Coexistent double gammopathy, myeloproliferative disorder, and malignant lymphoma.

The authors report a patient with coexistent double gammopathy, a Philadelphia chromosome-negative, bcr rearrangement-negative myeloproliferative disease resembling chronic myelocytic leukemia and a malignant lymphoma of B-cell origin. The double gammopathy consisted of IgM (kappa) and IgG (kappa). Peripheral blood, spleen, and marrow lymphocytes had primarily an IgG (kappa) isotype, whereas lymph node lymphocytes had predominantly an IgM (kappa) surface isotype. Increased numbers of marrow lymphocytes stained doubly for both IgM (kappa) and IgG (kappa). The results suggest that doubly isotypic as well as single isotypic lymphocytes contributed to the double gammopathy. Organ localization differed for lymphocytes with different antibody isotypes. This cluster of findings has not been described previously.

[Pre- and postoperative magnetic resonance tomography of osteochondrosis dissecans].

Thirty-five patients treated surgically and three treated conservatively were examined by MRT. This proved to be an imaging method which could provide information concerning the state of the bone fragments, the continuity of the joint cartilage and the state of reintegration of the dissected fragment. This was fixed by means of fibrin, autologous spongiosa and resorbable pins.

Ty1 transposition in Saccharomyces cerevisiae is nonrandom.

A large collection of Ty1 insertions in the URA3 and LYS2 loci was generated using a GAL1-Ty1 fusion to augment the transposition frequency. The sites of insertion of most of these Ty elements were sequenced. There appears to be a gradient of frequency of insertion from the 5' end (highest frequency) to the 3' end (lowest frequency) of both loci. In addition we observed hotspots for transposition. Twelve of the 82 Ty1 insertions in the URA3 locus were inserted in exactly the same site. Hotspots were also observed in the LYS2 locus. All hotspots were in the transcribed part of the genes. Alignment of the sites of insertion and of the neighboring sequences only reveals very weak sequence similarities.

[Sonographic leg length measurement].

After brief presentation of the clinical and radiological methods to measure the leg length and the leg length difference the authors outline the new diagnostic method for measuring the leg length and the leg length difference by means of real time sonography. Postmortem tests and clinical examples show that ultrasound is ideal to determine exactly the length of femur and tibia. The joint gaps on the hip, knee and upper ankle joint can be demonstrated by means of a 5 MHz linear scanner. A 1 mm strong metal bar on the skin and under the scanner is placed at right angles to the longitudinal axis of the body so that the bar can be seen in the centre. A measuring device gives the distances of the joint gaps in cm so that the difference correspond to the real length of femur and tibia. This standardised measuring is done by a particularly developed bearing and measuring device. The results of the sonographical measurements on 20 corpses and checking after consecutive dissections showed in 75% of cases a 100% sonographic measuring accuracy of the total leg length. The separately considered results for femur (85%) and tibia (90) were even better. The maximum sonographic measuring fault was 1.0 cm for the femur (in one case) and 0.5 cm for the tibia, respectively. All sonographic measurements were performed with the Sonoline SL-1 of the Siemens Company (Erlangen, W-Germany). Thus, sonographical measuring of the leg length offers a reliable, non-invasive method that can be repeated as often as necessary and is simply executed.(ABSTRACT TRUNCATED AT 250 WORDS)

[Does technetium scintigraphy in combination with gallium scintigraphy improve the differential diagnosis of aseptic and septic endoprosthesis loosening?].

In the majority of cases, 3-phase scintigraphy using Tc99m phosphate complexes is sufficient to diagnose a septic (infective) loosening of an endoprosthesis (hip or knee). Additional Ga 67 citrate scintigraphy should be considered only if technetium scintigraphy fails to supply an unequivocal finding. This is explained by the authors in detail.

[Measuring leg length and leg length difference with the method of real time sonography].

The maximum sonographic measuring fault was 1.0 cm for the femur (in one case) and 0.5 cm for the tibia, respectively. Thus, sonographic measuring of the leg length offers a reliable, non-invasive and easily performed new method that can be repeated any number of times. It is ideal for the development control of therapeutically influenced as well as spontaneous transformations of leg length differences.

[Refixation in osteochondrosis dissecans with resorbable material using magnetic resonance tomography in follow-up monitoring].

A new method for refixation of osteochondritis dissecans and osteochondral flake fractures by using absorbable pins, fibrin adhesive and autologous spongiosa transplantation is described. The diagnostics can be improved by using MR (magnetic resonance imaging) to demonstrate the preoperative status of the defect as well as the postoperative reintegration of the flake, bone vitality and continuity of the joint cartilage. MR information is an excellent addition to the conventional radiograms for preoperative planning as well as for postoperative decision when to allow daily and sports activities. The clinical and radiological preliminary results of the described method show a complete reintegration in all cases of osteochondritis dissecans of the femur condyle.

[A holding device for standardized roentgenologic detection of ulnar, palmar and radial capsule ligament lesions of the metacarpophalangeal joint of the thumb. Hyperextensometer for the index finger metacarpophalangeal joint for determining laxity of the ligament].

A device has been designed to hold the limb for standardized radiological demonstration of lesions of the ulnar and radial collateral and palmar ligaments of the metacarpophalangeal joint of the thumb. The device also serves as a hyperextensometer of the metacarpophalangeal joint of the index finger with which to measure joint laxity. The simple mechanism allows routine application. Standardized stressed roentgenograms allow exact quantification of the extent of all lesions and consequently leads to adequate treatment.

A primary genetic linkage map for human chromosome 12.

A primary genetic map for human chromosome 12 has been constructed from data on 23 restriction fragment length polymorphic systems collected in 38 normal families with large sibships. Linkage analysis of the genotypic data has ordered 16 loci into a continuous genetic map of 111 cM in males and 258 cM in females. Although most of the genetic map reflects a higher rate of recombination in females relative to males, significantly more frequent recombination was observed in males than in females in intervals between loci on the distal portion of the short arm of the chromosome. The mapping data shown here will serve as a first step toward a high-resolution genetic map for human chromosome 12.

[A holder for plain x-ray visualization of ulnar, volar and radial injuries of the capsule and ligaments of the basal thumb joint. A hyperextensometer for the determination of the ligamentous laxity of the index finger basal joint].

We have constructed a device for standardized radiodiagnostics of ulnar, volar, and radial capsulo-ligamentous lesions of the basal thumb joint. Used as hyperextensometer at the basal joint of the index finger, the device is of equal value in the diagnosis of ligamentous laxity.

Bacterial mutagenicity and chemical analysis of polycyclic aromatic hydrocarbons and some nitro derivatives in environmental samples collected in West Germany.

Snow and air particulate samples collected in Upper Frankonia, Federal Republic of Germany, have been analyzed for nitro-polycyclic aromatic hydrocarbons (PAH) and PAH content. A novel clean-up technique has been developed enabling interfering organochlorine environmental contaminants to be removed prior to analysis of the hydrocarbons by GC-MS. Mass fragmentation patterns are presented for 1-nitropyrene, 6-nitrobenzo(a)pyrene, 6-nitrochrysene, and 3-nitrofluoranthene. The level of these compounds found in air samples was in the range of 0.2-2.0 ng.m-3 with the exception of 6-nitrobenzo(a)pyrene, which was not detected. This compares with PAH values of between 1 and 6 ng.m-3. The freshly fallen snow sample collected at the side of a motorway had no detectable PAHs or nitro-PAHs. Parallel studies on the bacterial mutagenicity of the collected air samples using Salmonella typhimurium TA98 and TA100 in the presence and absence of aroclor-induced rat liver "S9" revealed both "direct" and "indirect" activity. Larger numbers of mutants were induced in the presence of S9 than in its absence. The snow sample was devoid of mutagenic activity. These studies show the utility of the biological approach to screen environmental samples prior to expensive and time-consuming chemical analysis.

Non-vascular transplantation of the rabbit uterus.

Transplantation of the uterus may be an optimal solution in young hysterectomized patients, in women with agenesis of the uterus or in severe uterine anomalies. In the human model, attempted tubal and ovarian homografts have usually failed. Eighteen rabbits underwent unilateral non-vascular uterine transplantation. Four rabbits underwent total unilateral uterine autograft. Three uteri showed severe inflammation and necrosis 72 h, 1 week and 1 month postoperatively. Thereafter, the operative technique was modified to avoid contamination from the vagina and four rabbits underwent supravaginal unilateral non-vascular autograft. Three uteri had preserved endometrial and myometrial structure on relaparotomy. The following four rabbits underwent homotransplantation of the uterus and showed acute rejection of the transplant. Under cyclosporine therapy three homotransplanted rabbits developed pelvic abscesses while three other rabbits showed preserved myometrial and endometrial structures.

Representativity of mosses as biomonitor organisms for the accumulation of environmental chemicals in plants and soils.

The suitability of mosses for air pollution monitoring of benzohexachloride isomers and polyaromatic hydrocarbons is shown by residue data of different samples from Europe. The interpretation of the results makes it obvious that next to regional pattern analysis, hypotheses for atmospheric transport and deposition processes of different environmental chemicals can also be formed. An evaluation of these kinds of bioindicator methods is presented by a quantitative comparison of air pollution data and accumulated residues in plants. The results indicate a high retention efficiency of mosses for pollutants dominantly adsorbed to particulate matter in the air, like polyaromatic hydrocarbons and heavy metals. The comparison of residue data of trace pollutants in mosses and other plants underlines the indicator functions of lower plants for air monitoring patterns with the exception of chlorinated hydrocarbons. They are more effective enriched by coniferous plants which contain ingredients able to absorb and transport these groups of environmental pollutants in the organism.
