Circadian patterns of plasma cortisol, 17-hydroxyprogesterone, and testosterone in congenital adrenal hyperplasia.

In 11 children aged between 2 and 17 years with (nonsalt-losing) congenital adrenal hyperplasia (21-hydroxylase deficiency) blood was drawn at 90-minute intervals during a 24-hour period and levels of 17-hydroxyprogesterone, testosterone, and cortisol were measured. Levels of 17-ketosteroids and pregnanetriol were measured too in 24-hour urine samples. These measurements were taken under different regimens of treatment and after interruption of treatment. Cortisol level rose and fell rapidly after administered corticosteroid, and reached unphysiologically high levels. Testosterone levels showed pronounced variations but stayed in the normal range for most of the time even in untreated patients; thus testosterone provides a poor control parameter. Levels of 17-hydroxyprogesterone showed extreme fluctuations and very high peak levels in untreated patients; standard treatment with two or three daily doses of corticosteroids did not prevent a pronounced rise in its level after midnight. After the first morning dose of hydrocortisone a very steep fall was observed. The 24-hour pregnanetriol excretion correlated well with the corresponding total integrated 17-hydroxyprogesterone area. It is concluded that single 17-hydroxyprogesterone values are unlikely to give adequate information about the quality of treatment.

[Serum levels of 25-hydroxycholecalciferol (25-OH-D3) in children of different age groups (author's transl)].

Serum 25-OH-D3 levels were determined in 78 children aged 6 weeks to 15 years, in March 1977 and March 1978 in Vienna. The total number of probands was divided into age groups, each comprising a range of 3 years. The average value with 1 SD. found over the whole age range was 48.3 +/- 25.75 nmol/l (i.e. 19.4 +/- 10.34 ng/ml). Somewhat higher levels were found in the age groups 3 to 6 and 6 to 9 years, with a slight decrease in the later age groups, but no significant differences were detected. The obtained values seem to represent the "normal" 25-OH-D3 serum levels of our population in the above-mentioned age range.

[Rickets prophylaxis by daily administration of vitamin D tablets (author's transl)].

The efficiency of a daily vitamin D prophylaxis with tablets ("Laevovit D3"), containing 1000 I.U. cholecalciferol was studied in newborn babies (most of them "prematures") over a period of about 5 weeks. The blood-values of 25-hydroxycholecalciferol were taken as parameter and determined 1 week and 5 weeks after the start of vitamin D prophylaxis. The data obtained were--with very few exceptions-- in a range considered to be protective against development of vitamin D deficiency. No significant difference could be found comparing vitamin D prophylaxis with these "microtablets" vs. liquid vitamin D preparations.

[Plasma 25-hydroxycholecalciferol after daily vitamin D administration in comparison with massive single-dose prophylaxis (author's transl)].

The effect of daily administration of 1200 IU vitamin D3 was compared with the response to a single oral dose of 200,000 IU in 21 young infants by determination of the plasma 25-OHCC levels. After about one week the mean value recorded in group 1 was 27 +/- 13 ng/ml; the corresponding value in group 2 was 127 +/- 78.4 ng/ml. At the second control examination about one month subsequently, the mean values were 61 +/- 26.2 ng/ml and 104 +/- 69.0 ng/ml respectively. The wide range of values found in the single-dose group clearly demonstrates the advantage of daily vitamin D administration for routine prophylaxis against rickets in infancy.

[The familial incidence of various types of hyperlipoproteinaemia (author's transl)].

Serum lipid values were determined in the first degree relatives of 31 children within the framework of a screening programme for cardiovascular-disease risk factors in schoolchildren of either sex (11 to 12 years old). Familial hyperlipoproteinaemia was not present in 5 families. The investigations revealed 12 families with hypercholesterolaemia and 2 with hypertriglyceridaemia. 12 families showed that form of hyperlipoproteinaemia which has been described in the literature as "familial combined" or "multiple type" hyperlipoproteinaemia. Spontaneous variation in serum lipids was observed in 4 out of the 12 above-mentioned children when determinations were repeated. The variability in expression of hyperlipoproteinaemia in the individual case, as well as in families, is one of the reasons for the controversy over the classification of hyperlipoproteinaemias. It is well known that hyperlipoproteinaemia, especially of the familial type, represents a significant risk factor for cardiovascular disease. Thus, it is essential to treat persons with elevated serum cholesterol or triglyceride values and to examine their relatives for hyperlipoproteinaemia.

[Scintigraphic studies of skeletal abnormalities during childhood (author's transl)].

An extensive application of nuclear medical examination in the field of pediatric diagnostics was only made possible by introduction of short-lived radionuclides. On the one hand, they cause low radiation exposure, on the other they allow short-term check-ups. Analysis carried out on examination results of 47 children revealing various skeletal disorders showed a combination of radiologic-nuclear medical examination methods to produce maximal information suitable for both diagnostics and therapeutical planning. Specific questions such as termination of pathologic alteration process within the skeleton, can be answered only by the use of the scintigraphy. With reference to 4 cases the wide range of possibilities and limitations of skeletal scintigraphy in pediatrics are shown.

[A new antigonadotropin in the treatment of precocious puberty and pubertal gynaecomastia (author's transl)].

A synthetic steroid compound derived from testosteron (isoxazol-ethisterone), Danazol, with gonadotropin-depressing activity, was used in the treatment of 4 cases of idiopathis sexual precocity (age 2 1/2 to 4 years) and in 10 cases of severe pubertal gynaecomastia. In sexual precocity the suppression of menstruation as well as of breast-enlargement was good, while the suppression of acceleration of longitudinal growth and bone maturation was inferior compared with cyproteron-acetate. In most boys with gynaecomastia a marked regression of breast enlargement occurred within a few weeks or months. With the dosage used (200-300 mg/day in the sexual precocity patients, 300-400 mg in the gynaecomastia patients) the changes in plasma hormone levels (LH, FSH, progesterone, estradiol, testosterone) were within a non significant range. Depression of testosterone seemed to be a rather regular finding. No untoward side-effects of the medication were noticed in the 14 patients studied. In summary, Danazol did not show any advantages compared with the compounds used in the treatment of isosexual precocity sofar. In contrast, the drug proved to have useful effects in pubertal gynecomastia, a condition which in severe degrees certainly deserves medical treatment.

[Chronic hypophosphatemic osteopathy ("rachitis"). Clinico-osteological review].

Chronic hypophosphatemia is the most common type of "resistant" rickets. Bone deformities, dwarfism and X-chromosomal dominant heredity, are regular findings in this "inborn error" of phosphate metabolism. Continumous administration of high dosage of vitamin D results in some improvement of the rickets-like bone lesions. Oral administration of phosphate seems to improve longitudinal growth. A summary of our experiences in 44 patients is presented.

[So-called "pseudorheumatoid" nodules in childhood (author's transl)].

This report concerns 4 patients, 2 to 2,5 years of age, with subcutaneous "pseudorheumatoid" nodules in the occipital region. None of the children suffered from any real rheumatic or rheumatoid disease, nor were there any biochemical signs of those diseases detectable. The course of the nodules was benign, they disappeared within several years, and no later rheumatic disease occurred. Extirpation and histology was performed in all 4 cases, the microscopic diagnosis being uniformly "rheumatic granuloma". The own observations are confronted with the literature. It is concluded that, inspite of the histological similarity of true rheumatic granuloma with the "pseudorheumatic" nodules, no etiologic relations between the two conditions seem to exist. The benignity of the pseudorheumatic nodules is underlined, and the diagnostic and prognostic importance of the condition is stressed.

[Serum levels of LH and FSH in patients with adreno-genital syndrome (AGS) (author's transl)].

Serum levels of LH, FSH, testosterone and 17beta-estradiol were estimated by radioimmunoassay in 13 children suffering from AGS. Hormone levels were determined during and after substitution therapy and were compared with values registered in normal subjects. After therapy was stopped a statistically significant rise of testosterone and 17beta-estradiol was observed, but no changes in the serum levels of LH and FSH was noted. The discrepancies between the two observations are discussed.

[Results of the exercise (ergometer) test as a diagnostic screening procedure in children with short stature].

The so-called Exercise Test of Lacey and coworkers (1973) is based on the fact that a functioning pituitary can be stimulated by physical exercise. The test was performed in 28 children of short stature as an out-patient screening test. The results were satisfactory since a growth-hormone deficiency could be excluded in 70% of the cases. The convenience of this test, which entails the investigation of only two blood samples, is stressed in respect to paediatric practice.

[Gonadal dysgenesis, A follow-up study of 25 cases (author's transl)].

Gonadal dysgenesis (GD) is a rather frequent type of sex chromosome aberration, the rate being approximately 1 in 2000 newborn females. Many problems arise from this condition, not only in terms of symptomatology and diagnosis but mainly concerning the long term medical care. 25 patients with the "pure" GD-syndrome (cytogenetic type 45 X), or mosaicism without Y-cells, ranging in age between 4 and 35 years were studied. The data of the family- and birth history, symptoms of dysmorphism, cytogenetics, longitudinal growth, "maturation" with and without hormone therapy will be presented. From our series it could be confirmed that the full phenotype of Turner's syndrome occurs only in the 45 X-type. Mosaic cases often lack some of the Turner's-symptoms, but dwarfism was always present. Adult final height resulted between 140 and 145 cms, usually. No positive influence of hormone substitution on body-growth could be found. An important part of the study was the follow-up of psychosocial development of 6 patients between 20 and 35 years of age, who showed a better integration into normal life than anticipated. Details on all the patients from psychosocial view points can be found in the following paper of this issue, by Dr. L. Haselbacher.
