[Microscopy and endoscopy of the cerebellopontine angle in the retrosigmoid approach].

The retrosigmoid approach is one of the ways to the cerebellopontine angle. It's simple to perform and offers a good view in this difficult to reach area. It's a way to surgical cure of vertigo, microvascular compression syndromes and some kinds of acoustic neuromas. The operating microscopes is a basic tool of the surgeon in otoneurosurgery. For last few years straight rigid microscopes were introduced to have a better visualization without cerebellar mechanical retraction. They are useful complementary tools to existing methods. The authors present their experience on cadaver studies.

[Quality of life in children with cochlear implants and their families (Program Schwarzenberg)--initial report].

The aim of the study was to present the results of international evaluation program "Quality of Life in children and their families--Schwarzenberg Initiative". The parents of children under 15 years old was asked to fill out specially prepared questionnaires. 1240 children from 9 countries participated in the study. In Poland 60 families filled out the questionnaires. 80 questions concerned several topics, including reaction of parents to the first information about impaired hearing of their child, using hearing aids before and after implantation, evaluation of behaviour in kindergarten and in school. This initial report describes only a few important topics. In 27.7% of Polish families the impaired hearing of their child was a cause to stop the work of one of the parents, to decrease time of work or to change the place of work in 9.1%. In most of the children the perception of sound was noticed. Only a few families from Austria, Slovenia, Swiss and Italy reported unsatisfactory results after implantation. Implantation increases quality of life of implanted children and their families.

[Palliative HDR and PDR brachytherapy in treatment of head and neck cancer relapses].

The preliminary results of palliative HDR and PDR brachytherapy in head and cancer relapses were presented. 47 patients with locoregional recurrenceses were treated by means of HDR and PDR brachytherapy between 05.06.2001 and 31.12.2002 in Great Poland Cancer Center and ENT Dept. K. Marcinkowski University of Medical Sciences. All patients were rejected from salvage surgery or distant field irradiation because of tumor advancement and previous treatment. The mean time of the recurrence after the diagnosing of the disease and first treatment was 9, 7 month. 34 patients were treated by the means of the HDR brachytherapy with the 5 doses of 4 Gy per fraction, 13 patients were treated by the means of the PDR brachytherapy with the one stage treatment (9 doses) or two stage treatment (4 doses) with total dose ranged between 20-40 Gy. The remission after 1, 3, 6 months after treatment and early complications were assessed. In 7 (15%) patients complete remission (CR), in 31 (65.9%) partial remission (PR) and in 5 (10.6) no response for the brachytherapy (NR) were stated; in the other 4 (8.5%) patients there were progression of the tumor. Percentages of remission after 3 and 6 months were as follows: CR - 5 (10.6%) and 3 (6.4%), PR - 25 (53.2%) and 17 (36.2%), NR and Progr - 17 (36.2%) and 27 (57.4)%. In 35 (74.5%) cases superficial necrosis was observed in 6 month time. The both methods had similar percentages of side effects. HDR or PDR brachytherapy are effective method of palliative treatment in head and neck cancer relapses. Early complications are frequent and demands intensive pharmacological treatment.

[Transversal resection of trachea as the method of treatment of postintubational stenosis].

We present an experience with transversal tracheal resection with end-to-end anastomosis in 3 patients with extensive subglottic stenosis because of prolonged intubation. All the patients were treated chirurgically based on the Pearson's method. In order to attein the fixation of tracheal stumbs, poroplastic splint was applied into the lumen of the trachea and patency of airways was protected by tracheotomy.There were no major complications during treatment and time of the treatment was significally shortening.

[Diagnosis of genetically determined hearing impairment--difficulties and problems].

In terms of growing interest in molecular analysis of genetically determined hearing impairment, it is of clinical importance to know problems and obstacles that arise on the way to molecular diagnosis. Although for clinicians a genetic factor as a reason for hearing problem is easier accepted, still an understanding the limits of such identification remains unclear. Technical issues, as well as social and legal aspects of genetic analysis are described. Authors try to explain why such impressive progress in understanding of hearing loss physiology, especially from the genetic point of view, does not have its transfer into clinical practice.

[Contraindication for cochlear implantation in Wildervanck's syndrome].

A case of 4-year-old boy with Wildervanck's syndrome and associated profound sensorineural hearing loss is presented. The child was disqualified from cochlear implantation following the CT scan of petrous pyramids which revealed inner ear malformation. The authors emphasize role of CT imaging in the management of profound sensorineural hearing loss in children before final qualification for cochlear implantation.

[The changes in the distinctness and discrimination of phonems of cochlear implants children].

Hearing impaired children do not develop oral speech in the same time with the hearing ones. Cochlear implants may change this situation. Implanted deaf children may develop oral speech but only after long speech rehabilitation. Acquisition of language skills should be assesed correctly in every patient. It is important because this therapy is quite expensive. There are many quantitative and qualitative methods used all over the world. In some cases score is calculated as percentage of correctly repeated words. Sometimes it did not reflect true level of language development. 240 people were implanted in Poznań in years 1994-2004. Two years ago, evaluation of changes in the distinctness and discrimination of phonems of cochlear implants children was begun. We found differences between standard speech audiometry and distinctness and discrimination of phonems.

[Neuroendocrine tumors of the larynx].

The classification, diagnosis, treatment and prognosis of neuroendocrine tumors are presented. Rare case of typical carcinoid of the larynx is described. This was the second primary tumor after squamous lung cancer treatment. The diagnosis was based on routine histological investigation, and then confirmed by immunohistochemical tests with synaptophysin, chromogranin, S-100 protein and cytokeratin markers. The clinical presentation of the patient, age, gender, localization, macroscopic appearance and slow growth dynamic was in accordance with cases described in the literature.

[Diagnostic difficulties in the laryngeal malignant peripheral nerve sheath tumor (MPNST)].

The malignant tumor deriving from the peripheral nerve sheet, previously described as malignant Schwannoma or neurosarcoma is extremely rare as malignancy localized in the larynx. The half of cases has been developing on the basis of neurofibromatosis in von Recklinghausen disease type I or seldom, type II. The high grade of malignancy end tendency to reccurences and distant metastases is typical for this tumors. The case of 64 year old man with larynx neurosarcoma was presented. The diagnostic difficulties were caused by clinical presentation of the smooth tumor covered by unchanged mucosa and typical histological features of the tumor. The final histological assessment was complemented by positive immunohistochemical reaction (antigens against protein S-100, NSE and PG 9.5).

Oxidative DNA base modifications and polycyclic aromatic hydrocarbon DNA adducts in squamous cell carcinoma of larynx.

Tobacco smoke, recognized as a major etiological factor for cancers of the upper aerodigestive tract, represents an abundant source of reactive oxygen species (ROS), which are believed to play a significant role in mutagenesis and carcinogenesis. An additional source of ROS in tissues exposed to tobacco smoke may be metabolic oxidation of polycyclic aromatic hydrocarbons (PAH). To investigate the relationships between oxidative DNA lesions and aromatic DNA adducts, six modified DNA bases 5-hydroxyuracil, 5-hydroxycytosine, 7,8-dihydro-8-oxoguanine, 7,8-dihydro-8-oxoadenine, 2,6-diamino-4-hydroxy-5-formamidopyrimidine and 4,6-diamino-5-formamidopyrimidine and the total level of PAH-related DNA adducts were measured in cancerous and the surrounding normal larynx tissues (68 subjects), using gas chromatography/isotope-dilution mass spectroscopy with selected ion monitoring and the 32P-postlabeling-HPLC assay, respectively. The levels of oxidative DNA lesions in cancerous and adjacent tissue were comparable; the differences between the two types of tissue were significant only for 5-hydroxypyrimidines (slightly higher levels were observed in the adjacent tissue). Comparable levels of DNA lesions in cancerous and the surrounding normal tissues observed in the larynx tumors support a field cancerization theory. The surrounding tissues may still be recognized as normal by histological criteria. However, molecular alterations resulting from the chronic tobacco smoke exposure, which equally affects larynx epithelia, may lead to multiple premalignant lesions. Thus, a demonstration of similar levels of DNA damage in cancerous and the adjacent tissue could explain a frequent formation of secondary tumors in the larynx and the frequent recurrence in this type of cancer. A weak, but distinct effect of tumor grading and metastatic status was observed in both kinds of tissue in the case of 5-hydroxyuracil, 5-hydroxycytosine, 7,8-dihydro-8-oxoguanine, 7,8-dihydro-8-oxoadenine. This effect was displayed as a gradual shift in the data distribution toward high values from G1 through G2-G3 and from non-metastatic to metastatic tumors. Since the levels of oxidative DNA base modifications tended to increase with the tumor aggressiveness, we postulate that the oxidative DNA lesions increase genetic instability and thus contribute to tumor progression in laryngeal cancer. No associations between aromatic adduct levels and oxidative DNA lesions were present, suggesting that the metabolism of PAH does not contribute significantly to the oxidative stress in larynx tissues, remaining the tobacco smoke ROS as a major source of oxidative DNA damage in the exposed tissue.

[Distinctness and discrimination of phonemes in children with cochlear implants--preliminary report].

There are many tests prepared for speech evaluation for different age groups and with any disabilities. The percent of correct repeated mono- or bisyllabes words create the final result. Sometimes this score is lower than real oral communication skill of CI patient. In the years 1994-2002 in Poznań 158 cochlear implantations were performed. Since few months the evaluation of the distinctness and discrimination of phonems of cochlear implants children are being conducted. It was found some differences between distinctness and discrimination of phonems and discrimination of words. It means that scores achieved by children in standarized words--tests do not reflect the real level of speech development of each child.

[Gastro-oesophageal reflux disease presenting as laryngeal pathology--epidemiology, symptoms, diagnosis. Preliminary report].

UNLABELLED: Symptoms from the ears, throat, nose and larynx--the laryngeal mask's manifestations of gastroesophageal reflux disease (GERD) are important clinical problem of significant occurrence. AIM: (1) Determination of GERD's frequency of occurrence, especially within the group of patients with the pathology of the posterior laryngeal commissure. (2) Evaluation of the effective algorithms for detecting and management of patients demonstrating symptoms of laryngeal mask, which develop on the ground of gastroesophageal reflux disease. The investigation included 9 patients who were hospitalized in the Department of Otolaryngology of Karol Marcinkowski University of Medical Sciences in Poznań. Every patient was carefully interviewed by means of 3 questionnaires: the first one being Karlsson's general questionnaire, the second one--author's questionnaire designed in the Department of Otolaryngology of Medical Academy in Poznań which emphasises laryngeal symptoms; and the third one being the inquiry including serious past diseases, regularly taken medications, and stimulants. The objective condition of the hypopharynx and larynx was estimated on the basis of indirect laryngoscopy, followed by manometric examination of the oesophagus and 24-hour pH-metry carried out in Motility Laboratory of Surgical Department. 8 patients (88.9%) out of 9 were diagnosed as having GERD on the ground of the questionnaires as well as on the base of the results from the 24-hour pH-metry, and among 7 patients (87.5%) out of the 8 motility disfunction of the oesophagus was detected. Within the group of 8 patients with diagnosed GERD on the ground of additional tests 3 patients (37.5%) were diagnosed as having posterior laryngitis, 2 patients (25%) were diagnosed as having chronic laryngitis and the last 1 (12.5%) suffering from dysphagia. Among 3 patients (37.5%) with tumors of the larynx of rare location within the posterior laryngeal commissure coexistence of gastroesophageal reflux disease (GERD) was proved. The remaining 1 patient (11.1%) with the larynx cancer had no confirmation of GERD. The connection between chronic laryngitis and gastroesophageal reflux disease and/or motility disfunction of the oesophagus described in the literature was confirmed within the group of examined patients. Awareness of GERD and diagnostics carried out in that direction is needed in the group of patients with typical symptoms of the laryngeal mask. Significant frequency of coexistence of gastroesophageal reflux disease and pathology of the posterior laryngeal commissure requires effective algorithms for management of these patients.

[Usefulness of TAPS (Polish version) for evaluation of CI children's progress in speech rehabilitation--7 years of observation].

The speech rehabilitation of pre- and perilingually deafened children with cochlear implants it is a very long process. It should be individualized for needs of every young patient. It is possible only after correct evaluation of speech level. There are many tests for this evaluation. Among them same are more known like TAPS test. Test of Auditory Perception of Speech (TAPS) was elaborated in English at Basel University in Swiss. It was translated to Polish at Adam Mickiewicz University and Karol Marcinkowski University School of Medical Sciences in Poznań. The authors present their own 7-years observations.

[Evaluation of an animal model in endoscopic surgery of the cerebello-pontine angle].

In the last few years one can observe a dynamic evolution of the minimal invasive surgery, particularly in the field of the skull base surgery. We are now in a front of a big problem with human specimen for dissection. They become rare. Those circumstances force us to look for an animal model to perform dissections. The swine is a very well known model for general and thoracic surgery but we can hear also some attempt of using it in the field of the skull base surgery. Authors tried to deal with the endoscopic anatomy of the cerebello-pontine angle through the retrosigmoid approach. During this study they found that we could find structures which are analogical to human's one. Animal models will not take place of human specimens in surgical training but they can considerably reduce their number. Difficult technique and unusual tools demands a lot of hours of training, and a big part of it can be easily performed on animal models. Authors present their own experience.

[Merkel cell carcinoma of the nose].

Despite an increasing number of reports Merkel cell carcinoma still is a rare neoplasm. We therefore present the imaging feature of the case of Merkel cell carcinoma involving the nasal region in 65-year woman with lymphoma malignum lymphoplasmocytoides and its rapid progression with fast metastasis to lymph nodes. Surgery and adjuvant radiotherapy, chemotherapy appeared to provide optimal local control.

[Enlarged vestibular aqueduct syndrome in patients with hypoacusis and deafness].

UNLABELLED: In 1978 Valvassori and Clemis have reported that enlarged vestibular aqueduct can cause progressive perceptive hearing loss and finally deafness. AIM: Analysis of CT scans of patients with cochlear implants. Analysis of radiological data of 120 implanted patients was performed. CT examinations were performed during qualification to cochlear implantation. Most of the CT scans were performed using high resolution computed tomography (HRCT). A special attention was paid to patients with progressive perceptive hearing loss and CT of two patients with "gusher" during CI surgery. In one case (6-years old girl with bilateral profound perceptive hearing loss diagnosed in 2nd year of age) an enlarged vestibular aqueduct was found radiologically. In the second case (16 years old girl with progressive perceptive hearing loss since 1st year of age) no enlarged vestibular aqueduct was detected. Diagnostics and treatment of enlarged vestibular aqueduct syndrome are presented.
