Fluperlapine vs haloperidol: a comparison of their neuroendocrinological profiles and the influence on serum lipids.

Twelve male patients suffering from acute paranoid schizophrenia, randomly receiving fluperlapine - a new dibenzodiazepine very similar to clozapine - or haloperidol were included in a double blind study. Plasma levels of triglyceride, cholesterol and HDL-cholesterol, prolactin, thyroxine, triiodothyronine, ACTH and cortisol were determined. There was no evidence, that fluperlapine has any relevant influence on those hypothalamic-pituitary axis mediated hormones with the possible exception of plasma lipids. These findings, together with the clinical experiences, suggest that fluperlapine might have a different mode of action than conventional neuroleptics.

Piracetam in alcoholic organic mental disorder: a placebo controlled study comparing two dosages.

The following paper presents an investigation of the efficacy of piracetam in alcoholic organic mental disorder. A double-blind placebo-controlled study design was used to compare 2 dosages of the substance (2 X 3 g vs. 2 X 12 g). Cognitive function was assessed on days 0, 7, 14, 28, 42. Analysis of the results from 24 patients showed a clear-cut amelioration of cognitive functions in all 3 treatment groups. No difference could be demonstrated between the administration of placebo and the lower dose of piracetam. Patients on the higher dose showed an earlier improvement on one of the tests, but the final scores were similar in all three treatment groups.

Epidemiology of benzodiazepine dependence.

The charts of 10,861 patients of the Innsbruck University Department of Psychiatry were evaluated for this study. The patients were in- and outpatients admitted between January 1978 and September 1981. WHO criteria were used for the diagnosis of dependence. 1,743 inpatients and 2,130 outpatients were dependent on psychotropic substances (most of them being alcoholics). 57 (1.1%) resp. 73 (1.3%) suffered from polydrug abuse including benzodiazepines. Only nine inpatients and 21 outpatients were addicted to benzodiazepines only. Details of benzodiazepine dependence are discussed.

[Bromocriptin in organic depression (author's transl)].

10 patients suffering from endogenomorphic depression on the basic of organic brain lesion were treated with 40 mg bromocriptin daily. Therapeutical efficacy was evaluated by various psychometric methods and by clinical impression. A negative correlation between performance and the depressive syndrome could be shown. Amelioration proved to be short lived, only 2 patients showed satisfactory response over a longer period of time. nausea and vomiting were the most common side effects, one patient showed a fully reversible manic syndrome. Reasons for the application of bromocriptin on the basis of a "dopamine-deficit hypothesis", and the results of the study are discussed.

Time after bone marrow transplantation as an important variable for quality of life: results of a cross-sectional investigation using two different instruments for quality-of-life assessment.

Quality of life (QoL) was investigated in 56 BMT recipients. The objective was to compare QoL in terms of physical, emotional, and social functioning between patients within the first year after BMT (n = 15) and patients who were alive more than 1 year after BMT (n=41). The Functional Assessment of Cancer Therapy Scale (FACT-BMT) and the EORTC-Quality of Life Questionnaire (EORTC-QLQ C30) were used to evaluate QoL as perceived by the patients. Results show a significantly reduced general QoL in patients within the first year after BMT. Specific differences were identified on the dimensions of physical and emotional well-being and the symptom scales of appetite loss, fatigue, pain, dyspnea, and nausea and vomiting. QoL improves significantly with time after BMT. We suggest that there should be more integration of QoL expectancy into the pre-BMT information process. Patients should be informed about potential deficits in physical and emotional well-being within the first year after BMT. This could enhance insight and compliance in the critical period early after BMT.

Localized proton and phosphorus magnetic resonance spectroscopy following electroconvulsive therapy.

Animal studies show that cerebral lactate increases after electrically induced seizures. We investigated three adult psychiatric patients by means of localized proton and phosphorous magnetic resonance spectroscopy in order to evaluate if such effects can be observed after electroconvulsive therapy (ECT). None of the patients had changes in cerebral energy metabolism following ECT. Within the limitations of in-vivo spectroscopy in a clinical setting, our results suggest that if lactate production increases after ECT, this effect is either very short or increased perfusion causes an efficient efflux of cerebral lactate.

Mood-altering effects of biperiden in healthy volunteers.

There has been considerable evidence that, aside from various other effects, synthetic anticholinergics also possess a significant euphoriant property. A double-blind placebo-controlled study was conducted in an attempt to answer the following questions: Can euphoria be induced by experimental intravenous administration of biperiden in therapeutic dosages? Is the individual's psychic response to anticholinergics associated with particular personality traits? In a study with a double-blind placebo cross-over design 28 healthy volunteers were given biperiden intravenously. The evaluation of their reaction to the drug using the Bech-Rafaelsen Mania Scale (MRS) revealed two groups of about the same numerical size demonstrating a considerable difference in their reaction to the drug.

Is schizophrenia linked to alteration in cellular immunity?

To reinvestigate the viral hypothesis of schizophrenia as well as possible immunological dysfunction, neopterin, which is an indicator of the activity in the cellular immune system, was determined in acute and chronic schizophrenics. Both diagnostic groups showed neopterin levels within the normal range. Patients with chronic schizophrenia of the residual type presented a significant dependency between neopterin concentrations and Brief Psychiatric Rating Scale (BPRS) total scores. In a long-term observation of 14 acute schizophrenic (paranoid type) inpatients, neopterin levels were found to be lowest at day 0. During the first week of treatment neopterin concentrations increased significantly whereas a concomitant decrease of the psychopathological symptoms could be observed. In comparison to healthy controls patients showed significantly lower neopterin levels at baseline. These findings will be discussed referring to the viral hypothesis of schizophrenia as well as to possible immunological alterations caused by stress or by changes in neurotransmitter synthesis.

Quality of life in breast cancer patients--not enough attention for long-term survivors?

In a study of breast cancer survivors, the authors used the European Organization for Research and Treatment of Cancer core questionnaire + breast module (EORTC QLQ-C30/+BR23) and the Functional Assessment of Cancer Therapy-Breast (FACT-B)for the assessment of quality of life (QoL). The main focus of this study was to look at the effect of time elapsed since initial treatment on the patients' QoL. Eighty-seven female patients (average age of 53.9 +/- 8.7 years) were included in the study. All women had received curative cancer therapy. The average time elapsed since start of initial treatment was 4.7 +/- 4.3 years. Reduced QoL, especially in the areas of emotional, social, and sexual functioning, was found not only after initial treatment (1-2 years) but also after long posttreatment survival (> 5 years). From these findings, needs for specific psycho-oncological interventions are derived. The findings imply that besides recovering from the acute consequences of cancer therapy, long-term survivors of breast cancer (> 5 years after initial treatment) still may have a special need for psycho-oncological support.
