatic enzymes is unclear. In the present study, a rat hepatic cDNA encoding the enzyme was cloned. Sequence analysis demonstrated that this cDNA corresponds to the full-length mature pancreatic carboxylester lipase (EC# 3.1.1.13). In individual animals the hepatic and pancreatic cDNA sequences were identical. However, among rats there were sequence variations, suggesting a polymorphic nature for this rat gene.

[Uptake of Escherichia coli lipopolysaccharide and expression of tumor necrosis factor-alpha-mRNA by isolated rat intrahepatic bile duct epithelial cells].

OBJECTIVE: To study the uptake of E. coli lipopolysaccharide (LPS) and expression of tumor necrosis factor-alpha-mRNA in isolated intrahepatic bile duct epithelial cells. METHODS: Fluorescent immunohistochemical and in situ hybridization techniques and observations with a confocal laser scanning microscope. RESULTS: Positive reactions to LPS were found in the cytoplasm of isolated intrahepatic bile duct epithelial cells after incubation with S type LPS (S-LPS) for 15 minutes, and the FITC fluorescent intensity against LPS was significantly higher than that of controls. After incubation with S-LPS for 3 hours, FITC fluorescent intensities to the expression of tumor necrosis factor (TNF)-alpha-mRNA by flurescent in situ hybridization in the cytoplasm and nuclei of cultured bile duct epithelial cells were significantly higher than that of controls. The increase of FITC fluorescent intensity to TNF-alpha-mRNA expression showed a peak at 6 hours after incubation and at various time points after incubation with LPS, the increase of fluorescent intensities in the cytoplasm were much higher than that in the nuclei. CONCLUSION: It is suggested that LPS could act on and enter into isolated intrahepatic bile duct epithelial cells and stimulate the expression of TNF-alpha-mRNA.

[Immunohistochemical change of endothelin-1 in acute lung injury and the role in its pathogenesis].

On the rat acute lung injury model produced by intraperitoneal cavity injection of zymosan showed that the respiratory rate became slow at first time (1st day), subsequently became rapid. Pulmonary histology showed that there was an accumulation of neutrophils in pulmonary microvessels, neutrophils infiltration in interstitial tissue, interetital and olveolar edema, hyaline membrane, atelectasis. Positive expression of ET-1 in experimental rats was stronger than than contral rats. Image analysis showed that OD value was elevated, especially at the fist day. The quantity of ET-1 was corresponding to the degree of pathologic change of acute lung injury. It suggests that the ET-1 might play an important role in the mechanism of acute lung injury.

[Mechanisms of thrombin induced proliferation and detachment of glomerular endothelial cells].

To clarify the mechanisms of thrombin contributing to the progression of glomerular diseases by injuring glomerular endothelial cells (GECs), we studied the effects of thrombin on GECs in vitro. Cell proliferation was detected with MTT incorporation, total plasminogen activator (PA) and tissue type PA (t-PA) activities were detected with fibrin plate and chromatogenic substrate methods and fibronectin was detected with ELISA as well as indirect immunofluore scence. 0.4-3.2 NIH U/ml thrombin promoted GEC proliferation significantly (P < 0.05). Thrombin promoted cell detachment, which can be inhibited by hirudin or aprotinin. Thrombin enhanced total PA and t-PA activities of GECs significantly (P < 0.01). Fibronectin in the supernatants of thrombin-stimulated GECs decreased significantly (P < 0.01) and in the extracellular compartment also decreased. The decrease was inhibited by hirudin and aprotinin. In conclusion, thrombin can induce GEC proliferation and GEC detachment. The latter is probably related to PA-mediated over-degradation of extracellular matrices such as fibronectin, which are needed for cell attachment.

[Hyperosmotic glucose and antibiotics induce production of plasminogen activator inhibitor and expression of TGF beta mRNA by rat peritoneal mesothelial cells].

To investigate the mechanism of fibrosis in peritoneal dialysis, we observed the effects of the hyperosmotic glucose and antibiotics such as gentamicin and cefazolin on rat peritoneal mesothelial cells with cell culture, fibrin-plate lyzing and Northern blotting analysis method. The peritoneal mesothelial cells may express PAI-1 mRNA. The expression of PAI-1 mRNA in peritoneal mesothelial cells and activities of PAI in the supernatants were enhanced by 11.2 mmol/L glucose, gentamicin and cefazolin at 12 hr. By 24 hr, the activity of PAI-1 also increased in the supernatants with 11.2 mmol/L glucose. The expression of PAI-1 mRNA and production of PAI activities by the peritoneal mesothelial cells were increased by gentamicin, and the TGF-beta mRNA increased in the peritoneal mesothelial cells was induced by 11.2 mmol/L glucose, gentamicin and cefazolin. The results suggested that 11.2 mmol/L glucose, gentamicin and cefazolin may increase the expressions or productions of PAI and TGF-beta in peritoneal mesothelial cells. So by perosmotic glucose, cefazolin and gentamicin promote the peritoneal fibrosis in patients with CAPD.

Acupuncture methods for hemiplegic spasm.

Remitting hemiplegic spasticity in apoplexy (HSA) is an important problem in today's clinical study. Through teaching and clinical practice, the authors summed up the effective acupuncture methods for remitting HSA: puncture deeply the acupoints on the superior-spasm side (SSS) by filiform needles so as to obtain the intensive needling sensations in the deep tissues (ISDT) until the superior spasm is immediately remitted; tap the skin on the inferior-spasm side (ISS) by skin needles until the corresponding muscle contracts. The methods have showed a significant immediate and long-term therapeutic effect.

[Immunohistochemical study of type IV collagen chain distribution within basement membrane in patients with Alport syndrome].

To observe the distribution of type IV collagen chains within basement membrane in patients with Alport syndrome (AS), discuss the pathogenesis of AS and assess the diagnostic value of indirect immunofluorescent study of type IV collagen chains. By using indirect immunofluorescence technique, the distribution of different chains of type IV collagen in specimen of renal (6 specimens) and skin (5 specimens) basement membrane of 8 AS patients belonging to 7 kindreds were investigated. In 4 male patients with X-linked dominant (XD) AS, the specific monoclonal antibodies against NC1 domains of the alpha 3, alpha 4 and alpha 5 chains of type IV collagen failed to localize on glomerular basement membrane (GBM) and the alpha 5 chain of type IV collagen were also absent in the epidermal basement membrane (EBM). The results of immunofluorescent study were positive in 3 autosomal dominant and 1 autosomal recessive AS patients as same as that in control. AS presents a biochemical abnormality of type IV collagen. The pathogenesis of XD-AS may associate with the abnormal products of alpha 5 chain. The immunofluorescent study of type IV collagen chains distribution within GBM and EBM by using monoclonal antibodies is useful in confirming the diagnosis of AS, especially XD-AS.

[Effects of low-molecular-weight heparin on proliferative glomerulonephritis].

To investigate the effects of LMWH on proliferative nephritis, we randomized 41 patients who had obvious mesangial cell proliferation testified by renal biopsy into 2 groups according to age, course, clinical manifestations and the pathological lesions. The patients in the control group were given steroid, cyclophosphamide and bennazipril, and the patients in the treated group LMWH (fraxiparin) with the dosage of 60 anti Xa IU.kg-1.day-1 besides the medicine used in the control group. We observed a significant decrease in proteinuria and increase in serum albumin in the treated group compared with those in the control group after 4 weeks of treatment (P < 0.05). The improvement of creatinine clearance in the treated group was significant after 8 and 12 weeks treatment compared with that in the control group (P < 0.05). Our data indicated that LMWH can decrease the proteinuria in the patients who have intraglomerular coagulation and proliferative nephritis with various degrees of sclerosis, and it is helpful to relieve nephrotic syndrome. Because the less risk of hemorrhage, LMWH can be used for long-term treatment to prevent glomerulosclerosis and the process of renal function loss without special mornitoring.

[Analysis of antibiotic resistance and production of enterotoxin of MRSA].

100 separate Staphylococcus aureus (S. a) isolates at Beijing Jishuitan Hospital during Oct. 1994-Apr. 1995 were submitted to determine the proportion of methicillin-resistant S. a (MRSA) and susceptibility test to 12 antibiotics. MIC determinations of the antibiotics were performed. Also, production rate of enterotoxin was measured with RPLA method. The results showed that MRSA constituted 60% of all cultures. (Burns Dept. 67.4%, Internal Medicine 62.5%, Orthopedics Dept. 44%). The antibiotics with resistant rate of MRSA below 50% included vancomyin (3.3%), norvancomyin (5%) and amikacin (21.5%). The production rate of enterotoxin was 100% in MRSA and 20% in MSSA (methicillin-sensitive S. a). In this group, 35% of MRSA and 5% of MSSA produced more than 2 types of enterotoxin.

[Observation on the content of plasma lipoperoxide and serum superoxide dismutase in NIDDM nephrosis patients].

OBJECTIVE: In order to explore the changes of MDA, SOD in each period of diabetic nephrosis patients, the contents of MDA and SOD from 40 cases of NIDDM nephrosis patients in various stages and 15 cases of normal persons (control group) were measured. METHODS: By using method of sulpho-barbiamino acid to determine the concentration of serum MDA (the reagents are provided by Juli Biomedical Engineering Institute of Nanjing). By using method of radioimmunization to determine the concentration of SOD (The reagents are provided by Huaqing Biological Technic Institute of Beijing), blood sugar, T-cho, TG, Bun, Ccr, UAE, etc. in serum were simutaneouslly determined. By adopting statistical method to analyse the mean difference in each groups and the interrelation of each data. RESULTS: The results show that the content of MDA in each groups exceeds normal control group and the content of MDA of the group of diabetic kidney failure (DKF) exceeds group of diabetic kidney early stage (DKE) obviously (P < 0.01). Only trace of MDA and SOD can be detected in the liquid of peritoneal dialysis in group of DKF. The content of SOD in each groups exceeds normal control group without exception and the content of SOD in group of DKF exceeds that of the group of diabetic kidney function injury stage obviously (P < 0.01). CONCLUSION: Our results suggest that content of SOD might have some clinical value for the diagnosis and prognosis in NIDDM nephrosis and situation of kidney function injury.

[Quantitative histological study of adult condylar proliferation zone].

The thicknesses and cell numbers of 43 adult condylar proliferation zone were observed by serial section and multiarea measurement in order to investigate the relationships between them and age. The samples were divided into three groups by age (19-45, 46-59, 60-74). By way of statistical analysis, the cell numbers and thicknesses of different areas of the same age group, different age of the same area, and the relationship between thicknesses and age was analysed. The results show that the average proliferation zone thickness is different among different age groups, it becomes thinner with the increase of age, and the cell number is decreased. There is high significant difference among different age groups (P < 0.01). But there is no significant difference of the proliferation zone thickness between the group of 45-59 years old and the older group (60-74)(P > 0.05). There is diverse correlation between thicknesses and age increase.

[A study of multi-channel visual evoked potentials in exotropic children without amblyopia].

OBJECTIVE: To investigate the clinical significance of multi-channel visual evoked potentials (VEPs) in constant exotropic children without amblyopia and the mechanism of exotropia. METHODS: With 14 active cup electrodes, and full and half field stimulation, the checkerboard pattern was reversed and the VEPs were recorded in control and experimental groups. RESULTS: With full field stimulation, the latent period 1 (LP1) of binocular VEPs was not longer and the amplitude N1-P1 (AN1P1) not higher respectively than those of monocular VEPs in experimental group. These properties were different from those of control group. The AN1P1 of non-dominant eye was decreased, and the LP1 of dominant and non-dominant eye was longer in experimental group than that of the control group. There were no significant differences in comparing LP1 and AN1P1 between nasal-retina and temporal-retina of experimental group. CONCLUSIONS: VEPs can provide a scientific basis for abnormal binocular vision in constant exotropia. Although the visual acuity of constant exotropic children is normal, their VEPs are abnormal. Therefore, there might be dysfunction in primary visual cortex. With half field stimulation, there is no suppression of temporal-retina in monocular VEPs of constant exotropic children.

[Analysis and prevention of road traffic fatalities in Sichuan Province].

This is an epidemiological survey of the fatalities from road traffic accidents in Sichuan. All fatal accidents that occurred in Sichuan from January 1994 to December 1994 were analysed. The mortalities per million kilometers of vehicle travel, per 100,000 registered motor vehicles and per 100,000 resident population were 4.35, 42.41 and 3.92 respectively. These figures were compared with international death rates and those reported by other countries. The results showed that Sichuan had higher mortalities per million kilometers of vehicle travel and per 100,000 registered vehicles but lower mortality per 100,000 resident population, compared with some industrialized and rich develeping countries. The majority of the victims were males aged 21 to 40. Road traffic fatalities constituted the leading cause of all accident deaths. The cost of road accidents in Sichuan was estimated to be sixty million Yuan. Further strategies and methods to minimize the rate of such accidents have been suggested.

[Relationship of primary pulmonary carcinoma with P16 protein expression].

This study sought to address the relationship of primary pulmonary carcinoma (PPC) with P16 protein expression. The expression of P16 protein in 65 cases of PPC was examined with immunohistochemical staining. The rate of loss of P16 protein expression in PPC was 36.92% +/- 18.45%. The rate of loss of P16 protein expression in adenocarcinoma (AC) was 28.47 +/- 16.33%, that in squamous cell carcinoma (SCC) was 35.95% +/- 17.36%, and that in adenosquamous cell carcinoma (ASCC) 57.88% +/- 10.18%. There significant differences were in the rate of loss of P16 protein expression between those aged < 60 with SCC and those aged > 60 with SCC, between low differentiated PPC and intermediate differentiated PPC, between AC and ASCC, and between SCC and ASCC (P < 0.05). There were no significant differences in the rate of loss of P16 protein between males and females with PCC, between left and right PPC, and between PPC and metastatic pulmonary carcinoma of lymph node (P > 0.05). These suggest that P16 protein expression may be one of the important standards in estimating the prognosis of patients with PCC.
