[Urinary retention without subvesical obstruction. Differential diagnostic considerations].

Acute urinary retention without subvesical obstruction is associated with neurogenic or nonneurogenic disorders of bladder function. Urodynamic investigations differentiates sensoric and/or motoric disturbances of the micturation. Neurologic and laboratory examinations are necessary to clear the etiology. Symptomatic urological treatment and specific etiological therapy should be used to manage this entity.

[Testicular and epididymal metastasis of prostate cancer].

Metastatic testicular carcinoma is unusual. A case of carcinoma of the prostate with metastases to the testis and epididymis is reported. Routes of metastatic spread of neoplasm to the testis are discussed. Easily removable cancer may remain in the scrotum when using the socalled subcapsular orchiectomy without complete removal of the scrotal contents.

[Selective embolization of the internal iliac artery in life-threatening hemorrhaging prostatic cancer].

The catheter embolization of the a. iliaca interna is a quick, little straining and effective method in order to control bleedings threatening life in the region of the organs of the small pelvis. It is superior to surgical ligature because of the better result and the less straining intervention. With the illustration of a case report the method will be demonstrated again and indications and complications will be discussed.

[The risk of cancer in neuropathic bladders].

Neuropathic bladder is on number six in a list of predisposing factors for bladder cancer. The incidence is reported to be between 0.25 and 9.7%. For these patients mortality is about twenty times higher than in patients with normal bladder function. Chronic infection of the urinary tract and long-term indwelling catheter increase the risk of cancer. Pure squamous cell and squamous cell carcinoma with elements of transitional cell carcinoma is noticed to be surprisingly frequent. In comparison, carcinoma in defunctionalized bladder is rare. For prophylaxis, instead of using a long-term catheter, intermittent catheterization is supposed to be applied as well as consequent cystoscopic examination once a year and 'mapping' of a neuropathic bladder which exists longer than 10 years.

[Therapy of stress incontinence recurrence: comparison of fascia sling-plasty and Zoedler band in 117 patients].

We report on 117 patients with urinary stress incontinence, 44 with previous attempts at surgical correction. The inguinovaginal fascial sling operation as modified by Narik and Palmrich was used in 33 women and had an early success rate of 91%. The Zoedler method used in 84 women was successful in 87%. 52% of women with fascial sling procedure (mean duration of follow-up 1.1 year) and 45% with Zoedler method (mean duration of follow-up 5.7 years) stayed completely continent. The most common reasons for failure are discussed.

[Penile revascularization surgery in vascular impotence].

The application of objective diagnostic methods to patients complaining of erectile dysfunctions has proved that 30 to 40% are based on pathologic changes of the penile vascular system. The most important diagnostic procedures (NPT, Doppler, BCR-latency time, dynamic cavernosography with simultaneous passive erection, penile angiography) are emphasized and possible pitfalls with reference to misinterpretation of the diagnostic findings are discussed. Technique and results of current revascularization procedures (epigastricopenile anastomosis, revascularization of cavernous and dorsal arteries using vein grafts, deep dorsal vein arterialization) are discussed and first own experiences with aforementioned techniques presented.

A microtechnique for the rapid analysis of macromolecule synthesis in minicultures of mammalian cells.

A new micromethod, called the Stanzen technique, is described for the rapid determination of DNA and protein content as well as the incorporation rates of radioactively labeled precursors into macromolecules in cells growing in replica minicultures on plastic petri dishes. The procedure yielded reproducible results assaying only minimal cell numbers per sample and was applied for studying both primary or early passaged cell cultures (mouse epidermal cells and fibroblasts) and a malignantly transformed epidermal cell line. In four well defined circular areas (called Stanzen) marked on the bottom of tissue-culture plastic petri dishes (by heated stamps), 0.2 to 4 x 10(5) cells per area were plated and grown as four individual cultures in one dish. Both treatment and labeling with radioactive precursors of these Stanzen cultures were performed as with normal petri dishes. After fixation and extraction of the cultures, the singular Stanzen areas (with the cells fixed onto them) were sawed out and transferred into vials for liquid-scintillation counting or determination of DNA and protein. The obtained values of specific activity corresponded well whether the samples compared were derived from the minicultures of the same dish or from several dishes. By modifications of the known colorimetric methods for DNA and protein determination, the sensitivity of these procedures was improved down to values of 1 microgram DNA or 5 micrograms protein per individual culture. These micromodifications yielded the same values as the standard methods whether applied to cell suspensions or to cell cultures. Finally, cell proliferation was not influenced by the growth conditions in the small Stanzen areas and proceeded as in normal dishes or larger areas similarly stamped on the bottom of petri dishes. Since this method proved valuable for biochemical studies using primary cultures of mouse epidermal cells (saving cell material by a factor of 10, test substances and time), it might also be advantageous for other purposes as well where the availability of cells or test substances are limiting factors for large test series.

Effects of promoters on DNA synthesis in C3H/10T1/2 mouse fibroblasts.

The synthesis of DNA has been studied by autoradiography and by measurements of tritiated thymidine ([3H]TdR) incorporation in cultured C3H/10T1/2 mouse embryo fibroblasts. The cells were first treated with 3-methylcholanthrene as an initiator and then with promoters according to schedules that produce oncogenic transformation. The levels of 3-methylcholanthrene used did not affect the growth or [3H]TdR incorporation of the cells. Treatment during the log phase of growth with 12-O-tetradecanoyl-phorbol-13-acetate, phorbol didecanoate, or 4alpha-phorbol didecanoate produced a transient inhibition of [3H]TdR incorporation with the maximum at 12 hr after treatment. This resulted in a temporary delay of growth followed by recovery of the normal cell-doubling time. Phorbol did not produce these effects, suggesting that the inhibition of DNA synthesis is associated with the process of promotion. Although treatment of the cells with 12-O-tetradecanoyl-phorbol-13-acetate during stationary phase resulted in a 2- to 3-fold stimulation of [3H]TdR incorporation, multiple treatments spanning log and stationary phases were found to be necessary for promotion.

[Enuresis--symptom of delayed micturition development or an obstructive urinary tract anomaly?].

In a review of the literature the question of origin of enuresis was studied. Most authors agree that diminished bladder capacity and involuntary detrusor contractions are the main cause of primary enuresis. Urodynamic investigations are necessary to differentiate developmental delay in bladder function form true anatomic infravesical obstruction. Therapeutic measures are successful in 2/3 of enuretics. Enuresis has a great tendency of spontaneous disappearance around 15% per year.
