[Correlations of clinical symptoms and treatment efficacy in patients with rheumatoid arthritis treated with Chinese herbal drugs or Western medicine].

OBJECTIVE: To evaluate the correlations between clinical symptoms and treatment efficacy in patients with rheumatoid arthritis (RA). METHODS: Four hundred and thirteen patients were included in the clinical trial from 9 clinical centers. They were randomly divided into Western medicine-treated group with 204 cases and Chinese herbal drug-treated group with 209 cases. Eighteen clinical symptoms were evaluated before and after treatment. The Western medicine therapy included voltaren extended release tablets, methotrexate and sulfasalazine. The Chinese herbal drug therapy included glucosidorum Tripterygii totorum tablets and Yishen Juanbi Tablets combined with treatment based on syndrome differentiation. The American College of Rheumatology 20 (ACR20) was used as efficacy evaluation criteria. RESULTS: In the Chinese herbal drug-treated group, clinical symptoms such as arthralgia and tenderness of joints were positively correlated with the efficacy after 12-week treatment, while frequent urination at night was negatively correlated. In the same group, tenderness of joints and fever were positively correlated with the efficacy after 24-week treatment, while deep-colored and turbid urine was negatively correlated. In the Western medicine-treated group, tenderness of joints and thirst were positively correlated with the efficacy after 12-week treatment, while vertigo was negatively correlated. And in the same group, tenderness of joints was positively correlated with the efficacy after 24-week treatment, while heaviness of limbs was negatively correlated to the efficacy. The statistical results showed that the treatment efficacy was improved when the correlated symptoms were included in the indications. CONCLUSION: The treatment efficacy of RA is correlated with some symptoms, so further studies should proceed on these correlations in order to achieve better treatment outcome.

[GFP fused to the cytoplasmic tail of integrin alphaIIb allows the normal expression of alphaIIb beta3 compound in CHO cells].

To investigate the effect of GFP fused to C terminal of integrin alpha(IIb) on the biosynthesis and expression of alpha(IIb) beta(3) compound, the alpha(IIb) GFP expression plamid, named palpha(IIb) GFP, the cDNA of alpha(IIb) was constructed from p3.1-2b and fused to pEGFP-N1 in frame. When the sequence of palpha(IIb) GFP was confirmed by sequencing it was transferred to Chinese Hamster Ovary (CHO) cells with or without p3.1-3a expressing integrin beta(3). Then the expression of alpha(IIb) GFP fusion protein was confirmed by Western blot and then its subcellular localization was determined with laser confocal scanning microscopy. The results showed that the target gene was cloned into recombinant vector by restriction analysis and sequencing. Overexpression of the fusion protein in the transfected CHO cells was identified with Western blot. Subcellular localization analysis confirmed that alpha(IIb) GFP was expressed in CHO cells and could be transferred from endoplasmic reticulum to Golgi apparatus. It is concluded that the eukaryotic expression plasmid containing alpha(IIb) GFP fusion gene is successfully constructed. GFP fused to the cytoplasmic tail of integrin alpha(IIb) allows the normal expression of alpha(IIb) beta(3) in CHO cells.

[Effect of glycoprotein alpha II bA2334C mutation on the biosynthesis and transportation of alpha II bbeta3 complex].

OBJECTIVE: To study the effect of glycoprotein (GP) alpha II bA2334C mutation on the biosynthesis and expression of alpha II bbeta3 complex. METHODS: The GP alpha II bA2334C eukaryotic expression plasmid pc3.1-2334M2b was constructed. Chinese hamster ovary (CHO) cells were transfected with the plasmid with or without integrin beta3 expression plasmid pc3.1-3a. The whole expression of alpha II bA2334C was confirmed by Western blot and the membrane expression was analyzed by flow cytometry. A newly constructed alpha II bA2334C GFP fusion protein expressing plasmid was used to determine its subcellular localization by laser confocal scanning microscopy. RESULTS: Expression of the mutant protein, alpha II bA2334C, in the transfected CHO cells was confirmed by Western blot with a lower rate of the mature type than the wild type control. The expression on membrane was only 25% of the normal. Subcellular localization analysis showed that alpha II bA2334C GFP was able to be expressed in CHO cells and could be transported from endoplasmic reticulum to Golgi apparatus. CONCLUSIONS: The mutant alpha II bA2334C can be synthesized in CHO cells and form alpha II bbeta3 complex. However, only a small fraction of the premature alpha II bA2334C can be transported to Golgi apparatus and transformed to mature alpha II b. The possible pathogenesis of this type II thrombasthenia may be that the misfolded alpha II bA2334C is partially degraded in the endoplasmic reticulum causing lower expression of alpha II bbeta3 complex on the membrane and resulting in impared function of platelets than normal alpha II b.

[Study of anti-myeloma activity of interleukin-2 activated bone marrow in vitro].

OBJECTIVE: To study the anti-myeloma activity of interleukin-2 activated bone marrow (ABM). METHODS: Bone marrow mononuclear cells (BMMNC) from multiple myeloma and iron-deficiency anemia patients were cultured in the presence of rIL-2. The anti-myeloma activity of ABM against U266 cells, cells expressing surface CD45, CD38, CD138, the levels of TNF-alpha and IFN-gamma in ABM culture supernatant were measured with MTT method, flow cytometry and ELISA method respectively after bone marrow was activated with rIL-2 for 24 and 72 hours. RESULTS: The tumor-killing activities against U266 cells of ABM were significantly increased compared with that of non-activated bone marrow (NBM) at 72 hours [(69.70 +/- 26.57)% vs (43.20 +/- 12.39)%, P < 0.05] and 24 hours [(34.25 +/- 11.93)% vs (26.53 +/- 5.48)%]. The CD45(-)CD38(+)CD138(+) cells of ABM from myeloma group at 72 hours were decreased from (8.46 +/- 3.66)% to (4.79 +/- 1.56)% (P < 0.05). TNF-alpha and IFN-gamma were detectable after cultured for 24 hours in both normal control group and myeloma group and went higher at 72 hours. The level of TNF-alpha and IFN-gamma were significantly increased in ABM compared with that in NBM (P < 0.05). Meanwhile, there was a positive relationship between the level of TNF-alpha, IFN-gamma and cytotoxicity of ABM from normal control group at 24 hours and 72 hours (P < 0.05), and was a negative relationship between TNF-alpha and IFN-gamma levels and the CD45(-)CD38(+)CD138(+) cells in myeloma group at 72 hours (P < 0.05). CONCLUSION: Normal BMMNCs activated with rIL-2 have tumor-killing activities against U266 cells. Myeloma cells and tumor burden were decreased in myeloma bone marrow after the marrow was activated with rIL-2. Production of TNF-alpha and IFN-gamma from bone marrow cells including T cells, monocyte-macrophages and NK cells activated with rIL-2 might be involved in anti-myeloma activity of ABM.

[Preparation and surface modification of the magnetic fluid as a targeted drug carrier].

Magnetic fluid containing Fe(3)O(4) as mother nucleus was prepared by chemical coprecipitation and treated with dextran-70 for surface modification. The dextran-70-modified magnetic fluid was reacted with 6-bromohexanoic acid and their chemical stability was studied, with the structure characterized by using laser granulometer, X-ray diffraction, Fourier-transform infrared analysis and transmission electron microscopy. The results showed that the magnetic fluid with surface modification by dextran-70 exhibited high stability in the presence strong alkali or acid. No chemical changes occurred in the magnetic Fe(3)O(4) nucleus surrounded by alkali-treated dextran-70 and the organic chain linked to the surface dextran rendered the reaction to the anti-cancer drug such as mitomycin C possible, suggesting the potential of the preparation as a drug carrier.

[Ectopic osteogenic ability of BMSCs on various scaffold materials in nude mice].

PURPOSE: To explore the ectopic osteogenic abilities of bone marrow stromal cells (BMSCs) on different scaffold materials in nude mice. METHODS: Canine BMSCs in vitro cultured were seeded at the density of 1x10(6)cells/cm(2) onto scaffolds of freeze-dried demineralized bone matrix (fdDBM), tricalcium phosphate (TCP), 200microm coralline hydroxyapatite (CH200) and 500microm coralline hydroxyapatite (CH500). The cell adherence and growth on the materials were examined by SEM and H&E staining after 4 to 7 days of in vitro culturing. To observe the ectopic osteogenic capacity, four 10mmx5mmx2mm cell/scaffold complexes (fdDBM, TCP, CH200, CH500) and a blank control scaffold (fdDBM only) were implanted subcutaneously into each of athymic mice (n=8). The implants were harvested after 9 weeks and H&E stained. Newly formed trabecular bone volume (TBV) was measured by IMAGER-PRO PLUS software, and statistical analysis was performed by ANOVA with SAS software. RESULTS: Canine BMSCs adhered to all 4 types of scaffolds and spread well in their pores 7-8 days after seeding. Athymic subcutaneous ectopic osteogenesis were observed in all the specimens of TCP, CH200, CH500 but were observed only in 75% of BMSCs/fdDBM and 25% of fdDBM. The TCP group showed significant higher TBV percentage (28.2%+/-2.86) than CH200 group (24.1%+/-4.12) and CH500 group (18.1%+/-4.66) (P<0.01). CONCLUSIONS: BMSCs adhered to all 4 scaffolds. When combined with BMSCs, TCP exhibited the greatest osteogenic potential, which indicates that it might be an excellent scaffold material in the field of bone tissue engineering. [Skeleton reconstruction of oblique facial clefts using mandibular outer table].

OBJECTIVE: This paper presents a new method of skeleton reconstruction for oblique facial clefts using autogenous bone of the mandibular outer table. METHODS: In the operation, the mandibular outer table was harvested through the intraoral approach. Assisted with internal rigid fixation technique, the mandibular outer table was used to reconstruct the naso-orbital framework as inlay or onlay bone graft. RESULTS: From 1993 to 2001, seven cases of oblique facial clefts were repaired with mandibular outer table bone graft. Postoperative follow-up for 6 months to 3 years demonstrated that the grafted bone healed well with the adjacent bones. No obvious bone resorption was observed. The facial appearance was improved greatly. CONCLUSIONS: The mandibular outer table, with similar bone density to the calvarium, is easy to harvest without donor site scar. The method is quite ideal for skeleton reconstruction of oblique facial clefts.

[Myocardial protective effect of puerarin injection in children with severe pneumonia].

OBJECTIVE: To investigate the myocardial protective effect of puerarin injection (PI) in children with severe pneumonia. METHODS: One hundred children with pneumonia enrolled were divided into two groups according to their condition of illness. Forty-eight children of mild condition in the common group (CG) were treated with conventional treatment and 52 children of serious condition in the severe group (SG) were treated with conventional treatment with additional PI or fructose-1,6-disphosphate (FDP) for myocardial protection. The parameters of serum myocardial zymogram, ECG and cardiac blood flow were measured and the effects of PI and FDP in patients of serious condition were compared. RESULTS: Change of serum myocardial zymographic parameters was indirectly proportional to the severity of childrens condition. The reduction of myocardial contractive function was demonstrated by the change of cardiac blood flow parameters in Ultrasound Doppler examination (P < 0.01). Both PI and FDP had the effect in protecting myocardium and improving heart function, showing insignificant difference between their efficacy (P > 0.05), but PI had less stimulation to vessels and without any obvious adverse reaction. CONCLUSION: Severe pneumonia in children can lead to myocardial damage and heart failure, and early applying of myocardial protective measure can significantly improve the prognosis of patient. Myocardial zymographic parameters, especially the CK-MB, are sensitive and important indexes for detecting myocardial injury. PI has effects in protecting myocardium and improving heart function, and therefore, is suitable for spreading and applying in clinical practice.

[Effects of mast cell infiltration on the development and metastasis of gastric carcinoma].

OBJECTIVE: To investigate the relationship between mast cell infiltration and the development and metastasis of gastric carcinoma. METHODS: Alcian blue-Safranin O staining and image analysis system were employed to observe, classify and quantify the mast cells in 74 gastric carcinoma specimens. RESULTS: Mast cells resided predominantly in the marginal area of the tumor and most of them were mature type, active and capable of discharging granules into the cytoplasm. The mast cells in the marginal area of the tumor were significantly more numerous than those in the matrix of the tumor (P<0.01) and there was a positive correlation between them (r=0.303, P<0.01). The density of the mast cells was inversely correlated to the depth of tumor invasion and the number of lymph node metastasis. CONCLUSIONS: Mast cell infiltration in the matrix of the tumor plays an important role in preventing the invasion and metastasis of gastric carcinoma possibly by release of heparin and other bioactive substances. Mast cell quantity in the marginal area of the tumor may serve as an indicator for prognostic assessment of gastric carcinoma.

Combined surgical therapy for hepatolithiasis.

OBJECTIVE: To evaluate the combined surgical therapy for calculi of intrahepatic duct. METHODS: One hundred and eight cases of hepatolithiasis treated in our hospital from January 1986 to September 2003 were summarized and analyzed retrospectively. The primary surgery included 57 cases of biliary tract exploration and cholangiolithotomy, 26 cases of cholangiojejunostomy, and 25 cases of partial hepatectomy. Of these cases, 156 operations were performed totally. There were 36 patients undergoing two or more operations for cholangiolithiasis problem. Fiberoptic choledochoscopy was employed to examine and remove the residual stones during and after operation in all cases. RESULTS: The residual stone rates were 57.89% (33/57), 26.92% (7/26), and 24.0% (6/25) for biliary tract exploration and cholangiolithotomy, cholangiojejunostomy, and partial hepatectomy, respectively. Fiberoptic choledochoscopy was utilized pre- and post-operatively for each patient, which was performed 2 to 9 times on each case. Following the combined treatment, the rate of residual stone after operation decreased to 12.96% (14/108). CONCLUSIONS: The combined surgical therapy is valuable for resolving the problem of hepatolithiasis in most of the patients. Regarding the clinical outcomes of different surgical procedures, partial hepatectomy is superior to cholangiojejunostomy or biliary tract exploration and cholangiolithotomy. Fiberoptic choledochoscopy is also important to reduce the occurrence of residual stones and the rate of reoperation.
