[Bleeding following tracheostomy: the brachiocephalic trunc].

Life threatening haemorrhage following lesions of the great supraaortic vessels has to be expected after tracheostomy in a ratio of 1:150. 0.6 to 1.1% of these lesions involve the brachiocephalic trunc. In most cases the acute haemorrhage can be controlled by hyperinflation of the cuff of the endotracheal tube. The following immediate emergency operation consists of ligature or resection of the brachiocephalic trunc. No cerebral damage is to be expected afterwards. The indication for vascular reconstruction of the trunc should be considered as a secondary matter.

[Thrombendarterectomy of the femoro--popliteal region for chronic occlusion (author's transl)].

218 patients who underwent subintimal dissection were followed up over a period of 21/2 years. The results (72,6% patients at the end of 21/2 years) come up to those of the saphenous vein bypass. the rate of secondary amputations (7.21%) is low, the primary mortality rate comes up to 2.75%. In our opinion thrombendarterectomy has satisfactory late results. In the author's opinion chronic arterial occlusion of the femoro-popliteal region should surgically be treated first of all by thrombendarterectomy. This enables the surgeon to perform an autogenous venous bypass procedure in case of relapse.

[Does oral contraception cause deep venous thromboses? (author's transl)].

From 1975 to 1980 48 women suffering from deep venous thrombosis were seen at the Surgical Dept. of the Medizinische Akademie Magdeburg. In 22 cases no objective cause could be found, 16 women, however, had been put under oral contraceptives for many months and even for years. Although there is no causal evidence between oral contraception and deep venous thrombosis until now, oral contraception has to be considered as a risk factor concerning the formation of deep venous thrombi in women.

[Should thrombendarterectomy of the superficial femoral artery still be practiced? (author's transl)].

112 subintimal dissections are follwed up over a period of 3 years. The results (67,6% patent at the end of the third year) come up to those of the saphenous vein bypass. The rate of secondary amputations (8%) is low, the mortality rate comes up to 3,6%. In our opinion thrombendarterectomy has satisfactory late results. There is no reason to abstain from it in favour of the vein bypass procedure.
