[Early onset irritant skin damage in apprentice hair dressers].

In the course of a cohort study addressing the importance of constitutional and occupational risk factors for the development of irritant hand eczema, 859 hairdressing apprentices were examined in 14 vocational training schools in autumn 1993, after an average of 8 weeks of professional exposure. In this cross-sectional survey at the start of training, 38.2% were suffering from skin damage, in most cases mild to moderate and only rarely severe; we observed almost exclusively irritant skin damage, mostly localized interdigitally. Logistic regression analysis revealed previous hand eczema, an elevated ¿atopy score¿ (Diepgen) and, particularly, unprotected work for several hours daily with wet hands as significant risk factors for this early onset irritant damage/eczema. The high wet workload is mainly caused by very frequent shampooing and by permanent waving without wearing gloves; however, other tasks are also often performed without appropriate protective gloves. The implications of the considerable inter-observer variability are described and discussed.

Patch test results with serial dilutions of nickel sulfate (with and without detergent), palladium chloride, and nickel and palladium metal plates.

Clinical experience suggests the existence of different degrees of sensitivity in nickel-allergic patients. For quantification of this phenomenon, 462 consecutive patients with previously diagnosed or strongly suspected nickel allergy were tested with serial dilution patch tests with 5 ppm to 5% nickel sulfate in pet. (Ni), and 5 ppm to 1% nickel sulfate in pet. with 1% detergent (Ni/D). Additionally, nickel and palladium metal plates were tested in 103, and cobalt salts, dichromate and palladium chloride (PdCl2) in most patients. 332 patients reacted positively to Ni or Ni/D. The influence of a concomitantly administered detergent was not significant. A significant correlation was found between positive reactions to low concentrations of Ni (or Ni/D), i.e., 0.1% or less (N = 166), and concomitant reactions to nickel metal plates, cobalt salts and PdCl2 and a history of ear piercing with metal intolerance. The clinical relevance of reactions to PdCl2 is at present not clear. A subgroup of nickel-allergic patients with "high sensitivity" can be defined. In future studies further addressing the clinical relevance of high versus low sensitivity, patch testing with 0.01, 0.1, 1.0 and 5% nickel sulfate in pet. is recommended instead of routine tests with 5% only.

[Chilblain lupus erythematosus].

Chilblain lupus erythematosus is a chronic form of lupus erythematosus that is seen predominantly in women and first manifests itself during cold weather as painful purplish acral swellings. These symptoms and possible progression to systemic lupus erythematosus are discussed with regard to two cases. Patients with recurrent or persistent "chilblains" should be investigated histologically and serologically as well as by immunofluorescence.

[Animal experiment model of keratomycosis].

A model of experimental keratomycosis in the rabbit eye is described. Candida albicans strain DSM 70010 (10 microliters; 2.5 X 10(5) cells) is injected intracorneally without employing immunosuppressive measures. This strain is characterized by marked germ-tube formation, which apparently is a major cause of its virulence. All 17 eyes developed an infiltration of the cornea two days after injection. On Day 6 (mean value; standard error +/- 2.34 days) this infiltration developed synchronously to a severe corneal ulcer with hypopyon. The infection remained active for about two weeks and was either complicated by a descemetocele or perforation or led to a reparatory stage with extensive vascularization and successive leukoma. The model presented is reproducible for the first time and is therefore recommended for experimental evaluation of new therapeutic concepts.

[Experimental studies of local therapy of Candida keratomycosis with amphotericin B].

The efficiency of Amphotericin B drops was studied using a newly developed keratomycosis model (defined strain Candida albicans DSM 70010, which leads reproducibly to a corneal infection with descemetocele without prior local or systemic immunosuppression in the rabbit). Penetration of the drug (administered ten times a day) into the cornea and aqueous humor was only demonstrated after abrasion of the corneal epithelium. Three groups were studied: (I) therapy with abrasion, (II) therapy without abrasion, and (III) a control group. Both clinically (descemetocele or perforation, hypopyon) and with regard to microbiology (reculture of Candida) the results obtained in Group I were significantly better than those obtained in Group II (p less than 0.001). Repeated corneal abrasion is therefore recommended for treatment of Candida keratitis with Amphotericin B.

Clotrimazole and bifonazole in the topical treatment of Candida keratitis in rabbits.

Using a reproducible model of Candida albicans keratitis in rabbits we studied the effect of topical clotrimazole and bifonazole. Candida albicans DSM 70010 (2.5 X 10(5) cells) was injected into the corneal stroma of both eyes of 28 rabbits. All eyes developed a corneal ulcer. Fourty-eight hours after inoculation the animals were divided into four groups: I (14 eyes) receiving 10 X clotrimazole 1% drops and subsequently removing the epithelium; II (14 eyes) receiving only clotrimazole drops; III (8 eyes) receiving 6 x bifonazole 1% drops and IV (19 eyes) serving as control (0.9% NaCl castor oil, untreated), 6 eyes of this group were also debrided. A further 6 rabbits were used respectively to judge if the drugs penetrated into the cornea and aqueous humor. There was a significant difference between the clotrimazole group with debridement (I) and the bifonazole group (IV) concerning hypopyon and complications (descemetocele, corneal perforation). Clotrimazole penetrated into the cornea and after debridement into the aqueous humor. Bifonazole could not be identified in the cornea or aqueous humor.

[Allergic reactions to glucocorticoids].

The possibility of anaphylactoid or contact-allergic reactions to corticosteroids should always be considered when the underlying disease deteriorates or specific signs of allergy or pseudoallergy appear during treatment with corticosteroids. Appropriate diagnostic procedures should be extensive enough to exclude hypersensitivity to other constituents of the commercial preparation used as well as multiple or cross-allergies to other corticosteroids.

Epidemiology of contact dermatitis. The information network of departments of dermatology (IVDK) in Germany.

In 1988, the multicenter project "Information Network of Departments of Dermatology (IVDK)", including eight university skin hospitals was founded. It now comprises 30 participating centers. At these centers, all patch test results are recorded, together with important anamnestic details and, twice a year, these are sent to the data center at the Göttingen University Skin Hospital. Regular, descriptive analyses of data collected by this epidemiological surveillance system serve the different purposes discussed in this paper: 1) observation of "sentinel events"; e.g. an increase in the proportion of patients tested who were sensitized to a certain allergen, or the identification of certain subgroups at particular risk as a starting point for further research or preventive action, and 2) advancement of good clinical practice in patch testing. In October 1996, a working group of the European Society of Contact Dermatitis (ESCD) called the "European Surveillance System on Contact Allergies (ESSCA)" was established with a similar aim of communal data collection and analysis.
