Cerebrospinal fluid changes in experimental cardiopulmonary bypass using hemodilution with glucose water.

Cardiopulmonary bypass using hemodilution with isotonic glucose water was performed on seven dogs. Intense systemic metabolic acidosis, hyponatremia, hypochloremia, and hyperglycemia were accompanied by only comparatively small changes in the corresponding cerebrospinal fluid values. The data suggested that in the present study, cardiopulmonary bypass was not associated with gross disruptions of the barriers for bicarbonate, sodium, chloride, and glucose between blood and cerebrospinal fluid.

A review of the current impact of measles in the People's Republic of China.

Before the vaccine era, epidemics of measles occurred in alternate years on the mainland of China. During epidemic years the annual incidence was as high as 1,000-5,000 cases/100,000 population, with a fatality rate of 1% to 2%. In 1965, when highly attenuated measles vaccine was developed in China, a vaccination campaign was launched throughout the country. In its early years, the immunization program was not implemented simultaneously at all places, and outbreaks that continued to occur in some areas resulted in a moderate local incidence of measles despite a decline in the overall incidence. From the late 1970s on, stronger measures were taken by local health authorities with regard to measles vaccination. By 1980 the incidence had dropped further; in that year the number of reported cases was 570,037, with 3,862 deaths. Both morbidity and mortality related to measles dropped drastically after vaccination from levels in the prevaccine era, but measles still accounts for an appreciable proportion of all cases of infectious disease.

Introduction to the control of measles by vaccination in the People's Republic of China.

Measles vaccine is provided free of charge in China. In big cities the section clinics are responsible for measles vaccination. In rural areas, barefoot doctors go from door to door to administer the vaccine, or parents are told to bring their children to the cooperative medical station for vaccination. In most parts of China, eight months is considered the optimal age for primary vaccination. The seroconversion rates are usually greater than 90%. With the elapse of time, titers of hemagglutination-inhibiting antibody decreased regularly and even declined to an undetectable level. As a result some people who had been vaccinated became ill with measles. Thus, one vaccination cannot guarantee persistence of antibody for life. It would be appropriate to carry out revaccination for maintenance of adequate immunity against measles. In view of the situation in China as a developing country, the main goal of the campaign against measles in the near future should be the broadening of effective vaccination coverage for control of epidemics, the reduction in incidence, and the lowering of the fatality rate. Measles can definitely be eliminated as has already been done for smallpox.

Undifferentiated carcinoma: an immunohistochemical and ultrastructural study.

Twenty-eight undifferentiated carcinomas (UCs) were immunohistochemically investigated with antibodies against cytokeratins (CKs), vimentin, p53 protein, c-erbB-2 protein and CEA. The diagnoses were based on the findings of conventional histopathology, immunohistochemistry and electron microscopy. CKs8, 18 and 19 were the CKs most frequently present in these tumors, in 61%, 61% and 82% of the cases, respectively. Nine of the 28 (32%) UCs were CKs5/6 positive. Expression of CK20 was found in three (11%) cases. Four UCs were sub-type Cks negative, but one of them was confirmed AE1/AE3 positive. P53 protein overexpression was found in nine (32%) cases. One of the patients with p53 protein positive tumors has been alive for 174 months. Nine (32%) UCs expressed vimentin, which included all of the three thyroid UCs. Comparing with our previous study of squamous cell carcinomas, we found that vimentin and CK18 are more frequently expressed in UCs. The overexpression of p53 protein is similar in the two groups of carcinomas and thus, p53 protein is not a differentiation marker in these tumors. Finally, we recommend the use of a CK "cocktail of antibodies" in the diagnosis of UCs.

Lack of expression of c-erbB-2 oncoprotein in human esophageal squamous cell carcinomas.

Fifty-one cases of esophageal squamous cell carcinoma were immunohistochemically investigated with monoclonal c-erbB 2 oncoprotein antibody. No tumor showed plasma membrane immunostaining for c-erbB-2 protein. Occasionally, unspecific cytoplasmic staining was found in minor tumor cell populations. The present results show that c-erbB-2 oncoprotein is not overexpressed in esophageal squamous cell carcinomas and does not play a central role in the tumorigenesis.

Canadian recommended nutrient intakes underestimate true energy requirements in middle-aged women.

To examine whether Health Canada's Recommended Nutrient Intakes (RNI) and FAO/WHO/UNU (Food and Agriculture Organization, World Health Organization, United Nations University) values provide accurate indices of true energy requirements, energy expenditure was determined using doubly labelled water (DLW) over 13 days in a group of 29 middle-aged women. Energy intakes were calculated from weighed food intake, and energy expenditures and intakes were then compared with individual calculated RNI requirements. The mean energy requirement as determined by DLW expenditure (9.56 +/- 0.53 MJ/d) was higher (p < 0.0001) than reported energy intake (7.08 +/- 0.30 MJ/d) and was higher (p < 0.004) than RNI mean energy requirement (7.97 +/- 0.18 MJ/d). The mean RNI for energy was also lower (p < 0.0001) than that derived from FAO data. These results suggest that current Health Canada RNIs are inadequate in predicting the energy needs of Canadian middle-aged women.
