[Endoscopic polypectomy in the colon, a calculated risk (author's transl)].

5774 colonoscopies were performed in 5 years, --including 2877 total colonoscopies and 1338 polypectomies. This confirms the diagnostic and therapeutic value of this method. Increasing experience of the endoscopist, better techniques as well as adequate pre-and postoperative care contribute to decreasing the risk of the polypectomy. If coagulation current is used exclusively the risk of bleeding can be lowered considerably.

[Angio-immunoblastic lymphadenopathy (author's transl)].

The general clinical and pathological findings of angio-immunoblastic lymphadenopathy are reviewed and illustrated by a case-report with involvement of the tonsils. Our patient showed all the characteristic signs of this disease, including fever, pruritus, rash, generalized lymphadenopathy and hepatosplenomegaly. Histologically the wellknown triad of arborizing postcapillary vessels, proliferation of immunoblasts and plasma-cells, as well as deposition of PAS-positive interstitial material was found. Laboratory findings included a polyclonal hyperglobulinemia and a hemolytic anemia. Treatment consisted of corticosteroids and supportive medications. The prognosis is generally poor, with a median survival of 13 months. At present, the cause is unknown.

[Differential diagnosis of primary and secondary erythrocytosis by means of in vitro culture of hematopoietic stem cells].

Peripheral blood precursor cells from all of 12 patients with polycythaemia vera (PV) formed hemoglobinized colonies in vitro without addition os was strictly epo-dependent in 30 normals and 8 patients with erythrocytosis of other origin. By addition of 1 U epo/ml to the cultures, colony formation was increased up to 5-fold in PV. Untreated patients with a short history had the highest numbers of such epo-responsive precursor cells. In 3 patients with concomitant myelofibrosis, erythroid in vitro growth was abundant, but scattered, single colonies could hardly be identified and their hemoglobinization was poor. This picture did not change with addition of epo, and hemoglobinization did not improve. In vitro grown colonies in PV had morphological abnormalities as compared to colonies grown from normal precursor cells: all stages of erythroid differentiation and up to 50% necrotic cells were found within single colonies. Scattered colonies contained an excess of large, immature, vacuolated erythroblasts. It is concluded that these functionaland morphological abnormalities of in-vitro erythropoiesis are a reliable indicator of PV. Further, transition to myelofibrosis is recognized by a characteristic growth pattern with decreased epo-responsiveness.

Tuberculin and dinitrochlorobenzene (DNCB) tests in cancer patients before and after cytostatic drug therapy.

In 137 patients with different kinds of cancer and different cancer stage, cell-mediated immunity was investigated by DNCB (dinitrochlorobenzene) and tuberculin test. These two skin tests were performed before and after cytostatic drug combination therapy. For a collective of cancer patients we found a positive correlation between skin reactions and prognosis and a negative correlation between skin reactions and cancer stage. After cytostatic drug therapy skin reactions could be significantly stronger. This could be observed in 50% when one test was positive before chemotherapy and in only 20% when both tests were negative before chemotherapy. There existed a significant correlation between an increased reaction after cytostatic drug therapy and objective tumor regression. When skin reactions decreased, tumor progression was seen in all cases. Due to these observations we use skin reactions as a good parameter for therapy results. When delayed cutaneous hypersensitivity impairs 2--3 weeks after chemotherapy, we then change the cytostatic drug combination immediately. We cannot say at this moment, whether an improvement of cytostatic drug therapy can be reached in this way.

Evidence for a 'silent allele' GLO0 at the glyoxalase I locus.

In a three-generation family, the segregation of an apparent silent allele at the GLO I locus in association with the rare HLA haplotype 'AW30-CW4-BW35' was observed in four members. In two cases the assumption of homozygosity at the GLO locus would lead to mother-child exclusions. Phenotypically, the GLO activity in the GLO0 carriers is clearly diminished.

[On fingertips of "washerwoman's hands" (author's transl)].

An investigation method is proposed to find out quantitatively the beginning and the degree of "washerwoman's hands" at the fingertips. To answer the question, for how long there was contact with a fluid medium, systematic investigations are necessary to measure the width of the "cristae papillares" of several test persons with an onlight-photomicroscope under similar fluid conditions.

Adenosine-3':5'-monophosphate-binding proteins from bovine kidney. Isolation by affinity chromatography and limited proteolysis of the regulatory subunit of protein kinase II.

Affinity chromatography on cyclic AMP columns allowed a two-step isolation of the cyclic-AMP-binding proteins from bovine kidney cytosol. An AMP-binding protein (apparent molecular weight approximately 60 000) and large amounts of a low affinity binding protein ('P35'; apparent subunit size approximately 35 000) were obtained in practically pure form besides the high affinity binding proteins of the R type. Among the R proteins the dimer R2 of the regulatory subunit of protein kinase II (apparent subunit size approximately 54 000) represented the bulk material. Small amounts of monomer, of higher aggregates, and of a protein 'P49' (subunit size approximately 49 000) presumably identical with the regulatory subunit of protein kinase I were also detected. The R protein fraction of kidney also contained a high affinity binding protein of smaller size (designated as R'; molecular weight approximately 37 000) which appeared to be derived from protein R2 of protein kinase II by limited proteolysis. At all stages of purification, R protein and its aggregates could be quantitatively transformed into R' protein (or a closely related polypeptide) by several proteases including the relatively unspecific proteinase K. The degradation product exhibited unchanged cyclic-AMP-binding capacities but had largely lost the ability to inhibit the catalytic subunit C of protein kinase, to be phosphorylated by C, and to form a dimer. Preliminary experiments indicate that protein R' may be a natural component of kidney tissue.

Dose-dependent changes in sulfamethazine kinetics in rapid and slow isoniazid acetylators.

The pharmacokinetics of sulfamethazine (SMZ) was studied in 44 healthy volunteers. After an oral dose of 10 mg/kg we found the usual distribution of rapid and slow acetylators. The differences in the t1/2s between the phenotypes were not due to variations in renal clearance. Repeat studies at this dose showed very little intraindividual variation in t1/2s. After a 40 mg/kg dose there was an increase in the t1/2 of SMZ in every subject. The implications of these results on methods of phenotyping are discussed.

Testosterone, LH and FSH in patients with treated testicular carcinomas.

Patients with testicular cancer were treated by semicastratio and in some cases by retroperitoneal surgery. The diminution of testicular parenchyma to about 50% results to a low serum level of testosterone, but elevated serum level of LH and FSH. 3 years after surgery and irradiation this is not a pathological finding but appears to be a typical pattern. As it is difficult to differentiate embryonal carcinomas and choriocarcinomas from pure embryonal carcinomas an estimation of serum level of LH and FSH may be of use by the diagnosis.

Light-induced alterations in cell shape and pigment displacement in chromatophores of the sea urchin Centrostephanus longispinus.

Alteration in cell shape of the ligh-sensitive chromatophores of Centrostephanus longispinus are described. Upon illumination a centrifugal pigment movement starts within extremely thin filopodia which radiate from the cell body. With continued pigment migration the cellular processes increase in length and diameter and give the cell an irregular stellate appearance. Pigment movement within the cellular processes is discontinuous in space and time and may occur independently in single filopodia. The motion of single granules shows characteristic features of a saltatory movement.

[Differentiation of lymphocyte populations in the blood of breast-cancer patients].

The number of T lymphocytes has been determined in the peripheral blood of 90 women (65 patients with breast cancer and 25 healthy controls) using a sheep-erythrocyte rosetting technique and a cytochemical method for intracellular localization of a nonspecific acid esterase. Both tests were in agreement in showing a significant depression of T-cell percentage in a group of patients with progressive disseminated breast cancer under hormonal and chemotherapeutic treatment. After radiotherapy a fall in T cells was observed only with the rosetting method. Possible reasons for this phenomenon are discussed.

[Adjuvant chemotherapy of the colonic and rectal carcinoma: concepts and uptodate results].

The aim of adjuvant chemotherapy is the destruction of micrometastases after surgical removal of a malignant tumor. This treatment modality is gaining in importance in the light of experimental data and lcinical success in pediatric tumors. Results of ongoing studies in colo-rectal cancer show a marginal effect of prophylactic treatment with 5-fluorouracil. The treatment benefits in trials with historical controls are much greater than in studies with simultaneous controls. Use of historical controls is therefore of doubtful value. Ongoing trials use the combination of 5-fluorouracil and methyl-CCNU, which has been shown to double the remission rate in advanced gastrointestinal cancer. Adjuvant chemotherapy of colo-rectal cancer is still experimental and justified only in the framework of clinical trials.

Ultrastructure of the basiepithelial nerve plexus of the sea urchin, Centrostephanus longispinus.

The basiepithelial nerve plexus of the Echinoid, Centrostephanus longispinus, contains strands of nerve fibres, which measure between 0.1 and 2.0 mum -in diameter. Nonciliated epithelial supporting cells do not contribute to the plexus. Within the fibres different cesicular inclusions are found. By means of serial sectioning the structure of multipolar cell bodies within the plexus could be demonstrated.
