[Lobar restricted nodular form of pulmonary alveolar proteinosis].

A case report of a 53-year-old man with lobar restricted pulmonary alveolar proteinosis is presented. The way from an accidental X-ray examination until the final histological diagnosis of the specimen taken at operation is followed. Typical diagnostic pitfalls are discussed.

Prospective evaluation of the contraction stress and nonstress tests in the management of post-term pregnancy.

Eight hundred and nineteen patients were evaluated at greater than or equal to 280 days' gestation. All patients underwent nonstress test (NST) and breast stimulation to induce contraction stress test (CST), except where contraindicated. If CST was nonqualifying (less than three contractions per ten minutes), Pitocin (oxytocin) was used to complete the CST if there was a nonreactive NST. Delivery was instituted for any abnormal CST, even with a reactive NST, based on the last test within seven days of delivery. There were 747 reactive NST and 72 nonreactive NST. Breast stimulation for CST was done in 655 instances--315 (48 per cent) had nonqualifying CST and 340 (52 per cent) had qualifying CST. There was an increased incidence of induction in the nonqualifying CST group and abnormal CST group. There were no statistically significant differences in perinatal outcomes in the group with reactive NST, irrespective of the CST result. There were no antepartum fetal deaths.

The heterogeneity of solid human xenotransplant tumours.

Fifteen xenograft tumour lines (4 osteosarcomas and 11 squamous carcinomas) were established on nude mice in numerous passages. Over a period of 4 years, samples were taken and studied systematically in more than 4000 histological slides. Of the tumour lines, 12 (4 osteosarcomas and 8 squamous carcinomas) showed remarkable dedifferentiation, but also redifferentiation towards a higher grade. There was a correlation between the rate of tumour takes in mice and poor prognosis in patients, but none between tumour doubling time and the phases of the cell cycle. Altogether, the results showed a great heterogeneity of human xenotransplants in terms of biological behaviour and morphological changes.

Depression and suicidal ideation in sexually abused children.

Sixty-five sexually abused children were administered the Children's Depression Inventory (CDI) following investigation of their abuse to determine the relationship of victims' depression and suicidal ideation with victims' age, post-investigation residence, nature of the abuse, and mothers' belief of victim and compliance with treatment recommendations. Results indicated that older victims were more likely to describe themselves as being depressed. In addition, older victims and those whose mothers were less compliant were more likely to have CDI scores that were above the 90th percentile reported by Smucker, Craighead, Craighead, and Green (1986). Suicidal ideation was more likely in victims who were older, whose mothers were less compliant, who were not removed from their homes, and who had been molested by a family member.

[Radiotherapy of osseous metastases in breast cancer].

1121 patients were treated for breast cancer over a period of 10 years in the Department of Radiotherapy-Radio-Oncology-of the University of Münster. 959 of these patients were regularly cared for in the tumor outpatient department after the treatment. Of these, 354 patients had, or developed later, distant metastases. Metastases were distributed frequently in the skeleton in 167 cases (47.2%). Radiation therapy was carried out under high-voltage conditions. As a rule, pain-free condition was achieved after 15 to 30 Gy (86.3% of all cases). After 30 to 40 Gy, in cases of extreme osteolyses also after higher focal doses, recalcification was evident in most cases. The average survival time after diagnosis of bone metastases was 11.8 months and 11.2 months without therapy.

[Experimental development of carcinoma in rats caused by N-nitrosopiperidine with vitamin C].

In animal experiments with Wistar rats we attempted to determine if oral application of Vitamin C inhibits or delays carcinogenesis due to oral administered N-Nitrosopiperidine (NTP). One group of animals received NTP only, two other groups NTP and Vitamin C either alternating or simultaneously. A fourth group receiving only Vitamin C served as control. Carcinomas of the esophagus, stomach and liver developed in response to NTP application. The additional administration of Vitamin C neither inhibited nor delayed the development of carcinomas.

Intrahospital transport of critically ill patients.

Severe complications sometimes occur in critically ill patients during intrahospital transport. Possible causes may be inadequate ventilation, insufficient monitoring, interrupted application of vasoactive drugs, or disconnections and accidental extubation. We constructed a transport unit equipped with a respirator; capnometer; monitor to measure ECG, arterial and intracranial pressures, and temperature; and two syringe pumps that can be connected easily to the patient's bed. Gas is supplied by cylinders with oxygen and air. Electrical power is supplied by two accumulators connected to recharger and transformer devices that deliver 220 V (110 V). Since this transfer unit was introduced, we have had no unanticipated problems during intrahospital ICU patient transport.

Risk of recurrent stricture following internal urethrotomy. Prospective ultrasound study of distal male urethra.

A group of 60 patients was examined prior to surgery in a prospective ultrasound study of the distal male urethra to predict the individual rate of stricture recurrence after internal urethrotomy. The patients were divided into 2 groups according to the ultrasound findings. One group had no evidence of scarred tissue and no recurrence was predicted. The other group did have periurethral scarring and stricture recurrence within approximately 6 months was predicted. We present the ultrasound findings and discuss the accuracy of the predictions. Correct prognoses were made in 92.9% of cases.

Genomic variation of Trypanosoma cruzi: involvement of multicopy genes.

By using improved pulsed field gel conditions, the karyotypes of several strains of the protozoan parasite Trypanosoma cruzi were analyzed and compared with those of Leishmania major and two other members of the genus Trypanosoma. There was no difference in chromosome migration patterns between different life cycle stages of the T. cruzi strains analyzed. However, the sizes and numbers of chromosomal bands varied considerably among T. cruzi strains. This karyotype variation among T. cruzi strains was analyzed further at the chromosomal level by using multicopy genes as probes in Southern hybridizations. The chromosomal location of the genes encoding alpha- and beta-tubulin, ubiquitin, rRNA, spliced leader RNA, and an 85-kilodalton protein remained stable during developmental conversion of the parasite. The sizes and numbers of chromosomes containing these sequences varied among the different strains analyzed, implying multiple rearrangements of these genes during evolution of the parasites. During continuous in vitro cultivation of T. cruzi Y, the chromosomal location of the spliced leader gene shifted spontaneously. The spliced leader gene encodes a 35-nucleotide RNA that is spliced in trans from a 105-nucleotide donor RNA onto all mRNAs in T. cruzi. The spliced leader sequences changed in their physical location in both the cloned and uncloned Y strains. Associated with the complex changes was an increase in the infectivity of the rearranged variant for tissue culture cells. Our results indicate that the spliced leader gene clusters in T. cruzi undergo high-frequency genomic rearrangements.

[Comparative experimental animal studies for testing the usefulness of formalin- and glutaraldehyde-pretreated umbilical cord veins as artery replacements].

Formalin-fixed (F) and glutaraldehyde-pretreated (G) umbilical veins (n = 24) were interposed in the infra-renal aorta of dogs. The follow-up period was 1 year. Early complications with significant differences included 2 occlusions and 3 deaths (41.6%) in group F, 1 occlusion and 1 death (16.6%) in group G. Ten animals of group G and 7 of group F were alive at 1 year, 7 transplants of each of both groups were angiographically and macroscopically patent (25%). Four aneurysms (33.3%) existed angiographically and showed good collateral circulation angiographically (25%). Four aneurysms (33.3%) existed angiographically and macroscopically in group F (statistically significant). Histologically, group G was found to have a delayed resorption and an earlier beginning of connective-tissue proliferation as compared to group F. Considering early and late findings, G-pretreatment is preferable to F-treatment of umbilical veins.

[New results with delayed progression in the cell cycle of solid squamous cell carcinomas of the head-neck area after single and fractionated irradiation with cobalt-60 and neutrons].

Cell-kinetic changes of xenograft tumors have been investigated after radiation therapy at different times. The growth-locking lasted clearly longer compared with the one of cell cultures. Against this was there were no cell-kinetic changes between centrum and borders of the xenograft tumors. Adding hypoxic sensitizers a further prolongation of the G2M block could be attained. Fractionated irradiation did not adduce qualitative or quantitative changes of the cell kinetics. There was also no difference between low LET and high LET radiation.

Characterization of extracellular matrix proteoglycan transcripts expressed by vascular smooth muscle cells.

Northern blot analysis using intraspecies and cross-species cDNA probes encoding a variety of proteoglycan (PG) core proteins was employed to examine in vitro and in vivo vascular smooth muscle cell (VSMC) extracellular matrix (ECM) PG gene expression. Similar studies were performed using a cDNA probe encoding the rat chondrosarcoma link protein. VSMCs maintained in vitro as well as in vivo expressed a 1.8 kb transcript homologous to a cDNA encoding the bovine bone small PGII. Densitometric analysis revealed an 11-fold increase in the in vivo level of this transcript when compared to an equivalent amount of RNA extracted from rat VSMCs maintained in vitro. Rat aorta medial tissue contained approximately a 10-fold higher level of the bovine bone PGII homologous transcript when compared to rat chondrosarcoma tissue. A 2.9 kb transcript homologous to a cDNA encoding the human bone small PGI (biglycan) was detected in poly(A)+ RNA isolated from rat VSMCs maintained in vitro as well as within rat thoracic aorta medial tissue. Densitometric analysis revealed a 0.55-fold increase in the in vitro level of the PGI transcript when compared to in vivo levels. Rat chondrosarcoma expressed approximately a 3-fold higher level of a human bone PGI homologous transcript when compared to rat aorta medial tissue. A 7.3 kb and an 8.4 kb transcript homologous to a cDNA encoding the rat chondrosarcoma large hyaluronic acid (HA) binding chondroitin-sulfate (CS) PG core protein was detected in poly(A)+ RNA extracted from rat VSMCs maintained in vitro.(ABSTRACT TRUNCATED AT 250 WORDS)
