Late asthmatic reaction caused by naphthylene-1,5 diisocyanate.

After occupational exposure to napthylene-1,5 diisocyanate (NDI) for 2 years, a 33-year-old female worker developed work-related asthma attacks. Occupational-type exposure tests with NDI resulted in a late asthmatic response, whereas a corresponding challenge test with 4,4'-methylenediphenyl diisocyanate was negative. We conclude that NDI exposure in the range of its occupational exposure limit caused occupational asthma in this subject.

Comparison of four methods to assess fungal alpha-amylase in flour dust.

The aim of the study was to compare four different immunological methods to analyse fungal alpha-amylase in flour dust samples. Three European research groups have independently developed four immuno assay based methods to measure alpha-amylase in air samples. Three of the methods use polyclonal antibodies and one method uses monoclonal antibodies. Eighty personal samples have been collected during two to three work-shifts in four bakeries. Sampling was performed with PAS-6 inhalable dust samplers and aliquots from each sample were analysed by the three laboratories. The agreement between the four methods was high compared with agreement between immunological methods to measure other allergens in the air, e.g. for rat allergens. For the three methods with polyclonal antibodies the mean differences for individual samples was less than a factor of two. The arithmetic means (AM) of the estimated alpha-amylase exposure were 12.5, 11.3, 8.6 and 25.9ng/m(3) for the respective methods with values ranging from below the detection limit to 192, 215, 207 and 615 ng/m(3). The AM for all samples analysed by the methods with polyclonal antibodies varied by about a factor of 1.5. About one-third of the values were below or at the detection limit for all methods. In a regression analysis the squared correlation coefficients (R(2)) between the methods varied between 0.91 and 0.95 for the log transformed values. For workplace monitoring, results from the methods using polyclonal antibodies will be relatively comparable. High levels of alpha-amylase might differ in absolute numbers with a factor of two or more between the different methods but will anyway be considered as high and should result in preventive actions. On the other hand, this study also shows that despite the relative agreement between methods, there is a clear need for standardization.

[Evaluation of residual working capacity with lung function studies].

Lung diseases are one of the most frequent causes of permanent occupational disability. The assessment of the insured person's remaining ability to work by evaluating the respiratory parameters is by no means trivial. Nevertheless, no clear overview has been published yet. In this paper tables and evaluation schemes are presented for the quick and reliable assessment of the degree of the occupational disability of persons with respiratory diseases.

Outcome of occupational asthma due to platinum salts after transferral to low-exposure areas.

OBJECTIVE: To assess the health effects of transferral to low-exposure jobs on subjects with occupational asthma (OA). METHODS: We performed a cross-sectional survey of 83 workers in 2 platinum (Pt) refineries and 2 catalyst production plants, all with diagnosed OA due to Pt salts. In all, 9 workers (group A; 11%) continued to be exposed at the time of examination, 16 subjects (group B; 19%) had been transferred to other workplaces within the production building, 39 subjects (group C; 47%) had been transferred to areas outside the building but inside the plant, and 19 subjects (group D; 23%) had left the plant. The median period from transferral to examination was 54 months (lower quartile 23 months, upper quartile 84 months, n = 74). Symptoms, skin-prick tests (SPT) with environmental allergens and Pt salt, total serum IgE, lung function, and bronchial hyperresponsiveness were assessed by standard procedures. RESULTS: Asthma symptoms were reported by all subjects in the production area but by only 37 of 74 subjects (50%) after transferral, with no difference being found between groups B, C, and D. Both the proportion of subjects sensitized to Pt salts (100%, 88%, 67%, and 42% in groups A, B, C, and D, respectively) and the degree of sensitization were associated with exposure (P < 0.0002). Total IgE was higher in group A as compared with groups B, C, or D (P < 0.002). There was no association between bronchial hyperresponsiveness and exposure. Although no univariate difference in FEV1 was found between the groups, regression analysis showed an association between FEV1 and exposure (P < 0.002). In all, 13 subjects demonstrated a predicted FEV1 value of <80% (22%, 6%, 10%, and 32% in groups A, B, C, and D, respectively). They were older (median 49 versus 37 years; P < 0.002) and had worked over longer periods in high-exposure areas in spite of symptoms (median 43 versus 10 months; P < 0.003). Fifteen subjects of groups C or D had been removed from areas B previously, eight of them due to respiratory symptoms. CONCLUSIONS: For the majority of subjects with OA due to Pt salts transfer to low exposure areas as defined in this study may not be associated with a more unfavorable outcome as compared with complete removal from exposure sources.

Comparison of lung-function reference values.

OBJECTIVE: Lung-function reference values play an important role in medical surveillance examinations of occupational and environmental respiratory diseases, in stipulation of preventive measures, in initiation of therapeutic measures, and in granting of benefits to which individuals with lung injuries or occupational lung diseases are entitled (e.g., bronchial asthma, pneumoconiosis, or farmer's lung disease). Prediction equations most widely used are based on studies performed more than 20 years ago and may not represent the findings obtained in today's population. METHODS: We recorded case histories and lung function values of 139 healthy subjects (spirometric and plethysmographic data, T(LCO)) and computed the differences of measured values minus predicted ones as recommended by different authors. RESULTS: The means of these differences can be seen as shifts in our group versus the theoretical (predicted) values of various authors. These shifts, the standardized mean values of residuals, and their probability as determined under the assumptions of the respective regression model and the number of subjects below the threshold limits are given. FVC, FEV1, and FEV1%FVC show mostly good agreement with the recommendations by Crapo et al., Roca et al., Glindmeyer et al., Brändli et al., and Berger et al. Our IVC and FVC values do not differ significantly from each other. Normative flow-volume curve parameters of various authors deviate widely, however, and are not compatible with the values of our control group. Plethysmographically measured volumes (FRC, TLC, and RV) are only insufficiently reflected by Quanjer's reference equations. Measured T(LCO) and K(CO) show good agreement with the predictions of Cotes et al. [11] for females but display less concurrence with the predictions for males. CONCLUSIONS: Our results indicate that the ERS values of FVC, FEV1, and FEV1%FVC mainly applied in Europe should be verified. The much better-evaluated formulas of Brändli et al. are recommended. Furthermore, the age range between 60 and 70 years should be extrapolated from these formulas until better epidemiological data on lung function are available.

IgE binding of the recombinant allergen soybean profilin (rGly m 3) is mediated by conformational epitopes.

BACKGROUND: Soybean proteins are constituents of a number of food products and represent a panel of potential allergens. Thus far, little is known about the molecular characteristics of soybean allergens. OBJECTIVE: The aim of this study was to identify the soybean profilin by PCR-based complementary (c)DNA cloning and to elucidate its allergenic characteristics. METHODS: Highly degenerate profilin-specific primers were used to identify, by means of PCR, 2 soybean profilin isoforms (GmPRO1 and GmPRO2) by using soybean cDNA as a target. One isoform (GmPRO1) with a length of 394 bp corresponding to 131 amino acid residues was subcloned and expressed in fusion with the maltose-binding protein. Moreover, 3 overlapping recombinant soybean profilin fragments comprising amino acid residues 1-65, 38-88, and 50-131 were also prepared as maltose-binding protein fusion proteins. IgE-binding reactivity of the recombinant proteins and the cross-reactivity of soybean profilin with birch profilin was studied by immunoblotting, enzyme-linked allergosorbent assays (EASTs), and competitive inhibition experiments by using serum samples from 13 soybean-sensitized subjects. RESULTS: Results of immunoblot analysis, EAST, and EAST-inhibition experiments indicate the presence of profilin in soybean extract. The recombinant soybean profilin (rGly m 3) was recognized by IgE in 9 (69%) of the 13 sera tested. Only the full-length rGly m 3 was able to bind with IgE antibodies, whereas the 3 soybean profilin fragments did not show significant binding reactivity, indicating that the IgE binding to rGly m 3 depends on the integrity of a conformational structure, which was not present in the overlapping profilin fragments. The rGly m 3 cross-reacted with birch pollen profilin (Bet v 2), and the IgE binding to Bet v 2 could be inhibited by rGly m 3. CONCLUSIONS: rGly m 3 represents a new soybean allergen with well-characterized primary sequence, and its IgE-binding reactivity is mediated by conformational epitopes.

Latex type I sensitization and allergy in children with atopic dermatitis. Evaluation of cross-reactivity to some foods.

Recent studies have demonstrated that allergy to natural rubber latex (NRL) is associated with cross-reactivity to certain foods. The aim of this study was to investigate the prevalence of NRL sensitization and allergy in children with atopic dermatitis (n=74). We also examined cross-reactions between latex and foods, and compared the frequency of suspected latex cross-reacting fruits in children with and without NRL-specific immunoglobulin E (IgE). Twelve of the 74 atopic children studied (16.2%; 95% confidence interval (CI), 8.7-26.6%) had circulating IgE antibodies to latex. These NRL-sensitized children were older and they showed significantly higher total IgE values (p<0.003) when compared with the group of children without NRL sensitization. Of the specific food IgE evaluations, 18.4% (93 out of 505) were positive, and 69.9% were observed in the group of children with latex-specific IgE, most frequently to potato, tomato, sweet pepper, and avocado. An isolated latex-specific IgE response without food-specific IgE was never observed. Exclusively in the latex-positive group, conformity with the report of allergic symptoms after ingestion of food and increased food-specific IgE was found. Twenty children without proven latex sensitization showed increased food-specific IgE, most frequently to potato, banana, and chestnut. Avocado-specific IgE was never determined in this patient group. No significant differences were detected concerning the sensitization to potato, banana, and kiwi between NRL-sensitized children and the group of 20 children without latex-specific IgE. The competitive CAP inhibition using sera from children with specific IgE to both latex and food showed different cross-reactivities between latex and the specific food. A close relationship existed between latex and avocado (median inhibition: 100%), whereas sensitization to latex and kiwi seemed to be independent in our study group (inhibition: <25%). In particular, for potato, cross-reactivity and co-sensitization existed. Our study demonstrated that children with atopic dermatitis are a high-risk group for latex sensitization. Increasing age, additional sensitization to ubiquitous inhaled allergens, and enhanced total serum IgE values seemed to be important variables for latex sensitization and further sensitization to the latex-associated foods. Cross-reactivity and, in some cases, co-sensitization to specific fruits and vegetables, were observed.

Class I endochitinase containing a hevein domain is the causative allergen in latex-associated avocado allergy.

BACKGROUND: In the medical literature immunoglobulin (Ig)E-mediated sensitization to avocado is rarely reported. On the other hand, more than 50% of subjects having IgE-mediated natural rubber latex allergy are sensitized to avocado fruit as demonstrated by skin-prick testing and/or specific IgE measurements and about 10-20% report hypersensitivity reactions after ingesting avocado. OBJECTIVE: The underlying pathomechanism of latex-associated avocado allergy is still unknown. The conserved hevein domain of the major latex allergen prohevein (Hev b 6.01) is a ubiquitous chitin-binding protein structure that can be found in several plant proteins and may be responsible for the observed cross-reactivity between latex and avocado fruit. METHODS: Chitin-binding avocado proteins (CBAPs) were isolated by affinity-chromatography and their IgE-binding characteristics were studied by immunoblotting using the sera from 15 avocado-sensitized latex patients. Inhibition experiments using isolated hevein and CBAPs as inhibitor solutions were performed to study the immunological cross-reactivity between both protein species and to assess the role of the CBAPs as mediators in latex-associated avocado allergy. RESULTS: In 80% of avocado-sensitized subjects (n = 15), IgE antibodies directed against a 31-kDa allergen were detected by immunoblotting. This IgE-binding protein was identified by protein sequencing to be a class I endochitinase containing a hevein domain at the N-terminus. Purified native and digested (using simulated gastric fluid) endochitinase were able to completely block all avocado-specific IgE antibodies in six out of seven avocado patients. CONCLUSIONS: Sensitization to endochitinase class I containing a hevein domain is the main underlying pathomechanism in latex-mediated avocado allergy.

Subchronic exposure to diisocyanates increases guinea pig tracheal smooth muscle responses to acetylcholine.

OBJECTIVE: In order to study the threshold concentrations of isocyanates (IC) for induction of lung disorders, constrictive responses of tracheal smooth muscles to acetylcholine (ACH) in guinea pigs with and without diisocyanate [toluene diisocyanate (TDI), hexamethylene diisocyanate (HDI) and diphenylmethane diisocyanate (MDI)] exposure were investigated. METHODS: An IC-induced increase in smooth muscle responsiveness was studied by measuring cumulative ACH dose responses (10(-10) to 10(-4) M ACH). Basal ACH dose-response curves, measured twice in intervals of 1 h using tracheal preparations of 11 guinea pigs previously not exposed to IC, were reproducible. RESULTS: Subchronic in vivo exposures to TDI, HDI, and MDI atmospheres of 10 and 20 parts per billion (ppb) on 5 consecutive days led to significantly (p < 0.05) increased ACH responsiveness of tracheal smooth muscle, whereas concentrations of 2.5 and 5 ppb were not effective. Exposure to HDI atmospheres of 10 ppb for 1, 2, 4, or 8 weeks resulted in a time-dependent increase in ACH responses (p < 0.05) of guinea pig tracheal smooth muscle. Increased tracheal muscle responses to ACH were transient since tracheal preparations from animals exposed to 10 and 20 ppb MDI for 4 weeks and with an exposure-free interval of 8 weeks before preparation did not show enlarged ACH responses, which were present in preparations at the end of the exposure period (p < 0.05). Exposure to low IC concentrations as present in workplaces cause increased ACH responsiveness of guinea pig tracheal smooth muscle. The increased responsiveness of the airways seems to be largely reversible, since normal responses were found after 8 weeks of IC avoidance. CONCLUSION: Reversibility of IC-induced airway hyperresponsiveness is of great occupational and preventive medical importance. Workers with acquired airway hyperresponsiveness might escape lung damage if the changes are detected in an early stage before alterations in lung function are in a chronic stage.

[Pathologic mechanisms of diseases caused by isocyanates--review of current knowledge].

During recent years in industrial countries, isocyanates have been one of the main causes of occupational asthma. The mechanisms of isocyanate-induced disorders are still unknown but recent evidence suggests immunological reactions, including cell-mediated immune responses. Immune responses to isocyanates may result in different illnesses, some of which represent cell- and/or antibody-mediated entities. In addition, irritative, toxic, and mutagenic effects may occur. This review summarizes current knowledge of the pathomechanisms, including immunological and nonimmunological (genotoxic and mutagenic) aspects of isocyanate disorders.

[Prognosis of bronchial asthma].

Little is known about the prognosis of asthma. Disease remission in adolescence has been reported to vary between 20-80%, but bronchial hyperresponsiveness can still be detected in most subjects even if they are without symptoms. The relapse rate of asthmatics who were without symptoms during adolescence amounts to about 30%. Severe disease at an early stage or in adolescence, atopy, atopic dermatitis, a high degree of bronchial hyperresponsiveness, severely impaired lung function and both active and passive smoking are risk factors and closely associated with a poor prognosis. The relevance of gender, age at onset or recurrent viral infections has not been clarified yet. The effect of environmental factors on the increasing incidence and the prognosis of asthma remains to be quantified.
