Neuronal apoptosis induced by pharmacological concentrations of 3-hydroxykynurenine: characterization and protection by dantrolene and Bcl-2 overexpression.

We have studied neurotoxicity induced by pharmacological concentrations of 3-hydroxykynurenine (3-HK), an endogenous toxin implicated in certain neurodegenerative diseases, in cerebellar granule cells, PC12 pheochromocytoma cells, and GT1-7 hypothalamic neurosecretory cells. In all three cell types, the toxicity was induced in a dose-dependent manner by 3-HK at high micromolar concentrations and had features characteristic of apoptosis, including chromatin condensation and internucleosomal DNA cleavage. In cerebellar granule cells, the 3-HK neurotoxicity was unaffected by xanthine oxidase inhibitors but markedly potentiated by superoxide dismutase and its hemelike mimetic, MnTBAP [manganese(III) tetrakis(benzoic acid)porphyrin chloride]. Catalase blocked 3-HK neurotoxicity in the absence and presence of superoxide dismutase or MnTBAP. The formation of H(2)O(2) was demonstrated in PC12 and GT1-7 cells treated with 3-HK, by measuring the increase in the fluorescent product, 2',7'-dichlorofluorescein. In both PC12 and cerebellar granule cells, inhibitors of the neutral amino acid transporter that mediates the uptake of 3-HK failed to block 3-HK toxicity. However, their toxicity was slightly potentiated by the iron chelator, deferoxamine. Taken together, our results suggest that neurotoxicity induced by pharmacological concentrations of 3-HK in these cell types is mediated primarily by H(2)O(2), which is formed most likely by auto-oxidation of 3-HK in extracellular compartments. 3-HK-induced death of PC12 and GT1-7 cells was protected by dantrolene, an inhibitor of calcium release from the endoplasmic reticulum. The protection by dantrolene was associated with a marked increase in the protein level of Bcl-2, a prominent antiapoptotic gene product. Moreover, overexpression of Bcl-2 in GT1-7 cells elicited by gene transfection suppressed 3-HK toxicity. Thus, dantrolene may elicit its neuroprotective effects by mechanisms involving up-regulation of the level and function of Bcl-2 protein.

[Primary repair of cartilage defect accompanying patellar fracture with free auto-periosteal graft].

OBJECTIVE: To evaluate the results of free auto-periosteal graft in primary repair of cartilage defect accompanying severe comminuted fractured of patella. METHODS: From January 1992 to August 1998, seventeen cases with extensive cartilage defect due to severe comminuted fracture of patella were primarily repaired with free auto-periosteal graft. In these cases, there were whole patellar fracture in 9 patients, upper two third patellar fracture in 3 patients and lower two third patellar fracture in 5 patients. During operation, "S"-shaped incision along medial side of knee through intra-cavity pathway were used. After fixation of the patellar fracture and clearance of the residual cartilage in the fracture area, the cancellous bone was exposed and trimmed. The free periosteum was incised from the anterior medial side of upper tibia and then transplanted to the region of cartilage defect. The size of grafted periosteum ranged from 3 cm x 4 cm 5 cm 6 x cm. The knee joint was received passive motion at 7 days after operation. RESULTS: All cases were followed up 8 to 74 months. There were excellent recovery in 12 patients and the function of knee joint was normal, better recovery in 4 patients and the function of knee joint was nearly normal, and moderate recovery in 1 patient and the function of knee joint was limited mildly. CONCLUSION: Free auto-periosteal graft is a simple and effective treatment in primary repair of cartilage defect accompanying patellar fracture. It is valuable to apply in clinical practice.

[Study on relationship of activated protein C resistance and recurrent spontaneous abortion].

OBJECTIVE: To investigate whether activated protein C resistance (APC-R) is associated with recurrent spontaneous abortion. METHODS: APC-R was detected by activated partial thromboplastin time (APTT) in the presence and absence of APC. Subjects included 40 nonpregnant parous women (control group) and 55 patients with recurrent spontaneous abortion. RESULTS: There was significant difference for APC-R between controls and the patients (P < 0.01). Among the study group, the prevalence of APC-R positive rate had no relation to the numbers of abortion. The positive rate of APC-R in patients with late abortion was significantly higher than those with only early abortion (38% and 12% respectively). CONCLUSION: APC-R is partly associated with recurrent spontaneous abortion, especially late abortion. It has no relation to the numbers of abortion.

[Posterior atlantoaxial facet screw fixation in rheumatoid arthritis].

OBJECTIVE: To clarify the characteristics of the clinicopathological factors in cases of atlantoaxial instability due to rheumatoid and to introduce a new method for the treatment of this disease. METHODS: Fifteen patients with atlantoaxial instability due to rheumatoid were treated with posterior facet screws to obtain immediate rigid fixation of C(1 - 2). In this series of patients, screw fixation was augmented with an interspinous C(1 - 2) strut graft which was wired in place to provide three-point stabilization and to facilitate bone fusion. RESULTS: In each patient fixation was satisfactory, and C(1 - 2) alignment and stability were restored without complication due to instrumentation. All patients showed osseous union (a mean follow-up period of 10 months; range 6 - 14 months). CONCLUSION: Posterior atlantoaxial facet screw fixation provides immediate three-dimension rigid fixation of C(1 - 2) that is biomechanically superior to wiring or other fixation.

[Cause and management of dislocated nuclear fragments during phacoemulsification].

OBJECTIVE: To investigate the cause and management of posteriorly dislocated lens nucleus in the vitreous cavity during phacoemulsification. METHODS: The authors performed a retrospective study of 18 patients with dislocation of nuclear fragment into vitreous cavity during phacoemulsification at Tong Ren Hospital from August 1994 to January 1999. RESULTS: 4 cases with dislocated nuclei floated in the anterior vitreous were removed by a lens loop from limbal extensive incision. 2 cases with their dislocated nuclei less than 1/4 of the normal size were under follow-up for 3 - 4 years, and no complications were observed. The final outcome in 1 case with a dislocated nucleus half of its normal size was ocular atrophy as a result of phacoanaphylactic endophthalmitis. 11 cases underwent pars plana vitrectomy. In the operation the nuclei were floated anteriorly by injection of perfluoro-1, 3-dimethylcyclohexane, then they were removed by a lens loop through the limbus and in 2 cases they were fragmented by ultrasound. The main post-operative complication was corneal edema. The final visual acuities generally were improved in varying degrees in 1 month to 4 years of follow-up. CONCLUSIONS: During phacoemulsification, the dislocation of a nucleus is liable to first occur in the peripheral sculpting stage, and secondly in central sculpting stage. A radial tear extending posteriorly from a discontinuous anterior capsulorrhexis is the major risk factor predisposing to posterior dislocation of the nucleus or nuclear fragment. During phacoemulsification, vitrectomy should be performed as soon as possible for a nucleus dislocated into middle or posterior vitreous cavity, and it is a safe and effective method for management of dislocated nucleus.

[The epidemiological study of HIV infection among paid blood donors in one county of China].

OBJECTIVE: To observe the prevalence of HIV infection among paid blood donors and risk factors. METHODS: Selected the residents between 18 to 50 years old from two villages in one county of China in 1997. We observed the prevalence of HIV infection and keep following them to 2000. RESULTS: Nine hundred sixty-three residents were investigated and the prevalence of HIV was 1.8% (17/963). There were 88 paid blood donors among 963 interviewed subjects, the HIV prevalence was significantly higher (17.0% or 15/88) than those who didn't have the experiences of paid blood donations (0.2% or 2/875). Only one person out of 668 who were followed from 1997 to 2000 has converted to sero-positive, the incidence of HIV infection is 0.06 per 100 person-years. Nearly half of sero-negative and sero-positive people have the experiences of muscle injection. Among those with HIV-positive spouse, we found no one converted to sero-positive. The CD(4) count of those sero-positive people declined every year. HIV subtype and C2-V3 region sequence analysis were made for five HIV-positive people in 1998, the result indicated that all of them were infected with HIV-1 subtype B' and the genetic divergence rate was 2.4%. CONCLUSION: There was a high prevalence of HIV among paid blood donors in some regions, and paid blood (plasma) donations were the main risk factor for HIV infection. Although no evidence suggests that during the follow-up period the epidemic of HIV infection has spread locally, it still shall take measurements to prevent further transmission of HIV through paid blood donors.

[Expression and activity analysis of the human glutathione-S-transferase in Lactococcus lactis].

The glutathione-S-transferase A1 cDNA was amplified from human liver total RNAs by RT-PCR and was cloned into a Escherichia coli expression vector pET23b, then the recombinant plasmid pET23bhgst was introduced into E. coli BL21 (DE3) and induced by IPTG, the high-level expression of hGSTA1 appeared in the E. coli cells. The cDNA encoding hGSTA1 was subcloned into pMG36e, a lactococcal expression vector, and introduced into Lactococcus lactis MG1363 by electroporation. In the positive transformants, the hGSTA1 was expressed as a fusion protein which was verified by SDS-PAGE and Western blot. The hGSTA1 from both E. coli and L. lactis was purified by affinity chromatography on glutathione-agarose and all showed enzymatic activity. The potential application of the recombinant Lactococcus lactis in functional food was also discussed.

[Liver sinusoidal endothelial cell damage during normothermic ischemia and reperfusion of rat].

OBJECTIVE: To observe the damage to liver sinusoidal endothelial cells during normothermic ischemia and reperfusion. METHODS: The afferent vessels of rat liver were clamped for 20, 40, 60 and 90 min respectively. The blood flow to the liver then was restored by removing the clamp. Ultrastructural changes in the sinusoidal endothelial cells in various periods were observed. RESULTS: After 20 or 40 min ischemia, fenestrations of the endothelial cells were enlarged and became mesh-like, but reperfusion for 120 min, led to recovery. After 60 or 90 min ischemia, the sieve plates and cytoplasmic processes of the endothelial cells were destroyed and the perikaryon tended to be desquamated. Reperfusion for 120 min led to no recovery, but resulted in the aggravation of injury and irreversible damage. The liver sinusoids were filled with numerous blood cells and blebs derived from hepatocytes, resulting in circulatory disturbance. CONCLUSIONS: Warm ischemia results in severe endothelial cell destruction. Endothelial cell damages may have unfavorable effects on the microcirculation of the reperfusion liver after prolonged ischemia.

Differentiation between senescence (M1) and crisis (M2) in human fibroblast cultures.

Normal human fibroblasts undergo only a limited number of divisions in culture and eventually enter a nonreplicative state designated senescence or mortality stage 1 (M1). Expression of certain viral oncogenes, such as the SV40 large T antigen (SV40 T-Ag), can elicit a significant extension of replicative life span, but these cultures eventually also cease dividing. This proliferative decline has been designated crisis or mortality stage 2 (M2). BrdU incorporation assays are commonly used to distinguish between senescence (<5% labeling index) and crisis (>30% labeling index). It has not been possible, however, to ascertain whether the high labeling index, indicative of ongoing DNA replication, was caused by the presence of T-Ag. We used gene targeting to knock out both copies of the p21(CIP1/WAF1) gene in presenescent human fibroblasts. p21 -/- cells displayed an extended life span but eventually entered a nonproliferative state. In their terminally nonproliferative state both p21 +/+ and p21 -/- cultures were positive for the senescence-associated beta-galactosidase (SA-beta-gal) activity; in contrast, the labeling index of p21 +/+ cells was low (<5%) whereas the labeling index of p21 -/- cells was high (>30%). The observation that p21 -/- and SV40 T-Ag-expressing cells behave identically with respect to life span extension as well as the high labeling index in the terminally nonproliferative state indicates that crisis is not a phenomenon induced solely by viral oncogenes, but a physiological state resulting from the bypass of normal senescence mechanisms. The widely used biomarker for senescence, SA-beta-gal, cannot distinguish between senescence and crisis. We propose that all SA-beta-gal-positive cultures should be further examined for their BrdU labeling index.

Calcium signals in cell lines derived from the cerebral cortex of normal and trisomy 16 mice.

We established two immortalized cell lines from cerebral cortex of normal (CNh) and trisomy 16 (CTb) mouse fetuses, an animal model of human trisomy 21. Those cells loaded with the fluorescent Ca2+ dyes, Indo-1 and Fluo-3, exhibited increments of intracellular Ca2+ ([Ca2+]i) in response to external glutamate, NMDA, AMPA and kainate. CTb cells exhibited higher basal Ca2+ concentrations and had higher amplitude and slower time-dependent kinetics in the decay than CNh cells, suggesting an impaired Ca2+ buffering capacity in the trisomy 16-derived cell line. Nicotine also induced increments of [Ca2+]i. The CTb cell line could represent a model for studying cellular alterations related to Down syndrome.

Update on sequential isologous rat organ transplantation: pancreaticoduodenal and kidney transplantations.

This periodic report includes intermittent results of consecutive pancreaticoduodenal (Pd) and kidney (Kt) transplants in inbred rats and results on double kidney transplants that did not follow sequential transplant protocol. Eight 24-month-old Lewis pancreas, kidney, and aorta served histological controls showing normal histological architecture with no atherosclerosis developed in the aorta. Thirty-four month old pancreas and thirty-two month old kidneys, which resided in young hosts for at least three occasions, appeared as youthful Pd and Kt grafts. They show normal histological appearance for more than the expected life span of a Lewis rat. The fact that not only pancreases but also kidneys outlived their host leads to the study of other different organs' viability as aged valuable grafts. Nevertheless, the threats by the development of atherosclerosis in graft-associated aortas resulted in slow progression of the follow-ups.

Diagnosis of disorders of platelet function by flow cytometry.

The platelet membrane glycoprotein (GP) IIb/IIIa, GPIX were detected by immunogold assay and flow cytometry, respectively, in two cases of Glannzmann's thrombasthenia (GT). The immunogold assay showed that the GP IIb/IIIa gold granules in GT were decreased obviously, compared with that in normal controls. Flow cytometry results showed that GP IIb/IIIa in two cases of GT was 0.1% and 0.5% of normal control, respectively. While GPIX showed no difference between the GT and normal control. The patients' father showed no bleeding symptoms, whose GP IIb/IIIa accounted for 36.99% of normal control. The experiment results suggested that flow cytometry is a quick, simple and sensitive method for the diagnosis of GP disorders.

Effect of SJAMP on ATP release of platelet.

The aggregation and ATP release of placelet of normal subjects were measured by platelet lumi-aggregometer. It was found that the aggregation curve induced by SJAMP at the concentration of 100 mg/L was a typical second phase aggregation. There existed a certain lag between platelet aggregation and secretion. The secretion actually began slightly after the second phase of aggregation, suggesting that the second phase aggregation induced by SJAMP is not dependent upon the release of contents of dense granule alone. If platelets were incubated with cyclo-oxygenase inhibitor, the second phase aggregation was inhibited and no ATP was released. The results indicated that the aggregation and release reaction induced by SJAMP were dependent upon the generation of prostaglandin endoperoxides and TXA2 in normal subjects. The amount of ATP release was 0.69 +/- 0.22 nmol/10(8) platelets as stimulated with SJAMP (100 mg/L). But the amount of ATP release were 1.60 +/- 0.25 and 1.37 +/- 0.15 nmol/10(8) platelets when platelets were stimulated with ADP (5 mumol/L) and collagen (5 mg/L). The amount of ATP release induced by SJAMP was significantly lower than that of ADP and collagen. These findings indicated that SJAMP was a weaker agonist than ADP in terms of platelets release reaction.

Endothelial function and carotid artery wall thickening in patients with early essential hypertension.

To investigate the relationship between endothelial function and carotid artery wall thickening in patients with early mild essential hypertension, the percent dilatation of humerus diameter and intimal-medial thickening (IMT) of common carotid artery was measured by using high-resolution ultrasound in 20 patients with early mild essential hypertension and 18 patients with normotension. The patients with hypertension had not been treated and their history of increased blood pressure was less than 12 months. In essential hypertension group, the percent dilatation of humerus diameter decreased significantly (2.65 +/- 0.98% vs 6.38 +/- 1.61%); IMT of carotid artery increased (0.88 +/- 0.16 mm vs 0.58 +/- 0.08). There was significant negative correlation between IMT and the percent dilatation of humerus diameter (gamma = -0.82, P < 0.05), and no correlation between IMT and 24 h mean systolic, diastolic pressure (gamma = 0.12 and gamma = 0.07, respectively; P > 0.05). Our results suggested that there was endothelial dysfunction in early mild essential hypertension. Endothelial dysfunction may not only contribute to the pathogenesis of hypertension but also serve as the most important inducing factor leading transformation from hypertension to atherosclerosis.

Detection of the level of urinary FPA in chronic nephritis with renal failure and its clinical implication.

The level of urinary FPA was assayed by high performance liquid chromatography (HPLC) in 42 normal controls, 57 cases of chronic glomerulonephritis, including 24 with normal renal function, 12 with renal insufficiency and 21 with uremia. Their levels were 24.40 +/- 10.30 micrograms/L, 26.99 +/- 5.77 micrograms/L, 38.81 +/- 6.28 micrograms/L, 79.74 +/- 18.76 micrograms/L, respectively. The level of urinary FPA in renal insufficiency function group was significantly higher than those of the control group and normal renal function group (P < 0.01). The patients with uremia presented dramatically higher level of urinary FPA than those in the renal insufficiency group (P < 0.01). A positive correlation was found between the level of urinary FPA and the blood creatine (r = 0.9120, P < 0.01). It was suggested that a hypercoagulable state existed in the patients with chronic nephritis with renal failure, in which the severity was closely related with the occurrence and development of the disease. The urinary FPA could serve as a good indicator for renal function.

Isolation of a mutant of Kluyveromyces sp. Y-85 resistant to catabolite repression.

The production of inulinase in Kluyveromyces sp. Y-85 is repressed by the carbon catabolites. To screen for derepression mutants, the strain of Kluyveromyces sp. Y-85 was treated with the mutagenic agent, ethyl methane sulfonate (EMS). Initially, mutants resistant to catabolite repression by 2-deoxy-D-glucose were isolated. One such mutant, Kluyveromyces sp. Y-85 K6, was found to exhibit stable, 2-fold higher expression of inulinase than the wild-type in glucose-containing medium, indicating that it is a promising strain for industrial production.

Free split and segmental latissimus dorsi muscle transfer in one stage for facial reanimation.

The authors report the experience in facial reanimation using free innervated split and segmental latissimus dorsi muscle flap one-stage transfer in 86 patients with longstanding facial palsy. The segmental latissimus dorsi was taken from the distal part of the muscle so that the muscle flap had an ultra-long neurovascular pedicle of 13 to 17.5 cm in length. The muscle flap was made thinner by splitting the segmental muscle. The split segmental muscle flap was transferred to the paralyzed side of the face with its ultra-long neurovascular pedicle passing through a tunnel in the upper lip to the normal side of the face. The neurovascular pedicle of the muscle flap was anastomosed with the facial nerve, artery, and veins, respectively, on the normal side of the face. The operation was designed without the cross-facial nerve graft stage. From 1986 to October of 1997, 86 patients with long-standing facial paralysis were treated in our department. The duration of facial palsy in this series ranged from 1.5 to 51 years. A satisfactory result was obtained in 80 cases, evaluated at 8 months to 2 years postoperatively. The expression movement of the soft tissues of the face can be seen not only over transferred muscle but also on the paralyzed muscle covered by the splitting muscle flap. It is supposed that this is the result of muscle-muscle neurolization. Study of 66 specimens of latissimus dorsi muscle in the cadavers is discussed.
