[Incidence and mortality of myocardial infarction in inhabitants of the Lublin province].

In the years 1993-1995 the incidence of myocardial infarction in the professionally engaged inhabitants of Lublin province was 16.4 per 10,000 population. The myocardial infarction attack rate in men (25.8 per 10,000 population) was three times as high as that in woman (7.4 per 10,000 population). The overall mortality rate of myocardial infarction in the adult inhabitants of Lublin province reached the value of 39.8%. After adjustment for sex fatality rates in men and in women were 41.7% and 33.4%, respectively. The percentage of out hospital deaths (25.2%) was significantly higher than that of in-hospital deaths (19.6%).

[François Girardot (1773-1831) the surgeon of Light - Cavalrymen Regiment of Napoleon I Guard].

The paper is the first wider biographic study presentif life and activity of François Girardot (1773-1831), a French physician, a surgeon of Polish light-cavalrymen regiment of Napoleon I guard for many years; since 1816 permanently domiciled in the Congress Kingdom of Poland, in Warsaw and Opinogóra, as a family doctor of Krasińskis family. There are his relations with Poland recalled, started in 1798 already, there are shown some famous patients of Girardot - Klementyna Hoffman of Tańskis, Zygmunt Krasiński and Fryderyk Chopin. There is also the attention drawn to his medical activity among poor population of Warsaw. The fact is underlined, that Girardot as the only physician was made by Napoleon the baron of Empire at the battle field (Craonne, 7th March, 1814), directly after injury, the effect of which there was an amputation of leg. It is also indicated, that despite unquestionable military merits, confirmed by numerous awards and the title of baron granted, and also medical merits of Girardot, he is still remaining a personage not well known to the historians of Napoleonic period.

[Mortality of patients with myocardial infarction at a young age].

Among patients with myocardial infarction, patients up to 40 years of age accounted for 5.1% (5.4% men and 4.1% women). Early mortality due to myocardial infarction was in the all material 42.6% and depended mainly on deaths of men in the prehospitalization period. Prehospitalization mortality varied between men and women. It was 45.3% for men and 11.2% for women. Hospital mortality was low, accounting for 7.8% of the treated subjects.

[Diagnostic difficulties in a case of polyarteritis nodosa].

A case of polyarteritis nodosa is presented in view of diagnostic difficulties which it caused. In a 47-year-old patient, in the clinical picture the following predominated: paroxysmal abdominal pain, symptoms and signs of polyneuropathy, and paralysis of the left peroneal and left radial nerves. Besides that, hypertension, tachycardia, sub-febrile states, significant body weight loss, and skin lesions were observed. In laboratory investigations high ESR, leucocytosis, HBe antigenaemia with the presence of all antibodies, and significant hypergammaglobulinaemia were found. In the differentiation, apart from polyarteritis nodosa, the following was taken into account: porphyria, neoplastic abdominal diseases with polyneuropathy, neurological diseases. Of decisive importance for making the diagnosis was microscopic examination of calf muscle biopsy specimen.

[Tuberculosis of the retroperitoneal lymph nodes].

The authors present an extremely rare location of retroperitoneal lymph node tuberculosis, imitating in ultrasonography and computed tomography a neoplastic tumour of this region. The diagnostic difficulties associated with the described case are analysed.

[Prehospital death rate in acute myocardial infarction].

The aim of the study was the analysis of prehospital death rate caused by AMI depending on sex, age and place of habitation in an agricultural-industrial region. Prehospital death rate resulting from AMI reached 34.6% (32.8% men and 39.3% women). 30.4% of patients died before obtaining medical help. The incidence of deaths increased with the more advanced age (until 70 was between 20-25% whilst, over 70 as high as 62-94%).

[Acute myocardial infarction. Analysis of the activities of the ambulance "R"].

Frequency of emergency calls for ambulance help to patients with acute myocardial infarction (MI) as well as a number of infarct complications and the influence of immediate professional help on survival of prehospital phase of acute MI is reviewed. The diagnosis of acute MI was based on a typical history of chest pain and electrocardiographic findings. Acute MI was also diagnosed in all cases of sudden cardiac death. Out of 3674 calls for ambulance help, MI was diagnosed in 379 patients what amounts for 10.1% of all interventions in life-threatening cases and for 61% of patients with acute MI in the analysed period of time. Complicated MI was observed in 61.7% of all patients, including 70.5% of men and 49.3% of women. Arrhythmic complications occurred in 54.5% of patients. Ventricular ectopic activity was the most frequent arrhythmia and amounted to 46.6%. The II0 or III0 atrioventricular block occurred in 4.5% of patients. Haemodynamic complications occurred in 12.3% of cases. Sudden cardiac death occurred in 23.6% of patients with acute MI, including 21.9% cases of ventricular fibrillation or flutter and 1.7% of asystole. 28.5% of those patients were successfully resuscitated. 61% of patients died in the prehospital phase of acute MI including 63.4% of males and 57.2% of females. Out of 61% of patients who died before admission to the hospital, 53% had died before ambulance team arrived and 7.9 died being under the care of the ambulance team. 80% of patients who survived prehospital phase of acute MI were admitted to the hospital within 60 min after the call for the ambulance.(ABSTRACT TRUNCATED AT 250 WORDS)
