[Value of preoperative diagnosis in mesenteric vascular occlusion. A prospective study].

In a prospective study the value of preoperative diagnostics in acute mesenteric vascular occlusion was investigated in 46 patients. Seventeen patients had an arterial thrombosis, 17 a venous thrombosis, 10 had an emboli and 2 patients suffered from a non-occlusive mesenteric infarction. All patients had leucocytosis with an average of 21,000/nl; serum lactate was increased in 91.4% and phosphate was increased in 36.4%. In 82.6% of the cases loops of the small bowel were seen on the X-ray of the abdomen, but in none of the patients an acute mesenteric vascular occlusion was suspected based on the X-ray alone. In 5 of the 46 cases the occlusion could be seen sonographically. The angiography had a sensitivity of 77%.

[Conclusions from field studies of stress on miners caused by dry-hot climate conditions].

The physical strain of miners in hot salt mines based on analyses of heart-rates, deep body temperatures, and sweat-rates during real work was studied. Based on these results changes in the working conditions have been realized (air-conditioning of cabins or ambient air, technological changes, workpause-regimes). Cooling-wastes are not sufficient to reduce the thermal strain in hot-dry climate in normal work of salt-mines, independent of the principle of cooling. During fire-fighters work the physical work is the more dominating stressor in relation to the thermal strain.

[Value of diagnostic localization of insulinoma].

The value of diagnostic localization of insulinoma The value of diagnostic localization statements are contradictory. Basing on our own patient material (n = 41) the preoperative localization of an insulinoma was correct with sonography in 57.7%, with computed tomography in 21.4%, with computed tomography with bolus injection of contrast medium in 73.3%, with angiography 63.9% and with percutaneous transhepatic portal vein catheterisation with selective test of hormones (PTP) in 76.9%. Intraoperative 38 of 41 insulinomas were palpable and twelve of 16 insulinomas were seen during intraoperative sonography. Although we palpate more than 90% of all insulinomas we support a preoperative diagnostic localization for easier intraoperative palpation.

The LDL-receptor-related protein, LRP, is an apolipoprotein E-binding protein.

The low-density-lipoprotein (LDL) receptor is a cell-surface protein that plays an important part in the metabolism of cholesterol by mediating the uptake of LDL from plasma into cells. Although LDL particles bind to the LDL receptor through their apolipoprotein B (apo B) and apolipoprotein E (apo E) moieties, other apo E-containing particles, like chylomicron remnants, are not dependent on the LDL receptor for uptake into cells. Chylomicrons formed in the intestinal mucosa during the absorption of the products of digestion, are processed by the peripheral circulation by lipoprotein lipase, which catalyses the breakdown of triglycerides in chylomicrons to free fatty acids and glycerol. The resulting chylomicron remnants, which are cholesterol-rich lipoproteins, are subsequently taken up in the liver. A second distinct protein that binds to apo E-containing lipoproteins, but not to LDL, has been proposed to be the receptor mediating the clearance of chylomicron remnants from the plasma. This protein has a relative molecular mass (Mr) of 56,000 (56K). More recent studies have failed, however, to establish whether this protein is a cell-surface receptor. Here we describe crosslinking experiments in which apo E liposomes were found to bind specifically to the cell surface of hepG2 cells and to human liver membranes. The size and immunological cross-reactivity of the protein to which the liposomes bound was indistinguishable from that of the recently cloned and sequenced LDL-receptor-related protein, LRP. We therefore conclude that the LRP might function as an apo E receptor.

[The dynamics of injuries caused by thrown, razor-sharp shurikens].

In 10 tests, the linear velocity and rotation frequency of a thrown, pointed, razor-sharp shuriken were investigated using a high-speed camera. The linear velocity (5.1-9.87 m/s) and rotation frequency (0-4.46 m/s) determine the impact (0.91-3.6 nm) of this Asiatic weapon. An impact of 3.6 Nm is sufficient to cause a penetration depth of more than 3.5 cm within the regio ossis temporalis, the regio collis lateralis and the regio intercostalis. Legally, the use of this new generation of pointed, razor-sharp shurikens should be prohibited as "sports" apparatus.

[Prognostically relevant factors in cancer of Vater's ampulla].

In order to evaluate the relevant factors for prognosis for patients with cancer of Vater's papilla our own patient material between 1. 1. 1978 and 31. 12. 1987 was studied retrospectively. The influence of age, sex, duration of anamnesis, tumor size, tumor morphology and tumor stage on the prognosis were analyzed in 32 patients with cancer of Vater's papilla. The 5 year survival rate after Whipple procedure was 56.1%. Statistically significant factors for prognosis were tumor size, lymph node metastases and tumor stage.

Lack of effect of acitretin on the hypothrombinemic action of phenprocoumon in healthy volunteers.

A possible influence of the aromatic retinoid acitretin on the hypothrombinemic action of phenprocoumon was investigated in 10 healthy male volunteers, 22-33 years of age. In a run-in period of 17 days the prothrombin complex activity (Quick) of the subjects was adjusted by individual doses of phenprocoumon (range 1.5-3 mg/day) to a therapeutic level of around 20% (INR = 3.6). Afterwards the dosage regimens were kept unchanged until day 40 when 60 mg oral vitamin K1 were administered. From day 18 to day 28 once daily doses of 50 mg acitretin were applied concurrently with phenprocoumon. The mean (+/- SD) prothrombin complex activity increased slightly and steadily from 19 +/- 1% on day 10 over 22 +/- 1% on day 18 to 24 +/- 4% on day 28 and reached 27 +/- 4% on day 38 (the corresponding INRs were 3.30, 2.91, 2.71 and 2.46 respectively). There were, however, no relevant changes of the Quick values in any subject which could be attributed to acitretin. It can be concluded that, under the present experimental conditions, acitretin did not affect significantly the anticoagulant action of phenprocoumon.

Bioequivalence and pharmacodynamics of a modified glibenclamide formulation in healthy volunteers.

A glibenclamide preparation (Glycolande N) with a modified galenic formulation was compared with a marketed standard preparation for bioequivalence and hypoglycaemic action after single oral administration of 3.5 mg. Twelve healthy male volunteers participated in this open two-way cross-over study. The confidence intervals around the mean values of the standard preparation were all in the range of +/- 20%. No significant differences were found between both formulations for Cmax, Tmax, AUC1 and AUC3. The glucose profiles were virtually equal. Both preparations are regarded as bioequivalent and therapeutically equivalent.

[Prognostically relevant factors in ductal pancreatic cancer].

Prognosis of patients with pancreatic carcinoma is bad. Less than 1% of all patients and even after pancreas resection only 10% survive more than 5 years. To evaluate the factors which influence the prognosis we analyzed retrospectively all patients who were operated on a pancreas cancer (n = 484) from January 1, 1978 to December 31, 1987. Age, sex, perineural invasion and lymphangiosis carcinomatosa had no prognostic influence. The duration of symptoms, degree of cellular differentiation, tumor size, preoperative level of the tumor associated antigen CA-19-9, nodal involvement and stage were statistically significant for prognosis. R1 and R2-resections had no better prognosis than other palliative procedures.

[Opioid analgesia during extracorporeal shockwave lithotripsy of gallstones].

Gallstone lithotripsy is a new and noninvasive therapeutic option for approximately 20% of patients who harbor cholesterol gallstones. Technologically advanced second-generation lithotripters such as the Dornier MPL 9000 device have greatly simplified biliary lithotripsy with a consecutive reduction in anesthetic requirements. Despite these technical improvements, patients still can experience considerable pain and discomfort during biliary ESWL. Due to its relatively predictable pharmacokinetic profile and its short duration of action, alfentanil appeared to be a suitable drug for pain relief during stone fragmentation. In order to analyze the degree and distribution of pain during gallstone lithotripsy and to evaluate pain control by alfentanil, 44 consecutive patients (ASA I-II) with no previous ESWL therapy were studied. Pain intensity and degree of opioid-induced sedation during shock wave application were evaluated according to 5-point verbal scales that ranged from 0 = no pain to 4 = unbearable pain and 0 = patient awake to 4 = patient asleep. All patients were treated while in the prone position and received oxygen at 6 l/min. After stone focusing, an alfentanil infusion at an initial rate of 2.5 micrograms/kg min was started. Single shock waves were released as test shocks after approximately 2 min. If they were well tolerated, stone fragmentation was begun. If not, more alfentanil was allowed to accumulate until continuous treatment was tolerated. Further in- or decreases of the infusion rate were titrated according to patient response. Registered variables included the required alfentanil loading dose, maintenance and total doses, and the applied shock wave energy approximated by multiplication of shock wave number and voltage squared.(ABSTRACT TRUNCATED AT 250 WORDS)
