Computed tomography of the pelvis.

The computed tomography scan has revealed the location and size of abnormal pelvic masses as an adjunct to other diagnostic studies. We have extended its usefulness by air distension of the bladder, which accommodates to adjacent structures and, therby, highlights abnormalities in its own wall and in other organs. The patients are prepared according to a simple routine. As we attempt further economy and efficiency the cost of each study becomes justified by the information gained.

Immunostimulation by cryosurgery: an orthotopic model of prostate and bladder cancer in the rat.

In patients with prostate cancer, cryosurgery has been highly effective in eradication of the primary tumor. Certain clinical observations have suggested the presence of an additional systemic anticancer effect, presumably immunologic in nature, but proof is lacking. To study this problem further, we developed two animal tumor models. Instead of using the classical subcutaneous site, we transplanted the 11095A and R-3327 prostate tumors into rat prostate, the 4909 bladder tumor into rat bladder. In these respective orthotopic locations, the 3 tumors displayed favorable growth characteristics with good accessibility for cryosurgery. This model should be considered for studies involving any localized treatment of an established urologic tumor.

Rat prostate carcinoma 11095-A: profile of organ- and tumor-specific antigens.

The 11095-A squamous cell tumor is a transplantable prostate carcinoma of the F344 rat. No tumor-specific antigens could be demonstrated for this carcinoma by serologic tests or in host protection experiments. Preliminary data from two other assay systems indicate that tumor-specific antigens may be weakly expressed in this model.

R3327 adenocarcinoma of the Copenhagen rat as a model for the study of the immunologic aspects of prostate cancer.

The Dunning R3327 prostate adenocarcinoma of the Copenhagen rat was developed as a suitable model of human prostate cancer. Inoculation of tumor tissue mince or cells sc in the flanks of recipient rats produced tumors that had the macroscopic and microscopic characteristics of the human disease. The histologic picture of these tumors was that of a well-differentiated adenocarcinoma with the formation of glands and acid secretions within the acini. Tumors were also produced in the dorsolateral lobe of the prostate by the injection of cells. The intraprostate tumor, although initially confined to the injected lobe, grew to involve the surrounding tissues and eventually metastasized to the lymph nodes and lungs. Occasional metastatic lesions were found in other organs also. Acid phosphatase could be domonstrated by histochemical staining of frozen tumor sections, and elevated levels of the enzyme were seen in the sera of rats bearing long-term subcutaneous tumors. During investigation of the tumor, a fast-growing tumor line arose that grew equally as well in females as in males. The histology of this tumor was that of an undifferentiated anaplastic tumor.

Liver disease in renal transplant recipients.

Significant liver disease developed in 14 patients after renal transplantation. Nine patients had morphologic and functional evidence of chronic active hepatitis. In general, these patients had few symptoms of liver disease, even though the course of chronic active hepatitis was progressive. Despite large doses of prednisone, cirrhosis ultimately developed in five patients. The cause of chronic active hepatitis could not be related to azathioprine or methyldopa therapy because there was no perceptible change in the course of liver disease after treatment with these drugs was stopped. Three patients were persistently positive for hepatitis B surface antigen. Isolated instances of granulomatous hepatitis (Mycobacterium kansasii) and of prolonged intrahepatic cholestasis were encountered in patients with chronic active hepatitis. Two patients had acute cytomegalovirus hepatitis. There was one episode each of fulminant herpes simplex hepatitis and severe fatty metamorphosis.

Serum immunoglobulins in genitourinary malignancies.

Serum concentrations of immunoglobulins G, A and M (IgG, Ig A and IgM) were measured by immunodiffusion in a control population of 58 patients and in 165 patients with cancer of the prostate, bladder, kidney or testis. Tumor patients were ranked according to clinical staging. In a smaller group of patients serum levels of IgE were measured by radioimmunoassay. IgG, IgA and IgM concentrations did not correlate with staging or the presence of malignancy. However, a statistically significant correlation between increased IgE levels and advanced bladder cancer (staged D1 and D2) stimulated us to study more patients with bladder tumors. In a subsequent group of 22 bladder tumor patients this trend could not be reproduced. IgE levels did not correlate with a history of allergy in any of the patients tested. Concentrations of immunoglobulins G, A, M and E seem to be unchanged in genitourinary malignancies, although their functional role here has not been assessed qualitatively.

Complete duplication of the urethra with vaginal stenosis.

Complete urethral duplication, clitoral hypertrophy and vaginal urethra in a girl with normal adrenal function are described. These anomalies caused severe micturitional disturbance by several mechanisms. Reported cases of complete duplication form a continuous spectrum of abnormalities and were classified according to anatomical pattern.
