OBJECTIVE: To evaluate the strategy of the management of patients with fulminant hepatic failure at Queen Mary Hospital. METHODS: In the period 1994 through 1996, 30 patients with fulminant hepatic failure were managed by active supportive treatment in the Intensive Care Unit in preparation for liver transplantation. Liver transplantation was performed in 14 patients (aged 17-47), 3 of whom received liver grafts from brain-stem dead donors and 11 received live-related grafts from family members. The median duration from admission to liver transplantation was 3 days (range: 1-6 days). RESULTS: Thirteen patients (93%) survived the liver transplantation and are well after a median period of follow-up of 7 months. The only mortality was in a patient with pre-exiting renal transplantation and hepatitis B infection, who died from intra-abdominal candidiasis. CONCLUSION: The strategy of active supportive treatment and early liver transplantation using live-related liver graft is probably the key to the success of the management of fulminant hepatic failure in our series.
OBJECTIVE: To review the research carried out in Hong Kong on nasopharyngeal carcinoma (NPC) over the past 10 years. METHODS: The literature published in Hong Kong over the past 10 years on nasopharyngeal carcinoma were reviewed in the light of other important international publications on the same subject. RESULTS: The research carried out in Hong Kong covers etiology, natural behavior, treatment and prognosis of NPC. These studies not only elucidated the different aspects of NPC, many of these new findings will also guide future directions of research. CONCLUSIONS: Major efforts have been made to research into the various aspects of NPC with important findings which will translate into better treatment results. As this tumor is uncommon in other parts of the world, continued efforts to improve the prevention, early diagnosis, treatment and care of this tumor are required in China.
INDEPTH geophysical and geological observations imply that a partially molten midcrustal layer exists beneath southern Tibet. This partially molten layer has been produced by crustal thickening and behaves as a fluid on the time scale of Himalayan deformation. It is confined on the south by the structurally imbricated Indian crust underlying the Tethyan and High Himalaya and is underlain, apparently, by a stiff Indian mantle lid. The results suggest that during Neogene time the underthrusting Indian crust has acted as a plunger, displacing the molten middle crust to the north while at the same time contributing to this layer by melting and ductile flow. Viewed broadly, the Neogene evolution of the Himalaya is essentially a record of the southward extrusion of the partially molten middle crust underlying southern Tibet.
The crust north of the Himalaya is generally electrically conductive below depths of 10 to 20 km. This conductive zone approaches the surface beneath the Kangmar dome (dipping north) and extends beneath the Zangbo suture. A profile crossing the northern Yadong-Gulu rift shows that the high conductivity region extends outside the rift, and its top within the rift coincides with a bright spot horizon imaged on the INDEPTH CMP (common midpoint) profiles. The high conductivity of the middle crust is atypical of stable continental regions and suggests that there is a regionally interconnected fluid phase in the crust of the region.
The major limitation of adult-to-adult living donor liver transplantation is the adequacy of the size of the graft that can be safely harvested from the donor. The present report describes a 22-year-old woman with stage 4 hepatic coma due to fulminant hepatic failure who was successfully treated using a small-for-size left lobe graft from her father. The graft weight was 0.6% of the recipient's body weight, or 25% of her ideal liver weight. Avoidance of warm ischemia, short cold ischemic time, and early treatment of rejection are important elements in optimizing small-for-size graft function. Since the left lobe represents 23-36% of the total liver volume of an adult, it is possible, in most cases, to harvest a left lobe graft of adequate size from a donor of similar size as the recipient.
Antitrypsin in human serum was determined by using both the SAW-impedance sensor system and spectrophotometry, indicating that the mean value for women was significantly higher than the mean value for men; the value for acute pancreasis patients is about 2-folds of the normal values, and there is no significant difference between the acute pancreasis patients and the pancreatic cancer patients.
The sex difference in the midsagittal area of the adult rat corpus callosum (CC) has been shown to be mediated, in part, by gonadal steroids in early development, with the sensitive period of hormone action in the female extending at least up to postnatal day 25. Given this prolonged sensitivity, the current study attempted to delineate organizational vs. activational influences of gonadal hormones on the female rat CC. In Experiment 1, callosal size was examined across the estrous cycle at 52 and 90 days of age. In Experiment 2, females were ovariectomized at 78 days and CC parameters assessed at 110 days. Last, in Experiment 3, females were ovariectomized at 78 days and sacrificed at 110 days; in addition, sham females were sacrificed during proestrus or estrus. Neither stage of estrous cycle nor adult ovariectomy affected midsagittal CC size. These results provide evidence for organizational effects of ovarian steroids on the female callosum, with the sensitive period of hormone action ending sometime between days 25 and 78.
Two classes of bacterial genes are involved in the elicitation of the plant hypersensitive response (HR) in resistant plants: hrp genes and avr genes. hrp genes have been shown to be involved in the production and secretion of a new class of bacterial virulence/avirulence proteins, including harpin of Erwinia amylovora and harpinPss of Pseudomonas syringae. The ability of avr genes in the elicitation of the HR/resistance is dependent on functional hrp genes. The relationships between harpins and avr gene products are not known. This study investigates the plant genes induced by harpins and the effect of avr genes on the expression of such plant genes. A tobacco gene highly induced by harpins was isolated by a subtractive hybridization method. Induction of hin1 by P.s. pv. syringae 61 (Pss61) was found to be dependent on functional bacterial hrp genes. P. fluorescens (a saprophyte) or hrp mutants defective in the Hrp secretion pathway did not induce hin1 significantly. A hin 1-related gene in tomato cv. Rio Grande-PtoR was found to be rapidly induced by P. s. pv. tomato T1 (a virulent bacterium on Rio Grande-PtoR) containing the avrPto gene, which mediates the elictation of the HR/resistance in a Pto plant resistance gene-dependent manner. The induction of hin1 by bacteria correlates with production of harpins in planta. The putative open reading frame of hin1 encodes a novel protein of 221 amino acids. The data suggest that harpins and the avrPto-mediated signal induce a common plant gene in the elicitation of the HR.

OBJECTIVE: To investigate the changes of urinary fibrinopeptide A (FPA) level in pregnancy induced hypertension (PIH) and its clinical significance. METHODS: High performance liquid chromatography (HPLC) was employed in determination. Urinary FPA was measured in 96 cases of normal pregnancy and 49 cases with PIH. RESULTS: The urinary FPA levels in mild and moderate PIH groups were significantly higher than that in normal pregnancy (71.65 +/- 18.53 micrograms/L vs 40.17 +/- 20.26 micrograms/L, P < 0.01). The levels of urinary FPA in patients with preeclampsia increased significantly (146.65 +/- 32.53 micrograms/L) when compared with the mild and moderate PIH groups (P < 0.001). The levels of urinary FPA in patients with eclampsia (422.93 +/- 81.46 micrograms/L) were much higher than that in group of preeclampsia (P < 0.001). CONCLUSIONS: It showed that the assay of urinary FPA might be helpful for earlier diagnosis, classification, treatment and judgement of prognosis in PIH.
In hematopoiesis; the human CD 34 protein is a strict developmental stage-specific antigen that marks hematopoietic stem/progenitor cells, suggesting that it plays an essential role in hematopoiesis. More recently, it has been demonstrated that hematopoietic cells expressing the CD 34 antigen constitute various heterogeneous cell populations in which each CD 34+ subset was associated with commitment to a particular lineage, and differed from other's in reconstituting hematopoiesis. In this report, we have assessed the different functional subpopulation of CD 34+ hematopoietic stem/progenitor enriched from human bone marrow using Isolex TM 50 system according to the strategy on immunomagnetic separation of positive selection. CD 34+ cell population of high purity (> 90% CD 34+) was analyzed by means of double staining procedure of flurorescein conjugated monoclonal antibodies on the two-color FACAcan or FACS 440. Eight cell subsets of at least of bone marrow CD 34 hematopoietic stem/progenitor cells have been defined by undertaking a comparative coexpressing of CD 71, CD45, CD 33 and HLA-DR antigens on CD 34 hematopoietic stem/progenitor cells. The frequencies of various subsets in two illustrative are as following: 1). CD 34+/CD 71- and CD 34+/CD 71+ (23.43%-56.6% versus 33.4%-66.6%): 2). CD 34+/CD45- and CD 34+/CD45+ (80.8%-82.5% versus 8.1%-11.2%): 3). CD 34+/CD 33- and CD 34+/CD 33+ (20.4%-80.6% versus 14.6%-64.8%): 4). CD 34+/DR- and CD 34+/DR+ (6.3%-11.0% verus 82.8%-85.5%). Immunological double color staining of IGSS-APAAP was also used to further analyse the CD 34+ cell subsets as above-mentioned, the results were very similar to those obtained by FACScan. Our data indicate that CD 34+ hematopoietic cell fraction is far from being a uniform cell population, and many works regarding biological properties and regulation mechanism of different subsets of CD 34+ hematopoietic stem/progenitor cells are being carried out.
A bacterial strain, designated as Jin-1, was isolated from a water sample taken from the Jinyang Lake located in the suburbs of Taiyuan City, Shanxi Province. Its morphologic and tinctorial properties, nutritional requirements, growth characteristics, biochemical reactions, as well as results of DNA and peptide analysis overall met the criteria for identification of Legionella pneumophila. In serogrouping, extensive cross-reactions between Jin-1 and the type strains of L. pneumophila serogroup 1 through 14 were observed in both slide and tube agglutination tests mediated by the IgM antibodies. However, Jin-1 revealed the L. pneumophila serogroup 5 specificity in IFA, ELISA and dot-ELISA, all mediated by the IgG antibodies. Jin-1 was finally identified as a strain of L. pneumophila serogroup 5 with the more complex antigenic composition than the ATCC type strain of this serogroup. The taxonomic conclusion has been confirmed by CDC. A Legionella pneumophila strain which serologically cross-reacting with so many serogroups of this species has never been reported in the published articles. It is presumed that the cross-reacting antigen of Jin-1 belongs to the thymus-independent antigen by nature.
In order to investigate the patterns of cervical lymph node metastases from head and neck SCC, serial sections were performed on 384 radical neck dissection (RND) specimens. Positive lymph node was found in 60.4% RNDs. The cervical lymph node spread from SCC in the head and neck regions including oral cavity, oropharynx, hypopharynx and larynx has some predictable patterns, i.e., for primary SCC of the oral cavity, the majority of cervical lymph node metastases were clustered at levels I, II and III; and for primary carcinoma of the oropharynx, hypopharynx and larynx, a majority of node metastases were located at levels II, III and IV. The positive lymph nodes mainly distributed at only one level or consecutive levels. The rates of pathologically positive lymph node and extranodal spread grew with the increase of the clinical N-staging. It is suggested that supraomohyoid neck dissection (levels I, II and III) is particularly applicable to carcinomas of the oral cavity, and lateral neck dissction (levels II, III and IV) is applicable to carcinomas of the oropharynx, hypopharynx and larynx in patients with limited (N0 and N1) neck nodules, but for patients with N2 and N3 nodules, RND is neccessary to eradicate the nodal metastases. Moreover, the postoperative radiotherapy is indispensable for ruling out the occult cervical lymph node metastaese in selective neck dissection.
Previous reports have shown that bone mass and architecture will partially recover by remobilization (RM) in immobilization (IM)-induced osteopenia. The aim of this study was to test whether PTH can accelerate the recovery during RM from the IM-induced osteopenia. Six-month-old Sprague-Dawley female rats were divided into aging and IM groups. The right hindlimb of rats was immobilized against the abdomen by elastic bandages for 18 weeks, then groups of rats were further IM or RM for 2, 10 and 20 weeks and given 30 or 80 micrograms hPTH (1-38)/kg/d s.c. Secondary spongiosa of proximal tibial metaphyses (PTM) were studied. Immobilization reduced the trabecular area, number and thickness at the first 18 weeks post IM, then plateaued. Ten weeks of RM restored 40% trabecular bone from IM controls due to thickening of the trabeculae. However, the trabecular area was still -14% and -13% lower than that of aging controls at 10 and 20 weeks. Two weeks of 30 micrograms PTH/kg/d in IM rats restored the bone to aging control levels, two weeks of 30 micrograms PTH/kg/d in RM rats and of 80 micrograms PTH/kg/d in both RM and IM rats added extra bone. Extending the treatment to 10 and 20 weeks, the same amount of total bone was added to both IM and RM rats. These findings indicate PTH is a powerful agent that can accelerate the recovery and add extra bone to osteopenic cancellous bone in both IM and RM rats.

AIM: To determine the effects of pineal body and melatonin (Mel) on lymphocyte proliferation and dinoprostone production in rat spleen. METHODS: Pinealectomy (PE), lymphocyte proliferation assay and dinoprostone radioimmunoassay were used. RESULTS: A circadian rhythm of splenic lymphocyte proliferation which peaked at 22:00 was obliterated by PE in rats. PE led to an impairment of lymphocyte proliferation and an increase of dinoprostone production, which were restored by ip Mel 10 micrograms.kg-1.d-1 at 16:00 for 7 d. Mel promoted lymphocyte proliferation and inhibited dinoprostone production in intact rats. A negative correlation between the change in lymphocyte proliferation and dinoprostone production was seen. CONCLUSION: The pineal body and its main hormone Mel play a regulatory role in circadian lymphocyte proliferation which is related to dinoprostone production in rat spleen.
